REZUMAT<br />
Scopul acestei cercetări a fost identificarea unor caracteristici specifice vârstei, utile în procedura de diagnostic în cazul copiilor cu<br />
cefalee tip migrenă. Această cercetare a fost realizată în perioada 1988 – 2008 în Prov<strong>in</strong>cia de Nord a Serbiei. Studiul s-a efectuat<br />
asupra a 30363 de copii cu vârste cupr<strong>in</strong>se între 3 și 17 ani. În 2644 de cazuri s-a evidențiat cefaleea tip migrenă.<br />
În primul rând, la copii, cefaleea tip migrenă debutează la vârsta de 5 ani și 1,8 luni. Durerea cu caracter pulsatil, ca cel mai<br />
prezent simptom la copii (76,3%) a fost dom<strong>in</strong>antă la vârsta de 12 ani. Semnificația ridicată ca durere unilaterală a fost clară după<br />
vârsta de 12 ani. Cefaleea tip migrenă la copii durează câteva m<strong>in</strong>ute la l0,89% d<strong>in</strong>tre copii, până la 1 oră la 4.43% d<strong>in</strong>tre copii<br />
și până la 2 ore la 27.76% copii. Greața, senzația de vomă și vărsăturile sunt elemente de bază ale atacurilor de migrenă la copii,<br />
mult mai frecvente la copiii mai mici.<br />
În migrene, modul de manifestare al durerii și localizarea, durata atacurilor și simptomele care le însoțesc par a fi dependente de vârstă.<br />

abstract<br />
The aim of this <strong>in</strong>vestigation was to identify age specific characteristics of children migra<strong>in</strong>e headaches useful <strong>in</strong> diagnostic procedure.<br />
This research was carried out from 1988 to 2008 <strong>in</strong> the North Prov<strong>in</strong>ce of Serbia. Study was conducted on 30363 children<br />
aged 3-17 years. Migra<strong>in</strong>e headache was reported from 2644 <strong>in</strong>terviewed.<br />
First migra<strong>in</strong>e headache onset at the age of 5 years 1.8 months. Pulsat<strong>in</strong>g pa<strong>in</strong> as the most present <strong>in</strong> children migra<strong>in</strong>e headaches<br />
(76.3%) was dom<strong>in</strong>ant at age of 12. High significance of unilateral pa<strong>in</strong> was clear after 12 th birthday. Migra<strong>in</strong>e headaches<br />
<strong>in</strong> children last a few m<strong>in</strong>utes <strong>in</strong> 0.89% children, up to 1 hour <strong>in</strong> 4.43% children and up to 2 hours <strong>in</strong> 27.76% children. Nausea,<br />
vomit<strong>in</strong>g impulse and vomit<strong>in</strong>g are basic elements of migra<strong>in</strong>e attacks <strong>in</strong> children, much more frequent <strong>in</strong> younger children.<br />
Migra<strong>in</strong>e headache pa<strong>in</strong> quality and localization, attacks duration and accompany<strong>in</strong>g symptoms seem to be age depend.<br />

INTRODUCERE:<br />
Cefaleea tip migrenă este subestimată și diagnosticată<br />
greșit la copii, d<strong>in</strong> cauza lipsei schimbărilor<br />
anatomice, markerilor biologici specifici și <strong>in</strong>strumentelor<br />
de cercetare specifice sau a tehnicilor de<br />
imagistică cerebrală (1,2,3,4).<br />
Diagnosticarea migrenei la copii dep<strong>in</strong>de de excluderea<br />
sistematică a tulburărilor secundare și de<br />
identificarea sistematică a caracteristicilor specifice<br />
migrenei dependente de vârstă. Simptomele migrenei<br />
se manifestă la nivel senzorial, vegetativ și afectiv,<br />
s-au descoperit doar la om și au variații calitative<br />
și cantitative cu un aspect marcant al dependenței<br />
de vârstă (5,6,7).<br />
Nu există suficiente cunoșt<strong>in</strong>țe priv<strong>in</strong>d semnele<br />
și simptomele la copii cu migrene pentru a dist<strong>in</strong>ge<br />
