The most important step of treatment<br />
Lung cancer, which starts with persistent coughs, is known as the<br />
second most common type of cancer. Although it is referred to as scary,<br />
we can easily allege that it is now successful thanks to the developing<br />
technology. The robotic surgery method, which was developed in the<br />
light of technological developments and considered as the last point<br />
of surgery, provides important advantages such as less bleeding, less<br />
pain and lower risk of complications. Surgery, which will be applied<br />
according to the treatment plan, can be performed with open and<br />
closed methods, and the field of use is expanding further with the<br />
development of technology. At the same time, the robotic surgery<br />
technique can be safely applied in all types of lung cancer deemed<br />
suitable for surgery.<br />
This technique, which provides great convenience to both the patient<br />
and the physician, reduces the complication rates that may develop<br />
by returning the person to his daily life more easily. We should not<br />
forget that early diagnosis of lung cancer, which is mostly diagnosed<br />
between the ages of 50-70, has a great importance for treatment.<br />

Tedavinin en önemli basamağı<br />
İnatçı öksürüklerle başlayan akciğer kanseri, en sık görülen ikinci<br />
kanser türü olarak biliniyor. Korkutucu olarak anılsa da gelişen<br />
teknoloji sayesinde artık başarıyla sonuçlanabildiğini rahatlıkla<br />
söyleyebiliriz. Teknolojik gelişmeler ışığında geliştirilen ve cerrahinin<br />
geldiği son nokta olarak değerlendirilen robotik cerrahi yöntemi<br />
hastalara daha az kanama, daha az ağrı ve daha düşük komplikasyon<br />
riski gibi önemli avantajlar sağlıyor. Tedavi planına göre uygulanacak<br />
olan cerrahi, açık ve kapalı yöntemler ile yapılabiliyor ve teknolojinin<br />
gelişmesi ile kullanım alanı daha da genişliyor. Aynı zamanda robotik<br />
cerrahi tekniği, ameliyata uygun görülen bütün akciğer kanseri<br />
türlerinde güvenle uygulanabiliyor.<br />
Hem hastaya hem de hekime büyük bir kolaylık sağlayan bu<br />
teknik, kişiyi günlük yaşantısına daha kolay döndürüp gelişebilecek<br />
komplikasyon oranlarını da düşürüyor. Çoğunlukla 50-70 yaşları<br />
aralığında tanısı konulan akciğer kanserinin erken teşhis edilmesinin<br />
tedavi için büyük önem taşıdığını da unutmamamız gerekiyor.<br />

Turk Expo hosted over 5 thousand visitors<br />
Turk Expo 5 bin üzerinde ziyaretçi ağırladı<br />
T.C Ticaret Bakanlığı koordinasyonunda ve<br />
İstanbul Maden ve Metaller İhracatçı Birlikleri<br />
(İMMİB) organizasyonuyla düzenlenen Türkiye<br />
İhraç Ürünleri Fuarı, 19-21 Mart tarihleri arasında<br />
Suudi Arabistan’ın Riyad kentinde düzenlendi.<br />
Türkiye ile Suudi Arabistan arasındaki ticari<br />
ilişkilerin yeniden tesis edilmesi ve ihracat<br />
hacminin hızla artırılması amacıyla düzenlenen<br />
fuar, 3 gün boyunca 5 binin üzerinde ziyaretçiyi<br />
ağırladı.<br />
Organized under the coordination of the Republic<br />
of Türkiye Ministry of Commerce and with the<br />
organization of Istanbul Minerals and Metals<br />
Exporters’ Association (IMMIB), Turkish Export<br />
Products Fair was held in Riyadh, Saudi Arabia,<br />
between 19 and 21 March. The fair, which was<br />
organized with the aim of re-establishing the<br />
commercial relations between Türkiye and Saudi<br />
Arabia and increasing the export volume rapidly,<br />
hosted over 5 thousand visitors for 3 days.<br />
The Saudi Arabian Turkish Export Products Fair was held by<br />
the General Secretariat of the Istanbul Minerals and Metals<br />
Exporters’ Association (IMMIB) with the coordination<br />
of the Ministry of Commerce between 19 and 21 March<br />
<strong>2023</strong> at the Riyadh International Congress and Exhibition<br />
Center on an area of approximately 5 thousand square<br />
meters. The fair, supported by the Saudi Arabian Ministry<br />
of Commerce, the Federation of Chambers of Commerce<br />
and the Riyadh Chamber of Commerce, is an important<br />
platform for re-establishing commercial relations between<br />
the two countries. The fair opened by T.R. Trade Minister<br />
Dr. Mehmet Muş, Saudi Arabian Trade Minister Abdullah Al<br />
Qasabi and İMMİB Coordinator President Adil Pelister, was<br />
met with great interest. In the fair, which was held with the<br />
participation of 100 companies from Türkiye, mainly from<br />
the industrial products, packaging, home life / furniture,<br />
food / agriculture, cosmetics / cleaning, medicine / medical,<br />
textile / fashion, building materials and service sectors, and<br />
6 Exporters’ Associations, more than 5 thousand visitors<br />
were welcomed.<br />
Suudi Arabistan Türk İhraç Ürünleri Fuarı, İstanbul Maden<br />
ve Metaller İhracatçı Birlikleri (İMMİB) Genel Sekreterliği<br />
tarafından Ticaret Bakanlığı koordinasyonu ile 19-21 Mart<br />

Making an evaluation about the fair, Istanbul Minerals<br />
and Metals Exporters’ Associations (IMMIB) Coordinator<br />
President Adil Pelister said, “We see it as an extremely<br />
important step taken for the future of our exports. Our<br />
fair was met with great interest by Saudi business people<br />
and was very productive. We aim to develop and grow the<br />
commercial business volume between the two countries<br />
and to increase our cooperation within the framework of<br />
Saudi Arabia’s 2030 vision. We attach great importance to<br />
contributing to the new structuring of Saudi Arabia within<br />
the scope of the 2030 vision, increasing the number of<br />
Turkish companies and making partnerships. We expect<br />
Turkish companies to take part in every field, whether in<br />
tourism, construction, chemistry and petrochemistry. Our<br />
trade volume with Saudi Arabia reached $6.55 billion in<br />
2022. We hope to exceed these figures this year. We want<br />
to increase the cooperation and trade volume between<br />
the two countries. I hope that this fair will be beneficial,<br />
auspicious and fruitful for both sides.”<br />
“Our exhibitors are pleased with the intense<br />
interest”<br />
Armağan Vurdu, Secretary General of İMMİB, said that<br />
within the scope of the Turkish Export Products Fair, which<br />
lasted for three days, 6 Exporters’ Association General<br />
Secretariat and 100 Turkish companies operating in various<br />
sectors had the opportunity to promote their products<br />
and services and they hosted approximately 5 thousand<br />
people at the fair. Expressing that they are pleased with<br />
the intense interest of the companies, Vurdu said, “We<br />
organized a fair in Saudi Arabia for the first time where<br />
only Turkish brands and services were exhibited. We saw<br />
that Saudi buyers and importers were satisfied as well as<br />
our participating companies. Our work will continue in the<br />
months and years to come.”<br />

birliğimizi artırmayı hedefliyoruz”<br />
Fuar ile ilgili değerlendirmede bulunan İstanbul Maden ve<br />
Metaller İhracatçı Birlikleri (İMMİB) Koordinatör Başkanı<br />

ikili ticari ilişkilerin geliştirilmesine yönelik oluşturulan<br />
yol haritasında belirlenen hedeflerden biri olan ve 5<br />

“Katılımcı firmalarımız yoğun ilgiden memnun”<br />
İMMİB Genel Sekreteri Armağan Vurdu ise üç gün süren<br />
Türk İhraç Ürünleri Fuarı kapsamında 6 İhracatçı Birliği<br />
Genel Sekreterliği ve çeşitli sektörlerde faaliyet gösteren<br />
100 Türk firmasının ürünlerini, hizmetlerini tanıtma fırsatı<br />
bulduğunu ve yaklaşık 5 bin kişiyi fuarda ağırladıklarını<br />
söyledi. Firmaların yoğun ilgiden memnun olduklarını<br />
dile getiren Vurdu, “Suudi Arabistan’da ilk defa sadece<br />
Türk marka ve hizmetlerinin yer aldığı bir fuar düzenledik.<br />
Katılımcı firmalarımızın yanı sıra Suudi alıcıların ve<br />
ithalatçıların da memnun olduklarını gördük. Gelecek<br />
aylar ve yıllarda çalışmalarımız devam edecek” dedi.<br />

at greatest risk for eating disorders. 75 percent of people<br />
diagnosed with Anorexia Nervosa and 83 percent of people<br />
diagnosed with Bulimia Nervosa are between the ages of<br />
12 and 25.<br />
Causes mental illness and depression<br />
Indicating that eating disorders are often associated<br />
with other psychological and physical disorders such<br />
as depression, anxiety disorders, substance use and<br />
personality disorders, Dietitian Hanife Kara said, “About 55<br />
to 97 percent of people diagnosed with an eating disorder<br />
have a mental illness comorbid condition, about 45 to<br />
86 percent have comorbid depression, about 64<br />
percent have an anxiety disorder, and 58<br />
percent have a comorbid personality<br />
disorder. Again, research shows<br />
that anxiety disorder (especially<br />
social anxiety) may precede<br />
the onset of an eating<br />
disorder. Higher rates of<br />
disordered eating are<br />
described in chronic<br />
health conditions<br />
that require dietary<br />
change, including<br />
celiac disease, cystic<br />
fibrosis, and diabetes.<br />
People with diabetes<br />
(both Type 1 and Type<br />
2) are twice as likely<br />
to develop disordered<br />
eating or eating disorders,<br />
possibly due to the nature<br />
of the disease, including<br />
factors such as weight gain, food<br />
obsession, and feelings of loss.”<br />
75% of people with eating disorders do not<br />
seek professional help<br />
Stating that it is estimated that 75 percent of people with<br />
eating disorders do not receive professional help, Dietician<br />
Hanife Kara said, “The reasons for refusing or not receiving<br />
treatment include exclusion, embarrassment, denial of the<br />
disease, inability to perceive the severity of the disease, the<br />
cost of treatment, lack of incentives and information on<br />
how to access help resources. be considered a deficiency.<br />
The most effective treatment for an eating disorder is<br />
person-centered multidisciplinary care tailored to the<br />
individual’s illness, condition, and needs. The average time<br />
it takes to recover from all types of eating disorders after<br />
receiving treatment is one to six years. Full recovery and a<br />
good quality of life can be achieved for most people with<br />
eating disorders when treatment is provided with skilled<br />
and knowledgeable healthcare professionals.”<br />
12 ila 25 yaş arası riskli grup<br />
Yeme bozuklukları her yaştan insanı etkileyebiliyor ancak<br />
araştırmalar yeme bozukluğunda 12 ila 25 yaş arasındaki<br />
ergenlerin en büyük risk grubu olduğunu gösteriyor.<br />
Anoreksiya Nervoza teşhisi konan kişilerin yüzde 75’i ve<br />
Bulimia Nervoza teşhisi konan kişilerin yüzde 83’ü 12 ila 25<br />
yaş arasında yer alıyor.<br />
Akıl hastalığı ve depresyona sebep oluyor<br />
Yeme bozukluklarının sıklıkla depresyon, anksiyete<br />
bozuklukları, madde kullanımı ve kişilik bozuklukları gibi<br />
diğer psikolojik ve fiziksel bozukluklarla ilişkili olduğunu<br />
söyleyen Diyetisyen Hanife Kara, “Yeme bozukluğu<br />
teşhisi konan kişilerin yaklaşık yüzde 55 ila 97’sinde akıl<br />
hastalığı komorbid durumu, yaklaşık yüzde<br />
45 ila 86’sında eşlik eden depresyon,<br />
yaklaşık yüzde 64’ünde anksiyete<br />
bozukluğu, yüzde 58’inde eşlik eden<br />
kişilik bozukluğu gözleniyor.<br />
Yine araştırmalar, kaygı<br />
bozukluğunun (özellikle sosyal<br />
kaygı) yeme bozukluğunun<br />
başlangıcından önce<br />
gelebileceğini gösteriyor.<br />
Çölyak hastalığı, kistik<br />
fibroz ve diyabet dahil<br />
olmak üzere diyet<br />
değişikliği gerektiren<br />
kronik sağlık koşullarında<br />
daha yüksek düzensiz<br />
yeme oranları tanımlanıyor.<br />
Diyabetli kişilerde (hem<br />
Tip 1 hem de Tip 2), kilo<br />
alma, yemek takıntısı ve kayıp<br />
duyguları gibi faktörler de dahil<br />
olmak üzere muhtemelen hastalığın<br />
doğası nedeniyle düzensiz yeme veya<br />
yeme bozukluğu geliştirme olasılığı iki kat<br />
daha fazla” diyor.<br />
Yemek bozukluğu olan kişilerin yüzde 75’i<br />
profesyonel yardım almıyor<br />
Yeme bozukluğu olan kişilerin yüzde 75’inin profesyonel<br />
yardım almadığının tahmin edildiğini belirten Diyetisyen<br />
Hanife Kara, “Tedaviyi reddetme ya da tedavi olmama<br />
nedenleri arasında dışlanma, utanma, hastalığı inkâr,<br />
hastalığın ciddiyetini algılayamama, tedavinin maliyeti,<br />
teşvik eksikliği ve yardım kaynaklarına nasıl ulaşılacağı<br />
konusunda bilgi eksikliği sayılabilir. Yeme bozukluğu için en<br />
etkili tedavi, bireyin hastalığına, durumuna ve ihtiyaçlarına<br />
göre şekillendirilmiş kişi merkezli multidisipliner bir<br />
bakımdır. Tedavi gördükten sonra tüm yeme bozuklukları<br />
türlerinden kurtulmak için geçen ortalama süre bir ila altı<br />
yıldır. Yetenekli ve bilgili sağlık uzmanları eşliğinde tedavi<br />
süreci sağlandığında yeme bozukluğu olan kişilerin çoğu<br />
için tam iyileşme ve iyi bir yaşam kalitesi elde edilebilir”<br />
şeklinde konuşuyor.<br />

