e. Diyette Protein K›s›tlamas›: Proteinden zengin diyet;<br />

renal kan ak›fl› <strong>ve</strong> glomerül hidrostatik bas›nc› artt›rarak<br />

GFH’n› yükseltir. Diyabetli bireylerde diyette protein al›m›n›n<br />

azalt›lmas›n›n, glomerül hiperfiltrasyon <strong>ve</strong> mikroalbüminüriyi<br />

azaltt›¤›n› ileri sürmektedir. Ayn› zamanda düflük<br />

proteinli diyetin aflikar diyabetik nefropatiye ilerlemeyi yavafllatt›¤›na<br />

iliflkin <strong>ve</strong>riler de bulunmaktad›r, ancak uzun süreli<br />

kontrollü klinik çal›flmalar halen yoktur (58,59).<br />

Sonuç olarak, diyabetes mellitus giderek artan s›kl›kta<br />

<strong>ve</strong> mikroalbüminüri de diyabetik hastalar›n yaklafl›k üçte<br />

birinde görülmektedir. Mikroalbüminüri, böbrek hastal›¤›<br />

<strong>ve</strong> kardiyovasküler hastal›k aç›s›ndan bir risk faktörüdür <strong>ve</strong><br />

glisemik kontrol, mikroalbüminüri geliflimini önleyebilmektedir.<br />

Anjiotensin tedavisi <strong>ve</strong> kan bas›nc›n›n kontrolü, idrardaki<br />

albumin düzeyini düflürmekte <strong>ve</strong> böbrek fonksiyonlar›n›<br />

korumaktad›r. Bu nedenle tip 2 diyabetli hastalar›n tümü,<br />

y›lda bir kez mikroalbüminüri aç›s›ndan taranmal›d›r.<br />
