A 31-year-old male patient was admitted to the hospital<br />
with coughing and dyspnea. One month ago he<br />
was treated with inhaler drugs with the diagnosis of<br />
occupational asthma. His history revealed that he<br />
smoked five packets of cigarettes per year (he was an<br />
ex-smoker for 8 years) and worked in the furniture<br />
industry for 3 years. He also added that he was exposed<br />
to glue inhalation in the work place; he did not<br />
use a mask. The physical examination revealed normal<br />
vital signs, and oxygen saturation was 95%.<br />
Rales in the base of right lung were also noted. Diffusion<br />
capacity was found to be very low. The thoracic<br />
CT revealed ground-glass densities in the right lower<br />
lobe and air trapping in the left lung. Fiber optic<br />
bronchoscopy and transbronchial biopsy were performed.<br />
The transbronchial biopsy specimen was<br />
insufficient for a specific diagnosis. Right thoracotomy<br />
and wedge resection were performed for the specific<br />
diagnosis. The pathological diagnosis was reported<br />
to be hypersensitivity pneumonitis (HP). The patient<br />
was treated with methylprednisolone (1mg/kg/day)<br />
for six months. After the treatment and removal from<br />
the antigen exposure, clinical and radiological improvements<br />
were obtained. In conclusion, detailed<br />
working anamnesis is important in the patients with<br />
suspicion of occupational HP.<br />

Otuzbir yaşında erkek hasta nefes darlığı, öksürük<br />
şikâyetleri ile başvurdu. Hasta 1 ay önce mesleksel<br />
astım tanısıyla inhaler tedavi kullanmıştı. Özgeçmişinde<br />
5 paket yıl sigara hikâyesi (8 yıldır sigara içmiyor)<br />
vardı. Hasta 3 yıldır mobilya fabrikasında çalışıyor<br />
ve çalışma ortamında maske kullanmadığını ve<br />
yapıştırıcı inhalasyonu olduğunu tarifliyordu. Fizik<br />
muayenesinde vital bulguları normaldi. Oksijen saturasyonu<br />
%95, sağ akciğer bazalinde raller vardı.<br />
Difüzyon kapasitesi ise çok düşüktü. Toraks BT'de sağ<br />
alt lobda buzlu cam densitesi, sol akciğerde hava<br />
hapsi mevcuttu. Fiberoptik bronkoskopi yapıldı ve<br />
transbronşiyal biyopsi alındı. Ancak biyopsi materyali<br />
kesin tanı için yeterli değildi. Sağ torakotomi yapılan<br />
hastadan wedge biyopsi alındı. Histopatolojik tanı ise<br />
hipersensitivite pnömonisi (HP) olarak raporlandı.<br />
Hastaya 6 ay metilprednizon (1mg/kg/gün) tedavisi<br />
verildi. Tedavi sonrası ve antijen maruziyetinin kesilmesi<br />
ile belirgin klinik ve radyolojik düzelme sağlandı.<br />
Sonuç olarak detaylı meslek anamnezinin mesleksel<br />
HP şüphesi durumunda önemli olduğunu vurgulamak<br />
istiyoruz.<br />

Hypersensitivity pneumonitis (HP) is a pulmonary disease<br />
with symptoms of dyspnea and coughing, resulting from<br />
the inhalation of an antigen to which the patient has been<br />
previously sensitized (1,2). Chemical-induced hypersensitivity<br />
pneumonitis has been so far rarely described. Reactive<br />
low molecular weight chemicals, many of which are<br />
known causes of occupational asthma, may act as haptens<br />
and cause HP (1). Occupational exposure to some<br />
low-molecular-weight chemicals such as isocyanate vapors,<br />
and aerosols that are found in foam, glue, and<br />
spray paint may induce typical hypersensitivity pneumonitis<br />
(3). The current report presents a case of HP due to<br />
glue inhalation that was misdiagnosed as occupational<br />
asthma.<br />
CASE<br />
A 31-year-old male patient was admitted to a reference<br />
hospital for chest diseases with the symptoms of coughing<br />
and dyspnea. He had effort-associated dyspnea and a<br />
nonproductive cough for one year. Symptoms progressed<br />
over the past three months. He had no sputum, hemoptysis,<br />
chest pain, or fever. He had an involuntary 6 kg<br />
weight loss in the last year. One month prior, he was<br />
treated with inhaler drugs with the diagnosis of asthma,<br />
with only mild improvement. His history revealed that he<br />
smoked five packets of cigarettes per year (he was an exsmoker<br />
for 8 years) and worked in the furniture industry<br />
for 3 years. He also added that he was exposed to glue<br />
inhalation in the work place; he did not use a mask. The<br />
physical examination revealed normal vital signs, and<br />
oxygen saturation (SO 2) was 95%. The chest examination<br />
revealed rales in the base of the right lung. The examination<br />
of the other systems was normal. The chest radiograph<br />
revealed non-homogenous infiltration in the paracardiac<br />
area and the lower zone at the right side. Hemogram,<br />
biochemical tests, CRP level, total IgE and urine<br />
test were all normal; sedimentation rate was 30 mm/hr.<br />
Results of the pulmonary function test were as follows:<br />
forced expiratory volume (FEV1): 42 % (1.57 liters),<br />
forced vital capacity (FVC): 44% (1.92 liters), FEV1/FVC:<br />
82%, FEF 25-75: 37% (1.70 liters/sec), and diffusing<br />
capacity: 36%. The thoracic CT revealed ground-glass<br />
densities in the right lung basal segments and air trapping<br />
in the left lung (Figure 1). Fiberoptic bronchoscopy was<br />
performed; vocal cords and trachea were normal. Both<br />
bronchial systems were open up to the subsegmental<br />
levels. Transbronchial biopsy and bronchoalveolar lavage<br />
(BAL) were obtained from the right lung basal segment.<br />
The BAL analysis produced the following, total cell count:<br />
1020/mm³, lymphocytes 70%, macrophages 20%, neutrophils<br />
9%, and eosinophils 1%, and CD4/CD8: 0.32.<br />
Transbronchial biopsy was insufficient for the specific<br />
diagnosis. Bronchial lavage cytology was negative for<br />
malignancy. Bronchial lavage was negative for acid resistant<br />
bacilli (ARB). For a specific diagnosis, a wedge<br />
resection was performed through right thoracotomy. The<br />
histopathological examination revealed lymphocyte infiltration<br />
and non-necrotizing granulomas with the diagnosis<br />
of hypersensitivity pneumonitis (extrinsic allergic alveolitis)<br />
(Figure 2). No clinical and radiological improvements<br />
were obtained in spite of removal from antigen<br />
exposure. Thus, the patient was treated with methylprednisolone<br />
(1mg/kg/day) for six months. Following treatment,<br />
the patient experienced clinical and radiological<br />
improvement. In the thoracic CT, ground-glass densities<br />
in the right lower lobe were regressed (Figure 3). The<br />
pulmonary function test results, in particular, significantly<br />
improved after the treatment as follows: FEV1: 69% (2.56<br />
L), FVC: 75% (3.25 L), FEV1/FVC: 79%, FEF 25-75: 54%<br />
(2.45 L/sec) and diffusing capacity was 93%.<br />

DISCUSSION<br />
Patients with HP might be misdiagnosed with occupational<br />
asthma, as they have similar symptoms and a history of<br />
exposure in the work place. Low molecular weight chemicals,<br />
especially isocyanates, are known to cause occupational<br />
asthma (1). In the current report, the patient was<br />
working in the furniture industry and had nonspecific<br />
symptoms as dyspnea and cough, and thus he was treated<br />
with bronchodilator drugs with the diagnosis of asthma.<br />
However, his chest radiograph was not normal and<br />
the diffusion capacity for carbon monoxide was low. Furthermore,<br />
the thoracic CT revealed ground-glass densities<br />
in the right lower lobe, which is not expected in patients<br />

with asthma. In the BAL analysis, the CD4/CD8 level was<br />
low and the histopathological examination was compatible<br />
with HP.<br />

measurement of dust concentration are important. Many<br />
agents may cause HP and the most important factor in<br />
the treatment is the cessation of the antigen exposure (5).<br />
The decision to initiate steroid therapy should be based<br />
on the severity of symptoms and physiological abnormalities.<br />
The effect of antigen type on the prognosis is unclear.<br />
Genetics and the duration of antigen exposure are important<br />
for the prognosis (5-7). In the current patient,<br />
there was no clinical and radiological improvement despite<br />
of removal from antigen exposure. Thus, he was<br />
treated with methylprednisolone for six months, which<br />
resulted in clinical and radiological improvement.<br />
In conclusion, recognizing HP is clinically challenging<br />
because among different patients, the disease might vary<br />
according to the clinical presentation, severity, and natural<br />
history. Detailed working anamnesis is important in<br />
patients with suspicion of occupational HP.<br />

Some low-molecular-weight chemicals that by themselves<br />
are not solely antigenic may combine with the host’s<br />
proteins to form haptens, which then may provoke hypersensitivity<br />
pneumonitis. For example, the agents implicated<br />
in a chemical worker’s lung are isocyanates, and are<br />
found in products such as foam, glue, and spray paint (4).<br />
Clinical suspicion is important in the diagnosis of occupational<br />
HP. In particular, the finding of immunoglobulin G<br />
precipitating antibodies in the serum of patients with suspected<br />
HD is a helpful diagnostic clue (2,5). However,<br />
the researchers of the current study could not measure<br />
precipitins in the serum of the patients.<br />
Hypersensitivity pneumonitis is not an atopic disease.<br />
There is no increase in IgE levels and eosinophil count.<br />
Precautions to decrease the antigen exposure must be<br />
taken both at home and in workplaces. In particular, the<br />
use of filtered masks, air conditioning, and the regular<br />

Pulmoner alveoler mikrolitiazis (PAM), her iki akciğerde<br />
intraalveolar mikrolitlerin birikimi ile karakterize<br />
az rastlanan kronik bir akciğer hastalığıdır. Tanı genellikle<br />
spesifik radyolojik bulgularla konmakla birlikte<br />
kesin tanı için patolojik inceleme yapmak gerekebilir.<br />
Kemik sintigrafisi tanıya yardımcı olan diğer bir<br />
görüntüleme yöntemidir. Kemik sintigrafisinde birçok<br />
vakada akciğerlerde diffüz Tc-99m metilen difosfonat<br />
(MDP) tutulumu rapor edilmiştir. Çalışmamızda kemik<br />
sintigrafisinde bilateral akciğerlerde diffüz artmış Tc-<br />
99m MDP tutulumu gösteren PAM tanılı bir olguyu<br />
sunduk.<br />

Pulmonary alveolar microlithiasis (PAM) is a rare<br />
chronic lung disease characterized by the deposition<br />
of intra-alveolar microliths in both lungs. Although<br />
diagnosis is usually based on specific radiological<br />
findings, pathological examination may be necessary<br />
for final diagnosis. Bone scintigraphy is another imaging<br />
method that aids in the diagnosis. Diffuse Tc-<br />
99m methylene diphosphonate (MDP) uptake was<br />
reported in the lungs on bone scintigraphy in many<br />
cases. The current study presents a case diagnosed<br />
with PAM who had diffuse increased Tc-99m MDP<br />
uptake in both lungs on bone scintigraphy.<br />

Pulmoner alveoler mikrolitiazis (PAM), her iki akciğerde<br />
intraalveolar yaygın küçük taş yapılarının birikimi ile karakterize<br />
az rastlanan kronik bir akciğer hastalığıdır (1).<br />
Etyoloji tam olarak bilinmemekle birlikte otozomal resesif<br />
geçişli genetik bir hastalık olabileceği düşünülmektedir (2).<br />
Hastalar genellikle yakınmasız olup tanı sıklıkla akciğer<br />
görüntülemesi sırasında rastlantısal olarak konmaktadır<br />
(3). Direkt grafide kum fırtınası tarzındaki yaygın kalsifiye<br />
mikronodüler görünüm karakteristiktir (3,4). Yüksek rezolüsyonlu<br />
bilgisayarlı tomografi (YRBT) tanıda önemli bir<br />
yere sahiptir. Ayrıca kemik sintigrafisinde akciğerlerde Tc-<br />
99m metilen difosfonat (MDP) tutulumu birçok olguda<br />
rapor edilmiştir (5-7). Kesin tanı akciğer biyopsisi ya da<br />
bronkoalveoler sıvıda mikrolitlerin gösterilmesi ile konmaktadır<br />
(3). Bilinen kesin bir tedavisi yoktur. Çalışmamızda<br />
kemik sintigrafisinde bilateral akciğerlerde diffüz<br />
artmış Tc-99m MDP tutulumu gösteren PAM tanılı bir<br />
olguyu sunmak istedik.<br />
OLGU<br />
Otuz dört yaşındaki erkek hasta yaklaşık 2 yıldır devam<br />
eden kuru öksürük nedeni ile göğüs hastalıkları polikliniğine<br />
başvurdu. Yapılan fizik muayenede oskültasyonda<br />
bilateral solunum seslerinde hafif azalma saptandı. Hemogram,<br />
biyokimya, kan elektrolitleri ve tam idrar tahlilini<br />
içeren rutin laboratuvar incelemeleri normal sınırlarda<br />
bulundu. Spirometrik incelemede FEV1: 2,64 L (%64),<br />
FVC: 3,12 L (%65), FEV1/FVC: %83 olarak hesaplanmıştı.<br />
Hastanın postero-anterior akciğer grafisinde bilateral<br />
akciğerlerde kum fırtınası tarzında yaygın mikronodüler<br />
opasiteler izlendi. YRBT incelemesinde bilateral akciğerlerde<br />
yaygın mikronodüler kalsifiye lezyonlar, plevral/subplevral<br />
kalsifikasyonlar ve interlobüler septal kalınlaşmalar<br />
saptandı (Şekil 1). Tanıyı desteklemek için yapılan<br />
tüm vücut kemik sintigrafisinde bilateral akciğer alanlarında<br />
diffüz Tc-99m MDP tutulumları saptandı. Akciğer<br />
dışında vücudun başka bir yerinde patolojik aktivite tutulumuna<br />
rastlanmadı (Şekil 2). Hastaya kesin tanı için<br />
biyopsi planlandı. Fiberoptik bronkoskopide bronşlar<br />
doğal izlendi. Transbronşiyal biyopsi alındı. Materyalin<br />
histopatolojik incelenmesinde alveol boşluklarını dolduran<br />
tipik mikrolitler görüldü ve PAM tanısı doğrulandı.<br />
Hastanın aile öyküsü bulunmamaktaydı. Hastanın öksürük<br />
haricinde başka belirgin yakınması olmayıp göğüs hastalıkları<br />
tarafından klinik takibi devam etmektedir.<br />
TARTIŞMA<br />
PAM, her yaş grubunda görülmekle birlikte hastalığın<br />
alveoler gaz alışverişini etkileyebilecek düzeye gelinceye<br />