clar toate entitățile cl<strong>in</strong>ice ale s<strong>in</strong>dromului de migrenă<br />
(8,9,10).<br />
Cefaleea tip migrenă la copii este greu de def<strong>in</strong>it,<br />
în special datorită faptului că ei nu pot să își def<strong>in</strong>ească<br />
în mod clar dificultățile dar și fi<strong>in</strong>dcă ele au<br />
trăsături care dep<strong>in</strong>d de vârstă (11). În op<strong>in</strong>ia unui<br />
mare număr de autori d<strong>in</strong> zilele noastre, cefaleea tip<br />
migrenă la copii, ca s<strong>in</strong>drom foarte complex, este<br />
încă o problemă de deducție. Se sugerează diferite<br />
criterii de diagnosticare a cefaleei tip migrenă la<br />
copii (12).<br />
Cefaleea tip migrenă la copii este o tulburare neurologică<br />
cronică în cea mai mare parte caracterizată<br />
pr<strong>in</strong> crize de cefalee episodice de o <strong>in</strong>tensitate,<br />
frecvență și durată diferite, în mod obișnuit unilaterală<br />
și asociată cu anorexia, greață, vărsături, fonofobie și<br />
fotofobie. Foarte des este precedată sau acompaniată<br />
de modificări senzoriale, motorii, vegetative, afective<br />
și diverse comb<strong>in</strong>ații ale schimbărilor neurologice,<br />
gastro<strong>in</strong>test<strong>in</strong>ale și vegetative. La copii, gradul exprimării<br />
și momentul apariției variază în funcție de<br />
vârstă, calitativ și cantitativ. Conform cu datele publicate<br />
în literatura de specialitate, în general, modul de<br />
manifestare al durerii și localizarea ei, durata, markerii<br />
comportamentali care însoțesc simptomele și rata<br />
apariției remisiei par să dep<strong>in</strong>dă de vârstă (13,14).<br />
MATERIALE ȘI METODE:<br />
Această cercetare s-a desfășurat pe teritoriul<br />
Voivod<strong>in</strong>ei, Prov<strong>in</strong>cia de Nord a Serbiei care are o<br />
populație totală de 2.031.992, după ultimul recensământ<br />
(în 2002). În timpul studiului, care a durat<br />
d<strong>in</strong> 1988 până în 2008, s-au adm<strong>in</strong>istrat în fiecare an<br />
câte 2000 de chestionare participanților care au fost<br />
selectați d<strong>in</strong> 23 de grăd<strong>in</strong>ițe și 42 de școli d<strong>in</strong> 9 orașe<br />
d<strong>in</strong> Voivod<strong>in</strong>a (Novi Sad, Subotica, Kik<strong>in</strong>da, Zrenjan<strong>in</strong>,<br />
Vrsac, Bela Crkva, Melenci, Futog și Temer<strong>in</strong>).<br />
În total au fost cupr<strong>in</strong>și 30363 de copii cu vârste între<br />
3 și 17 ani. Cefaleea recurentă s-a înregistrat la<br />
27.46% d<strong>in</strong> copiii <strong>in</strong>tervievați (8456), iar în 8.63%<br />
(2644) d<strong>in</strong> cazuri cefaleea a fost def<strong>in</strong>ită ca migrenă.<br />
Copiii au fost selectați în funcție de luna și anul<br />
nașterii, precum și după primele trei litere ale primului<br />
nume, pr<strong>in</strong>tr-un procedeu de eșantionare în ciorch<strong>in</strong>e,<br />
stratificat, în mai multe etape. În acest fel, s-a<br />
exclus posibilitatea ca un copil să <strong>in</strong>tre în studiu de<br />
două ori pe parcursul cercetării care a durat o perioadă<br />
îndelungată. Li s-a cerut subiecților și/sau păr<strong>in</strong>ților<br />
lor să completeze un chestionar la locul lor de<br />
reșed<strong>in</strong>ță. Chestionarele au fost distribuite copiilor și/<br />
sau păr<strong>in</strong>ților lor selectați pr<strong>in</strong> eșantionare aleatoare.<br />
Chestionarul semistructurat, care a fost alcătuit special<br />
pentru acest studiu de către autor, a fost proiectat<br />
în conformitate cu criteriile Societății Internaționale<br />