AstraZeneca Türkiye talked about the importance<br />
of everyone’s equal access to cancer treatments<br />
in his statement on April 1 – 7, National Cancer<br />
Week, and talked about the projects they have<br />
implemented in this context.<br />
Cancer is one of the leading causes of death globally,<br />
causing nearly 10 million deaths and nearly one in six<br />
deaths in 2020. According to the data of the same year,<br />
it is estimated that 18.1 million cancer cases are seen<br />
worldwide. For this reason, 1-7 April National Cancer Week,<br />
which brings together public and private institutions that<br />
raise awareness about cancer and carry out studies in<br />
this field, is very important in terms of aiming to prevent<br />
millions of preventable deaths.<br />
Pharm. Serkan Barış, Country Manager, AstraZeneca Türkiye<br />
said, “Immunotherapy, which enables the immune system<br />
to target and eliminate cancer cells, and the emergence of<br />
treatments that personalize cancer treatment according<br />
to the genetic characteristics of the patient are positive<br />
developments. Implementation of diagnosis and screening<br />
programs also ensures that cancer can be detected in<br />
the early stages when it is most suitable for treatment.<br />
However, many people do not have access to these<br />
innovations for reasons such as cost or the lack of specialist<br />

bahsetti ve bu kapsamda hayata geçirdikleri<br />
projeleri anlattı.<br />
2020 yılında yaklaşık 10 milyon kişinin ölümüne ve<br />
neredeyse her altı ölümden birine neden olan kanser,<br />
küresel çapta başlıca ölüm nedenlerinden biri. Aynı<br />
yılın verilerine göre dünya genelinde 18,1 milyon kanser<br />
vakasının görüldüğü tahmin ediliyor. Bu nedenle kanser<br />
konusunda farkındalığı artıran ve bu alanda çalışmalar<br />
yürüten kamu ve özel kurumları bir araya getiren 1- 7 <strong>Nisan</strong><br />
Ulusal Kanser Haftası, milyonlarca önlenebilir ölümün<br />
önüne geçmeyi amaçlaması açısından oldukça önem<br />
taşıyor.<br />
Son yıllarda kanserin önlenmesi ve tedavisine yönelik<br />
çok önemli ilerlemeler kaydedildiğini söyleyen ve<br />

Stating that they are among the institutions leading the<br />
revolution in the field of oncology for cancer treatments,<br />
Barış added that they carry out applications covering four<br />
fields by following science in order to discover and develop<br />
life-changing treatments, put them at the service of health<br />
and increase the treatment potential. Declaring that these<br />
areas are clinical trials, drugs, patient experience, and<br />
employees, Barış said, “Our clinical strategy is designed to<br />
intervene earlier and smarter in cancer to increase survival<br />
time. Our portfolio of drugs for cancer treatment focuses<br />
on the most challenging types of cancer, such as breast<br />
and lung cancers, gastrointestinal cancers, genitourinary<br />
cancers and some blood cancers. As AstraZeneca, we<br />

Scientific collaborations to raise awareness<br />
against cancer<br />
In addition to its clinical studies, AstraZeneca Türkiye also<br />
has important studies to increase social awareness in the<br />
field of cancer in our country.<br />
With the unconditional support of AstraZeneca Türkiye, the<br />
website www.akcigerimizokuzun.com was launched as part<br />
of the “Akciğerimizi Okuyoruz” (means We Are Reading Our<br />
Lungs) project implemented by Turkish Society of Medical<br />
Oncology. This website, which has been visited by more<br />
than 40 thousand people so far, contains information under<br />
the headings of lung cancer symptoms and risk factors,<br />
which is the most common cause of death from cancer<br />
worldwide and in Türkiye.<br />

Kansere karşı farkındalığı artırmak için bilimsel iş<br />
birlikleri gerçekleştiriyor<br />

yanı sıra ülkemizde kanser alanında toplumsal farkındalığı<br />
artırmaya yönelik önemli çalışmaları da bulunuyor.<br />
AstraZeneca Türkiye’nin sağladığı koşulsuz destek ile Türk<br />
Tıbbi Onkoloji Derneği’nin hayata geçirdiği “Akciğerimizi<br />
Okuyoruz” projesi kapsamında www.akcigerimiziokuyoruz.<br />

Within the scope of the “Know Yourself, Early Diagnosis<br />
Breast Cancer Awareness Movement” carried out by the<br />
Needs Map and Cancer Fighters Association with the<br />
unconditional support of AstraZeneca Türkiye, it is aimed<br />
to inform women about breast cancer by explaining the<br />
importance of control and diagnosis and to carry out<br />
screenings at the Cancer Early Diagnosis, Screening and<br />
Education Centers (KETEM) closest to them. To date,<br />
approximately 400 women have been given information<br />
training in Antalya and Trabzon. These women were<br />
provided with breast screening at the nearest KETEM. More<br />
than 10% of the trainees were referred to hospitals for<br />
further testing.<br />
Five associations, each of whom are experts in their fields,<br />
came together for the first time with the unconditional<br />
support of AstraZeneca Türkiye and prepared the “Genetic<br />
Diagnosis Consensus Report” specific to ovarian and<br />
prostate cancers for the first time in Türkiye. In the report<br />
prepared by the Federation of Turkish Pathology Societies,<br />
Turkish Society of Medical Genetics, Turkish Gynecological<br />
Oncology Association, Turkish Society for Radiation<br />
Oncology and Turkish Society of Urooncology it was stated<br />
that the cause of one out of every four ovarian cancers is<br />
hereditary, and it was noted that prostate cancer is also<br />
associated with hereditary breast and ovarian cancer.<br />
With our unconditional contributions, the TRGENTEST<br />
search engine was developed by Turkish Society of<br />
Medical Genetics in order to enable cancer patients to<br />
access molecular genetic tests by providing laboratory<br />
information. On the platform, nearly 200 tests that can be<br />
performed in 112 genetic testing laboratories consisting<br />
of university hospitals, state hospitals, private hospital<br />
laboratories and private laboratories throughout Türkiye<br />
can be questioned.<br />

İhtiyaç Haritası ve Kanser Savaşçıları Derneği’nin,<br />
AstraZeneca Türkiye’nin koşulsuz desteği ile yürüttüğü<br />
“Kendini Tanı, Erken Tanı Meme Kanseri Farkındalık<br />
Hareketi” kapsamında kontrol ve teşhisin önemi anlatılarak<br />
kadınların meme kanseri konusunda bilgilendirilmesi ve<br />
kendilerine en yakın Kanser Erken Teşhis, Tarama ve Eğitim<br />
Merkezi’nde (KETEM) taramalarının yapılması amaçlanıyor.<br />
Şu ana kadar Antalya ve Trabzon’da yaklaşık 400 kadına<br />
bilgilendirme eğitimi verildi. Bu kadınların en yakın<br />
KETEM’de meme taramalarının yapılması sağlandı. Eğitim<br />

özel “Genetik Tanı Uzlaşı Raporu”nu hazırladı. Patoloji<br />
Dernekleri Federasyonu, Tıbbi Genetik Derneği, Türk<br />
Jinekolojik Onkoloji Derneği, Türk Radyasyon Onkolojisi<br />
Derneği ve Türk Üroonkoloji Derneği tarafından hazırlanan<br />
raporda her dört yumurtalık kanserinden birinin nedeninin<br />
kalıtsal olduğu belirtilirken prostat kanserinin de kalıtsal<br />
meme ve yumurtalık kanseri ile ilişkili olduğuna dikkat<br />
çekildi. Kanser hastalarının moleküler genetik testlere,<br />
laboratuvar bilgilerini de vererek ulaşmalarını sağlamak<br />
amacıyla koşulsuz katkılarımızla Tıbbi Genetik Derneği<br />
tarafından TRGENTEST arama motoru geliştirildi.<br />
Platformda, Türkiye genelinde üniversite hastaneleri,<br />
devlet hastaneleri ve özel hastane laboratuvarları ile özel<br />

“Infertility is defined as the inability of a couple to have<br />
children after 12 months of unprotected care. It is seen<br />
in 10-15% of couples and this rate has not changed in the<br />
last 50 years. Women of reproductive age have a 15-18%<br />
chance of getting pregnant in one menstrual period, and<br />
85-90% of them get pregnant at the end of a year. The<br />
incidence of infertility increases with the increasing age<br />
of women. Delaying the age of having children due to<br />
women’s more active participation in business life has led<br />
to a proportional increase in age-related factors among the<br />
causes of infertility.”<br />
Causes of infertility<br />
1. Factors Related to Women<br />
35% of infertility causes are women-related factors.<br />
a. 5-10% of these are factors related to the cervix.<br />
b. Factors related to the uterus include congenital<br />

kadınların bir adet döneminde gebe kalma olasılığı %15-<br />
18 olup, bir yılın sonunda %85-90’ı gebe kalmaktadır.<br />
Kadın yaşının artmasıyla birlikte infertilite sıklığı da<br />
artmaktadır. Kadınların iş yaşamına daha aktif katılmaları<br />
nedeniyle çocuk sahibi olma yaşının geciktirilmesi,<br />
infertilite nedenleri arasında yaşa bağlı faktörlerin<br />
oransal olarak artmasına neden olmuştur.”<br />