kadar yakınmasız seyretmesi nedeniyle tanı sıklıkla erişkin<br />
yaşlarda genellikle 30–50 yaşlarında konmaktadır (3).<br />
Hastamızın tanı yaşı 34 idi. Hastalığın otozomal resesif<br />
geçişli olduğu düşünülmektedir ve hastaların yarısında<br />
pozitif aile öyküsü mevcuttur (2,3). Olgumuzda aile öyküsü<br />
bulunmamaktaydı. Hastalık genellikle diğer sağlık<br />
problemleri nedeniyle yapılan akciğer görüntülemesi<br />
sırasında tesadüfen saptanır (3). PAM’ın tipik radyolojik<br />
bulgusu, daha çok alt ve orta zonları tutan ve kum fırtınası<br />
olarak adlandırılan bilateral diffüz mikronodüler kalsifik<br />
lezyonlardır (3,4). Buzlu cam opasiteler, interlobüler septal<br />
kalınlaşmalar, plevral-subplevral kalsifikasyonlar ve<br />
kistler YRBT’de görülen tipik bulgulardır (8). Olgumuzda<br />

her iki akciğerde yaygın kalsifiye mikronodüller, plevral/subplevral<br />
kalsifikasyonlar ve interlobüler septal kalınlaşmalar<br />
mevcuttu. YRBT hastalığın yaygınlığını belirlemede<br />
ve klinik izlemde akciğer grafisine kıyasla daha<br />
duyarlıdır (3). Olgumuzda kesin tanı transbronşial biyopsi<br />
ile kondu. Kemik sintigrafisi tanı ve ayrıcı tanıya yardımcı<br />
olan diğer bir görüntüleme yöntemidir. Kemik sintigrafisinde<br />
kullanılan Tc-99m MDP, PAM’da izlenen akciğer<br />
alveollerindeki mikrolitler tarafından yüksek tutulum gösterir.<br />
Mikrolitler ekstrasellüler kalsiyum depozitleri olup<br />
kalsiyum, apatit ve fosfor içerirler. Tc-99m MDP şüpheli<br />
olgularda hastalık tanısının desteklenmesinde faydalı<br />
olabilmektedir (7). Tüm bunların yanında akciğerlerinde<br />
Tc-99m MDP tutulumu göstermeyen PAM olguları da<br />
bildirilmiştir (10). Bu durumun alveoler mikrolitlerin azalmasından<br />
ya da sistemik dolaşım bozukluğundan kaynaklanabileceği<br />
düşünülmektedir (11). Hastalığın klinik yakınmaları<br />
ve radyolojik bulguları açısından büyük uyumsuzluklar<br />
mevcuttur (12). Hastalar yaygın radyolojik lezyonlardan<br />
beklenmeyecek kadar az yakınmalıdırlar. Olgumuz<br />
bu profile uymakta olup yaygın akciğer lezyonlarına<br />
karşın hastamızın sadece kuru öksürük şikâyeti mevcuttu.<br />
Olguların çoğunda normal akciğer fonksiyonları<br />
izlenmekle birlikte bazı olgularda restriktif tip solunum<br />
fonksiyon bozukluğu ve difüzyon kapasitesinde azalma<br />
izlenmektedir. Hastamızda hafif restriktif tip solunum fonksiyon<br />
bozukluğu saptandı. Semptomatik hastalarda tipik<br />
yakınmalar dispne, kuru öksürük, göğüs ağrısı ve halsizliktir.<br />
Hastalık genellikle yavaş ilerlemekte ancak bazı<br />
olgularda zamanla pulmoner fibrozis, solunum yetmezliği<br />
ve kor pulmonale gelişebilmektedir. Hastalığın spesifik bir<br />
tedavisi yoktur. Sistemik kortikosteroid tedavisi ve bronkoalveolar<br />
lavaj gibi palyatif tedavi yöntemleri etkisizdir.<br />
Disodyum etidronat erken tanı alan olgularda kullanılabilir.<br />
Akciğer transplantasyonu, hastalığın ileri dönemlerinde<br />
uygulanabilen bir tedavi seçeneğidir (6,7). Ancak<br />
etkinliği tartışmalıdır. Sonuç olarak, kemik sintigrafisi PAM<br />
tanısında ilk sırada tercih edilmese de hastalığın tanısını<br />
destekleyen önemli bir görüntüleme yöntemidir.<br />

Pulmoner Langerhans hücreli histiositozis daha çok<br />
genç, sigara içen erişkinlerde görülen etiyolojisi bilinmeyen<br />
nadir bir hastalıktır. Otuz altı yaşında bayan<br />
hasta 4 aydır devam eden öksürük ve balgam çıkarma<br />
yakınması ile polikliniğimize başvurdu. Yedi yıl<br />
günde 1 paket sigara anamnezi olan hastanın çekilen<br />
Yüksek rezolüsyonlu toraks bilgisayarlı tomografisinde<br />
her iki akciğer parankim alanlarında üst ve orta zonlarda<br />
daha belirgin olarak, alt zonlarda ise daha hafif<br />
derecede ve küçük olarak izlenen kaviter lezyonlar,<br />
çevresinde buzlu cam dansitesinde infiltratif tutulumlar<br />
ve milimetrik buzlu cam dansitesinde nodüler<br />
görünümler mevcuttu. Tanı açık akciğer biyopsisinde<br />
immünhistokimyasal boyama ile CD1a yüzey antijeni<br />
pozitifliği gösteren Langerhans hücrelerin görülmesiyle<br />
konuldu.<br />

Pulmonary Langerhans cell histiocytosis is a rare<br />
idiopathic disorder mostly seen in young adult smokers.<br />
A 36-year-old female patient referred to our<br />
clinic with the complaints of cough and sputum production<br />
for the last four months. She had a smoking<br />
history of 7 packets per year (current smoker). In high<br />
resolution computed tomography, cystic appearances,<br />
which were more marked in upper lobes, ground<br />
glass opacities near the cystic lesions, and millimetric<br />
nodules around them were observed. A surgical<br />
biopsy showed a lymphocytic lung infiltrate with<br />
Langerhans cells immune staining with CD1a antigen<br />
positivity.<br />

Langerhans hücreli histiyositoz (LHH) kemik iliği kökenli<br />
dendritik hücre yapısındaki histiyositlerin anormal proliferasyonu<br />
ve dokularda granülom oluşturacak şekilde infiltrasyonu<br />
ile karakterize nedeni bilinmeyen bir hastalık<br />
grubudur (1-2). Erişkin yaş grubunda nadiren görülürken<br />
çocuklarda daha sık saptanır (3). Langerhans hücrelerinin<br />
infiltrasyonu eozinofilik granülom, Hand-Schuller-<br />
Christian hastalığı veya Letterer-Siwe hastalığı olarak<br />
isimlendirilen 3 farklı klinik tablo ile ortaya çıkabilir (4).<br />
Erişkinlerde pulmoner LHH (PLHH) neredeyse yalnızca<br />
sigara içenlerde meydana gelen ve nadir görülen bir<br />
bozukluktur. PLHH daha çok genç erişkinleri etkilemekte<br />
ve 20 – 40 yaş arasında en sık görülmektedir (5). Biz de<br />
nadir bir hastalık olması nedeni ile olgumuzu burada<br />
tartışmayı uygun bulduk.<br />
histopatolojik incelemesinde langerhans hücreleri, histiosit<br />
eozinofil, lökosit ve diğer enflamatuar hücrelerin oluşturduğu<br />
multipl interstisiyel nodüler infiltrasyonlar izlendi. Bu<br />
alanlara uygulanan IHK ile S100 pozitif ve CD1a pozitifliği<br />
gözlendi (Şekil 3). IHK bulguları ve histopatolojik<br />
bulgular ile olguya LHH tanısı konuldu. Başka bir organ<br />
tutulumu saptanmayan hastaya sigara bırakma danışmanlık<br />
hizmeti verildi, tedavi sürecinde sigarayı bırakan<br />
hasta klinik takibe alındı.<br />
OLGU<br />
Otuz altı yaşında bayan üç aydır devam eden öksürük ve<br />
balgam çıkarma yakınmaları ile başvurdu. Yedi paket/yıl<br />
sigara anamnezi vardı. Öz ve soy geçmişinde bir özellik<br />
yoktu. Fizik muayenesinde genel durum iyi, bilinci açık ve<br />
koopereydi. Solunum sistem muayenesinde solunum<br />
sayısı 16/dk, bilateral solunum sesleri olağandı. Nabız:<br />
80/dk, tansiyon arteriyel 130/80 mmHg, ateş yoktu, kalp<br />
sesleri olağandı. Diğer sistem muayenesi olağandı. Hemogram<br />
ve biyokimya değerleri normal sınırlarda olan<br />
hastanın eritrosit sedimentasyon hızı 32mm/saatti.<br />
PA akciğer grafisinde bilateral üst ve orta zonlarda daha<br />
belirgin yer yer retiküler ve nodüler heterojen dansite<br />
artımları izlendi (Şekil 1). Solunum fonksiyon testinde<br />
FEV1: 2,82 L (%101) FVC: 2,98 L (%93) FEV1/FVC: %94<br />
olarak saptandı. Karbonmonoksit difüzyon testinde<br />
(DLCO) hastanın difüzyon kapasitesinin orta derecede<br />
azalmış (%50) olduğu gözlendi. YRBT’de her iki akciğer<br />
parankim alanlarında üst ve orta zonlarda daha belirgin<br />
olarak, alt zonlarda ise daha hafif derecede ve küçük<br />
olarak izlenen kaviter lezyonlar, çevresinde buzlu cam<br />
dansitesinde infiltratif görünümler ve milimetrik buzlu cam<br />
dansitesinde nodüler görünümler mevcuttu (Şekil 2). Hastaya<br />
fiberoptik bronkoskopik muayene yapıldı. Orta lob<br />
medial segmentten yapılan BAL hücre sayımında %89<br />
makrofaj, %5 eozinofil, %4 nötrofil lökosit %1 lenfosit<br />
izlendi. S100 ve CD1a immünhistokimyasal (IHK) tetkikinde<br />
boyama saptanmadı. Hasta bronkoskopi sırasında<br />
yapılan transbronşial biyopsiyi tolere edemedi ve hasta<br />
göğüs cerrahisi ile konsülte edilerek açık akciğer biyopsisine<br />
yönlendirildi.<br />
Yapılan açık akciğer biyopsisinde üst lob wedge alt lob<br />
superior ve bazal segment wedge rezeksiyon materyallerin<br />

TARTIŞMA<br />
PLHH’in etiyolojisi ve patogenezi tam olarak bilinmemektedir.<br />
Hastalığın hemen hemen sadece sigara içenlerde<br />
görülmesi çevresel bir faktörün hastalığı tetiklediğini düşündürmektedir<br />
(5). Önceleri belirgin bir erkek baskınlığı<br />
bildirilmişken günümüz verileri kadın erkek oranının eşitlendiği<br />
yönündedir (5,6). Bu farklılıkların olasılıkla zaman<br />
içerisinde sigara içme alışkanlıklarındaki değişiklikleri<br />
yansıtabileceği düşünülmektedir. Nadir görülen bir hastalık<br />
olması nedeniyle prevalans belirlemek güçtür, ancak<br />
Japonya’da bir yıl içerisinde 160 yeni olgu belirlenmiş ve<br />
prevalans erkeklerde ve kadınlarda 100.000’de 0,27 ve<br />
0,07 olarak tahmin edilmiştir (7). Bizim olgumuz da orta<br />
yaşlı bir bayan olup aktif sigara içicisidir.<br />
PLHH’de, patolojik sürecin oluştuğu bronşiyollerde çok<br />
sayıda LH birikimi olur. Lezyonlar fokaldir, distal bronşiyollerden<br />
bazılarını tutar, hepsinde bulunmaz. Bu dağılım,<br />
olasılıkla sigara içilmesi nedeniyle uyarılan bronşiyol epitelin<br />
mikro çevresine bağlıdır. Bronşiyollerde LH birikimi<br />
açısından bu değişimin önemli olduğu düşünülmektedir.<br />
Çok sayıda LH tarafından bronşiyollerin infiltrasyonu, LH<br />
öncülerinin LH oluşturması, lokal LH proliferasyonu<br />
ve/veya apopitozu uyaran mekanizmalara karşı azalmış<br />
LH duyarlığı ile ilgili olabilir (8). Hastalığın nadiren de<br />
olsa sigara içmeyen erişkinlerde ve küçük çocuklarda<br />
ortaya çıkması hastalığın patogenezinde sigara dışında<br />
farklı etkenlerin de olduğunu düşündürmektedir (8).<br />

