family”, “self-employed in agriculture”<br />

− limiting the right to obtain social assistan-<br />
ce by refusing to issue the necessary certificates;<br />
in 2008 – allocated MDL 50 .0 thousand, in<br />
2009 – MDL 116 .1 mln; October 2008 – October<br />
2009 → recor<strong>de</strong>d 51,170 applications, of<br />
which 33 260 → approved for payment .<br />
b) Much was achieved through <strong>de</strong>centralization<br />
of primary health care system – it is still necessary<br />
to emphasize efficiency and effectiveness .<br />
Shortage of healthcare professionals: in urban<br />
areas – surplus of doctors, shortage of<br />
nurses; in rural areas – the situation is reversed<br />
. In villages – the number of active family<br />
physicians <strong>de</strong>creases, increasing the number<br />
of retired doctors; young professionals – basically,<br />
do not return to work here;<br />
accessibility to health services remains an obstacle<br />
– people have to travel long distances<br />
to receive PHC services, even if they can be<br />
obtained in local administrative units in the<br />
surrounding areas;<br />
the choice of a doctor remains an administrative<br />
and territorial principle;<br />
some PHC institutions → distribute compensated<br />
drugs, partially compensated, cases<br />
treated + sell pharmaceutical products;<br />
PHC institutions premises do not meet operation<br />
standards, are fully worn out or in a <strong>de</strong>plorable<br />
state, not autonomous, <strong>de</strong>pen<strong>de</strong>nt<br />
on rayon FMC;<br />

The current system of regulation and management<br />
of solid waste collection and disposal<br />
is not effective and does not minimize environmental<br />

The imperfect regulatory framework leads to<br />
inefficient solid waste management, to the<br />
formation of <strong>de</strong>posits inconsistent to standards<br />
→ consequence is negative environmental<br />
impact – 1415 .0 ha dumps area;<br />
Volume of waste (45 .7 mln m 3 ), modification<br />
of morphological structure of waste and its<br />
storage on land surface → danger of infiltration<br />
of toxins in the soil during the <strong>de</strong>composition<br />
process and emission of greenhouse<br />
gases in the atmosphere;<br />
Lack of automated services for waste collection,<br />
transportation, storage and processing<br />
leads to the formation of unauthorized waste<br />
dumps (annually > 3000);<br />
Medical waste incineration → lack of proper<br />
crematories creates the risk of contamination<br />
of soil and water .<br />
Joint efforts are nee<strong>de</strong>d – CPA + LPA – for preventing<br />
the ecological and epi<strong>de</strong>miological disasters<br />