1. Kadına Bağlı Faktörler<br />
İnfertilite nedenlerinin %35’ini kadına bağlı faktörler<br />
oluşturmaktadır.<br />

anomalies, pathologies such as fibroids, adenomyosis or<br />
polyps, infections and past infections, adhesions due to<br />
abortion or other surgical interventions.<br />
c. Obstruction of the tubes: Another important cause of<br />
female infertility is obstruction in the tubes due to reasons<br />
such as surgical interventions, infections or endometriosis,<br />
or structural deterioration even if it is open.<br />
d. Again, one of the most important factors related to<br />
women is ovulation disorders.<br />
e. Another important factor causing female infertility is<br />
endometriosis.<br />
f. Apart from these, problems such as stress and depression<br />
can also increase the risk of infertility, although not proven.<br />
2. Male factor:<br />
In 35% of infertile couples, the problem is caused by the<br />
man. Defects in sperm count, shape and movement are the<br />
most common male-related causes of infertility. In addition,<br />
varicocele, congenital occlusion of the channels through<br />
which sperm pass, or disorders in sperm production due to<br />
genetic reasons and sexual dysfunction can also cause male<br />
infertility.<br />
3. In 20% of couples, problems related to men and<br />
women can cause infertility together.<br />
4. Unexplained infertility: In 10-15% of couples, no problem<br />
can be detected in man or woman with routine tests. This<br />
group is called “unexplained infertility”.<br />
5. Environmental factors and toxins: Factors such as<br />
smoking, substance abuse, alcohol use, excessive diet<br />
and exercise can increase the risk of infertility in men and<br />
women. There are studies showing that Biphosphenol<br />

açık olsa bile yapısal olarak görülen bozulmalardır.<br />
d. Yine kadına bağlı en önemli faktörlerden bir tanesi de<br />
yumurtlama bozukluklarıdır.<br />
e. Kadın infertilitesine neden olan önemli faktörlerden bir<br />
diğeri ise, endometriozistir.<br />
f. Bunların dışında, stres ve depresyon gibi sorunlar da<br />
-kanıtlanmamakla birlikte- infertilite riskini artırabilmektedir.<br />
2. Erkek faktörü:<br />
İnfertil çiftlerin %35’inde sorun erkekten kaynaklanmaktadır.<br />
Sperm sayısı, şekil ve hareketindeki bozukluklar infertilitenin<br />
erkeğe bağlı en sık rastlanan nedenlerini oluşturmaktadır.<br />
Ayrıca, varikosel, doğuştan spermin geçtiği kanalların tıkalı<br />
olması veya genetik nedenlere bağlı sperm yapımında<br />
bozukluklar ve cinsel fonksiyon bozuklukları da erkek<br />
infertilitesine neden olabilmektedir.<br />
3. Çiftlerin %20’sinde erkek ve kadına bağlı sorunlar birlikte<br />
infertiliteye neden olabilmektedir.<br />
4. Açıklanamayan infertilite: Çiftlerin %10-15’inde rutin<br />
testlerle erkek veya kadında bir sorun saptanamamaktadır.<br />
Bu grup “açıklanamayan infertilite” olarak adlandırılmaktadır.<br />
5. Çevresel faktörler ve toksinler: Sigara kullanımı, madde<br />
bağımlılığı, alkol kullanımı, aşırı diyet ve egzersiz gibi faktörler<br />

(BPA), which is considered among the environmental toxins,<br />
causes infertility in men.<br />
Investigation of infertility<br />
Infertility investigation should be started in women up<br />
to the age of 35 who fail to conceive despite one year of<br />
unprotected intercourse, and after 6 months in women<br />
over the age of 35.<br />
History: The first stage of infertility research is to obtain a<br />
detailed history from the couples. Here, questions such as<br />
previous systemic and sexual diseases, surgical procedures,<br />
duration of infertility, previous pregnancy and miscarriage,<br />
and a history of previous marriage related to a second<br />
marriage are addressed.<br />
Tests to be done: Today, only 3 tests are sufficient for<br />
infertility. Detailed tests are for research purposes only and<br />
do not affect the treatment process. First of all, we start<br />
the research process with the test we call “spermiogram”.<br />
If the sperm values are normal, the uterine film (HSG) is<br />
checked to investigate the problems in the uterus and<br />
whether the tubes are open, and the progesterone levels<br />
in the blood at a certain period of the menstruation are<br />
checked for the evaluation of ovulation. Another important<br />
test is the evaluation of the ovaries in ultrasound for the<br />
evaluation of ovarian reserve and, if necessary, the level of<br />
AMH in the blood.<br />
Examination: Apart from a general physical examination,<br />
it is very important to evaluate the uterus and ovaries in<br />
women. If a problem is detected in the sperm values, the<br />
man should also be evaluated by the urology.<br />
Treatment<br />
Treatment in infertility is determined by factors such as the<br />
reason for not having children, the age of the woman, the<br />
duration of infertility and ovarian reserve. Couples under<br />
the age of 30, with good ovarian reserve and no significant<br />
problems detected in the tests, can be followed up to 3<br />
years without starting treatment. The most important<br />
factor affecting the treatment decision is the age of the<br />
woman. To summarize briefly:<br />
1. Ovulation disorders: In these couples, the ovaries can be<br />
stimulated for 6-12 months and pregnancy can be achieved<br />
by 40-50% as a result of stimulating the ovaries only with<br />
pills. In unsuccessful couples, “vaccination” and then “in<br />
vitro fertilization” methods are applied.<br />
2. Unexplained infertility: In these couples, treatment is<br />
usually started with “vaccination” and in case of failure, “in<br />
vitro fertilization” treatment is started.<br />
3. Blocked tubes: Depending on the woman’s age, the<br />
cause of the obstruction, the location and degree of<br />
obstruction in the tube, the tubes can be opened primarily<br />
by microsurgery, but nowadays, “in vitro fertilization”<br />
treatment is applied to most of these couples. In addition, if<br />
there is fluid accumulation in the tubes due to obstruction<br />
in these patients, it is recommended to remove the<br />
tubes to increase the chance of pregnancy before in vitro<br />
fertilization.<br />

bulunmaktadır.<br />
İnfertilitenin araştırılması<br />
İnfertilitenin araştırılmasına, kadınlarda 35 yaşına kadar<br />
bir yıllık korunmasız ilişkiye rağmen gebe kalamama<br />
durumunda, 35 yaşın üzerindeki kadınlarda ise 6 aydan<br />
sonra başlanması gerekir.<br />
Öykü: İnfertilite araştırmasının ilk aşaması, çiftlerden<br />
detaylı bir öykünün alınmasıdır. Burada daha önce geçirilen<br />
sistemik ve cinsel hastalıklar, cerrahi işlemler, çocuk sahibi<br />
olamama süresi, daha önce gebelik ve düşük olup olmadığı,<br />
ikinci evliliği olanlarda daha önceki evlilikle ilgi gebelik<br />
öyküsü gibi sorular yönlendirilmektedir.<br />
Yapılacak testler: Günümüzde infertilite ile ilgili sadece 3<br />
test yeterli bulunmaktadır. Detaylı testler sadece araştırma<br />
amacına yönelik olup, tedavi sürecini etkilememektedir.<br />
Öncelikle “spermiogram” dediğimiz testle araştırma<br />
sürecini başlatıyoruz. Eğer sperm değerleri normalse,<br />
rahimdeki sorunların ve tüplerin açık olup olmadığının<br />
araştırılması için rahim filmi (HSG) ve yumurtlamanın<br />
değerlendirilmesi için kanda adetin belirli bir dönemindeki<br />
progesteron düzeylerine bakılmaktadır. Diğer önemli<br />
bir test de yumurtalık rezervinin değerlendirilmesi için<br />
ultrasonda yumurtalıkların değerlendirilmesi ve gerekirse<br />
kanda AMH düzeyine bakılmasıdır.<br />
Muayene: Genel bir fizik muayenenin dışında, kadında rahim<br />
ve yumurtalıkların değerlendirilmesi oldukça önemlidir.<br />
Eğer sperm değerlerinde bir sorun saptanırsa, erkeğin<br />
üroloji tarafından da değerlendirilmesi gerekir.<br />
Tedavi<br />
İnfertilitede tedavi, çocuk sahibi olamama nedeni, kadının<br />
yaşı, infertilite süresi ve yumurtalık rezervi gibi faktörlere<br />
bağlı olarak belirlenmektedir. 30 yaşın altında, yumurtalık<br />
rezervi iyi olan ve testlerde önemli bir sorun saptanmayan<br />
çiftler 3 yıla kadar tedaviye başlanmadan izlenebilir. Tedavi<br />
kararını etkileyen en önemli faktör, kadının yaşıdır. Kısaca<br />
özetlersek:<br />
1. Yumurtlama bozuklukları: Bu çiftlerde 6-12 ay süreyle<br />
yumurtalıklar uyarılabilir ve sadece yumurtalıkların hapla<br />
uyarılması sonucu %40-50 oranında gebelik elde edilebilir.<br />
Başarı elde edilmeyen çiftlerde “aşılama” ve sonrasında<br />
“tüp bebek” yöntemleri uygulanmaktadır.<br />
2. Açıklanamayan infertilite: Bu çiftlerde tedaviye genellikle<br />
“aşılama” ile başlanır ve başarı sağlanamaması durumunda<br />
“tüp bebek” tedavisine geçilir.<br />
3. Tüplerin tıkalı olması: Kadın yaşı, tıkanıklığın nedeni,<br />
tüpteki tıkanıklık yeri ve derecesine göre, öncellikle<br />
mikrocerrahi yöntemiyle tüpler açılabilir, ancak günümüzde<br />
bu çiftlerin büyük bir bölümüne “tüp bebek” tedavisi<br />
uygulanmaktadır. Ayrıca, bu hastalarda tıkanıklığa<br />
bağlı tüplerde sıvı birikimi varsa, tüp bebek uygulaması<br />
öncesi gebelik şansının artırılması için tüplerin alınması<br />

4. Endometriosis: Depending on the degree of the<br />
disease, surgical treatment can be tried or reproductive<br />
techniques such as “vaccination” or “in vitro fertilization”<br />
can be applied directly.<br />
5. Male factor: Methods of obtaining sperm from the<br />
testicles are used in patients with no sperm seen in the<br />
tests, using the methods we call “in vitro fertilization”<br />
and TESA-TESE. Sperm can be found surgically in<br />
approximately 50% of this group of patients. Apart<br />
from this group, the treatment method to be chosen is<br />
determined according to the severity of the disorders in<br />
the spermiogram. In couples with mild disorders, first of<br />
all, “vaccination” can be done, while in severe disorders,<br />
“microinjection-in vitro fertilization” treatment is applied.<br />
Additional treatments that increase<br />
success in IVF<br />
In order to increase the chance of success in IVF,<br />
giving various vitamins and supplements, hormonal<br />
supplements such as testosterone and growth hormone,<br />
scratching the uterus, selecting normal embryos by<br />
looking at the genetics of the embryos, PRP applications<br />
to the uterus or ovaries, magnetic methods or largescale<br />
microscopes for sperm selection. Many methods<br />
are tried, such as using the best sperm. However,<br />
unfortunately, there is not enough data to definitively<br />
show the effectiveness of these methods. For this<br />
reason, it is necessary to be careful about these methods,<br />
which significantly increase the cost of treatment, and to<br />
present objective data when informing the couple.<br />
4. Endometriozis: Hastalığın derecesine bağlı olarak cerrahi<br />
tedavi denenebilir veya direkt olarak “aşılama” veya “tüp<br />
bebek” gibi üremeye yardımcı tekniklere başvurulabilir.<br />
5. Erkek faktörü: Yapılan testlerde hiç sperm görülmeyen<br />
hastalarda “tüp bebek” ve TESA-TESE adını verdiğimiz<br />
yöntemlerle testisten sperm elde edilme yöntemlerine<br />
başvurulmaktadır. Bu grup hastaların yaklaşık %50’sinde<br />
cerrahi olarak sperm bulunabilmektedir. Bu grup dışında<br />
seçilecek tedavi yöntemi spermiogramdaki bozuklukların<br />
şiddetine göre belirlenmektedir. Hafif bozukluk görülen<br />
çiftlerde öncelikle “aşılama” yapılabilir, şiddetli bozukluk<br />
olanlarda ise “mikroenjeksiyon-tüp bebek” tedavisi<br />
uygulanmaktadır.<br />
Tüp bebekte başarıyı artıran ek tedaviler<br />
Tüp bebek”te başarı şansını artırmak için çeşitli vitamin<br />
ve takviye edici gıdaların verilmesi, testosteron ve growth<br />
hormonu gibi hormonal takviyeler, rahime çizik atılması,<br />
embriyoların genetiğine bakılarak normal embriyoların<br />
seçilmesi, rahim veya yumurtalıklara PRP uygulamaları,<br />
sperm seçimi için magnetik yöntemler veya büyük ölçekli<br />
mikroskoplarla bakılarak en iyi spermin kullanılması<br />
gibi birçok yöntem denenmektedir. Ancak, ne yazık ki<br />
bu yöntemlerin etkinliğini kesin olarak gösteren yeterli<br />
veri bulunmamaktadır. Bu nedenle, tedavi maliyetini<br />
ciddi ölçüde artıran bu yöntemler konusunda dikkatli<br />
davranılması ve çifte bilgi verirken objektif verilerin<br />