PLHH klinik görünüm itibariyle değişkendir, yani farklı<br />
tablolar görülebilir (5–9). Yaygın akciğer tutulumuna<br />
karşın semptomlar oldukça siliktir veya yoktur. Olguların<br />
yaklaşık %25’inde hastalık hiçbir belirti vermez ve rutin<br />
göğüs filminde saptanır. Olguların yaklaşık üçte ikisinde<br />
tanı anında solunum belirtileri, kuru öksürük ve eforla<br />
nefes darlığıdır. Buna spesifik olmayan semptomlar eklenebilir<br />
(ateş, gece terlemesi ve kilo kaybı vb.). Olguların<br />
%10-20’si spontan pnömotoraks ile kendini gösterebilir.<br />
Pnömotoraks genç erkeklerde daha sık gelişir, hastalık<br />
seyrinde herhangi bir zamanda meydana gelebilir ve<br />
zorlu tedavi girişimleri gerektiren, iki taraflı ve/veya nüks<br />
eden tipte olabilir (5-9). Bizim hastamızın üç aydır devam<br />
eden öksürük ve balgam çıkarma yakınması mevcut idi.<br />
PLHH’li hastaların SFT’leri normal olabildiği gibi obstrüktif,<br />
restriktif, mikst tipte solunum fonksiyon bozukluğu görülebilir<br />
(5). Hastaların çoğunluğunda obstrüktif fonksiyonel<br />
bozukluk görülür. Radyolojik değişiklikler çok belirgin<br />
olmasına karşın solunum fonksiyonları normal olabilir.<br />
Bizim olgumuzun YRBT’sinde yaygın kistik ve nodüler<br />
görünümler olmasına karşın SFT parametreleri normal<br />
sınırlar içerisinde ancak difüzyon kapasitesi düşük idi.<br />
Akciğer grafisinde retikülomikronodüler infiltrasyon en sık<br />
görülen radyolojik görünümdür (8). YRBT tetkiki, PLHH<br />
tanısında büyük bir gelişme sağlamıştır ve bugün bu tanıdan<br />
kuşkulanıldığında yapılması gerekli olan bir tetkik<br />
halini almıştır. Tipik YRTB görünümü, fokal normal parankim<br />
ile birbirinden ayrılan ve daha çok üst orta zonlarda<br />
yayılım gösteren iyi sınırlı olmayan küçük nodülleri,<br />
kaviteli nodülleri, ince ve kalın duvarlı kistleri içerir (8).<br />
PLHH’de lezyonların dağılımı sentrilobülerdir. Bu hastalık<br />
ilerledikçe, kistik lezyonlar baskın bulgu haline gelir.<br />
BAL sıvısında yüksek makrofaj sayıları içeren, genç, semptomsuz<br />
sigara içicilerde akciğer alanlarının üst yarısında<br />
baskın nodüler ve kistik görünüm varlığı PLHH tanısı için<br />
tipiktir. Aksine, nadir saf nodüler formlar veya kaviteli<br />
akciğer nodülleri bulunan hastalarda, ayırıcı tanı listesinde;<br />
mikobakteri enfeksiyonunu, diğer enfeksiyonlar, sarkoidoz,<br />
Wegener granülomatozisi, kaviteli akciğer metastazları,<br />
bronşiyoloalveoler karsinom, septik emboli veya<br />
kaviteli P. jiroveci pnömonisi bulunmaktadır. Kadınlarda,<br />
saf kistik PLHH’nin lenfanjiyoleyomiyomatozisden ayırt<br />
edilmesi için anjiyomiyolipomlar gösterebilir (8). Hastamızın<br />
YRBT’sinde her iki akciğer parankim alanlarında üst ve<br />
orta zonlarda daha belirgin olarak, alt zonlarda ise daha<br />
hafif derecede ve küçük olarak izlenen kaviter lezyonlar,<br />
çevresinde buzlu cam dansitesinde infiltratif görünümler<br />
ve milimetrik buzlu cam dansitesinde nodüler görünümler<br />
mevcuttu.<br />
Kesin LHH tanısı, hastalığa yakalanmış bir dokuda LH<br />
granülomunun tanımlanması ile konur. PLHH hastalarında<br />
transbronşiyal biyopsi LH granülomlarını gösterebilmesine<br />
karşın, histolojik kanıt daha sık olarak cerrahi biyopsi<br />
yoluyla elde edilir. Langerhans hücreleri S-100 ile boyanır.<br />
Bu hücreler CD-1 pozitiftir ve spesifik monoklonal antikor<br />
ile tanınabilir. BAL sanılanın aksine çok düşük duyarlılığa<br />
sahiptir ve hastaların tanısında nadiren yararlıdır (10).<br />

Bizim olgumuzda da BAL’da langerhans hücresi saptanmadı.<br />
Hasta açık akciğer biyopsisine yönlendirildi. İHK<br />
bulguları ve histopatolojik bulgular ile olgumuza LHH<br />
tanısı konuldu.<br />
Hastalığın tedavisinde ilk basamak sigaranın bırakılmasıdır.<br />
Progresif, sistemik semptomu olan veya solunum<br />
yetmezliği ile başvuran hastalarda steroid kullanılabilir (8).<br />
Hastamıza sigara bırakma danışmanlık hizmeti verilerek<br />
sigara bırakma tedavisi uygulanmıştır. Hastanın semptomlarının<br />
ağır olmaması ve klinik olarak stabil olması<br />
nedeniyle steroid tedavisi başlanmamıştır. Hasta halen<br />
klinik gözlem altındadır.<br />
Hastalığın seyri oldukça değişken olup sigara bırakıldıktan<br />
sonra hastalığın düzelebileceği bildirilmiştir (11-13).<br />
Bununla birlikte, sigara bırakılmasının uzun dönemdeki<br />
sonuçları belirsizdir ve sigara bırakılmasına rağmen nüks<br />
eden olgular da bildirilmiştir (14).<br />
Her ne kadar akciğer Langerhans hücreli histiyositozisine<br />
yol açan hastalık patogenezi aydınlanmaya başlamış olsa<br />
da, hastalığın yerleşimi ve kliniğin ortaya çıkışı ile ilgili<br />
mekanizmalar açık değildir. Bu az rastlanan hastalık için<br />
tedavilerin geliştirilmesine katkı sağlayacak yeni çalışmalara<br />
ihtiyaç vardır.<br />

RESPIRATORY CASE REPORTS<br />
Pulmonary alveolar microlithiasis (PAM) is a rare lung<br />
disease characterized by the deposition of calcium in<br />
the alveolar spaces and bilateral diffuse micronodular<br />
‘‘sandstorm’’ radiographic pattern. The current<br />
report presents the case of a 48-year-old woman with<br />
pulmonary alveolar microlithiasis. The patient presented<br />
with exertional dyspnea for the past five years.<br />
The physical examination was within normal limits<br />
and respiratory sounds were normal. The chest x-ray<br />
revealed a bilateral diffuse micronodular pattern in<br />
the middle and the lower areas of both lungs. On the<br />
parenchymal window of thorax computed tomography<br />
(CT), bilateral diffuse widespread millimetric<br />
calcified nodules that were more prominent in the<br />
lower lobes were observed. Whole body bone scintigraphy<br />
with technetium-99m revealed bilateral, diffuse,<br />
heterogeneous, increased uptake in the pulmonary<br />
parenchyma. Regarding the current special radiological<br />
findings, further invasive diagnostic examination<br />
was not performed, the patient was diagnosed with<br />
PAM and continues to be followed-up on without<br />
treatment.<br />

Pulmoner alveoler mikrolitiyazis (PAM) alveollerde<br />
kalsiyum birikmesi ile karakterize nadir görülen bir<br />
akciğer hastalığıdır. Her iki akciğerde yaygın simetrik<br />
mikronodüler radyolojik patern mevcuttur. Bu yazıda<br />
48 yaşında PAM tanısı alan bayan hasta sunulmuştur.<br />
Hasta 5 yıldır eforla gelişen nefes darlığı ile başvurdu.<br />
Fizik muayene ve solunum sesleri normaldi. Akciğer<br />
grafisinde orta ve alt alanlarda bilateral yaygın mikronodüler<br />
patern saptandı. Bilgisayarlı toraks tomografisinde<br />
parankim penceresinde bilateral alt loblarda<br />
daha belirgin olmak üzere yaygın milimetrik kalsifiye<br />
nodüller izlenmekteydi. Teknesyum-99m ile yapılan<br />
tüm vücut kemik sintigrafisinde her iki hemitoraksta<br />
yumuşak doku alanlarında heterojen tarzda diffüz<br />
artmış aktivite tutulumu saptandı. Mevcut radyolojik<br />
bulguların uyumlu olması ile ileri girişimsel tetkik<br />
yapılmadı, hasta PAM tanısı ile tedavisiz takibe alındı.<br />

Pulmonary alveolar microlithiasis (PAM) is a rare lung<br />
disease with unknown etiology and characterized by accumulation<br />
of calcium and phosphate crystals in alveoli.<br />
The disease was first described by Friedrich in 1856 and<br />
then by Harbitz in 1918 and called “pulmonary alveolar<br />
microlithiasis” by Puhr in 1933 (1-3).<br />
Radiological findings from the posteroanterior chest x-ray<br />
are bilateral diffuse symmetric micronodular patterns<br />
resembling a “sandstorm”. Most cases have been reported<br />
from Turkey, followed by Italy and the United States,<br />
respectively. In general, PAM can be discovered in any<br />
age of patient, but usually remains asymptomatic until the<br />
fourth decade (4).<br />
The current article presents the case of a 48-year-old<br />
woman with shortness of breath, and typical radiological<br />
findings for PAM are presented.<br />

CASE<br />
A 48-year-old female patient presented with exertional<br />
dyspnea for the past five years that began to increase<br />
over the last three months. There was no history of smoking<br />
or environmental exposure. On physical examination,<br />
the patient's vital signs were within normal limits, has no<br />
cyanosis or clubbing of the fingers, respiratory sounds<br />
were natural, and there was no remarkable pathological<br />
findings on the other systems. Routine complete blood<br />
count and biochemical parameters were also normal.<br />
Radiological evaluation of the chest x-ray revealed bilateral<br />
symmetrical micronodular alveolar opacities obscuring<br />
cardiac and diaphragmatic shadows (Figure 1). In<br />
parenchymal window of the thorax computed tomography<br />
(CT), bilateral diffuse widespread millimetric nodules<br />
more prominent in the lower lobes were observed (Figure<br />
2).<br />
Arterial blood gases in room air were PCO 2: 36 mmHg,<br />
PO 2: 69 mmHg; pulmonary function tests were: FEV1:<br />
1.75 L (82%), FVC: 2.25 L (90%), FEV1/ FVC: 97%, and<br />
TLCO 93%. The six minute walking test, the patient<br />
walked 390m without desaturation.<br />
Sputum smears were negative for acid-fast bacilli, there<br />
was no hemoptysis or history of any exposure for pneumoconiosis.<br />
Regarding the current radiological findings,<br />
PAM was considered; whole body bone scintigraphy with<br />
technetium-99m revealed bilateral, diffuse, heterogeneous,<br />
increased uptake in the pulmonary parenchyma<br />
(Figure 3).<br />
The abdominal ultrasound examination revealed an<br />
18mm gallstone. Further invasive diagnostic examination<br />

DISCUSSION<br />
PAM is a disease that is incompatible with clinical presentation<br />
and radiological findings. Despite extensive radiological<br />
patterns, symptoms are usually less common. The<br />
current case was admitted to a health institution for the<br />
first time, although shortness of breath had been present<br />
for five years. Shortness of breath on exertion, cough, and<br />
chest pain are reported as the most common symptoms in<br />
PAM. Sputum, hemoptysis, and when alveolar gas exchange<br />
begin to deteriorate, hypoxia, cor pulmonale,<br />
and respiratory failure in the later stages of the disease<br />
can be encountered (5,6).<br />

Routine chest x-ray typically shows bilateral ill-defined<br />
micronodular patterns, predominantly on the bases (7).<br />
PAM is one of the disorders with diagnostic features in<br />
radiological images. Chest radiography is especially<br />
pathognomonic in familial cases; however, the clinical<br />
presentation of the disease is often incompatible with the<br />
intensity of the lesions on the chest radiograph. Bone<br />
scintigraphy with Tc-99m shows diffuse uptake in both<br />
lungs due to the deposition of calcium phosphate in the<br />
alveoli (8). Radiological findings are often sufficient for<br />
the diagnosis of the disease; however, transbronchial<br />
biopsy, bronchoalveolar lavage, or open lung biopsy may<br />
sometimes be necessary (5,8). In the light of radiological<br />
findings reflecting the typical characteristics of PAM,<br />
widespread millimetric nodules on thorax CT, or bone<br />
scintigraphy demonstrating the disease-specific findings,<br />
invasive procedures was not performed in the current<br />
case.<br />
An interstitial pattern can also be present in PAM. High<br />
resolution computed tomographic (HRCT) findings of<br />
PAM are similar to interstitial lung diseases in varying<br />
degrees; micronodules, parenchymal bands, ground<br />
glass opacity, subpleural interstitial thickening, interlobular<br />
septal thickening, paraseptal emphysema, centrilobular<br />
emphysema, bronchiectasis, peribronchovascular<br />
interstitial thickening, panacinar emphysema, pleural<br />
calcification. Deniz et al. (8) suggests that the degree of<br />
parenchymal alterations is also closely related to the<br />
degree of pulmonary function loss in patients with PAM.<br />
Mariotta et al. (9) reported an analysis of 576 cases published<br />
in literature, and the origins of the cases were<br />
mainly reported from Europe (42.7%) and Asia (40.6%).<br />
The gender predominance around the world is not known,<br />
but male predominance was reported from Turkey (4).<br />
Similar radiological findings may be seen in miliary tuberculosis,<br />
pneumoconiosis, pulmonary hemosiderosis or<br />
sarcoidosis. Pulmonary tuberculosis or sarcoidosis was<br />
misdiagnosed in 88 cases of the 576 (9).<br />
The disease can also be sporadic or familial (9,10). As<br />
familial transmission of PAM is also reported in nearly<br />
one-third of the cases, the current patient's family was<br />
also screened with chest x-ray. There was no radiological<br />
evidence for PAM in the family.<br />
Macroscopic evaluation of lung quite hard and heavy<br />
due to calcific micronodules in PAM. Histopathologically,<br />
round calcified structures of 250–750 μm diameters is<br />
observed (6).<br />
To date, corticosteroids, chelating agents, mechanical<br />
ventilation during the end stages of the disease, and<br />
bronchoalveolar lavage has been applied for the treatment<br />
of PAM, but achieved no significant benefit (11).<br />
Recently, lung transplantation for selected cases is promising<br />
(12,13).<br />
As a result, PAM is a rare disease with characteristic radiological<br />
features and has no specific treatment. Innovations<br />
are required for the diagnosis and treatment of this<br />
rare disease.<br />

Plastic bronchitis is a rare disease characterized by<br />
chronic cough, dyspnea, airway obstruction, and<br />
expectorating rubber-like secretions. This entity is<br />
mostly seen in children predominantly in association<br />
with an underlying congenital heart disease. We<br />
present an adult case who was treated for asthma for<br />
two years and finally diagnosed with plastic bronchitis<br />
via bronchoscopy.<br />

Plastic bronchitis is a rare condition in which<br />
rubber-like airway casts develop in the<br />
tracheobronchial tree causing dyspnea and<br />
cough (1). These casts can be seen in bronchoscopy<br />
or may be expectorated spontaneously.<br />
The most common symptoms are progressive<br />

görülmektedir. Bu yazıda 2 yıldır astım tanısı ile<br />
tedavi gören ve bronkoskopi ile plastik bronşit tanısı<br />
konulan erişkin hastayı sunmaktayız.<br />

