Farm Przegl Nauk, 2010, 12 no w stawach, jak i innych narządach, w przebiegu reumatoidalnego zapalenia stawów [35]. Rituksymab jest chimerycznym przeciwciałem monoklonalnym przeciwko CD20, będącym immunoglobuliną IgG1k, której cząsteczka składa się z łańcuchów lekkich mysich i łańcuchów ciężkich pochodzenia ludzkiego. Stosowanie tego preparatu u osób chorych na reumatoidalne zapalenie stawów powoduje zmniejszenie aktywności procesu zapalnego w stawach. Rituksymab może być stosowany zarówno w monoterapii, a także w skojarzeniu z metotreksatem lub cyklofosfamidem [28, 34, 40]. Abatacept, określany również jako CTLA-4lg to nowy lek klasy selektywnych modulatorów kostymulacji limfocytów T. Lek ten jest rozpuszczalnym rekombinowanym białkiem fuzyjnym, które wpływa hamująco na interakcje kostymulujące pomiędzy receptorem CD28 i ligandami CD80/ CD86, co przeciwdziała aktywacji limfotytów T. Abatacept – w odróżnieniu od inhibitorów TNF-α – nie neutralizuje bezpośrednio cytokin prozapalnych, ale swoiście hamuje aktywację limfocytów T. Wskazaniem do podania abataceptu jest umiarkowana lub ciężka postać reumatoidalnego zapalenia stawów, z niedostateczną odpowiedzią na jeden lub więcej leków modyfikujących przebieg choroby, takich jak metotreksat lub inhibitor TNF-α. Abatacept zmniejsza przede wszystkim progresję zmian strukturalnych w stawach [39, 41]. Podsumowanie Odpowiednio dobrana i wcześnie włączona terapia lekami modyfikującymi przebieg choroby skutecznie hamuje proces zapalny, powstawanie nadżerek oraz zapobiega uszkodzeniu tkanek i niepełnoprawności w przebiegu reumatoidalnego zapalenia stawów. W 2010 roku Europejska Liga do Walki z Chorobami Reumatycznymi opracowała nowe zalecenia dotyczące leczenia osób chorych na reumatoidalne zapalenie stawów klasycznymi (syntetycznymi) i biologicznymi lekami przeciwreumatycznymi modyfikującymi przebieg choroby (Ryc. 1) [42, 43]. W zaleceniach tych zwrócono szczególną uwagę na rozpoczęcie leczenia osób chorych na reumatoidalne zapalenie stawów syntetycznymi LMPCh, niezwłocznie po rozpoznaniu tej choroby. Grupa ekspertów ELUAR stwierdziła, że każde opóźnienie w zażywaniu leków modyfikujących przebieg choroby u pacjentów chorych na reumatoidalne zapalenie stawów pogarsza ich rokowania [42, 43]. W aktualnych zaleceniach Europejskiej Ligi do Walki z Chorobami Reumatycznymi zwrócono również szczególną uwagę na metotreksat, jako lek tzw. pierwszego rzutu, zarówno w monoterapii, jak i leczeniu skojarzonym, co wynika ze skuteczności i względnego bezpieczeństwa stosowania tego leku [43].