Elastofibroma Dorsi, a rare disease whose medical<br />
name is callus on the back, is mostly seen in<br />
women. This tumor, which is mostly benign, can<br />
negatively affect daily life. Experts emphasize<br />
that it should be surgically removed after<br />
diagnosis. Thoracic Surgeon Associate Professor<br />
Dr. Özkan Demirhan shared what you need to<br />
know about the subject<br />
Explaining the reasons for the formation of back calluses,<br />
Dr. Özkan Demirhan, “Elastofibroma Dorsi is a rare benign<br />
and slow growing chest wall tumor. It is a pseudotumor<br />
of fibroelastic structure that develops due to mechanical<br />
friction and compression between the scapula (scapula) and<br />
the chest wall, and is therefore considered reactive rather<br />
than tumoral. It usually affects the middle age group. It is<br />
more common in women over the age of 40 (female-male<br />
ratio 6/1), although it is mostly unilateral (single and double<br />
incidence) it is 3/1. Most unilateral lesions are seen on the<br />
right side.”<br />
Tıbbi adı sırt nasırı olan nadir hastalık<br />
Elastofibroma Dorsi, daha çok kadınlarda<br />
görülüyor. Çoğunlukla iyi huylu olan bu tümör<br />
günlük hayatı olumsuz etkileyebiliyor. Uzmanlar<br />
ise teşhis konulduktan sonra cerrahi olarak<br />
çıkarılması gerektiğini vurguluyor. Konuyla ilgili<br />
bilmeniz gerekenleri Göğüs Cerrahisi Uzmanı Prof.<br />
Dr. Özkan Demirhan anlattı.<br />
Sırt nasırlarının oluşum nedenlerini anlatan Dr. Özkan<br />
Demirhan,” Elastofibroma Dorsi nadir görülen iyi huylu<br />
ve yavaş büyüyen göğüs duvarı tümörüdür. Kürek kemiği<br />
(Skapula) ve göğüs duvarı arasında mekanik sürtünme ve<br />
basıya bağlı gelişen, bu nedenle tümöral olmaktan çok<br />
reaktif olarak kabul edilen fibroelastik yapıda bir yalancı<br />
tümördür (psödotümör). Genellikle orta yaş grubunu<br />
etkilemektedir. 40 yaş üstü daha çok kadınlarda görülür<br />
(kadın erkek oranı 6/1), çoğunlukla tek taraflı olmasına<br />
rağmen (tek ve çift görülme oranı) 3/1’dir. Tek taraflı<br />
lezyonların çoğu sağ tarafta görülmektedir.” dedi.<br />

More common in heavy workers<br />
Mentioning that the movements that force the arm<br />
trigger the back calluses, Dr. Demirhan said, “Whether<br />
Elastofibroma Dorsi is a true tumor or not is controversial,<br />
and its etiology can be said to be multifactorial. Studies have<br />
reported that the incidence may increase with repetitive<br />
traumas in those who work with muscle strength. The<br />
fact that back calluses are more common around the<br />
right scapula in people who use their shoulders and arms<br />
constantly or do heavy work supports this view. However,<br />
Elastofibroma Dorsi is also observed in people who do not<br />
force their arm throughout their life. It can also be seen in<br />
different locations other than the scapula.”<br />
“Swelling in the scapula and weakness in the arm<br />
can be seen”<br />
Stating that back calluses start to show symptoms when<br />
they are larger than 5 centimeters, Prof. Dr. Özkan Demirhan<br />
said, “50% of the cases do not usually show any symptoms.<br />
In symptomatic cases, most patients have swelling in the<br />
back around the scapula, weakness in the arm, and back<br />
shoulder pain. Other symptoms include a clicking sound<br />
(click) due to the scapula getting caught in the mass, which<br />
occurs with arm movement, joint stiffness, and the scapula<br />
being higher than the other side. Patients presenting with<br />
these complaints should be evaluated radiologically, even<br />
if there are no abnormal findings on physical examination.<br />
From the simplest to the most complex, Ultrasonography<br />
(USG), Computed Tomography (CT) and MRI examination<br />
are very useful imaging methods for this pathology. In<br />
radiology, it is sometimes defined as a malignant masslike<br />
lesion that cannot be clearly differentiated from the<br />
thoracic wall muscles. In fact, the location and clinic of the<br />
lesion is typical for Elastofibroma Dorsi, and there is no need<br />
for incisional biopsy because typical findings are obtained<br />
radiologically. In rare cases, biopsy may be required when<br />
imaging is not sufficient for the diagnosis of Elastofibroma<br />
Dorsi and it is thought to be an aggressive tumor. However,<br />

artabileceği bildirilmiştir. Omuz ve kolunu sürekli kullanan<br />
ya da ağır iş yapan kişilerde, sıklıkla da sağ kürek kemiği<br />
etrafında sırt nasırının daha fazla görülmesi bu görüşü<br />
desteklemiştir. Bununla birlikte Elastofibroma Dorsi, hayatı<br />
boyunca kolunu zorlamayan kişilerde de gözlenmektedir.<br />
Kürek kemiği dışında farklı yerleşimlerde de görülebilir.”<br />
şeklinde konuştu.<br />
“Kürek kemiğinde şişlik ve kolda kuvvetsizlik<br />
görülebilir”<br />
Sırt nasırlarının 5 santimden büyük oldukları zaman<br />
belirti vermeye başladıklarını ifade eden Prof. Dr. Özkan<br />
Demirhan,” Olguların yüzde 50’si genelde belirti vermez.<br />
Belirti veren durumlarda hastaların çoğunda sırtta kürek<br />
kemiği etrafında şişlik, kolda kuvvetsizlik, sırt omuz ağrısı<br />
görülmektedir. Diğer belirtiler arasında kol hareketiyle<br />
ortaya çıkan kürek kemiğinin kitleye takılmasına bağlı<br />
tıkırtı sesi (click), eklem sertliği, kürek kemiğinin diğer<br />
tarafa göre yüksek olması gibi yakınmalar sayılabilir.<br />
Bu yakınmalarla başvuran hastalar, fizik muayenede<br />
anormal bulgu yok ise de mutlaka radyolojik olarak<br />
değerlendirilmelidir. En basitinden komplekse doğru<br />
Ultrasonografi (USG), Bilgisayarlı Tomografi (BT) ve MRG<br />
inceleme bu patoloji için oldukça yararlı görüntüleme<br />
yöntemleridir. Radyolojide bazen toraks duvarı kasları<br />
ile net ayırt edilemeyen, malign kitle benzeri lezyon<br />
olarak tanımlanır. Aslında Elastofibroma Dorsi açısından<br />
lezyonun yeri ve kliniği tipiktir radyolojik olarak da tipik<br />
bulgular elde edildiği için insizyonel biyopsiye gerek<br />
yoktur. Elastofibroma Dorsi tanısında görüntülemenin<br />
yeterli olmadığında ve agresif bir tümör gibi düşünüldüğü<br />
zaman nadir de olsa biyopsi yapılması gerekebilir. Fakat<br />
derin yerleşimli yumuşak doku ve yağ dokusuna uzanan<br />
karakteristik lezyonlarda başka testlere gerek olmayabilir.<br />

other tests may not be necessary in characteristic lesions<br />
extending to deep-seated soft tissue and adipose tissue.<br />
CT and MRI may not be sufficient in small cases of<br />
Elastofibroma Dorsi. Then gadolinium can be used. The<br />
sensitivity of computed tomography in the diagnosis of<br />
Elastofibroma Dorsi is lower than MRI. However, in case of<br />
typical localization and bilateral lesion, CT can be helpful<br />
in diagnosis. In the presence of MRI findings, the diagnosis<br />
of elastofibroma dorsi can be easily considered. In the<br />
presence of atypical findings on MRI, the typical localization<br />
of the lesion, its bilateral nature, and the fact that the<br />
patient is elderly and female supports the diagnosis of<br />
elastofibroma dorsi. The fact that the lesion is bilateral can<br />
largely rule out malignant masses that can be considered<br />
in the differential diagnosis. Therefore, when the diagnosis<br />
of elastofibroma dorsi is clinically suspected, the chest wall<br />
should be evaluated bilaterally. Other chest wall tumors<br />
should also be considered in the differential diagnosis.”<br />
Solution with surgery in treatment<br />
Referring to the treatment methods of back calluses,<br />
Prof. Dr. Özkan Demirhan said, “A solution can be provided<br />
with surgery in the treatment. Surgery should be treated<br />
radically and approached like cancer surgery, and the<br />
slightest mass residue should not be left. In unsuccessful<br />
surgery, the mass may reappear, and a larger surgery may<br />
be required in the second operation. For this reason, the<br />
treatment takes place by completely clearing the tumor.<br />
If the tumor causes functional insufficiency, compression<br />
findings, pain and swelling in the chest wall in the arm and<br />
shoulder, and the diameter of the tumor exceeds 5 cm, it<br />
should definitely be removed. In smaller lesions that do not<br />
cause complaints, the patient can be followed up without<br />
surgical treatment. Since the “Dubling time”, which we call<br />
the doubling time of Elastofibroma Dorsi, is quite long,<br />
there is no evidence that the disease has turned into cancer.<br />
Radiotherapy is rarely used in unresectable masses.”<br />
Küçük Elastofibroma Dorsi olgularında BT ve MR yeterli<br />
olmayabilir. O zaman gadolinium kullanılabilir. Bilgisayarlı<br />
tomografinin Elastofibroma Dorsi tanısındaki duyarlılığı<br />
MRG’ye göre daha düşüktür. Ancak tipik lokalizasyonu ve<br />
iki taraflı lezyon olması durumunda BT tanıda yardımcı<br />
olabilir. MRG bulguları varlığında elastofibroma dorsi tanısı<br />
kolaylıkla düşünülebilir. MRG’de atipik bulguların varlığı<br />
durumunda ise lezyonun tipik lokalizasyonu, iki taraflı<br />
(bilateral) oluşu, hastanın ileri yaşta ve kadın hasta olması<br />
elastofibroma dorsi tanısını destekler. Lezyonun iki taraflı<br />
olması ayırıcı tanıda düşünülebilecek malign kitleleri büyük<br />
oranda ekarte edebilir. Bu yüzden elastofibroma dorsi<br />
tanısından klinik olarak şüphe edildiğinde göğüs duvarı<br />
mutlaka iki taraflı değerlendirilmelidir. Ayırıcı tanıda diğer<br />
göğüs duvarı tümörleri de akla gelmelidir.” şeklinde bilgi<br />
verdi.<br />
Tedavide cerrahi ile çözüm<br />
Sırt nasırlarının tedavi yöntemlerine değinen Prof.<br />
Dr. Özkan Demirhan,” Tedavide cerrahi ile çözüm<br />
sağlanabiliyor. Cerrahide radikal davranılarak kanser<br />
cerrahisi gibi yaklaşılır en ufak bir kitle kalıntısı<br />
bırakılmamalıdır. Başarısız cerrahide kitle yeniden ortaya<br />
çıkabilir ikinci ameliyatta daha büyük bir cerrahi gerekebilir.<br />
Bu nedenle tedavisi tümörün tamamen temizlenmesi ile<br />
gerçekleşir. Tümör kol ve omuz da fonksiyonel yetersizlik,<br />
bası bulguları, ağrı ve göğüs duvarında şişliğe neden<br />
oluyorsa ve tümörün çapı 5 santimi aşıyorsa kesinlikle<br />
çıkarılmalıdır.<br />
Daha küçük ve şikâyete yol açmayan lezyonlarda cerrahi<br />
tedavi uygulanmadan hasta izlenebilir. Elastofibroma<br />
Dorsinin ikiye katlanma zamanı dediğimiz “Dubling<br />
time” suresi oldukça uzun olduğu için hastalığın kansere<br />
dönüştüğüne dair delil yoktur. Rezeke edilemeyen kitlelerde<br />
nadir de olsa radyoterapinin kullanılmaktadır.” dedi.<br />