CASE<br />
A 56-year-old woman, who was treated for asthma for<br />
two years, was admitted with progressive cough and<br />
dyspnea for three months. She did not describe any allergies<br />
or other chronic diseases. She was a non-smoker<br />
and had no family history of lung disease. She was a<br />
house wife and was never employed. She was receiving<br />
inhaled salmeterol/fluticasone combination for asthma.<br />
She stated that her cough and dyspnea episodes dissolve<br />
whenever she expectorates tough, thread-like sputum.<br />
She was not hospitalized for any reason within last two<br />
years. On physical examination, her body temperature<br />
was 36.8 °C and respiratory rate was 18 breaths/min.<br />
There were inspiratory crackles on the bases of the both<br />
lungs on auscultation. Heart rate and sounds were normal.<br />
The examination of other systems was unremarkable.<br />
Her oxygen saturation was 95% on ambient air.<br />
episodes were diminished. Steroid treatment was continued<br />
for one month with tapered doses. N- acetylcysteine<br />
was added to her asthma medications. She is still in control<br />
with no complaints.<br />

White blood cell count was 8100/mm 3 and C-reactive<br />
protein was 0.8 mg/dl. Other laboratory test results were<br />
normal. Pulmonary function tests were as follows: FEV1:<br />
82% of predicted, FVC: 83% of predicted, FEV1/FVC<br />
ratio 84%, and PEF: 95% of predicted. Tiny ground glass<br />
opacities were detected on both bases of the lungs on<br />
computed tomography (Figure 1). A pale, tan, highly<br />
viscous material filling the right middle lobe was seen on<br />
the bronchoscopy. Because of the fragility of the material,<br />
it was impossible to remove. The patient expectorated the<br />
material spontaneously one hour later (Figure 2). The<br />
material was spongy and consisted of irregular branches<br />
resembling the bronchial tree. Microscopic examination<br />
revealed fibrinous, mucoid, and hypocellular material.<br />
Microbiologic evaluations were non-diagnostic and no<br />
growth was present on cultures. She was started on systemic<br />
steroids and N-acetylcysteine treatment. Cough<br />
DISCUSSION<br />
Chronic cough is a common and frequently debilitating<br />
symptom that is often viewed as an intractable problem<br />
(3). There are numerous factors provoking chronic cough.<br />
The most common reasons are follows: asthma, esophageal<br />
disease, rhinitis, chronic bronchitis and idiopathy (3).<br />
Plastic bronchitis, also known as fibrinous bronchitis or<br />
pseudomembranous bronchitis, is of unknown etiology,<br />
which is a rare reason for chronic cough. The bronchial<br />
casts, which develop acutely and recurrently, may take<br />
the form of the tracheobronchial tree and cause airway<br />
obstruction. The casts can be of variable sizes. Seear et al.<br />
(2) proposed a classification for the casts; inflammatory<br />
casts, and acellular casts. An inflammatory cast is composed<br />
of fibrin, eosinophils, and little mucin, and mostly<br />
seen in patients with underlying lung diseases like asthma<br />
and cystic fibrosis. However, acellular casts are mostly<br />
composed of mucin and little fibrin and no eosinophilic<br />
infiltrate. The patient was diagnosed with asthma and<br />
treated for two years but the patient’s bronchial cast’s<br />
pathologic examination revealed an acellular cast. In the<br />
literature, plastic bronchitis has been associated with<br />
numerous illnesses such as asthma, cystic fibrosis, congenital<br />
heart diseases, pneumonia, allergic bronchopulmonary<br />
aspergillosis (ABPA), bronchiectasis, and sickle<br />
cell acute chest syndrome (4-7). Plastic bronchitis is rare<br />
and the prevalence is, therefore, unknown but probably<br />
under diagnosed (7). Plastic bronchitis affects patients of<br />
all age groups, with the suggestion of a female predomi-<br />

nance (7). Due to nonspecific symptoms, a high index of<br />
suspicion is necessary to diagnose the disease. Chest x-<br />
ray is not diagnostic. Obstructive emphysema, pneumonia,<br />
and atelectasis may develop due to bronchial casts.<br />
Primarily bronchoscopy or spontaneous expectoration of<br />
the bronchial cast results in diagnoses. The treatment<br />
strategies in the literature depend on personal experiences<br />
due to inexistence of clinical trials. Recommended<br />
therapies are mostly supportive care, bronchoscopy, steroids,<br />
and treatment of underlying diseases. Support, via<br />
bronchoscopy or mucolytic agents, designed to remove<br />
or facilitate the expectoration of bronchial casts may have<br />
a role in the treatment. Saracoglu et al. (8) treated young<br />
plastic bronchitis patients with aerolised hyaluronidase in<br />
to addition corticosteroids in 1960. Aerosolized N-<br />
acetylcysteine can reduce viscosity by breaking disulfide<br />
bonds (2,4,5). Özer et al. (9) reported the success of<br />
intrabronchial lavage with N-acetylcysteine. The current<br />
patient was treated oral steroid and N-acetylcysteine and<br />
after one month of treatment, the complaints resolved.<br />
In conclusion, plastic bronchitis should be taken into<br />
consideration in patients with progressive dyspnea and/or<br />
chronic cough.<br />

Bilateral spontaneous pneumothorax is an extremely<br />
rare condition. The current study presents a primary<br />
synchronized bilateral spontaneous pneumothorax<br />
patient who was operated on by VATS in a semiseated<br />
position with single lumen intubation and<br />
bilaterally, simultaneously.<br />

Bilateral spontaneous pneumothorax is an<br />
extremely rare clinical condition representing<br />
approximately only 1% of all cases of<br />
spontaneous pneumothorax (1,2). Spontaneous<br />

underlying lung disease is termed primary<br />
spontaneous pneumothorax (PSP), while<br />

CASE<br />
A 16-year-old boy presented to the emergency department<br />
with a chief complaint of bilateral chest pain and<br />
shortness of breath. His symptoms suddenly began two<br />
days prior, without any history of trauma. The patient did<br />
not smoke. He had no past medical or surgical history.<br />
His heart rate was 130 beats per minute, blood pressure<br />
of 115/75 mmHg, respiratory rate was 20 breaths per<br />
minutes, and peripheral oxygen saturation was 97% with<br />
room air. Decreased breath sounds were auscultated<br />
bilaterally. Chest radiograph and thorax tomography<br />
were obtained. Bilateral pneumothorax was indicated<br />
(Figure 1 and 2). Bilateral chest tubes were applied and<br />
the patient was taken to the thoracic surgery service.<br />
Prolonged air leakage was determined on the right side<br />
on the sixth day. The decision was made to operate on<br />
the patient bilaterally due to bilateral pneumothorax simultaneously<br />
and prolonged air-leakage at one side. Single<br />
endotracheal intubation with low volume-high frequency<br />
ventilation and three-port VATS technique with<br />
semi-seated position were applied bilaterally. Bullae were<br />
bilaterally detected and excised using endoscopic linear<br />
stapler. Apical pleurectomy was performed. The patient<br />
was discharged from hospital on the third day following<br />
the operation. Recurrence was been observed six months<br />
after operation.<br />
are to provide complete lung expansion and to prevent<br />
recurrence. The researchers of the current study believe<br />
that surgery could be the best way to prevent recurrence.<br />
Sternotomy, bilateral thoracotomy, or videothoracoscopic<br />
surgery could be conducted for synchronized bilateral<br />
spontaneous pneumothorax (4). In our opinion, VATS<br />
should be used for early recovery and discharge from the<br />
hospital with less pain. Double lumen intubation is mostly<br />
performed for VATS, but in the current case, we used<br />
simple endotracheal intubation with low volume-high<br />
frequency ventilation. Furthermore, we performed surgery<br />
in the semi-seated position. In this way, it was not necessary<br />
to turn the patient side to side. The operation time<br />
was shortened in this way.<br />

Bilateral spontaneous pneumothorax could result from<br />
sarcoidosis, lymphangioleiomyomatosis, Marfan’s syndrome,<br />
metastasis, tuberculosis, and mesothelioma (5-8).<br />
For this reason, systemic physical examination should be<br />
conducted on these patients. In the current patient, no<br />
systemic diseases were observed, emphysematous changes<br />
were reported pathologically.<br />
In conclusion, the bilateral VATS approach in the semiseated<br />
position can be done safely in selected patients<br />
with bilateral spontaneous pneumothorax. The procedure<br />
is well tolerated. This technique avoided the need for<br />
subsequent operations.<br />

DISCUSSION<br />
The recurrence rate for the primary spontaneous pneumothorax<br />
ranges from 7.6% to 52% and secondary spontaneous<br />
pneumothorax ranges from 22.7% to 47%, most<br />
occurring between six months to two years after the first<br />

Respiratory failure and hemodynamic instability are<br />
responsible for up to 80% of the obstetric admissions<br />
to the intensive care unit. We report an unusual case<br />
of a multigravida with ruptured pulmonary hydatid<br />
cyst that mimics tension hydropneumothorax.<br />

Hydatid disease in pregnancy is a rare condition,<br />
with and incidence of 1/20,000 to 1/30,000<br />
deliveries (1,2). In late pregnancy, it carries the<br />
risk of enlargement with subsequent rupture and<br />
anaphylactic shock (3). Respiratory failure and<br />

A 21-year-old patient with multigravida, at 34 weeks of<br />
gestation, presented with syncope, vomiting, and respiratory<br />
failure. She explained that the symptoms had been<br />
present for ten days. She had gone to some medical<br />
centers, but the doctors diagnosed her symptoms as being<br />
related to the last trimester of pregnancy. There was<br />
not any thoracic imaging up to now. The patient’s physical<br />
examination revealed severe respiratory distress with a<br />
respiratory rate of 35 breaths/min, tachycardia of 160-<br />
183/min, fever of 38.0 °C, blood pressure of 90/60<br />
mmHg, and no breath sounds at the left hemithorax.<br />
Oxygen saturation was 90% with a facemask at 10 L/min.<br />
The chest x-ray that was taken on presentation to the<br />
referring hospital revealed a high air-fluid level, which<br />
was surrounded by a thick membrane in the left hemithorax<br />
with the sliding of the trachea and heart to the<br />
right hemithorax, which mimics tension hydropneumothorax<br />
(Figure 1). Thoracentesis was carefully conducted to<br />
exclude hydropneumothorax, and neither fluid nor air was<br />
aspirated from the left hemithorax. A decision was made<br />
to proceed with a caesarian section, followed by left thoracotomy<br />
to save both the mother and the baby. Informed<br />
consent was obtained from patient. Following the caesarian<br />
section, a left thoracotomy was performed. A destroyed<br />
left lower lobe and huge ruptured hydatid cyst<br />
were detected. A left lower lobectomy was performed.<br />
The baby was sent to another hospital for admission to<br />
the pediatric intensive care unit. The mother was discharged<br />
from the hospital nine days after surgery without<br />
any complications.<br />
DISCUSSION<br />
A hydatid cyst is a kind of parasitosis, which affects mostly<br />
liver and/or lungs. It can cause different symptoms or be<br />
found incidentally. Cough, hemoptysis, and chest pain<br />
are the most common symptoms of hydatid cyst of the<br />
lung (5). There are very few cases about pregnancy and<br />
coexistence with hydatid cyst in literature (1,2,5,7). In the<br />
current case, we determined a ruptured hydatid cyst of<br />
the lung in the late period of pregnancy with respiratory<br />
failure.<br />
Cysts may rupture spontaneously or more commonly<br />
precipitated by a sudden rise in intrapulmonary pressure<br />
(6). It may follow an increase in intraabdominal pressure<br />
as in pregnancy (7).<br />
Radiologically, an intact cyst typically presents as a round<br />
or oval homogenous density with sharp margins. In case<br />
of a ruptured cyst, the solid component may fall into the<br />
cavity (mass within a cavity) (8). The current case revealed<br />
a high air-fluid level in the left hemithorax with sliding of<br />
the trachea and heart to the right hemithorax. A solid<br />
component of the cyst was not seen on the chest X-ray<br />
and it could be confused with tension hydropneumothorax<br />
in emergency conditions.<br />
Pregnancy and pulmonary hydatidosis occurring together<br />
are very rare and there is no consensus on standard approach<br />
(7). The researchers of the current study believe<br />
that definitive management of ruptured pulmonary hydatid<br />
cyst is surgery.<br />
Ruptured pulmonary hydatid cyst should be considered in<br />
patients living in endemic areas with suggestive radiologic<br />
findings, especially during pregnancy. Doctors should be<br />
alert for the patients who live in endemic areas for hydatid<br />
cyst and receive a chest x-ray before planning pregnancy.<br />

Akciğer apsesi, piyojenik mikroorganizmaların doku<br />
nekrozuna yol açması sonucu akciğer parankiminde<br />
oluşan kaviter, düzgün sınırlı, lokalize ve süpüratif bir<br />
lezyondur. Radyolojik olarak hava-sıvı düzeyi gösteren<br />
kavite görünümü ile tanınır ve bu olguların %10-<br />
15’i antibiyotik tedavisine rağmen iyileşmemektedir.<br />
Girişimsel tedavi yöntemleri arasında transtorasik tüp<br />
drenajı, bronkoskopi veya bilgisayarlı tomografi eşliğinde<br />
aspirasyon ve cerrahi rezeksiyon bulunmaktadır.<br />
Burada diyabeti olan 70 yaşındaki bir kadın hastayı<br />
sunmaktayız. Hastanın sağ akciğer alt lobunda periferik<br />
yerleşimli bir apse saptandı. İlk olarak medikal<br />
tedavi yapıldı. Yanıt alınamaması üzerine ultrasonografi<br />
eşliğinde ince iğne aspirasyonu ile apse drenajı<br />
yapıldı. Yaptığımız literatür araştırmasında, ultrasonografi<br />
eşliğinde ince iğne aspirasyonuyla tam olarak<br />
iyileşen başka bir akciğer apsesi olgusuna rastlamadık.<br />
Sonuç olarak toraks duvarına komşu periferik<br />
akciğer apselerinin tedavisinde; kolay ulaşılabilirliği,<br />
ucuz bir yöntem olması ve radyasyon maruziyetine yol<br />
açmaması nedeniyle ultrasonografi eşliğinde transtorasik<br />
apse drenajı, ilk tercih olarak düşünülmelidir.<br />