pentru Studiul Cefaleei. A fost un chestionar de screen<strong>in</strong>g<br />
(de depistare) care a fost completat de copii și/<br />
sau păr<strong>in</strong>ții lor împreună. El a <strong>in</strong>clus 3 secțiuni: (1)<br />
itemi despre caracteristicile socio-demografice ale copilului<br />
și ale familiei sale precum și ale școlii; (2) itemi<br />
referitori la dezvoltarea copilului și (3) itemi despre<br />
durerile de cap <strong>in</strong>clusiv toate caracteristicile, semnele<br />
și simptomele.<br />
Chestionarul a fost dezvoltat în trei etape. În prima,<br />
au fost organizate <strong>in</strong>terviuri semi-structurate cu<br />
medici pediatri și cercetători selecția domeniilor relevante.<br />
Domeniile pentru secțiunile referitoare la<br />
cefalee au fost selectate pe baza criteriilor Clasificării<br />
Internaționale a Tulburărilor legate de Cefalee – partea<br />
a II-a. Au fost identificați peste 150 de itemi posibili.<br />
În primul formular au fost <strong>in</strong>cluși itemi corespunzători,<br />
comprehensivi și preciși. Răspunsurile posibile<br />
au fost opțiuni deschise și judecăți categorice.<br />
În a doua fază, chestionarul a fost pre-testat în<br />
<strong>in</strong>terviuri semi-structurate aplicate unui grup mic de<br />
copii care fie sufereau, fie nu sufereau de cefalee (au<br />
fost <strong>in</strong>cluse 16 familii.) În această fază am urmărit<br />
să evaluăm validitatea de construct și de conț<strong>in</strong>ut a<br />
chestionarului. În plus, sensibilitatea a fost evaluată<br />
pr<strong>in</strong> corelarea datelor d<strong>in</strong> chestionar cu cele d<strong>in</strong> fișele<br />
medicale ale copiilor care aveau dureri de cap. În urma<br />
acestei faze a rezultat o versiune revizuită care a fost<br />
evaluată doar pe copiii sănătoși. C<strong>in</strong>cizeci de copii<br />

și adolescenți au completat chestionarul de două ori<br />
în trei săptămâni. Au fost analizate aspecte ca: rata<br />
non-răspunsului, distribuțiile răspunsurilor, prezentarea<br />
răspunsului grafic (<strong>in</strong>consecvența răspunsului) și<br />
greutățile legate de chestionar (timpul necesar completării,<br />
<strong>format</strong>ul, etc.). S-au modificat sau au fost<br />
elim<strong>in</strong>ați un număr de itemi astfel încât forma f<strong>in</strong>ală<br />
a cupr<strong>in</strong>s 93 de itemi pentru completarea cărora erau<br />
necesare 20 de m<strong>in</strong>ute.<br />
Doar de data aceasta au fost analizate datele despre<br />
alimentația la sân.<br />
Criteriile de <strong>in</strong>cludere au fost: vârsta 3-17 ani,<br />
consimțământul <strong>in</strong><strong>format</strong>, sub semnătură d<strong>in</strong> partea<br />
păr<strong>in</strong>ților. Criteriul de excludere a fost un diagnostic<br />
precedent al unei boli care are cefaleea drept simptom.<br />
Precizia chestionarului folosit în această cercetare<br />
s-a bazat pe clasificarea și criteriile de diagnostic<br />
pentru tulburările de tip cefalee, nevralgii craniene și<br />
durere facială și pe criteriile revizuite ale Societății<br />
Internaționale pentru Studiul Cefaleei (15).<br />
Vârsta medie a subiecților studiului a fost de 9 ani<br />
și 2,5 luni (rangul 3-17 ani, deviația standard (DS)<br />
3,44). Numărul mare al copiilor chestionați a permis<br />
concluzii def<strong>in</strong>ite.<br />
Studiul s-a efectuat în 2 faze: completarea chestionarului<br />
și, pentru cei cu cefalee recurentă (MR), un<br />
<strong>in</strong>terviu față-în-față. Pe baza datelor colectate pr<strong>in</strong><br />
chestionar, copiilor care aveau MR li s-a adm<strong>in</strong>istrat<br />