“Lens seçimi hastanın ihtiyacına göre yapılmalı”<br />
Explaining what is being wondered about<br />
multifocal intraocular lenses and other lens types,<br />
which have become increasingly popular among<br />
the public, Prof. Dr. Nur Acar Göçgil emphasized<br />
that the lense selection should be made according<br />
to the patient’s personal situation and needs.<br />
Noting that not every patient can be fitted with a multifocal<br />
lens, Prof. Dr. Göçgil said, “These lenses cannot be used<br />
in cases of uveitis and some diseases affecting the yellow<br />
spot. Before cataract surgery, considering the anatomical<br />
structure of the eye, health status, and the person’s life and<br />
needs, factors such as how the patient spends his day and<br />
expectations should be taken into account, and the most<br />
suitable lens for each eye should be selected.”<br />
Halk arasında akıllı lens adıyla son zamanlarda<br />
popülerliği gittikçe artan multifokal göz içi<br />
mercekler (lensler) ve diğer lens çeşitleri<br />
hakkında merak edilenleri aktaran Prof. Dr. Nur<br />
Acar Göçgil, mercek seçiminin hastanın kişisel<br />
durumu ve ihtiyacına göre yapılması gerektiğini<br />
vurguladı.<br />
Her hastaya multifokal mercek takılamayacağı konusunda<br />
uyarıda bulunan Prof. Dr. Göçgil, “Üveit, sarı noktayı<br />
etkileyen bazı hastalıkların olduğu durumlarda bu lensler<br />
kullanılamamaktadır. Katarakt ameliyatı öncesinde gözün<br />
anatomik yapısına, sağlık durumuna ve kişinin yaşamı<br />
ile ihtiyacına bakılarak, hastanın gününü nasıl geçirdiği,<br />
beklentisi gibi faktörler dikkate alınmalı ve her bir göze en<br />
uygun mercek seçilmelidir.” dedi.<br />

“As the adaptability of our natural lens begins to<br />
decline, we need close-up glasses”<br />
Stating that we have achieved an increase in the vision<br />
quality of the patients with the application of intraocular<br />
lenses, Prof. Dr. Göçgil said, “Our natural lens in our<br />
eyes adapts by moving forward or backward in order to<br />
perceive different distances more clearly when we look<br />
far or close, and allows us to see clearly. This feature<br />
works properly until the age of 40, although it differs from<br />
person to person after this age, the disorder begins to<br />
occur and we need to use close glasses. The patient has<br />
cataracts and if surgery is required, we can eliminate this<br />
problem to a certain extent thanks to the lenses we place<br />
in the eye during the surgery. We can also use refractive<br />
and diffractive intraocular lenses to help the patient see<br />
at different distances, with the principle of the same<br />
multifocal glasses, in order for the patient to experience<br />
relief in their far and near vision.”<br />
“Monofocal intraocular lenses are the most<br />
commonly used lenses”<br />
Indicating that intraocular lenses are divided into three<br />
main groups according to their optical properties, Prof. Dr.<br />
Nur Acar Göçgil said, “Toric lenses can be used by adapting<br />
to monofocal and multifocal intraocular lenses. Monofocal<br />
lenses are the most commonly used lenses today. Lenses<br />
that support near and intermediate vision are called<br />
multifocal lenses. Multifocal lenses provide two or more<br />
focal distances. In these lenses, the primary focus is far, the<br />
secondary focus is near, and the tertiary focus is mid-range.<br />
These lenses, which have very different optical principles,<br />
also have two different optical designs, refractive and<br />
diffractive. Today, if the patient’s astigmatism is caused by<br />
the cornea, which is the regular and frontmost transparent<br />
layer of the eye, astigmatic disorders of 0.75 D and above<br />
can be corrected with the application of a toric intraocular<br />
lens (T-IOL) during cataract surgery.”<br />
Recording that EDOF, extended dept of focus, intraocular<br />
lenses with increased focal depth can provide vision at<br />
a distance of 60 cm without deteriorating the patient’s<br />
distance vision quality, Prof. Dr. Göçgil said, “For example,<br />
the patient can see the text on the big screen or the<br />
indicator while driving, but if he/she is going to read a book<br />
at 35 cm or closer, he/she feels the need to use a pair of<br />
glasses. These lenses with high patient satisfaction are<br />
preferred when the use of multifocal and trifocal lenses is<br />
not suitable for the patient.”<br />
“The term smart lens is not the right term”<br />
Informing on the trifocal lenses, another lens type that<br />
has been increasing in popularity recently, under the name<br />
of “smart lens” among the public, Prof. Dr. Göçgil said,<br />
“The term smart lens is actually not a correct term. These<br />
lenses, which are included in the multifocal lens group,<br />
are known as smart lenses among the people and cause<br />
scientific misconceptions. Some of our patients refer to it<br />
as a method that can be easily inserted and removed, such<br />

baktığımızda farklı mesafeleri daha net algılayabilmek<br />
için öne ya da arkaya hareket ederek adapte olmakta ve<br />
bizim net görmemizi sağlamaktadır. 40 yaşına kadar bu<br />
özellik düzgün çalışırken bu yaştan sonra kişiden kişiye<br />
farklı olmakla birlikte bozukluk yaşanmaya başlar ve yakın<br />
gözlüğü kullanma ihtiyacı duyarız. Hastanın kataraktı<br />
var ve ameliyat olması gerekiyorsa ameliyatta göz içine<br />
yerleştirdiğimiz lensler sayesinde bu sorunu belirli oranda<br />
giderebilmekteyiz. Hastanın uzak ve yakın görüşünde<br />
rahatlama yaşaması amacıyla, aynı multifokal gözlükteki<br />
prensiple farklı mesafelerde görmeye yardımcı olacak<br />
refraktif ve difraktif göz içi lensleri de kullanabilmekteyiz.”<br />

“Monofokal göz içi mercekler en sıklıkla<br />
kullanılan lenslerdir”<br />
Göz içi merceklerin optik özelliklerine göre monofokal (tek<br />
odaklı), multifokal (çok odaklı; refraktif ve difraktif yapıda)<br />
ve torik olmak üzere üç ana gruba ayrıldığını belirten Prof.<br />
Dr. Nur Acar Göçgil, “Torik lensler monofokal ve multifokal<br />
göz içi merceklere de uyarlanarak kullanılabilmektedir.<br />
Monofokal lensler günümüzde en sıklıkla kullanılan<br />
lenslerdir. Yakın ve orta mesafede görmeyi destekleyen<br />
lenslere ise multifokal lens adını vermekteyiz. Multifokal<br />
lensler iki veya daha fazla odak mesafesi sağlamaktadır.<br />
Bu lenslerde birincil odak noktası uzak, ikincil odak<br />
noktası yakın, üçüncül odak noktası ise orta mesafedir.<br />
Çok farklı optik prensipleri olan bu bu lensler, refraktif ve<br />
difraktif olmak üzere iki farklı optik tasarımına da sahiptir.<br />
Günümüzde hastanın astigmatizması eğer düzenli ve gözün<br />
en öndeki şeffaf tabaka olan korneadan kaynaklanıyorsa,<br />
0,75 D ve üzerindeki astigmatik bozukluklar katarakt<br />
ameliyatı sırasında torik göz içi lens (T-GİL) uygulaması ile<br />
düzeltilebilmektedir.” Şeklinde konuştu.<br />

most positive results. It can take 6-24 weeks for the brain to<br />
adapt to the lenses and to improve vision.”<br />
“The choice of lens should be made according to<br />
the patient’s personal situation and needs”<br />
Pointing out that the patients that multifocal lenses cannot<br />
be placed on every patient, Prof. Dr. Nur Acar Göçgil added<br />
further, “For example, it is not recommended to wear<br />
multifocal lenses for most people with yellow spot disease.<br />
Lense selection should be made carefully according to<br />
the patient’s personal situation and needs. These lenses<br />
cannot be used in cases of certain diseases such as uveitis<br />
and advanced glaucoma. In the selection of the intraocular<br />
lens to be used, it is appropriate to inform the patient in<br />
detail, to learn the anatomical suitability of the eye, as well<br />
as the patient’s expectations and daily activities, and to<br />
carefully convey what type of lens they will be satisfied with<br />
after the surgery. As a matter of fact, these lenses take the<br />
task of our natural lens in our eyes and there is no option<br />
to put on and take off daily. Repeated surgeries also carry<br />
additional risks.<br />
“It is extremely important that the surgeon is<br />
knowledgeable and experienced”<br />
It is also extremely important that the surgeon be<br />
knowledgeable and experienced in terms of determining<br />
the intraocular lens to be selected. Considering the<br />
anatomical structure of the eye, the state of health, and<br />
the person’s life and needs, factors such as how the patient<br />
spends his day and expectations should be taken into<br />
account, and the most suitable lense for each eye should be<br />
selected.”<br />

göz içi merceklerin hastanın uzak görme kalitesi<br />
bozulmadan 60 cm mesafede de görme sağlayabileceğini<br />
belirten Prof. Dr. Göçgil, “Örneğin hasta büyük ekrandaki<br />
yazıları veya araba kullanırken göstergeyi görebilir; ancak<br />
35cm ve daha yakında kitap okuyacaksa bir yakın gözlüğü<br />
kullanma ihtiyacı hisseder. Hasta memnuniyetinin yüksek<br />
olduğu bu lensler multifokal ve trifokal merceklerin<br />
kullanımının hasta için uygun olmadığı durumlarda tercih<br />
edilmektedir.” ifadelerini kullandı.<br />
“Akıllı lens terimi doğru bir terim değil”<br />
Halk arasında “akıllı lens” adıyla son zamanlarda<br />
popülerliği gittikçe artan bir başka lens türü olan trifokal<br />
lensler hakkında da bilgi veren Prof. Dr. Göçgil, “Akıllı<br />
lens terimi aslında doğru bir terim değildir. Çok odakı<br />
lens grubuna dahil olan bu lensler halk arasında akıllı<br />
lens ismiyle anılmakta ve bilimsel olarak yanlış kanılara<br />
da sebep olmaktadır. Bazı hastalarımız bunu kontakt<br />
lens gibi kolay takılıp çıkarılabilen bir yöntem sanarak<br />
başvurmaktadırlar. Bu lensler uzağı, yakını ve ara mesafeyi<br />
göstermeyi hedefleyen çok odaklı merceklerdir. Trifokal<br />
lensler, gelen ışığın bölgeler arasında odaklanılan alana<br />
göre hastaya hem yakını hem de uzak nesneleri net bir<br />
şekilde görme kapasitesi sunmaktadır. Gözlüksüz olarak<br />
okuyabilme gibi beceriler kişiden kişiye değişmekle birlikte<br />
genellikle bu lenslerin her iki göze de yerleştirilmesi en<br />
olumlu sonuçları vermektedir. Lenslere beynin uyum<br />
sağlaması ve görmede iyileşmenin sağlanması 6-24 hafta<br />
gibi bir süreyi alabilmektedir.” açıklamasında bulundu.<br />
“Lens seçimi hastanın kişisel durumuna ve<br />