Lung abscess is a well-circumscribed and localized<br />
suppurative lesion in the lung parenchyma as a result<br />
of pyogenic microorganisms, which usually leads to<br />
tissue necrosis and cavitation. The lung abscess radiologically<br />
appears as an air-fluid level and despite<br />
the antibiotic treatments, up to 10%-15% of these<br />
lesions do not heal. In these cases, drainage by the<br />
transthoracic catheters and aspiration by the guidance<br />
of bronchoscopy or computed tomography and<br />
surgical resection are the choices of treatment modalities.<br />
Herein, we report a 70-year-old female<br />
patient with diabetes mellitus. A lung abscess was<br />
detected in the right lower lobe with peripheral localization.<br />
The patient did not recover by medical therapy<br />
and the content of abscess was aspirated by ultrasonography<br />
guidance. No cases with lung abscess<br />
that was treated completely with the fine needle aspiration<br />
by the guidance of ultrasonography were<br />
found in the literature. As a result, we recommend<br />
that transthoracic drainage of peripheral lung abscesses<br />
by the guidance of ultrasonography should<br />
be considered as a first choice treatment modality<br />
with the advantages of being affordable, convenient,<br />
and with no exposure to radiation.<br />

Akciğer apsesi, piyojenik mikroorganizmaların yol açtığı<br />
iyi sınırlı ve lokalize püy birikimi olup akciğer parankiminde<br />
doku nekrozu ve kavite oluşumuyla seyreder. Geliştirilen<br />
yeni antiyotiklerle birlikte akciğer apsesine bağlı mortalite<br />
giderek azalmakla birlikte bu olguların %10-15’inde<br />
antibiyotik tedavisine yanıt alınamamaktadır (1). Akciğer<br />
apsesi genellikle aspirasyon riskinin yüksek olduğu olgularda<br />
görülmektedir. Bilinç bozukluğu, alkol kullanımı,<br />
serebrovasküler olaylar, diyabetes mellitus, kronik akciğer<br />
hastalıkları ve kronik karaciğer hastalıkları önde gelen<br />
risk faktörleri arasında bulunmaktadır (2). Akciğer apsesinin<br />
başlangıç tedavisini uzun süreli etkin antibiyotik kullanımına<br />
dayalı medikal tedaviler oluşturur ve bu yöntemle<br />
olguların yaklaşık %85-90’ı tam olarak iyileşmektedir.<br />
Medikal tedavinin başarısız olduğu durumlarda ise apse<br />
drenajı, cerrahi rezeksiyon ve seçilmiş olgularda lobektomi<br />
gibi girişimsel bir takım tedaviler uygulanmaktadır.<br />
Başlıca apse drenaj yöntemleri; perkütan transtorasik tüp<br />
drenajı (PTD), endoskopik drenaj ve bilgisayarlı tomografi<br />
eşliğinde drenajdır. Perkütan transtorasik tüp drenajı,<br />
torakotomi ve lobektomiyi tolere edemeyecek olan hastalarda<br />
tercih edilmektedir (3,4). Endoskopik drenaj yöntemi,<br />
büyük hava yollarını tıkayan santral yerleşimli apselerde,<br />
apsenin bronşial sisteme açıldığı durumlarda ve<br />
pıhtılaşma bozukluğu olan olgularda PTD yöntemine<br />
alternatif olarak kullanılmaktadır (5). Bilgisayarlı tomografi<br />
eşliğinde ince iğne ile drenaj etkin ve güvenilir bir yöntem<br />
olmasına rağmen, pratik bir yöntem olmadığından<br />
günlük uygulamalarda daha az sıklıkla başvurulmaktadır<br />
(6). Burada sunduğumuz olgu, medikal tedaviyle yeterli<br />
yanıt alamadığımız ve ultrasonografi eşliğinde ince iğne<br />
aspirasyonu ile başarılı bir şekilde tedavi ettiğimiz periferik<br />
yerleşimli bir akciğer apsesi olgusudur.<br />
(Şekil 2, 3) çekilen hastaya klinik, laboratuvar ve radyolojik<br />
bulgular doğrultusunda akciğer apsesi tanısı konuldu.<br />
Göğüs hastalıkları kliniğine yatırılan hastaya antibiyotik<br />
tedavisi olarak parenteral yoldan ampisilin+sulbactam<br />
4x1,5 gr ve metranidazol 4x500 mg başlandı. Klinik,<br />
laboratuvar ve radyolojik bulgularda düzelme sağlanamadığından<br />
tedavinin yedinci gününde ampisilin+sulbactam<br />
kesilerek imipenem+silastatin 4x500 mg<br />
başlandı ve metranidazol tedavisine devam edildi. Yapılan<br />
tedavi değişikliğine rağmen iyileşme sağlanamadı.<br />
Çekilen akciğer grafisinde az miktarda hava sıvı seviyesi<br />
görüntüsü ortaya çıktı. Yapılan bronkoskopi işleminde<br />
havayollarının açık olduğu ve apse içeriğinin bronş içine<br />
drenajının olmadığı görüldü. Tedavinin 15. gününde<br />
yapılan toraks ultrasonografisinde periferik yerleşimli apse<br />
formasyonu tespit edildi (Şekil 4). Hastanın sırt bölgesinde<br />
sağ midskapular hat hizasında sekizinci interkostal aralıktan,<br />
ultrasonografi eşliğinde ince iğne (16 gauge) ile<br />
aspirasyon yapıldı ve yaklaşık 120 mililitre pürülan apse<br />
içeriği boşaltıldı (Şekil 5). Bu işlemden sonra hastanın<br />
kliniğinde belirgin düzelme gözlendi. Ayrıca enfeksiyon<br />
göstergelerinden CRP ve sedimantasyon değerleri azalırken<br />
lökosit sayısı normal düzeylere geriledi. Aspirasyon<br />
işleminden 3 gün sonra kontrol akciğer grafisi ve toraks<br />
bilgisayarlı tomografisinde apse çapının küçüldüğü ve<br />
apse içeriğinin azaldığı gözlendi (Şekil 6,7). İkinci kez<br />
ultrasonografi eşliğinde ince iğne ile 30 mililitrelik drenaj<br />
yapıldı. Bu işlemden 3 gün sonra hastanın kliniği ve laboratuvar<br />
bulguları normale dönerken akciğer grafisinde<br />
tama yakın düzelme görüldü (Şekil 8). Tedavisi tamamlanan<br />
hasta taburcu edildi.<br />
OLGU<br />
Kliniğimize yüksek ateş, öksürük, bol miktarda pürülan<br />
balgam, halsizlik ve iştahsızlık yakınmalarıyla başvuran 70<br />
yaşındaki kadın hastanın 30 yıllık Tip 2 Diabetes Mellitus<br />
ve son 6 yıldır insülin kullanma öyküsü bulunmaktaydı. İki<br />
hafta önce gripal bir infeksiyon geçirdiğini belirten hasta,<br />
asıl yakınmalarının son beş gün içerisinde giderek arttığını<br />
belirtiyordu. Olgunun fizik muayenesinde; aksiler bölgeden<br />
ölçülen vücut ateşi 38,5 °C, sağ akciğer orta ve alt<br />
alanlarda perküsyonla matite alınmakta ve oskültasyonda<br />
aynı bölgede solunum seslerinin azalmış olup her iki akciğer<br />
alt alanlarında ince raller duyulmaktaydı. Laboratuvar<br />
bulgularında CRP: 35 mg/dl, lökosit sayısı: 23000/ml<br />
(%88’i nötrofil), sedimantasyon hızı: 105 mm/saat olarak<br />

Son zamanlarda akciğer apsesinin gerek tanısında, gerekse<br />
tedavisinde bilgisayarlı tomografi eşliğinde iğne<br />
aspirasyonu yönteminin kullanımı giderek artmaktadır.<br />
Kelogrigoris ve ark. (6) akciğer apselerine bilgisayarlı<br />
tomografi eşliğinde perkütan olarak drenaj tedavisi uyguladıkları<br />
40 olguyu içeren seride %83 oranında bir tedavi<br />
başarısı sağlamışlardır.<br />

TARTIŞMA<br />
Medikal tedaviye dirençli akciğer apsesi, çözümlenmesi<br />
zor olan klinik sorunlar arasında yer almaktadır. Böyle<br />
durumlarda, tedavi yöntemi olarak cerrahi rezeksiyon ve<br />
tüp drenajı gibi mortalite ve morbidite riski olabilecek<br />
invazif yöntemler uygulanmaktadır.<br />
Ultrasonografi eşliğinde ince iğne aspirasyonu tedavileri,<br />
meme apsesinde, batın içi apselerde ve özellikle de karaciğer<br />
ve dalak apselerinde uzun süredir başarılı bir tedavi<br />
şekli olarak kullanılmaktadır (7,8). Bizim yaptığımız literatür<br />
araştırmasında erişkinlerde ultrasonografi eşliğinde<br />
ince iğne aspirasyonu ile akciğer apsesinin tedavisine<br />
rastlamadık. Fakat yeni doğan çocuklarda yapılan bir<br />
çalışmada, ultrasonografi eşliğinde basit perkütan iğne<br />
drenajı ile akciğer apsesinin etkin bir şekilde tedavi edildiği<br />
belirtilmiştir (9).<br />
Toraks ultrasonografisi, plevral sıvıların yapısını değerlendirmede,<br />
ankiste plevral sıvılardan örnek alınmasında ve<br />
periferik akciğer lezyonlarından biyopsi alınmasında yar-<br />

dımcı olabilecek son derece güvenli ve kolay bir yöntemdir.<br />
Akciğer apselerinde ultrasonografi eşliğinde aspirasyon<br />
uygulaması sayesinde etken olan mikrobiyal ajanlar tespit<br />
edilebilir (10). Bizim sunduğumuz olguda, akciğer apsesi<br />
yaklaşık olarak 10 cm çapında ve periferik yerleşimliydi.<br />
Bu nedenle apsenin değerlendirilmesi ve aspirasyonu<br />
oldukça kolay bir şekilde yapıldı. Ultrasonografi, hastalar<br />
radyasyona maruz kalmadan, yatak başında uygulanabilen,<br />
pratik, tekrarlanabilir ve kolay ulaşılabilir bir yöntemdir.<br />
Dolayısıyla klinisyen ve hasta için son derece avantajları<br />
vardır.<br />

Sonuç olarak, ultrasonografi eşliğinde aspirasyon uygulaması<br />
özellikle medikal tedaviye cevap vermeyen periferik<br />
akciğer apsesi olgularında, diğer invazif tedavi yöntemlerinden<br />
önce ilk tedavi seçeneği olarak göz önünde<br />

Keratosis lichenoides chronica (KLC) is a rare and<br />
chronic skin disease characterized by erythema, keratotic<br />
and lichenoid papules are distributed symmetrically.<br />
The histological examination of KLC reveals<br />
lichenoid dermatitis with hyper and parakeratosis.<br />
Although the etiology and pathogenesis of KLC are<br />
unknown, it is suggested that KLC is a variant of<br />
lichen planus or lichenoid drug eruptions. We present<br />
a case of generalized KLC that occurred during antituberculosis<br />
therapy including isoniazid, rifampicin,<br />
ethambutol, and pyrazinamide and dramatically<br />
improved after cessation of therapy. We report this<br />
generalized KLC case in order to draw attention to<br />
the side effects that can be seen during tuberculosis<br />
treatment.<br />

Keratozis Likenozis Kronika (KLK), simetrik dağılım<br />
gösteren, eritemli, likenoid, keratotik papüllerle karakterize,<br />
nadir görülen kronik bir dermatozdur. Histopatolojik<br />
incelemede hiperkeratoz ve parakeratozun<br />
eşlik ettiği likenoid dermatit bulgusu görülür. Etyolojisi<br />
tam olarak bilinmemekle birlikte liken planus veya<br />
likenoid ilaç reaksiyonlarının bir varyantı olduğu<br />
düşünülür. Tedavinin kesilmesiyle dramatik bir düzelme<br />
gösteren, isoniyazid, rifampisin, etambutol ve<br />
pirazinamidli antitüberküloz tedavi sırasında oluşan<br />
Jeneralize KLK olgusunu, tüberküloz tedavisi sırasında<br />
görülebilecek yan etkilere dikkat çekmek amacıyla<br />
sunuyoruz.<br />

Keratosis Lichenoides Chronica (KLC), or Nekam’s disease,<br />
is a rare chronic mucocutaneous dermatitis characterized<br />
by keratinization. It most commonly occurs in<br />
adolescents and young adults, with a slight predominance<br />
in males (M/F: 1.35:1) (1,2). Although its etiology<br />
is unknown, systemic diseases, infections, and drugs have<br />
been reported as possible trigger factors (3). We report a<br />
case of KLC that developed secondary to antituberculosis<br />
treatment while on treatment for pulmonary tuberculosis.<br />
the patient continued to receive the three-drug protocol.<br />
With this treatment protocol, no relapses were observed<br />
during nine months of follow-up. The patch test, which<br />
was performed after the treatment by using antituberculosis<br />
drugs, was positive for pyrazinamide but negative for<br />
other anti-tuberculosis drugs.<br />
CASE<br />
A 58-year-old male patient was admitted to our clinic<br />
with the complaint of a generalized rash with erythematous,<br />
scaly plaques. He was diagnosed with tuberculosis<br />
two months prior and isoniazid, rifampicin, ethambutol,<br />
and pyrazinamide treatment was initiated. However, on<br />
the tenth day of the treatment, he developed a rash starting<br />
from the torso disseminating throughout the body. In<br />
the dermatological examination, violaceous-erythematous,<br />
scaly, infiltrated plaques were found all over the body.<br />
Lesions were mostly located on the torso, and upper and<br />
lower extremities and showed a tendency for coalescence<br />
(Figure 1-3). Laboratory tests including blood biochemistry,<br />
complete blood count, anti-HIV, and hepatitis markers<br />
were all normal. A biopsy taken from the lesions revealed<br />
focal parakeratosis, irregular acanthosis, focal<br />
exocytosis, colloid bodies, basal vacuolar degeneration,<br />
necrotic cells, and lichenoid infiltration (Figure 4). The<br />
patient was diagnosed with KLC. The patient’s antituberculosis<br />
treatment was terminated and a topical<br />
methylprednisolone aceponate and oral cetirizine treatments<br />
were initiated. All lesions have improved on the<br />
15th day of the treatment. Therefore, we suspected that<br />
antituberculosis treatment triggered the current condition.<br />
However, since the antituberculosis treatment was mandatory,<br />
after consulting a pulmonologist, we decided to<br />
administer antituberculosis drugs intermittently, in a consecutively<br />
at precise intervals in order to determine the<br />
responsible drug. Accordingly, every week the patient was<br />
reintroduced his medications successively (isoniazid 300<br />
mg/day, ethambutol 1500 mg/day, and rifampicin 600<br />
mg/day, respectively), under the control of the pulmonary<br />
medicine unit, and the patient did not experience any side<br />
effects during that time. The last drug added to the intermittent<br />
treatment protocol was pyrazinamide 1500<br />
mg/day and the patient’s erythematous, scaly lesions<br />
relapsed on his torso on the third day of pyrazinamide<br />
treatment. Thinking that pyrazinamide was the triggering<br />
drug, it was excluded from the treatment protocol, thus,<br />