un <strong>in</strong>terviu ext<strong>in</strong>s și au făcut o exam<strong>in</strong>are neurologică<br />
Precizia chestionarului utilizat în acest studiu s-a<br />
bazat pe criteriile Societății Internaționale pentru<br />
Studiul Cefaleei. Folos<strong>in</strong>d codurile de clasificare ale<br />
societății, migrena a fost acceptată ca 1.1-1.7, migrena<br />
cu aură a fost 1.2.2 – 1.2.6, migrena fără aură a fost<br />
1.1 și alte s<strong>in</strong>droame de migrenă au fost 1.3 – 1.7.<br />
Cefaleea recurentă a fost acceptată ca toate celelalte<br />
tipuri de cefalee care au apărut de 3 ori pe lună, fără<br />
a le separa în funcție de caracteristici specifice. Toate<br />
tipurile de cefalee recurentă (idiopatică sau criptogenică)<br />
care nu erau migrene au fost considerate cefalee<br />
non-migrenă (16,17,18).<br />
D<strong>in</strong>tre chestionare, 1,5% au fost părt<strong>in</strong>itoare (distorsionate),<br />
datorită unui posibil <strong>in</strong>terviu dublu, și<br />
datorită lipsei de date d<strong>in</strong> chestionare. S-au analizat<br />
separat date în funcție de durerile de cap recurente<br />
pentru copiii care au răspuns complet la chestionar.<br />
Comparând caracteristicile cefaleei <strong>in</strong>dividuale la copiii<br />
cu cefalee tip migrenă și la cei cu non-migrenă<br />
s-au stabilit trăsături ale cefaleei tip migrenă la copii,<br />
care dep<strong>in</strong>d de vârsta acestora.<br />
Au fost descrise caracteristici sociale, cl<strong>in</strong>ice și<br />
demografice grupate în funcție de vârstă și sex, de<br />
prezența durerii de cap și de tipul acesteia.<br />
Testul Hi 2 , testul Lev<strong>in</strong> și ANOVA au fost utilizate<br />
drept metode statistice. A fost utilizat un nivel de<br />

ani și 5,7 luni (<strong>in</strong>tervalul 3-17 ani).<br />
La copiii cu vârsta medie 12 și 0,3 luni durerea în<br />
migrenă a fost cel mai mult unilaterală (79,48%), la<br />
copiii cu vârsta medie 11 ani și 2,2 luni, durerea a fost<br />
mai mult la început unilaterală și s-a răspândit bilateral.<br />
La copiii mai mici durerea a fost predom<strong>in</strong>ant<br />
variată, 6.4% (cu expresia maximă la vârsta medie 10<br />
ani și 7 luni) ți bilaterală 1.5% (cu expresia maximă la<br />
vârsta medie de 10 ani și 7 luni) (Tabelul 2 localizarea<br />

Migrena apare de obicei de mai multe ori pe an<br />
după împl<strong>in</strong>irea vârstei de 11 ani, dar în prima decadă<br />
de viață ea apare săptămânal și chiar zilnic. În general<br />
ea apare cu un an mai târziu decât cefaleea non-migrenă<br />
(Tabelul 3. Frecvența și tipul de cefalee în funcție<br />

Cefaleea tip migrenă, care afectează copiii mai<br />
mici, durează semnificativ mai puț<strong>in</strong> (la vârsta medie<br />
de 8 ani durează doar câteva m<strong>in</strong>ute, la vârsta de 9 ani<br />
și 3,7 luni durează până la o oră și la vârsta de 12 ani<br />

Greața, senzația de vomă și vărsăturile sunt mult<br />
mai frecvente la copiii mici. Doar 1.3 % d<strong>in</strong>tre copiii<br />
cu cefalee tip migrenă având vârsta medie de 6 ani și<br />
4,8 luni nu au greață, senzație de vomă și vărsături<br />
(Tabelul 5. Greață și vărsături asociate cefaleei în relație<br />
cu vârsta copiilor).<br />
Nu s-au descoperit corelații cu vârsta pentru alte<br />
simptome care însoțesc durerea de cap migrenă (fotofobia,<br />
fonofobia, amețeli, paloarea pielii, cearcăne,<br />
transpirație, respirație neregulată, dureri în piept și<br />
alte simptome vegetative, schimbări evidente în comportament)<br />
și nici felul de a face față raportării legate<br />
de migrenă la copiii studiați.<br />
DISCUȚII<br />
D<strong>in</strong>tre cei 30636 de subiecți studiați, cefaleea<br />