Her hastaya multifokal lens takılamayacağı konusunda<br />
hastaları bilgilendiren Prof. Dr. Nur Acar Göçgil,<br />
açıklamasının devamında şu ifadeleri kullandı: “Örneğin<br />
sarı noktada hastalığı olan kişilerin çoğuna multifokal<br />
lens takılması önerilmez. Mercek seçimi hastanın kişisel<br />
durumuna ve ihtiyacına göre dikkatle yapılmalıdır. Üveit,<br />
ileri evre glokom gibi bazı hastalıkların olduğu durumlarda<br />
da bu lensler kullanılamamaktadır. Kullanılacak göz içi<br />
lensin seçiminde hastanın detaylı bilgilendirilmesi, gözün<br />
anatomik uygunluğunun yanı sıra hastanın beklentilerini<br />
ve günlük aktivitelerini tam olarak öğrenerek ameliyat<br />
sonrasında hangi tip mercek seçimiyle memnun<br />
olacaklarını dikkatlice aktarmak uygun olur. Nitekim<br />
bu mercekler gözümüzün içerisindeki doğal lensimizin<br />
görevini almakta ve günlük takıp çıkarılma gibi bir seçenek<br />
bulunmamaktadır. Tekrar gereken ameliyatları da ek risk<br />
taşımaktadır.<br />
“Cerrahın bilgili ve deneyimli olması son derece<br />
önemli”<br />
Cerrahin konu hakkında bilgili ve deneyimli olması<br />
seçilecek göz içi merceğin tespiti açısından da son derece<br />
önemlidir. Gözün anatomik yapısına, sağlık durumuna ve<br />
kişinin yaşamı ile ihtiyacına bakılarak, hastanın gününü<br />
nasıl geçirdiği, beklentisi gibi faktörler dikkate alınmalı ve<br />
her bir göze en uygun mercek seçilmelidir.”<br />

Approximately 28 thousand young people received<br />
basic life support training<br />
Yaklaşık 28 bin genç temel yaşam desteği eğitimi aldı<br />
Within the scope of the “Run First for the First<br />
Aid” (in Turkish “İlk Yardıma İlk Sen Koş”) project<br />
carried out in cooperation with the Emergency<br />
Medicine Association of Türkiye (EMAT), Tuvana<br />
Foundation for the Education of Motivated<br />
Children (TOCEV), and AstraZeneca Türkiye,<br />
approximately 28 thousand students were given<br />
basic life support training in 75 schools in 11<br />
provinces. With the project, it is aimed to provide<br />
education to 55 thousand students until 2025.<br />
Only in 2021, 2,421 people died at the accident site in<br />
traffic accidents. Every year, 830 thousand children die<br />
as a result of preventable accidents in the world. On the<br />
other hand, although one person in every two people<br />
whose heart stopped outside the hospital worldwide is<br />
accompanied by a person, very few of these people can<br />
start basic life support. More than 140,000 people have<br />
died in our country since 1900 due to earthquakes.<br />
In addition to home, work or traffic accidents that can<br />
happen to anyone in daily life, after disasters such<br />
as earthquakes and in some situations that require<br />
emergency intervention, such as a heart attack, people’s<br />
lives can also be at serious risk if basic life support is not<br />
provided. Setting out from this fact, AstraZeneca Türkiye<br />
continues the “Run First for the First Aid” project with the<br />
Emergency Medicine Association of Türkiye (EMAT), and<br />
Türkiye Acil Tıp Derneği (TATD), Tüvana<br />
Okuma İstekli Çocuk Eğitim Vakfı (TOÇEV) ve<br />
AstraZeneca Türkiye iş birliği ile yürütülen “İlk<br />
Yardıma İlk Sen Koş” projesi kapsamında 11<br />
ildeki 75 okulda yaklaşık 28 bin öğrenciye temel<br />
yaşam desteği eğitimleri verildi. Proje ile 2025<br />
yılına kadar 55 bin öğrenciye eğitim verilmesi<br />
hedefleniyor.<br />
Sadece 2021 yılında trafik kazalarında 2 bin 421 kişi kaza<br />
yerinde hayatını kaybetti. Dünyada her yıl 830 bin çocuk ise<br />
önlenebilir kazalar sonucu hayatını kaybediyor. Öte yandan<br />
dünya genelinde hastane dışında kalbi duran her iki kişiden<br />
birinin yanında bir kişi bulunsa da bu kişilerin çok azı temel<br />
yaşam desteğine başlayabiliyor. Depremler nedeniyle<br />
ülkemizde 1900 yılından bu yana 140 binden fazla insan<br />
yaşamını yitirdi.<br />
Günlük hayatta herkesin başına gelebilecek ev, iş ya da<br />
trafik kazalarının yanı sıra deprem gibi afetlerden sonra ve<br />
kalp krizi gibi acil müdahale gerektiren bazı durumlarda<br />
temel yaşam desteği verilmediğinde insanların yaşamları<br />
da ciddi risk altına girebiliyor. Bu gerçekten yola çıkan<br />
AstraZeneca Türkiye, global ölçekte yürütülen Genç Sağlığı<br />
Programı kapsamında Türkiye Acil Tıp Derneği (TATD) ve<br />
Tüvana Okuma İstekli Çocuk Eğitim Vakfı (TOÇEV) ile “İlk<br />
Yardıma İlk Sen Koş” projesini sürdürüyor. Bu proje ile<br />

the Tuvana Foundation for the Education of Motivated<br />
Children (TOCEV) within the scope of the Youth Health<br />
Program carried out on a global scale. With this project,<br />
high school students in various cities of Türkiye are given<br />
basic first aid training by volunteer emergency medicine<br />
specialists of EMAT. In these trainings, subjects such as<br />
basic life support, heart massage and artificial respiration<br />
in adults and children, as well as the heimlich maneuver,<br />
airway obstruction due to foreign objects are explained<br />
practically on models. At the same time, the knowledge<br />
of the students participating in the training is measured<br />
by applying the first-post tests. TOCEV, on the other<br />
hand, provides coordination with the schools that attend<br />
the training every month, including the schools of the<br />
Provincial Education Directorates in different provinces,<br />
with the announcement of the Ministry of National<br />
Education, in order to determine the schools to be trained.<br />
Within the scope of the project, approximately 28<br />
thousand students have been reached in 75 schools in<br />
11 provinces so far. 54 employees from AstraZeneca<br />
also voluntarily supported the trainings held in different<br />
cities. With the project, it is aimed to provide education<br />
to 55 thousand students in 155 schools in approximately<br />
27 provinces until 2025. In addition, as of the <strong>2023</strong>-2024<br />
academic year, studies have been started to add the<br />
subject of first aid after the earthquake to the scope of the<br />
trainings.<br />
“CPR immediately after calling 112 can save lives”<br />
Delivering a speech about the project, EMAT President<br />
Prof. Dr. Serkan Yılmaz said, “The most important goal in all<br />
medical interventions is to keep people alive. Intervention<br />
in the most accurate and timely manner in cases of sudden<br />
cardiac arrest, which is extremely critical; It is extremely<br />
important that there are no deficiencies or errors in the<br />
application. It is estimated that an average of 150 thousand<br />
people per year in Türkiye suffer from sudden cardiac<br />
arrest and/or are affected by its consequences. 2 out of<br />
3 deaths due to sudden cardiac arrest occur before the<br />
person reaches the hospital. More than 95 percent of<br />
these losses occur because there is currently no rescue and<br />
rescue medical equipment. In sudden cardiac arrest, every<br />
minute that passes before the heart returns to its normal<br />
rhythm reduces the chance of survival by 10 percent. In<br />
the first 4 minutes of intervention, the chance of survival is<br />
around 60 percent with automatic shock device and heart<br />
massage application. Survival rates from cardiac arrest<br />
can also dramatically improve with programs that inform<br />
the public about these practices. Sudden cardiac arrest is<br />
an unexpected death that usually occurs within 1 hour of<br />
the onset of symptoms, whether or not there is a known<br />
heart disease, and these deaths can occur on the street, at<br />
home, at school, at work, at sporting events, in shopping<br />
centers or in hospitals. When we face this situation, in<br />
order to reduce the loss of life in unexpected sudden<br />
cardiac arrests, we should continue the first aid and basic<br />
life support trainings that will popularize the practices<br />
of calling 112 and performing heart massage in the early<br />
period.”<br />
Türkiye’nin çeşitli illerindeki lise öğrencilerine TATD’nin<br />
gönüllü acil tıp uzmanları tarafından temel ilk yardım<br />
eğitimleri veriliyor. Bu eğitimler içerisinde temel yaşam<br />
desteği, erişkin ve çocuklarda kalp masajı ve suni teneffüs<br />
uygulamasının yanı sıra heimlich manevrası, yabancı cisme<br />
bağlı hava yolu tıkanmaları gibi konular maketler üzerinde<br />
uygulamalı olarak anlatılıyor. Aynı zamanda ilk- son testler<br />
uygulanarak eğitime katılan öğrencilerin bilgileri de<br />
ölçülüyor. TOÇEV ise eğitim verilecek okulların belirlenmesi<br />
için Millî Eğitim Bakanlığı’nın duyurusu ile İl Eğitim<br />
Müdürlükleri’nin farklı illerdeki okullarını kapsayacak<br />
şekilde her ay eğitime gidilen okullarla koordinasyonu<br />
sağlıyor.<br />
Proje kapsamında şu ana kadar 11 ildeki 75 okulda yaklaşık<br />
28 bin öğrenciye ulaşıldı. Farklı illerde gerçekleşen<br />
eğitimlere AstraZeneca’dan 54 çalışan da gönüllü<br />
olarak destek verdi. Proje ile 2025 yılına kadar yaklaşık<br />
27 ildeki 155 okulda 55 bin öğrenciye eğitim verilmesi<br />
hedefleniyor. Ayrıca <strong>2023</strong>- 2024 eğitim öğretim dönemi<br />
itibarıyla eğitimlerin kapsamına deprem sonrası ilk yardım<br />
konusunun da eklenmesi için çalışmalara başlandı.<br />
”112’yi aradıktan hemen sonra kalp masajı<br />
yapmak hayat kurtarabilir”<br />
Proje hakkında açıklamada bulunan TATD Başkanı Prof.<br />
Dr. Serkan Yılmaz, “Bütün tıbbi müdahalelerde en önemli<br />
hedef, insanı hayatta tutmaktır. Son derece kritik olan ani<br />
kalp durması vakalarında müdahalenin en doğru şekilde<br />
ve zamanda yapılması; uygulamada hiçbir eksiklik veya<br />
hatanın olmaması son derece önemlidir. Türkiye’de yılda<br />
ortalama 150 bin kişinin ani kalp durması geçirdiği ve/<br />
veya sonuçlarından etkilendiği tahmin edilmektedir. Ani<br />
kalp durması sebebiyle gerçekleşen her 3 ölümden 2’si<br />
kişi hastaneye ulaşmadan önce meydana gelmektedir. Bu<br />
kayıpların yüzde 95’inden fazlası, hâlihazırda bir kurtarıcı<br />
ve kurtarıcı tıbbi ekipman bulunmadığı için olmaktadır.<br />
Ani kalp durması geçirildiğinde, kalbi normal ritmine<br />
döndürmeden önce geçen her dakika, hayatta kalma şansını<br />
yüzde 10 azaltmaktadır. İlk 4 dakikadaki müdahalede<br />
otomatik şok cihazı ve kalp masajı uygulaması ile hayatta<br />
kalma şansı ise yüzde 60 civarındadır. Halkı bu uygulamalar<br />
hakkında bilgilendiren programlar sayesinde kalp durması<br />
nedeniyle hayatta kalma oranları da çarpıcı şekilde<br />
iyileştirebilir. Ani kalp durması, bilinen kalp hastalığı<br />
olsun olmasın belirtiler başladıktan sonra genellikle 1<br />
saat içinde meydana gelen beklenmedik ölümler olup;<br />
bu ölümler sokakta, evde, okulda, iş yerlerinde, spor<br />
etkinliklerinde, alışveriş merkezlerinde veya hastanelerde<br />
olabilmektedir. Bu durumla karşılaştığımızda beklenmedik<br />
ani kalp durmalarında yaşam kayıplarının azaltılması<br />
için, 112’yi arama ve erken dönemde kalp masajı yapma<br />
uygulamalarını yaygınlaştıracak ilk yardım ve temel yaşam<br />
desteği eğitimlerini sürdürmeliyiz,” dedi.<br />
“Temel yaşam desteği vermenin önemine dikkat<br />
çekiyoruz”<br />