DISCUSSION<br />
Kaposi first described this entity in 1895 as lichen ruber<br />
acuminatus verrucosus et reticularis. The most current<br />
and widely accepted term is keratosis lichenoides chronica<br />
(KLC), as proposed by Margolis et al. (4) in 1972. It is,<br />
as in the current case, characterized by widespread symmetrical<br />
lichenoid hyperkeratotic papules that are typically<br />
arranged in a linear or reticulate pattern, and erythematous<br />
psoriasis-like, scaly plaques (1,2). In 75% of cases,<br />
rash-like or rosacea-like facial seborrheic dermatitis is<br />
present, which facilitates diagnosis. Nail dystrophy may<br />
be seen in 30% of these patients and palmoplantar<br />

keratoderma in approximately 40% of cases. This skin<br />
problem can also be associated with oral manifestations<br />
in 50% of cases presenting as recurrent aphthous ulcers<br />
and lesions (4). The current case has presented with erythematous,<br />
scaly plaques, which were very clinically similar<br />
to psoriasis and there was no accompanying nail or<br />
soft tissue involvement.<br />

In the histopathological examination of KLC, the presence<br />
of lichenoid dermatitis associated with numerous necrotic<br />
keratinocytes, focal parakeratosis, and remnant neutrophils<br />
are characteristic. There may be vacuolar alteration<br />
of the basal layer along the dermo-epidermal junction<br />
(2,5).<br />
Although KLC has some common clinical and dermatopathological<br />
features with lichen planus, it is considered<br />
to be a distinct entity (6). The etiopathogenesis of KLC is<br />
still unknown but its close similarity to lichenoid drug<br />
eruption induced by tuberculostatic and anti-malarial<br />
drugs suggest that KLC is a variant of lichenoid druginduced<br />
eruptions (3). It has been reported in association<br />
with systemic diseases (renal disease, diabetes mellitus,<br />
lymphoma, multiple sclerosis, mycosis fungoides, hypothyroidism,<br />
and chronic hepatitis), following trauma, and<br />
chronic infectious diseases such as toxoplasmosis, and<br />
tuberculosis (7). Our patient had no systemic pathology<br />
other than pulmonary tuberculosis. The researchers suspected<br />
that the KLC lesions developed secondary to the<br />
drugs because the eruptions first appeared on the tenth<br />
day of the antituberculosis treatment and regressed after<br />
the discontinuation of the treatment; however, the responsible<br />
drug could not be identified. Since the patient<br />
required treatment for pulmonary tuberculosis, antituberculosis<br />
drugs were administered to the patient consecutively<br />
at precise intervals. Scaly-lichenoid papules similar<br />
to the first reaction developed following pyrazinamide, the<br />
last drug added to the treatment. Subsequently, pyrazinamide<br />
was excluded from the treatment protocol and<br />
the treatment continued as three-drug regimen for nine<br />
months. During this period, no new lesions developed.<br />
Accordingly, the researchers determined that the triggering<br />
agent was pyrazinamide. The treatment of KLC is<br />
difficult because the disease is resistant to many different<br />
therapies, including topical or systemic corticosteroids,<br />
methotrexate, cyclosporine, antibiotics, antimalarials, and<br />
sulfones. In some cases, partial responses have been<br />
reported with the use of PUVA, narrowband UV-B, and<br />
systemic retinoids (8). In the current case, generalized<br />
eruptions which were presumed to be induced by pyrazinamide,<br />
resolved after the administration of symptomatic<br />
treatment and the abandonment of the suspected drug<br />
without any further treatment.<br />
As a result, the researchers suggest that KLC should be<br />
considered in the differential diagnosis of dermatosis<br />
presenting with erythematous, scaly lichenoid papules,<br />
and drug intake as the triggering agent must be questioned<br />
while recording the history.<br />

Lung cancers rarely metastasize to intracardiac ventricular<br />
wall and subsequently embolizes to systemic<br />
circulation, causing arterial occlusion. The current<br />
study reports a case of lung cancer with peripheral<br />
arterial embolism. This is unique case report because<br />
the first diagnostic tool of bronchogenic carcinoma<br />
was embolectomy material during hospitalization and<br />
the initial evaluation stage of the patient. Tumor<br />
embolization should be considered in the differential<br />
diagnosis in an abrupt cessation of peripheral arterial<br />
circulation.<br />

Akciğer kanseri, nadir olarak kalbin iç ventrikül duvarına<br />
metastaz yapar ve sonrasında buradan kaynaklanan<br />
emboli sistemik dolaşıma girerek atar damar<br />
tıkanıklığına yol açar. Burada, periferik atar damarında<br />
emboliye yol açan bir akciğer kanseri olgusunu<br />
sunmaktayız. Bu olgunun ilginç tarafı, hospitalize<br />
edilip ilk değerlendirme aşamasında, periferik arter<br />
embolektomi materyalinden akciğer kanseri tanısı<br />
konulmuş olmasıdır. Periferik arter dolaşımının ani<br />
olarak durmasında, ayırıcı tanıda tümör embolisi de<br />
akılda tutulmalıdır.<br />

Peripheral arterial embolism arising from lung cancer is<br />
uncommon (1-4). Malignant primary lung tumors invading<br />
pulmonary vessels, atrial myxomas and other primary<br />
cardiac tumors are the main causes of arterial embolism,<br />
which is a very unusual clinical entity (5). Metastatic tumors<br />
may involve the pericardium and signs of pericardial<br />
effusion and tamponed may be observed. In general,<br />
arterial embolism occurs either intraoperatively or in the<br />
immediate postoperative period of bronchogenic carcinoma.<br />
The most frequently reported sites of tumor emboli<br />
are aortic bifurcation, femoral vessels, and cerebral arteries<br />
(2). Nearly 200 cases of arterial embolism have been<br />
reported in the literature from malignant tumor tissue (3,<br />
6). Intravenous digital angiography, contrast enhanced<br />
computerized tomography, MRI, and echocardiography<br />
are the diagnostic methods. Regarding treatment, once<br />
the embolism has appeared, it should be surgically removed,<br />
and anticoagulation, vasodilator treatment<br />
should be initiated.<br />
In the literature only a few of reports related to peripheral<br />
arterial embolism arising from bronchogenic carcinoma<br />
have been reported. Arterial tumor embolism may be the<br />
first and most important issue during the course of the<br />
tumor. Herein, we report a quite rare occurrence of a<br />
metastatic bronchogenic tumor to the left ventricle and<br />
presented with sudden occlusion of the left brachial artery<br />
during the diagnostic stage of the patient.<br />
A 77-year-old male smoker with a history of COPD presented<br />
with chest pain and breathlessness for ten days.<br />
The chest radiograph and computed tomography of the<br />
chest revealed a heterogeneous, ill-defined mass originated<br />
from left upper lobe bronchus, extending and invading<br />
the hilar structures (Figure 1). Transthoracic echocardiography<br />
confirmed an intracardiac mobile mass,<br />
implanted in the lateral wall. Positron emission tomography<br />
revealed pulmonary malignancy and distant metastasis<br />
with increased FDG uptakes in a left hilar mass, satellite<br />
lesions on the upper lobe posteriorly destructing the<br />
fourth rib, subcarinal lymphadenopathy, left ventricle, and<br />
liver (Figure 2). Fiberoptic bronchoscopy revealed an<br />
endobronchial lesion in the lingular bronchus, which was<br />
biopsied. While the patient was hospitalized and awaiting<br />
pathologic results, he complained of pain, numbness in<br />
the right arm, and had cyanosis. An immediate angiography<br />
confirmed total occlusion of right brachial artery<br />
and an immediate embolectomy was performed and<br />
circulation restored. Subsequently, pathology of the bronchoscopic<br />
biopsies revealed squamous cell bronchogenic<br />
carcinoma and the cytology of embolectomy confirmed<br />
metastasis of lung cancer (Figures 3 and 4). Upon discharge,<br />
the patient was referred to the oncology department,<br />
but the patient died one month later.<br />

DISCUSSION<br />
A macroscopic peripheral arterial tumor embolus is an<br />
uncommon event. Large and centrally located bronchogenic<br />
tumors were most often responsible for arterial<br />
tumor emboli (1). The event could also be the presenting<br />
manifestation of a bronchogenic carcinoma, as presented<br />
here. Another common source of the arterial emboli is<br />
from the left atrial myxoma and sarcomas, primary or<br />
secondary bronchogenic carcinoma. Arterial embolization<br />
highlights the advanced disease, and poor prognosis with<br />
significant comorbidity. The source of tumor emboli could<br />
be the left atrium or pulmonary veins (7). The mechanism<br />
of primary and metastatic lung cancer embolism is considered<br />
to involve a tumor that invades and propagates<br />
within a pulmonary vein. When a tumor fragment breaks<br />
off in the pulmonary vein, it travels through the left heart<br />
chamber and subsequently lodges in a systemic artery.<br />
Infarction results when the tumor fragment is large<br />
enough to occlude the tissue’s arterial perfusion. Most<br />
tumor emboli occur during surgery; a few occur in the<br />
early postoperative period (8-12). The delayed occurrence<br />
of these emboli may be due to tumor fragments<br />
that remain loosely adhered to the site of pulmonary vein<br />
ligation and then embolize after surgery with coughing or<br />
other movement, or perhaps emboli are temporarily detained<br />
in the left ventricular trabeculations or chordae<br />
tendineae (1,2). Arterial embolism related to thoracic<br />
surgery is likely due to the manipulation of the pulmonary<br />
hilum or division of the pulmonary vein, which dislodges<br />
the tumor from the pulmonary vein. The other scenario of<br />
tumor embolization occurs in the absence of pulmonary<br />
surgery and is assumed to result from spontaneous fragmentation<br />
of the propagated tumor within a pulmonary<br />
vein (4,11). Xiromeritis et al. (3) identified 104 reported<br />
cases of arterial tumor emboli and 46 of the cases arose<br />
from primary lung cancer and 33 patients with metastatic<br />
lung cancer, and these were responsible for most of these<br />
cases. The most common cause of the metastatic lung<br />
neoplasms was sarcoma, which accounted for 14 cases.<br />
Arterial tumor embolism mostly occurred spontaneously<br />
and fewer of cases were associated with thoracic surgery.<br />
In the review of Whyte et al. (2), tumor embolization to<br />
cerebral arteries occurred in half of the cases, and to<br />
multiple locations occurred in one-third of patients. Immediate<br />
diagnosis of an arterial embolism is very critical<br />
and immediate intervention is required to restore circulation<br />
and to avoid tissue necrosis. Routine two dimensional<br />
or transesophageal echocardiography may be<br />
useful in diagnosing large and centrally located intracardiac<br />
tumors (8). This is a unique case with a metastatic<br />
mass invading the left ventricle and the source of peripheral<br />
embolism. Overall prognosis is poor in these cases<br />
despite immediate diagnosis and aggressive surgical<br />
interventions, as these patients tend to die from extended<br />
metastatic disease.<br />

Sağ arkus anomalisi erişkin çağda nadir görülen,<br />
genellikle asemptomatik bir durumdur. Trakea veya<br />
özefagusa bası durumunda hastalar bazen nefes<br />
darlığı veya disfaji gibi şikâyetlerle de başvurabilirler.<br />
Bu olgu sunumunda sağ arkus aorta anomalisi bulunan,<br />
yıllardır astım tanısı ile takip edilen 59 yaşında<br />
kadın hasta sunulmaktadır. Hastalarda astım ayırıcı<br />
tanısında vasküler ring gibi hava yolu basısına yol<br />
açan patolojilerin de düşünülmesi önemlidir.<br />

Right aortic arch is a rare condition that is usually<br />
asymptomatic in adulthood. Patients may present with<br />
dyspnea and dysphagia. This report presents a 59-<br />
year-old woman with a right aortic arch anomaly who<br />
had been followed-up with asthma for many years.<br />
The current report emphasizes the importance of the<br />
consideration of aortic arch anomaly in the differential<br />
diagnosis of asthma.<br />

damarsal anomalilerle birlikte olabileceği gibi,<br />
tek başına da olabilir. Uzun süredir astım tanısı<br />
ile izlenmiş olgumuzu, erişkin döneminde nadir<br />
görülen kardiyovasküler anomalilerden vasküler<br />
ring şekli olan sağ arkus aorta anomalisi saptanması<br />
nedeni ile sunduk.<br />

OLGU<br />
Uzun yıllardır astım tanısı ile çeşitli inhaler tedaviler kullanmış<br />
59 yaşında kadın hastanın özellikle düz yatmakla<br />
artan hırıltılı solunum, öksürük ve nefes darlığı yakınmaları<br />
vardı. Özgeçmişinde özellik olmayan hastanın ailesinde<br />
ve kendisinde atopi öyküsü yoktu. Sigara alkol kullanım<br />
öyküsü vermiyordu. Vital bulguları stabil idi. Solunum<br />
sistemi ve diğer tüm sistemik muayenesi normal olarak<br />
değerlendirildi. Tam kan sayımı ve biyokimyasal tetkikleri<br />
normal sınırlarda idi. PA akciğer grafisinde mediastende<br />
genişleme görüldü. Alerji testi normal olarak saptandı.<br />
Solunum fonksiyon testinde FEV1: %109, FVC: %101,<br />
MEF 25-75: %91, FEV1/FVC: %90, reversibilite saptanmadı.<br />
Hastada uygun tedaviye rağmen şikâyetlerinde<br />
gerilememe olmaması üzerine ayırıcı tanılar gözden geçirildi.<br />
Solunum fonksiyon testinde akım/volüm eğrisinde<br />
inspiratuar ve ekspiratuar akım hızlarında azalma, intratorasik<br />
üst hava yolu obstrüksiyonunu düşündürdü. Bu nedenle<br />
çekilen toraks bilgisayarlı tomografi (BT) anjiografi<br />
incelemesinde sağ arkus aorta mevcut olup trunkus brakiosefalik<br />
lokalizasyonundan daha sağdan çıkıp sol karotis<br />
arter ve sol subklavian arteri verdiği, trakeaya belirli bir<br />
alandan bası yaptığı saptandı (Şekil 1 ve 2). Ayırıcı tanı<br />
için hastadan gerekli onamlar alındıktan sonra yapılan<br />
fiberoptik bronkoskopik incelemede trakea ve bronş sisteminde<br />
herhangi bir anomali ve endobronşiyal lezyon<br />
saptanmadı. Trakea alt hizasında sağ lateral duvarda<br />
dıştan bası bulgusu vardı (Şekil 3). Ekokardiyografik incelemede<br />
herhangi bir ek anormal bulgu gözlenmedi. Hastamızın<br />
semptomatik olması nedeniyle vasküler ringe<br />
yönelik cerrahi operasyon yapılması amacıyla kalp damar<br />
cerrahisine servisine yönlendirildi.<br />

