що можуть також викликати ГІН. Нещодавно визначений<br />
гормонозалежний рецидивуючий ГІН становить<br />
важливу терапевтичну проблему. Хоча кортикостероїди<br />
широко використовуються при медикаментозному ГІН<br />
із метою прискорення відновлення функції нирок і уникнення<br />
хронічного захворювання нирок, їх ефективність<br />
не була перевірена в рандомізованих контрольованих дослідженнях.<br />
Необхідні нові діагностичні тести і біомаркери,<br />
перспективні терапевтичні дослідження для покращення<br />
діагностики і ведення ГІН.<br />

Відмінною рисою IgA-нефропатії (IgAN), добре<br />
відомою кожному студенту-медику, є наявність макроскопічної<br />
гематурії, що з’являється під час або<br />
відразу ж після гострого тонзиліту [1]. Цей симптом є<br />
настільки характерним, що пацієнтам із повторенням<br />
подібних епізодів встановлюють імовірний діагноз<br />
IgAN навіть без біопсії нирок, що виявляє IgA в мезангіумі.<br />
З цієї простої клінічної точки зору протягом<br />
довгого часу видалення мигдалин вважалось доцільним<br />
для лікування пацієнтів з IgAN; зниження частоти<br />
епізодів гематурії зазвичай спостерігається при<br />
рецидивуючому бактеріальному тонзиліті [2]. З іншого<br />
боку, через десятиліття після ідентифікації IgAN<br />
і його взаємозв’язку з інфекцією верхніх дихальних<br />
шляхів переваги від тонзилектомії у хворих з IgAN<br />
були значною мірою менш вражаючими, ніж очікувалося<br />
[3, 4].<br />
Деякі дослідження показують, що аномалії слизової<br />
лімфоїдної тканини є критичними для розвитку IgAN,<br />
а інфекції відіграють просту роль ініціюючого фактора<br />
[1]. Експериментальна IgAN може бути отримана у<br />
тварин тільки після відміни природного процесу толерантності<br />
слизової, що сприяє імунному захисту від патогенів<br />
[5]. IgAN швидше за все розвивається через збій<br />
усунення зі слизової оболонки антигена і зміненого<br />
синтезу IgA, що призводить до синтезу нефритогенного<br />
IgA1, який, як думають, ініціює події, що призводять<br />
до IgAN [6]. Крім того, при розгляді тонзилектомії як<br />
методу зменшення кількості слизової лімфоїдної тканини<br />
слід враховувати, що мигдалики являють собою<br />
лише 0,5 м 2 від 400 м 2 загальної поверхні слизової оболонки<br />
в людини.<br />
Разом із тим інші дані, як і раніше, акцентують<br />
увагу на можливій ролі мигдаликів при IgAN, наприклад<br />
у синтезі димерів і дегалактозильованого IgA1,<br />
яких більше у мигдаликах хворих на IgAN, ніж у здорових.<br />
Крім того, зменшення циркулюючого дегалактозильованого<br />
IgA1 спостерігається в пацієнтів<br />
із IgAN [7, 8]. У 2003 році проривний звіт із Китаю<br />

can also induce AIN. Cortico-dependant, relapsing<br />
AIN is a recently recognized entity that poses an<br />
important therapeutic challenge. Although corticosteroids<br />
are widely used in drug-induced AIN to<br />
speed kidney function recovery and avoid chronic<br />
kidney disease, their efficacy has not been tested by<br />
randomized controlled trials. New diagnostic tests<br />
and biomarkers, as well as prospective therapeutic<br />
studies are needed to improve AIN diagnosis and<br />

The hallmark of IgA nephropathy (IgAN), wellknown<br />
to every medical student, is macroscopic<br />
haematuria, coincidently or immediately following<br />
an acute tonsillitis [1]. This presentation is so typical<br />
that patients with recurrence of such episodes<br />
receive a likely diagnosis of IgAN even without a<br />
renal biopsy detecting IgA in the mesangial area.<br />
From this simple clinical standpoint, for a long time<br />
tonsillectomy was considered a suitable treatment<br />
for patients with IgAN, and a reduction in the frequency<br />
of episodes of haematuria is commonly observed<br />
when recurrent bacterial tonsillitis is present<br />
[2]. On the other hand, decades after the identification<br />
of IgAN and its relationship with upper respiratory<br />
tract infection, benefits from tonsillectomy in<br />
patients with IgAN have been largely less impressive<br />
than expected [3, 4].<br />
Several studies suggest that abnormalities of the<br />
mucosal-associated lymphoid tissue (MALT) are<br />
critical for the development of IgAN, with infections<br />
representing a simple role as a triggering event [1].<br />
Experimental IgAN can be produced in animals only<br />
after abrogation of the natural process of mucosal tolerance<br />
which favours the host defence against pathogens<br />
[5]. IgAN is likely to develop because of a failure<br />
of mucosal antigen elimination and altered IgA synthesis,<br />
leading to production of nephritogenic degalatosylated<br />
IgA1, which is thought to be the initiating<br />
event leading to IgAN [6]. Moreover, when considering<br />
tonsillectomy for reducing MALT surface, we<br />
have to consider that tonsils represent only 0.5 m 2 of<br />
the entire 400 m 2 of total mucosal surface in humans.<br />
However, other series of data continued to focus<br />
attention on a possible role of tonsils in IgAN, e.g.<br />
in dimeric and degalactosylated IgA1 production,<br />
which is greater in the tonsils of patients with IgAN<br />
than in healthy controls. Moreover, a reduction of<br />
circulating degalactosylated IgA1 has been observed<br />
in patients with IgAN [7, 8]. In 2003, a breakthrough<br />