TOCEV Chairman of the Board Ebru Uygun said, “As TOCEV,<br />
our biggest goal is to make our projects meet the needs<br />
of our children. In our projects and all our activities, our<br />
main subject is the child, but it is among our main goals<br />
that parents and other family members can take a stance<br />
for the benefit of the child along with the child. Your social<br />
responsibility; We are in an effort to spread the awareness<br />
that it is not a duty but a responsibility in our social<br />
development and development, to live more humanely, and<br />
to find an adequate response in the society. We met with<br />
AstraZeneca Türkiye in 2007 while we were thinking about<br />
what we can do with this effort. We started a beautiful<br />
adventure as a partner in this effort. We continue to add<br />
milestones to children’s lives with our years of cooperation.<br />
The projects we have done with different age groups have<br />
touched different areas in the lives of children and aimed<br />
to provide solutions to the needs of children with different<br />
issues. We have now recreated our first project that we<br />
started together with different collaborations and brand<br />
new content. With our “Run First for the First Aid” project,<br />
we draw attention to the importance of providing basic life<br />
support with simple but important techniques in cases of<br />
vitally important accidents that can happen to anyone in<br />
daily life due to demands from schools and students. This<br />
time, we brought a new breath to our content with the<br />
Emergency Medicine Association of Türkiye (EMAT), one of<br />
our project partners. Together with our teachers, we reach<br />
children theoretically and practically. The measurements we<br />
have made with our project are also extremely important<br />
for us to observe how much it raises awareness about first<br />
aid, and the value provided by the volunteers, consisting of<br />
AstraZeneca Türkiye employees, is growing even more. Our<br />
goal is to bring these trainings to 55 thousand children in<br />

“It is a need for people of all ages to have basic<br />
first aid information”<br />
Pointing out that they work with the desire to add value<br />
to the society in Türkiye as well as in all geographies where<br />
they operate, beyond developing innovative drugs and<br />
offering them to the service of health, Pharm. Serkan Barış,<br />
Country President, Astrazeneca Pharmaceuticals Türkiye,<br />
said, “Since the first day we started operating in Türkiye,<br />
we consider supporting the development of public health<br />
among our corporate responsibilities and priorities. For us,<br />
sustainability means using our competencies to make the<br />
most meaningful impact where society needs it. We want<br />

Young people, who are one of the most important building<br />
blocks of public health, are also the focal point of many<br />

to health services, which is one of our sustainability focuses.<br />
We realized this project within the scope of AstraZeneca’s<br />
Youth Health Program, which has been carried out in 39<br />
countries, including Türkiye. The program has touched<br />
“TOÇEV olarak en büyük hedefimiz, projelerimizin<br />
çocuklarımızın ihtiyaçlarının karşılığı olması. Projelerimizde<br />
ve tüm faaliyetlerimizde asıl öznemiz çocuk, fakat çocukla<br />
birlikte anne-baba ve diğer aile bireylerinin de çocuğun<br />
yararına bir duruş edinebilmeleri temel hedeflerimiz<br />
arasında bulunuyor. Sosyal sorumluluğun; toplumsal<br />
gelişim ve kalkınmamızda, daha insanca yaşayabilmemizde<br />
bir görev değil, bir sorumluluk olduğu bilincini<br />
yaygınlaştırmak ve toplumda yeterli karşılığı bulması<br />
çabası içindeyiz. Tam da bu çaba ile neler yaparız diye<br />
düşünürken 2007 yılında AstraZeneca Türkiye ile tanıştık.<br />
Bu çabamıza ortak olarak güzel bir serüvene başladık. Yıllar<br />
süren iş birliğimizle çocukların hayatına kilometre taşı<br />
eklemeye devam ediyoruz. Farklı yaş grupları ile yaptığımız<br />
projeler, çocukların hayatındaki farklı alanlara dokunmuş<br />
ve çocukların ihtiyaçlarına farklı konularla çözüm olmayı<br />
amaç edinmiştir. Birlikte başladığımız ilk projemizi şimdi<br />
farklı iş birlikleri ve yepyeni içeriklerle tekrar oluşturduk.<br />
“İlk Yardıma İlk Sen Koş” projemiz ile okullardan ve<br />
öğrencilerden gelen taleplerle hayati önem taşıyan, günlük<br />
hayatta herkesin başına gelebilecek kazalarda, basit<br />
ama önemli tekniklerle temel yaşam desteği vermenin<br />
önemine dikkat çekiyoruz. Bu sefer proje ortaklarımızdan<br />
biri olan Türkiye Acil Tıp Derneği ile içeriklerimize yeni<br />
bir soluk getirdik. Hocalarımızla birlikte çocuklara teorik<br />
ve uygulamalı olarak ulaşıyoruz. Projemizle yaptığımız<br />
ölçümlemeler de ilk yardım konusundaki farkındalığı ne<br />
derece artırdığını gözlemlememiz adına son derece önemli<br />
oluyor ve AstraZeneca Türkiye çalışanlarından oluşan<br />
gönüllülerinin de projeye olan katkısı ile sağlanan değer<br />
daha da büyüyor. Hedefimiz toplamda 55 bin çocuğumuza<br />
bu eğitimleri götürebilmek. Emeği geçen herkese çok<br />

“Her yaşta insanın temel ilk yardım bilgilerine<br />
sahip olması bir ihtiyaç”<br />
Yenilikçi ilaçlar geliştirmenin ve sağlığın hizmetine<br />
sunmanın ötesinde faaliyette bulunduğu tüm coğrafyalarda<br />
olduğu gibi Türkiye’de de topluma değer katma arzusuyla<br />
çalıştıklarına dikkat çeken AstraZeneca Türkiye<br />
Ülke Başkanı Ecz. Serkan Barış, “Türkiye’de faaliyet<br />
göstermeye başladığımız ilk günden bu yana toplum<br />
sağlığının geliştirilmesine destek vermeyi kurumsal<br />
sorumluluklarımız ve önceliklerimiz arasında görüyoruz.<br />

olan sağlık hizmetlerine erişim alanında hayata geçirdiğimiz<br />
değerli projelerimizden biri. Bu projemizi, AstraZeneca’nın<br />
Türkiye dahil şu ana kadar 39 ülkede yürüttüğü Genç<br />
Sağlığı Programı kapsamında hayata geçirdik. Program<br />

the lives of more than 9 million young people and 160<br />
thousand health workers all over the world so far. “Run<br />
First for the First Aid” is a very meaningful project that<br />
instills the importance of being equipped with first aid, as<br />
well as raising young people who are the guarantee of our<br />
future as healthy individuals. It is actually a need for people<br />
of all ages to have basic first aid information. We hope that<br />
our project will be beneficial for our youth and our society,<br />
and we thank everyone who contributed to its realization.”<br />
The numbers also show the importance of first aid<br />
In 2021, 5 thousand 362 people died in traffic accidents in<br />
Türkiye. 2,421 of these people died at the accident site. 2<br />
thousand 941 people lost their lives within 30 days due to<br />
the cause and effect of the accident after they were injured<br />
and transferred to health institutions. According to a study,<br />
1/3 of the reasons for applying to emergency services are<br />
home accidents. In addition, 830,000 children die every<br />
year in the world as a result of preventable accidents. Due<br />
to earthquakes, 92 thousand 463 people lost their lives in<br />
our country from 1900 to the earthquakes on 6 February.<br />
With the earthquakes that took place on February 6, the<br />
loss of life exceeded 140,000. On the other hand, less<br />
than 10 percent of those whose heart stopped outside the<br />
hospital worldwide survive. Outside the hospital, there is<br />
one person next to one of every two people whose heart<br />
stopped. However, unfortunately less than 1/3 of these<br />
people can start basic life support. However, if a person<br />
whose heart has stopped is given basic life support within<br />
4 minutes and shock can be given within 8 minutes, the<br />
survival rate can increase to 40-50 percent.<br />

Rakamlar da ilk yardımın önemini gözler önüne<br />
seriyor<br />
Türkiye’de 2021 yılında trafik kazalarında 5 bin 362 kişi öldü.<br />

dışında kalbi duran her iki kişiden birinin yanında ise bir kişi<br />
bulunuyor. Buna karşın bu kişilerin maalesef 1/3’ünden daha<br />
azı temel yaşam desteğine başlayabiliyor. Oysa kalbi duran<br />
bir kişiye 4 dakika içinde temel yaşam desteği uygulanır ve 8<br />
dakika içinde şok verilebilirse sağ kalma oranları yüzde 40-<br />
50’ye çıkabilir.<br />

The 30th Expomed Eurasia, International Istanbul<br />
Medical Analysis, Diagnosis, Treatment, Rehabilitation,<br />
Laboratory Product, Equipment and Hospitals Fair was<br />
held in Istanbul on March 16-18 this year. At the fair, which<br />
brought together hundreds of health representatives and<br />
companies from 18 countries, Russian origin manufacturers<br />
introduced the solutions they developed in the field of<br />
medical technology. In addition, the “Russian-Turkish Health<br />
Cooperation” roundtable was established to strengthen the<br />
commercial relations between Russia and Türkiye.<br />
The 30th Expomed Eurasia, International Istanbul Medical<br />
Analysis, Diagnosis, Treatment, Rehabilitation, Laboratory<br />
Product, Equipment and Hospitals Fair, which is organized<br />
regularly every year in order to introduce new medical<br />
technologies by Russian health care companies and to<br />

“Russia-Türkiye trade volume doubled in 2022”<br />
Stating that Expomed Eurasia Fair is an important event for<br />
health sector representatives from Türkiye, CIS countries,<br />
Europe, Asia and Africa, Expomed Eurasia Representative<br />
Alaudin Yarahmedov made the following statement about<br />
the fair, “The trade volume between Russia and Türkiye will<br />
double in 2022. This volume has now reached a value of<br />
65.3 billion dollars. We think that in the coming period, the<br />
increase in export supply to Türkiye will further strengthen<br />
cooperation including health and medical technologies. In<br />
this context, we bring together the experts, manufacturers<br />
and suppliers of the health sector at the Expomed Eurasia<br />
Fair, which we organize every year, and open the door to<br />

Stating that the interest of Russian companies in the<br />
Turkish market serves as a driving force for the expansion<br />

Stating that 2 to 3 out of every thousand babies<br />
are born with hearing loss in Türkiye, experts<br />
stated that early diagnosis is very important<br />
to prevent hearing loss in babies. Ear Nose<br />
and Throat Specialist Prof. Dr. Ozan Seymen<br />
Sezen, Istanbul Okan University Hospital gave<br />
information about hearing loss in babies.<br />
What causes hearing loss in babies?<br />
Prof. Dr. Ozan Seymen Sezen said, “Hearing is one of the<br />
most important sense organs that affect the whole life<br />
of individuals from birth. If hearing loss in newborns is<br />
not identified and treated in the early period, it harms<br />
the speech and language development of children. In<br />
our country, 2,500 newborns are born with hearing loss<br />
annually. Some infections of the mother during pregnancy,<br />
premature birth and low birth weight of the baby, staying in<br />
the intensive care unit or the baby using ototoxic drugs for<br />
the ear cause hearing loss.”<br />

İşitme kaybının çocukların sosyal yaşamını ve öğrenme<br />
becerilerini olumsuz yönde etkilediğini belirten Prof. Dr.<br />
Ozan Seymen Sezen “İşitme doğuştan itibaren bireylerin<br />
tüm hayatını etkileyen en önemli duyu organlarından biridir.<br />
Yenidoğanlarda işitme kaybı erken dönemde tanımlanıp<br />
tedavi edilmez ise, çocukların konuşma ve dil gelişimine<br />
zarar verir. Ülkemizde yılda 2 bin 500 yenidoğan işitme<br />
kaybı ile doğuyor. Annenin gebelik döneminde geçirdiği<br />
bazı enfeksiyonlar, bebeğin erken doğum ve düşük ağırlıklı<br />
doğması, yoğun bakım ünitesinde kalma veya bebeğin kulak<br />
için ototoksik etkili ilaç kullanması gibi durumlar işitme<br />