TARTIŞMA<br />
Konjenital bir anomali olan vasküler ringler, bronşiyal ark<br />
sistemini etkileyen anomaliler sonucunda trakea ve özefagusun<br />
bu ark sisteminden oluşan damarlar ile çevrelendiği<br />
ve bası altında kaldığı patolojilerdir. Anatomik açıdan<br />
trakea ve özefagusun ikisinin birden bu damarlar ile<br />
çevrildiği formuna “komplet”, tam olarak çevrilemediği<br />
trakea veya özefagusun bir kısmının bası altında kaldığı<br />
formuna ise “inkomplet” vasküler ring denilmektedir. Bu<br />
iki tip trakeaözefageal kompresyon anomalisinde aynı<br />
semptomların bulunması, benzer şekilde değerlendirilmesi,<br />
benzer cerrahi müdahalelerin uygulanması nedeniyle<br />
bazıları inkomplet olsa dahi aynı başlıkta, vasküler ring<br />
konusu içinde incelenmektedir (1- 4). Tanı akciğer grafisi<br />
(PA ve yan), baryumlu özefagus grafisi, transtorasik EKO,<br />
kontrastlı multidetektör BT, magnetik rezonans görüntüleme<br />
(MRG) ve anjiografi (DSA, MRG veya BT anjiyografi)<br />
ile konur (2,5). Günümüzde EKO, BT, MRG en sık tercih<br />
edilen tanı metotlarıdır (6,7). Bu hastalarda bronkoskopinin<br />
tanısal olarak kullanımı sınırlı olup, ayırıcı tanı açısın-<br />

dan yapılmaktadır. Erken dönemde dispne, vizing, stridor;<br />
geç dönemde ise egzersize bağlı nefes darlığı, disfaji ve<br />
göğüs ağrısı gibi semptomlara neden olabilir (4,8). Hastamız<br />
da benzer şekilde tekrarlayan hırıltılı solunum, öksürük<br />
ve nefes darlığı yakınmaları ile birçok merkeze başvurmuş,<br />
astımı taklit eden klinik nedeniyle hasta uzun süre<br />
astım tanısı altında takip edilmiştir. Hastada uygun tedaviye<br />
rağmen şikâyetlerinde gerilememe olmaması üzerine<br />
ayırıcı tanılar gözden geçirilmiştir. Solunum fonksiyon<br />
testinde akım/volüm eğrisinde inspiratuar ve ekspiratuar<br />
akım hızlarında azalma olması, PA akciğer grafisinde<br />
mediastende genişleme olması nedeniyle BT anjiografi ilk<br />
tetkik olarak tercih edilmiştir. Vasküler ringe bağlı ortaya<br />
çıkan klinik bulgular, vasküler ringin yerine ve havayoluna<br />
yaptığı bası derecesine göre değişkenlik gösterir. Vasküler<br />
ringe bağlı ortaya çıkan havayolu basısı belirgin ise yakınmalar<br />
yaşamın erken döneminde ve ağır olarak ortaya<br />
çıkmaktadır. Backer ve ark. (9) ameliyat ettikleri çift arkus<br />
aortalı 61 hastanın %92’sinin yaşamlarının ilk 6 ayı içerisinde<br />
belirti verdiklerini belirtmişlerdir. Özkaya ve ark. (10)<br />
yapmış olduğu 13 hastalık çalışmada, disfaji ve dispnenin<br />
en sık semptom olduğu ve hastaların %54’ünün semptomatik<br />
olduğunu belirtmişlerdir. Ancak havayolu obstrüksiyonu<br />
hafif ise bizim hastamızda da olduğu gibi yakınmalar<br />
geç dönemde ve hafif olarak ortaya çıkabilmektedir<br />
(10,11). Grathwohl ve ark. (12) izledikleri 25 vasküler<br />
halka anomalili hastada en çok çift aort arkı (n=12) ve<br />
sağ arkus aorta anomalisi (SAAA) (n=8) saptamışlar ve<br />
tüm hastaların %66’sının semptomatik olduğunu belirtmişlerdir.<br />
Hastalar bizim hastamızda olduğu gibi genellikle<br />
bronşiyolit, astım veya tekrarlayan pnömoni tanıları<br />
almakta ve vasküler ring tanısı geç koyulmaktadır (13).<br />
Fındık ve ark. (14) takip ettikleri sekiz SAAA hastasının<br />
dördünde çeşitli semptomların olduğunu bildirmiştir. Bu<br />
hastaların ikisinin SAAA tanısı almadan önce astım tanısıyla<br />
takip ve tedavi edildikleri belirtilmiştir. Havayoluna<br />
bası varlığı radyolojik olarak kanıtlanan semptomatik<br />
hastalarda, vasküler ringe yönelik cerrahi girişim önerilmektedir<br />
(15-17).<br />
Sonuç olarak, özellikle supin pozisyonla ilişkili olarak<br />
öksürük ve nefes darlığı gibi nokturnal semptomları olan<br />
hastalarda astım ayırıcı tanısında vasküler ring gibi hava<br />
yolu basısına yol açan patolojilerin de düşünülmesi gerekir.<br />

RESPIRATORY CASE REPORTS<br />
Hipersensitivite pnömonisi (HP); duyarlılaşmış kişilerde<br />
çeşitli allerjenlerin tekrarlayan inhalasyonu sonucu<br />
meydana gelen IgE aracılı olmayan bir hipersensitivite<br />
reaksiyonudur. Tanıda en önemli nokta HP düşünmek<br />
ve buna yönelik maruziyeti hem çevresel hem de<br />
mesleksel olarak ayrıntılı sorgulamaktır. Elli dört yaşında<br />
kadın ve 2 yıldır nefes darlığı, halsizlik ve miyalji<br />
şikâyeti olan hastanın Fizik muayenesinde fibrotik<br />
ralleri mevcuttu. Posteroanterior akciğer grafisinde<br />
retiküler ve mikronodüler görünümleri vardı. Difüzyon<br />
testi düşüktü. Yüksek çözünürlüklü bilgisayar tomografisinde<br />
interlobuler septaları kalın, buzlu cam dansitesinde<br />
alanlar ve mozaik atenüasyon görüldü.<br />
Hikâyesinde güvercin maruziyeti bulunan hastaya HP<br />
tanısı kondu. Uygun tedavi ve etkenden uzaklaştırılması<br />
ile bulgularında regresyon izlendi. Biz olgumuzu<br />
nadir görülmesi nedeniyle sunduk.<br />

Hypersensitivity pneumonitis (HP) is a non-IgEmediated<br />
hypersensitivity reaction which is caused by<br />
repeated inhalation of allergens in the patient has<br />
been previously sensitized. The most important point<br />
in the diagnosis is to consider HP and to profoundly<br />
question the environmental and occupational exposure.<br />
A 54-year-old, female suffering from shortness<br />
of breath, fatigue, myalgia for two years admitted to<br />
our hospital. Fine crackles were auscultated. Reticular<br />
and micronodular opacities were observed at chest X-<br />
ray. Diffusion capacity of the patient was low. High<br />
resolution computed tomography revealed ground<br />
glass opacities, interlobular septal thickening, and<br />
mosaic attenuation areas in the lung field. As the<br />
patient had contact history with pigeon she was diagnosed<br />
with HP. With proper treatment and removal of<br />
influential factors, such findings of regression were<br />
observed. The current case is presented due to its<br />
rarity.<br />

Hipersensitivite pnömonisi (HP); organik tozların veya<br />
reaktif kimyasalların tekrarlayan inhalasyonları sonunda<br />
oluşan anormal immün reaksiyonla karakterize interstisyel<br />
akciğer hastalığı olarak tanımlanır. Reaksiyona neden<br />
olan antijenler; kuş, mantar, termofilik bakteriler ve bazı<br />
kimyasal bileşiklerdir ve bunlar içerisinde 200'den fazla<br />
antijen tanımlanmıştır. (1-3) HP; akut, subakut ve kronik<br />
olarak sınıflanmakla birlikte bu klinik sendromlar birbirinin<br />
içine geçmiş vaziyette karşımıza çıkabilir (1). Belli bir<br />
çevresel ajana maruziyet sonrası ortaya çıkan semptomlar,<br />
laboratuvar bulguları ve radyoloji tanı için yeterlidir (4).<br />
Ancak klinik semptomlar HP'ne spesifik olmadığından<br />
maruziyetin sorgulanması açısından ayrıntılı anamnez<br />
önemli bir yer tutmaktadır.<br />
izlendi (Şekil 3). Hastanın tedavisi 1 yıla tamamlanmak<br />
istendi ancak hasta tedavinin devamını kabul etmediği<br />
için tedavi kesildi. Kontrol değerleri incelenen hastanın<br />
difüzyon testinde DLCO: %102, VA: %99,<br />
DLCO/VA %103, SFT'de; FEV: 2,07 L (% 95), FVC: 2,38<br />
L (% 93), % FEV1/FVC: %87 idi. Oda havasında alınan<br />
AKG incelemesinde; pH: 7,44, PCO 2: 24 mmHg, PO 2:<br />
85 mmHg, HCO 3: 17 mEq/L, SaO 2: %97 olarak bulundu.<br />
Sonrasında hasta tedavisiz olarak 1 yıldır üçer ay<br />
aralıklarla takibimizde olup, hastanın tekrar şikâyetleri<br />
olmadı.<br />
OLGU<br />
Elli dört yaşında kadın hasta 2 yıldır devam eden efor<br />
dispnesi, sürekli devam eden öksürük, halsizlik, ateş şikayeti<br />
ile başvurdu. Özgeçmişinde Tip 2 diyabetis mellitus<br />
(DM) ve hipertansiyon vardı. Ayrıca 9 paket/ yıl sigara<br />
hikâyesi mevcuttu ve 5 aydır içmiyordu. Fizik muayenesinde<br />
her iki akciğer bazallerinde inspiryum sonunda<br />
işitilen ince ralleri mevcuttu. Çekilen PA akciğer grafisinde<br />
bilateral orta ve alt zonlarda retikülonodüler patern izlendi<br />
(Şekil 1). Solunum fonksiyon testlerinde (SFT); FEV:<br />
1,75 L (% 82), FVC: 1,75 L (% 69), FEV1/FVC: %100 idi.<br />
Oda havasında alınan arter kan gazı (AKG) incelemesi<br />
pH: 7,49, PCO 2: 33 mmHg, PO 2: 72 mmHg, HCO 3: 25<br />
mEq/L, SaO 2: %95 idi. Difüzyon testinde DLCO: %57<br />
VA: %68, DLCO/VA: %83 idi. Hastanın çekilen yüksek<br />
çözünürlüklü bilgisayarlı tomografi (YÇBT)’sinde orta ve<br />
alt loblarda belirgin buzlu cam dansitesinde alanlar, mozaik<br />
atenuasyon, interlobuler septalarda kalınlaşma ve<br />
sentriasiner nodüller saptandı (Şekil 2). Hastanın kuş<br />
besleme hikâyesi mevcut değildi ancak mevcut klinik,<br />
radyolojik bulgularla ayrıntılı sorgulanan hastanın kardeşinin<br />
ve oğlunun evlerinin teras katında güvercin beslediği<br />
öğrenildi. Hasta 4–5 yıldır teras katında bulunan güvercinlerin<br />
yanına haftanın 2–3 günü çıkıp 1–2 saat oturduğunu<br />
ancak terasın ve kafeslerin temizliği ile ilgili olmadığını<br />
belirtti. Hastaya bronkoskopi yapıldı, bronkoalveolar<br />
lavaj (BAL) sonucunda %80 lenfosit, %10 nötrofil, %10<br />
alveolar makrofaj saptandı. Mevcut bulgularla hastaya<br />
HP tanısı kondu, etken maruziyetinden uzaklaşması önerildi.<br />
Tip 2 DM olan hastaya endokrin bölümü kontrolünde<br />
40 mg/gün metilprednizolon tedavisi başlandı. Dokuz<br />
ay devam edildi. Tedavinin 9. ayında semptomları gerileyen<br />
hastanın çekilen YÇBT'sinde buzlu cam dansitesi<br />
alanlarında, sentriasiner nodüllerde belirgin regresyon<br />

TARTIŞMA<br />
HP patogenezi; tekrarlayan antijen maruziyeti ve konakçının<br />
antijene karşı duyarlılaşması sonucu pulmoner ve<br />
sistemik belirtilere yol açan immün inflamatuvar yanıttan<br />
oluşmaktadır. Aerodinamik yapısı 5 µm’den daha küçük<br />
partiküller akciğer periferine ulaşarak HP oluşmasına yol<br />
açabilirler. Çeşitli antijenlerle tekrarlayan karşılaşmalar<br />

sonucunda TİP III ve TİP IV alerjik reaksiyonların aracılığıyla<br />
bu tablo oluşmaktadır. Ekstrensek alerjik alveolit<br />
olarak da adlandırılır. Kuş besleyici hastalığı, HP’nin bir<br />
alt grubudur. Kuşlara ait organik partiküllerin tekrarlayan<br />
inhalasyonu ile akciğer parankim harabiyeti oluşur. En sık<br />
rastlanılan tiplerinden birisi güvercin besleyici hastalığıdır.<br />
İlk kez 1960 yılında tanımlanmıştır ve güvercin “avian”<br />
protein antijenlerin inhalasyonuna bağlı olarak oluşur<br />
(1,3,5). Bizim olgumuzda da güvercin ile çevresel maruziyet<br />
mevcuttu. Teknik yetersizlikten dolayı presipitan antikorlar<br />
bakılamadı.<br />