migrenă a fost raportată la 8,63% (43,95% băieți și<br />
56,05% fete). Constatările noastre sunt foarte asemănătoare<br />
cu ale altor grupe de studiu europene. Studiul<br />
lui Wober –Bignol priv<strong>in</strong>d populația de copii și<br />
adolescenți cu migrenă a întâlnit prevalența migrenei<br />
la 1 – 1,5% d<strong>in</strong>tre copii și 3-17,6% d<strong>in</strong>tre adolescenți<br />
(19). În 1983, Sillanapaa a descris o prezența a migrenei<br />
mai mare: 37% d<strong>in</strong> 2941 de copii d<strong>in</strong> F<strong>in</strong>landa<br />
cu vârste între 7 și 14 ani (20). Hershey a constatat<br />
că migrena apare în pediatrie la 10,6% d<strong>in</strong>tre copiii<br />
cu vârste între 15 și 19 ani (21). În Turcia, Zencir a<br />
descris că prevalența migrenei era de 8,8%: 6,7% la<br />
băieți și 11,0% la fete (sau 1,7; 95% CI: 1,3 până la<br />
2,3) (22). Studi<strong>in</strong>d 1598 fișe medicale ale copiilor care<br />
au vizitat Centru Medical pentru Cefalee în perioada<br />
efectuării studiului, Raieli V. et all au constatat, folos<strong>in</strong>d<br />
criteriile Societății Internaționale pentru Studiul<br />
Cefaleei, 35,2% de cazuri de migrenă.<br />
În grupul de copii observat la vârsta medie de 9<br />
ani și 2,5 luni (<strong>in</strong>tervalul 3-17) cu deviația standard<br />
3,44, primele crize de migrenă au fost raportate la copii<br />
de la vârsta de 5 ani și 1,8 luni (DS 1,47, eroarea<br />
standard a mediei aritmetice 0,03). Balott<strong>in</strong> et al. Au<br />

Durerea pulsatorie este mai frecventă în cefaleea<br />
tip migrenă la copii, dom<strong>in</strong>antă la vârsta de 12 ani,<br />
în timp ce durerea nedef<strong>in</strong>ită, presiunea și durerea<br />
ascuțită, tip „mengh<strong>in</strong>ă” sunt de asemenea prezente<br />
până la vârsta de 7 ani. Cu cât copilul este mai mic, cu<br />
atât mai mare este probabilitatea că durerea va fi descrisă<br />
ca nedef<strong>in</strong>ită. Există o corelare clară, semnificativă<br />
d<strong>in</strong> punct de vedere statistic, între tipul de durere<br />
și vârsta copilului care pune sub semnul întrebării validitatea<br />
<strong>in</strong>sistenței de a <strong>in</strong>clude tipul de durere în criteriile<br />
de diagnosticare a cefaleei tip migrenă la copii.<br />
Copiii cu cefalee tip migrenă cu vârsta medie de 9<br />
ani și 2,5 luni și-au evaluat severitatea durerii folos<strong>in</strong>d<br />
o Scală de Evaluare a Durerii de la 0 la 9 cu 3,12.<br />
Evaluarea severității migrenei dep<strong>in</strong>de și de vârsta copilului.<br />
Coeficientul Pearson 0,43, p< 0,01 confirmă<br />
că există o asociere între vârsta copilului și evaluarea<br />
<strong>in</strong>tensității durerii, fapt care necesită evaluarea cl<strong>in</strong>ică<br />
precisă a fiecărui copil.<br />
Cefaleea tip migrenă este în majoritate unilaterală<br />
(79,4%) dar la copii, până la împl<strong>in</strong>irea vârstei de 11<br />
ani, sunt mai mult bilaterale. La vârsta de 11 ani durerea<br />
este mai mult unilaterală dar se ext<strong>in</strong>de bilateral și<br />
după împl<strong>in</strong>irea vârstei de 12 ani este clar unilaterală.<br />
Localizarea dist<strong>in</strong>ctă a durerii în migrene este corelată<br />
semnificativ cu vârsta copiilor și cu capacitatea lor de<br />
a o <strong>in</strong>terpreta. Maytal a constatat durerea unilaterală<br />
în 34,1% d<strong>in</strong> cazurile de migrenă pediatrică (25).<br />
Frecvența crizelor de durere în migrenă precum și<br />
alte caracteristici ale s<strong>in</strong>dromului de migrenă la copii<br />
dep<strong>in</strong>de de vârsta copiilor. Cele mai multe crize de<br />