“Neonatal screening tests have great<br />
importance”<br />
The ‘Newborn Hearing Test’ has a great<br />
importance in order to prevent hearing loss<br />
in newborn babies. Experts, who said that<br />
if hearing loss is not diagnosed and treated<br />
early, children fall behind in speech skills and<br />
language development, stated that hearing<br />
problems and screening tests in babies<br />
are very important. In order to understand<br />
whether babies have hearing problems, Prof.<br />
Dr. Sezen said, “If your baby does not startle<br />
at loud noises, does not cry or moves towards<br />
you when you call, if he has not made speech sounds<br />
yet, these symptoms are important for early diagnosis of<br />
hearing loss. However, for a definitive diagnosis, there is no<br />
other way to understand hearing loss other than hearing<br />
screening tests in newborns.”<br />
What is the hearing process like in a newborn?<br />
The newborn’s response to sound is a reflex event. Babies<br />
born with a sense of hearing react to sounds by blinking<br />
or startling. Depending on the intensity of the sounds, the<br />
rate of their response may also change. Babies exposed to<br />
high sound intensity, called the startle reflex, during the<br />
developmental period from birth to 2 years of age, make<br />
movements like jumping. These symptoms give a rough<br />
indication of the extent to which the sound is picked up by<br />

Yenidoğanda işitme süreci nasıldır?<br />
Yenidoğanın sese karşı verdikleri tepkiler bir refleks<br />
olayıdır. İşitme duyusuna sahip olarak doğan bebekler,<br />
seslere gözlerini kırparak veya irkilerek reaksiyon<br />
gösterirler. Seslerin şiddetine göre verdikleri tepkilerin<br />
oranları da değişebilir. Doğumdan itibaren 2 yaşına kadar<br />
olan gelişim sürecinde irkilme refleksi denilen yüksek<br />
ses şiddetine maruz kalan bebekler sıçrar gibi hareket<br />
yaparlar. Bu belirtiler, sesin bebek tarafından ne derece<br />

Autoimmune diseases are known as diseases that occur<br />
as a result of the unwanted immune response developed<br />

Aquaporin 4 develop in the central nervous system, and the<br />
complement system is activated due to these antibodies,<br />
causing damage to the myelin sheath of the central nervous<br />

that patients consult a physician with symptoms such as<br />

“There are three different ways to treat NMOSD. The first<br />
of these is the treatment of attacks. Steroid therapy is used<br />
in attacks. Apart from steroid treatment, plasma exchange<br />
(plasmapheresis) can be used as a treatment. In the<br />
plasma exchange treatment, the antibodies that cause the<br />
disease are cleared from the body. The second treatment<br />
is the prevention of attacks. Immune system-suppressing<br />
treatments are used to prevent attacks and prevent<br />
progression of disability in the patient. These treatments<br />
are long-term treatments and are treatment options<br />
that regulate the immune system against this disease.<br />
These treatments are preferred according to the clinical<br />
conditions of the patients and changes can be made in<br />
these treatments according to the response received. The<br />
third treatment option is the treatment of the symptoms<br />
that occur with the attack. Due to the symptoms occurring<br />
in the patients, treatments for these symptoms are applied<br />
by the relevant branch physicians (internal medicine,<br />
urology, physical therapy and rehabilitation, psychiatry,<br />
etc.).<br />

launched the You Are Not Alone (in Turkish Yalnız Değilsin)<br />
project to provide free psychological counseling to<br />
earthquake victims. Volunteer experts who want to provide<br />
psychological support to earthquake victims within the<br />
scope of the project can generate their profiles free of<br />
charge and share their knowledge with earthquake victims.<br />
DoktorTakvimi, the most used private health appointmentmaking<br />

patients, Dr. Ateş said, “Mitral valve problems are seen in<br />

problem in heart failure and heart valves, but it is not<br />

yaş grubu yüksek hastalarda görülür ve bu durumda<br />
hastalara cerrahi müdahale yapılması risk taşımaktadır.<br />
Evde odadan odaya geçemeyen hastalar düşünün; yani kalp<br />
yetmezliği ve kalp kapaklarında sorun var ancak ameliyat<br />
olması mümkün değil… Bu gibi cerrahi olarak reddedilmiş<br />
hastalara Mitraclip Yöntemi ile müdahale edebiliyoruz.”<br />
şeklinde konuştu.<br />
Hastaların hareket kapasiteleri artıyor<br />
Ateş sözlerine şöyle devam etti; “Mitraclip metodu anjiyo<br />
yöntemiyle uygulanıyor. Göğüste hiçbir kesinin olmadığı,<br />
kasıktan yapılan bu işlem 1-1,5 saat sürüyor. Bu yöntemle<br />
aynı anjiyo gibi sağ kasıktaki toplardamardan girerek<br />
mandallama yöntemi ile mitral kapak tamiri yapıyoruz.<br />
Mitral kapak ön ve arka yapraktan oluşuyor. Anjiyo<br />
metoduyla kateterle kasık toplardamarından girilip, karın<br />
içindeki büyük toplardamara geçilerek kalbe ulaşılıyor.<br />
Sonrasında damar içindeki kateter kanalıyla mitral kapak<br />
seviyesine kadar mitraclip cihazı ulaştırılıyor. Mitraclipin<br />
kanatları bu noktaya ulaştığında açılıyor. Problemli olan<br />
kalp kapak kısımları mitraclipin kanatları altına alınıyor<br />
ve hedeflenen kapak yapısı sağlanınca dikişler karşılıklı<br />

Patients’ mobility increases<br />
Ateş continued, “The Mitraclip method is applied with the<br />
angiography method. This procedure, which is made from<br />
the groin, where there is no incision in the chest, takes<br />
1-1.5 hours. With this method, we perform mitral valve<br />
repair by latching method by entering through the vein<br />
in the right groin, just like angio. The mitral valve consists<br />
of anterior and posterior leaves. With the angiography<br />
method, the heart is reached by entering the inguinal vein<br />
with a catheter and passing through the large vein in the<br />
abdomen. Then, the mitraclip device is delivered to the<br />
level of the mitral valve through the catheter in the vein.<br />
The mitraclip’s wings unfold when it reaches this point.<br />
The problematic heart valve parts are taken under the<br />
wings of the miraclip and the stitches are fixed to each<br />
other when the targeted valve structure is achieved. At<br />
this point, it is tested that sufficient closure is provided in<br />
the cover. In this way, the valve is repaired and the patients<br />
are discharged after 1 day to return to their normal lives.<br />
After the procedure, the patients’ complaints of not being<br />
able to breathe due to the heart and breathing irregularity<br />
decreased, as well as the growth of the heart stops and the<br />

The triclip method in the treatment of<br />
tricuspid valve<br />
Touching on the TriClip method, which has started to<br />
be applied in the treatment of tricuspid valve, and its<br />
advantages, Dr. İsmail Ateş said, “As the name suggests,<br />
TriClip treatment is a treatment method based on the<br />
principle of placing clips (latching) on the valve, which we<br />
call the tricuspid valve, instead of the mitral valve, similar to<br />
the MitraClip treatment. After making similar preparations<br />
as in the MitraClip treatment, the main treatment goal is<br />
to connect the appropriate leaflets of the tricuspid valve,<br />
which has three leaflets, with the clip and to reduce the<br />
valve leakage. In this method, the patient can recover in<br />
a short time and return to his daily life in a much more<br />
energetic and healthy way.”<br />
What is the Aortic Valve Renewal<br />
(TAVI) procedure?<br />
Dr. Ateş also gave information about the TAVI method,<br />
which is the aortic valve regeneration process, and said,<br />
“Aortic valve diseases are a serious health problem that can<br />
occur as a result of calcification of the valve, especially with<br />
advancing age, and can even result in death in a short time<br />
if not treated. Aortic valve diseases seen in elderly patients<br />
may also be accompanied by insufficiency of vital organs<br />
such as lung, kidney and liver.<br />
These accompanying diseases can make surgical<br />
interventions very risky. The TAVI treatment method<br />
is mostly based on the placement of special apparatus<br />
designed as a remote valve stent with a pressure-inflatable<br />
balloon into the aortic valve position via the inguinal<br />
artery. After this valve placement procedure, which can be<br />
performed in a short time of about 1 hour, the patient can<br />
stand up in a short time and continue his daily life.”<br />

Aort Kapak Yenileme (TAVİ) işlemi nedir?<br />
Dr. Ateş, aort kapak yenileme işlemi olan TAVİ<br />
yöntemi hakkında da bilgi vererek şunları söyledi:<br />
“Aort kapak hastalıkları özellikle yaşın ilerlemesi ile<br />
kapağın kireçlenmesi sonucu karşımıza çıkabilen ve<br />
tedavi edilmediği takdirde kısa sürede ölümle dahi<br />
sonuçlanabilecek ciddi bir sağlık sorunudur. İleri yaş<br />
hastalarda görülen aort kapak hastalıklarına akciğer,<br />
böbrek ve karaciğer gibi hayati organların yetmezlikleri<br />
de eşlik edebilmektedir. Eşlik eden bu hastalıklar cerrahi<br />
müdahaleleri oldukça riskli kılabilmektedir. TAVİ tedavi<br />
yöntemi çoğunlukla kasık atar damarı vasıtasıyla aort<br />
kapak pozisyonuna, uzaktan kapaklı bir stent halinde<br />
tasarlanmış özel aparatların basınçla şişebilen bir balon ile<br />
yerleştirilmesi temeline dayanır. Yaklaşık 1 saat gibi kısa<br />
bir sürede gerçekleştirilebilen bu kapak yerleştirme işlemi<br />

Another side effect of immobilization is decreased range<br />
of motion of the joints. This applies both to the joint to<br />
which the broken bone is attached and to the upper and<br />
lower joints of that region. Since the broken bone will be in<br />
the plaster application during the immobilization process,<br />
simple joint range of motion exercises and stretching<br />
exercises can be applied without compromising the union<br />
of the bone, in order to avoid limitation of motion in an<br />
upper and lower joint, but not in that joint. Superficial<br />
massages can be applied to increase circulation, and cold<br />
applications can be applied to eliminate edema.”<br />
How does the process progress after<br />
immobilization?<br />

on the post-immobilization process, said, “In the postimmobilization<br />
period; in other words, rehabilitation after<br />
plaster removal is much more important. Joint limitations<br />
and muscle loss in strength and mass are much higher.<br />
Balance and coordination disorders may also occur due<br />
to disuse, people create alternative movements instead<br />
of movements they cannot do and wrong movement<br />
patterns due to using them are settled. The rehabilitation<br />
process is important in order to eliminate all these<br />
problems. When starting the rehabilitation, first of all, it<br />

This requires radiological evaluation. This evaluation is<br />
especially important in order to provide weight bearing<br />
and to determine the amount of load to be given in lower<br />
extremity fractures such as foot and leg. Again, this process<br />
of union will determine the duration of rest and corset<br />
use in spinal fractures. Nerve injury may accompany the<br />
fracture, in this case, the work of nerves and muscles<br />
should be evaluated with EMG, and treatments for this<br />
problem should be added to the study when necessary.”<br />
How does the process progress in the treatment<br />
program?<br />

process, continued her speech as follows, “In the treatment<br />

için ses dalgalarından (ultrason) ve soğuk uygulamalardan,<br />
kas kuvvetini arttırmak için elektrik stimülasyonlarından<br />
faydalanılabilinir. Egzersiz programlarına bakacak olursak,<br />
kasları kuvvetlendirmeye yönelik kademeli artan kuvvet<br />
egzersizleri (izometrik, izotonik, dirençli egzersizler), eklem<br />
hareket açıklıklarını arttırmaya yönelik önce aktif yardımlı,<br />
sonra aktif hareketler en son da pasif germe egzersizleri<br />

of eating disorders?<br />
Dietitian Hanife Kara said that the COVID-19 epidemic<br />