Klinik prezentasyon; inhale antijenin büyüklüğü yoğunluğu,<br />
maruziyet sıklığı ve kişinin immün yanıtına göre akut,<br />
subakut ve kronik olarak değişmektedir. Akut form; yüksek<br />
miktardaki antijene maruziyetten 4–8 saat sonra ortaya<br />
çıkar ve akut viral enfeksiyonu taklit eder. Subakut<br />
form; duyarlı kişilerin antijeni sürekli olarak ve düşük<br />
konsantrasyonda inhale etmeleri sonucu gelişir. Semptomlar<br />
daha sinsi ilerler. Akut semptomlar olmaksızın<br />
haftalar aylar içinde artan progresif bir dispne söz konusudur.<br />
Fizik incelemede bibaziler raller mevcuttur. Kişinin<br />
antijenik temas ortamından uzaklaşması semptomların<br />
iyileşmesiyle sonuçlanır (1,3). Bizim olgumuzda düşük<br />
dozda maruziyet mevcuttu, ortamdan uzaklaştırılması ve<br />
tedavi ile semptomlarda tam iyileşme sağlandı. Olgumuz;<br />
düşük doz maruziyet ve sinsi seyreden nefes darlığı ve<br />
YÇBT de buzlu cam dansitesinde alanların mevcut olması<br />
nedeni ile subakut form olarak kabul edildi. Kronik form;<br />
düşük dozda kronik antijen maruziyeti sonucu oluşur ve<br />
akut ataklar olmaksızın geri dönüşümsüz akciğer hasarıyla<br />
seyreder. Progresif dispne, öksürük, huzursuzluk ve kilo<br />
kaybı başlıca semptomlardır. Bu formda interstisyel fibrozis<br />
başlıca bulgudur. Bibaziler raller duyulur, çomak parmak<br />
olguların %20-50’sinde görülür. Kronik olgularda<br />
pulmoner hipertansiyon ve kor pulmonale gelişebilir (3).<br />
Tanıda major ve minor kriterler belirlenmiştir. HP'de tanısal<br />
yaklaşımda ayrıntılı anamnez (meslek, çevre ve hobiler),<br />
fizik muayene bulguları, solunum fonksiyon testleri,<br />
BAL ve bronşiyal biyopsi için fiberoptik bronkoskopi yapılması,<br />
mikrobiyolojik inceleme için özel boya ve kültürler<br />
ana basamaklardır. Son yayınlanan literatürlerde bazı<br />
majör ve minör kriterler tanımlanmış ve HP tanısı için<br />
major kritelerin tamamı ve en az 4 minör kriter gereklidir.<br />
Majör kriterler; 1-HP ile uyumlu semptomlar, 2-Antijenle<br />
temas öyküsü veya serumda presipitan antikor ve/veya<br />
BAL'da antikor varlığı, 3-HP ile uyumlu radyolojik bulguların<br />
olması. Minör kriterler ise; 1-Bibaziler raller, 2-<br />
Egzersiz veya istirahatte hipoksemi, 3-DLCO'da azalma,<br />
4-BAL da lenfositoz, 5-HP ile uyumlu histolojik değişiklikler,<br />
6-Pozitif doğal provakasyon son olarak bildirilmiştir<br />
(5,6). Bizim olgumuzda major kritelerin tamamı; minör<br />
kriterlerden ise 4 tanesi mevcuttu. (bibaziller raller, hipoksemi,<br />
DLCO'da azalma, bronş lavajında lenfositoz ). HP<br />
de BAL' da lenfositik alveolit görülür. Lenfositlerin çoğu T<br />
lenfosittir ve artan subgrup CD8+ T lenfositlerdir ve buna<br />
bağlı olarak CD4+/CD8+ oranı düşer. Akut HP'li olguların<br />
BAL'ında nötrofil, plazma hücresi ve mast hücre<br />
sayısında artış saptanmıştır. Tekrarlayan maruziyetlerde<br />
bronşiyollerde lenfosit infiltrasyonu ile subakut evre gelişir<br />
(1,3,7,8). Bizim olgumuzda da subakut evre ile uyumlu<br />
olarak belirgin olarak artmış lenfosit hâkimiyeti saptandı.<br />
HP'nin YÇBT bulguları hastalığın evresine göre değişmektedir.<br />
Akut HP’de başlıca bulgu buzlu cam görünümüdür.<br />
Subakut HP’de sentrilobüler nodüller ve buzlu cam görünümü<br />
ile birlikte hava hapsi ve mozaik perfüzyon en sık<br />
rastlanan bulgulardır. Sentrilobüler nodüller aktif alveolit<br />
ve granülomların; mozaik perfüzyon kan akımı redistribüsyonunun;<br />
hava hapsi ise obstrüktif bronşiyolitin radyolojik<br />
yansımasıdır. Kronik HP’de fibrozis bulguları olan<br />
düzensiz lineer opasiteler, bal peteği akciğer ve traksiyon<br />
bronşektazisi YÇBT bulgularıdır. Bulgular üst ve orta zonlarda<br />
lokalizedir. Subakut evrenin tipik bulguları olan<br />
sentrilobüler nodüller ve buzlu cam kronik evrede de<br />
görülebilir. Bu devam eden antijen temasının göstergesidir.<br />
Amfizem sigara içmeyen kronik HP’lerin %20’sinde<br />
görülen bir bulgudur (1-5,7-9). Bizim olgumuzda da<br />
buzlu cam görünümleri, mozaik atenuasyon ve sentriasiner<br />
nodüler görünümler mevcuttu ve bu bulgular subakut<br />
formu desteklemekteydi.<br />
HP tedavisinde anahtar rol antijenden sakınmaktır. Antijenden<br />
sakınma hastalığın regresyonunu sağlarken, bazı<br />
şiddetli hastalarda kortikosteroid tedavisi gerekebilir ve<br />
klinik bulguların düzelmesini hızlandırır. Hastalığın akut<br />
formunda kortikosteroidlerin belirgin faydası olmasına<br />

karşın, tedavinin etkisini, hastalığın doğal seyrinden ve<br />
antijenden kaçınmanın etkisinden ayırt etmek zordur.<br />
Kortikosteroid tedavisi genellikle fonksiyonel kaybı olan<br />
subakut ve kronik hastalarda kullanılmaktadır. Tedavi<br />
fizyolojik bozuklukta olabilecek en iyi düzelmenin olduğu<br />
evreye kadar devam edilir. Tedaviye çoğunlukla sarkoidoz<br />
ve diğer intertisyel akciğer hastalıklarındakine benzer<br />
şekilde 40–50 mg kortikosteroid ile başlanır. Bir ay bu<br />
dozda kullanıldıktan sonra 2–3 aylık süre içinde 7,5–15<br />
mg’lık idame dozuna düşülür (1,8,10). Biz de olgumuza<br />
suabakut HP tanısı ile 40 mg/gün metilprednizolon tedavisi<br />
başladık ve tedavimize doz azaltarak 9 ay devam ettik.<br />
Hastamızda klinik ve radyolojik gerileme izledik.<br />
Sonuç olarak, biz bu olguda olduğu gibi sinsi nefes darlığı,<br />
öksürük, ateş, halsizlik gibi semptomları olan hastalardan<br />
dikkatli ve detaylı anamnez alınmasını, meslek ve<br />
çevresel maruziyetini ve çevresindekilerinde hobilerinin<br />
sorgulanmasını, ayırıcı tanıda mutlaka HP’lerinin akla<br />
getirilmesi gerektiğini vurgulamak istedik. Erken tanı konan<br />
olgularda maruziyetin sonlandırılması ve uygun tedavi<br />
ile tamamen düzelme sağlanmakta ve kronik forma<br />
gidiş engellenebilmektedir.<br />

RESPIRATORY CASE REPORTS<br />
Mounier-Kuhn syndrome is a rare syndrome characterized<br />
by tracheobronchomegaly resulting from the<br />
loss or atrophy of musculo-elastic fibers within the<br />
trachea and main bronchi wall. This syndrome is<br />
more common in the third or fourth decades of life.<br />
The diagnosis can usually be made by measuring the<br />
diameters of trachea and main bronchi radiologically.<br />
A 52-year-old male patient was referred to our outpatient<br />
clinic with chronic productive cough and<br />
recurrent respiratory tract infections. We detected<br />
tracheobronchomegaly, tracheal diverticula, and<br />
bronchiectasis in the chest CT scans. This rare case is<br />
presented due to later onset than expected and very<br />
demonstrative radiological findings.<br />

Mounier-Kuhn sendromu (MKS) trakea ve ana bronş<br />
duvarındaki muskuloelastik fibrillerin kaybolması veya<br />
atrofisi sonucu oluşan ve trakeobronkomegali ile<br />
karakterize nadir bir sendromdur. Bu sendromun<br />
tanısı daha çok hayatın 3. veya 4. dekatında konulmaktadır.<br />
Tanı genellikle trakea ve ana bronşların<br />
çaplarının radyolojik olarak ölçülmesi ile konur. Elli<br />
iki yaşında bir erkek hasta kronik prodüktif öksürük ve<br />
tekrarlayan solunum yolu infeksiyonları nedenleri ile<br />
polikliniğimize sevk edilmiş. Bilgisayarlı tomografi<br />
incelemesinde trakeobronkomegali, trakeal ve bronşiyal<br />
divertiküller ve bronşektazik alanlar saptadık. Bu<br />
nadir olguyu beklenenden daha geç başlangıçlı olması<br />
ve çok eğitici radyolojik bulgular nedeniyle<br />
sunduk.<br />

Anatomic abnormalities of the airways and lungs should<br />
also be considered in patients with recurrent respiratory<br />
tract infections. Mounier-Kuhn syndrome is a rare syndrome<br />
characterized by abnormal enlargement of the<br />
trachea and main bronchi. This syndrome is more common<br />
in the third or fourth decades of life. A radiological<br />
diagnosis is made by measuring the diameter of the trachea<br />
and main bronchi. The syndrome may be accompanied<br />
by bronchiectasis, diverticulum, and recurrent<br />
infections of the lower respiratory tract (1).<br />
CASE<br />
A 52-year-old male patient was admitted to our outpatient<br />
clinic with complaints of chronic cough and sputum.<br />
The patient had these symptoms for 16 years, particularly<br />
during the winter months, and he used antibiotics due to<br />
the increase in the amount of sputum. He had a smoking<br />
history of 20 packs/year and has been an ex-smoker for<br />
ten years. He was working in the technical service department<br />
of a lift company. The patient had hyperlipidemia<br />
and adenoidectomy operation 10 years prior. His family<br />
history was not significant.<br />
The examination of the respiratory system revealed inspiratory<br />
crackles at the left lower field. Pulmonary function<br />
tests (PFT) indicated mild bronchial obstruction. There<br />
was no significant finding in the flow-volume loop. Enlargement<br />
of the trachea and non-homogeneously increased<br />
density in the left lower zone were detected in the<br />
chest x-ray (Figure 1). The chest CT scan showed tracheobronchomegaly<br />
with an increase in the transverse diameter<br />
of the trachea, right and left main bronchus measured<br />
as 40 mm, 21.5 mm, and 19 mm, respectively.<br />
Minimal cystic bronchiectasis was detected in the right<br />
middle lobe; widely cystic bronchiectasis was seen in the<br />
lingula and the left lower lobe (Figure 2). There was also<br />
evidence of multiple diverticula involving trachea (sawtooth<br />
appearance and ondulation signs) (Figure 3).<br />
Laboratory tests revealed increased erythrocyte sedimentation<br />
rate (55 mm/h) and C-reactive protein level (8<br />
mg/dl; normal=0-5mg/dl). He was evaluated by rheumatologist<br />
and no accompanying connective tissue disease<br />
was detected. The patient was diagnosed as primary<br />
MKS. We offered him to have respiratory physiotherapy<br />
and prescribed appropriate antibiotic for infected bronchiectasis.<br />

DISCUSSION<br />
Mounier-Kuhn syndrome is a rare disease characterized<br />
by abnormal enlargement of the trachea and main bronchi.<br />
Clinical, radiological, and endoscopic findings of this<br />
syndrome were first identified by Mounier-Kuhn in 1932<br />
(2). It is slightly more common in males and in the third<br />
or fourth decades of life. However, as in our patient,<br />
there have been reported cases detected over the age of<br />
50 and in childhood (3,4). An association has been reported<br />
between tracheobronchomegaly and Marfan syndrome,<br />
Ehler-Danlos, Kenny Caffey syndrome, connective<br />
tissue diseases, Bruton-type agammaglobulinemia, ankylosing<br />
spondylitis, ataxia-telangiectasia, Cutis Laxa, and<br />
light chain deposition disease (5). However, as in this<br />
case, most cases are sporadic and the etiology is unknown.<br />
The loss and atrophy of elastic and smooth muscle<br />
tissue of the wall of trachea and main bronchi was<br />
observed in the autopsy series (6).<br />
Mucociliary clearance and cough mechanisms are not<br />
effective in tracheobronchomegaly. Therefore, patients<br />
usually visit physicians for recurrent respiratory tract infections,<br />
as in the current case. If there is no infection, the<br />
disease may be asymptomatic. Occasionally, the clinical<br />
picture may be accompanied by dyspnea and hemoptysis<br />
with the development of bronchiectasis. More rarely,<br />
spontaneous pneumothorax, massive hemoptysis and<br />
clubbing can be observed (3,7,8).<br />
Mounier-Kuhn syndrome is diagnosed with radiological<br />
imaging. In adults, measuring diameters for the trachea,<br />
the right main bronchus and the left main bronchus, over<br />
30 mm, 24 mm, and 23 mm, respectively, are diagnostic<br />
in the chest radiograph. Trachea and main bronchi diameters<br />
for the diagnosis of Mounier-Kuhn syndrome in<br />
the CT scans are given in Table 1 (5). Tracheobronchomegaly<br />
is often accompanied by bronchiectasis and<br />
tracheal diverticula. Bronchiectasis is usually bilateral and<br />
in saccular formation (9). In the current case, bronchiectasis<br />
was cystic in formation and bilateral, but more<br />
common in the left. Tracheal diverticula radiologically<br />
elicit a sawtooth appearance and ondulation sign. The<br />
CT scan of the current patient revealed gross enlargement<br />
of the trachea and evidence of diverticula was<br />
clearly observed on the trachea and main bronchi.<br />
Treatment of the disease involves respiratory physiotherapy<br />
to aid in the removal of secretions, and the administration<br />
of appropriate antibiotics during the attacks (3,10).<br />
Previous studies reported that tracheobronchoplasty and<br />
stent application improves respiratory function and quality<br />
of life in severe cases (11,12).<br />

As a result, assessment of the anatomy of the lungs and<br />
airways has great importance to determine the etiology of<br />
chronic productive cough and recurrent respiratory tract<br />
infections. Mounier-Kuhn syndrome should be kept in<br />
mind in the differential diagnosis of recurrent tract infections.<br />