migrenă (78,1%) se petrec de mai multe ori pe an la<br />
copiii cu vârsta medie de 11 ani și 1,8 luni.<br />
Durerile în migrenă durează mult mai mult (de la<br />
câteva m<strong>in</strong>ute în 0,89% d<strong>in</strong> cazuri până la 2 ore în<br />
27,8% d<strong>in</strong> cazuri și o zi întreagă sau și mai mult în<br />
21,8% d<strong>in</strong>tre copii. Procentul de cefalee tip migrenă<br />
care durează de la câteva m<strong>in</strong>ute, 0,89% d<strong>in</strong> cazuri,<br />
până la 1 oră în 4,4% d<strong>in</strong> cazuri și până la 2 ore<br />
(27,8%) exclude durata migrenei d<strong>in</strong> criteriile de diagnostic.<br />
Cefaleea tip migrenă care afectează copii mai<br />
mici durează semnificativ mai puț<strong>in</strong>: la vârsta medie<br />
de 8 ani, durează câteva m<strong>in</strong>ute, la vârsta medie de 9<br />
ani și 4.5 luni până la 1 oră și după 12 ani și 4 luni mai<br />
mult de 2 ore. În 1995, Wober – B<strong>in</strong>gol, analizând 409<br />
de copii a găsit că durata crizelor de migrenă la copii<br />
era de mai puț<strong>in</strong> de 2 ore la 19,0% d<strong>in</strong>tre pacienții<br />
cu migrenă (19). Maytala a descris durata crizelor de<br />
cefalee ca fi<strong>in</strong>d de 2 ore sau mai mult, la 55,7% d<strong>in</strong>tre<br />
copiii cu migrenă cu aură (25).<br />
De obicei, migrena dev<strong>in</strong>e evidentă pr<strong>in</strong> simptomele<br />
care o însoțesc și care dom<strong>in</strong>ă tabloul cl<strong>in</strong>ic, în<br />
special la o vârstă mai mică. Greața este prezentă <strong>in</strong><br />
98,7% cazuri, senzația de vomă în 97,7%, vărsăturile în<br />
77,2% și greață și /sau senzație de vomă și/sau vărsături<br />
în 83,7% d<strong>in</strong>tre copiii cu episoade de migrenă. Aceste<br />
rezultate def<strong>in</strong>esc clar greața, senzația de vomă și vărsăturile<br />
ca elemente de bază ale crizelor de migrenă la<br />
copii. Greața, senzația de vomă și vărsăturile sunt mult<br />
mai frecvente la copiii mai mici. Doar 1.3% d<strong>in</strong>tre copiii<br />
cu migrenă cu vârsta medie de 6 ani și 5,5 luni nu<br />
aveau greață, senzație de vomă și vărsături. Maytala a<br />
arătat că 47,7% d<strong>in</strong>tre copiii cu migrenă fără aură vomită<br />
în timpul crizelor de migrenă (25). Analizând 409<br />
copii, Wober – B<strong>in</strong>gol au descoperit că în diagnosticul<br />
diferențial al migrenei și al cefaleei de tensiune, <strong>in</strong>tensitatea<br />
durerii, agravarea durerii pr<strong>in</strong> activitate fizică,<br />
greață și vărsături sunt cele mai importante trăsături.<br />
Calitatea durerii, fono- și fotofobiile sunt mai puț<strong>in</strong><br />
importante și localizarea cel mai puț<strong>in</strong> importantă (19).<br />
Nu există o dependență clară de vârstă pentru<br />
fotofobie, fonofobie, amețeli, paloare tegumentară,<br />
cearcăne, transpirație, respirație neregulată, dureri<br />
precordiale și alte simptome vegetative, și modificări<br />
de comportament observabile.<br />
În anul 2000, analizând <strong>in</strong>fluența vârstei la declanșare<br />
asupra trăsăturilor cl<strong>in</strong>ice ale migrenei, Zambr<strong>in</strong>o a<br />
descoperit că localizarea durerii unilaterale și <strong>in</strong>tensi-<br />

tatea severă, prezența grețurilor, vărsăturilor, fonofobiei<br />
și fotofobiei erau trăsături care variau între subiecții<br />
cu migrenă sau fără migrenă. El nu a găsit că vârsta la<br />
declanșare este un factor care <strong>in</strong>fluențează caracteristicile<br />
cefaleei. Concluzia lui a fost că localizarea și calitatea<br />
durerii în cazul migrenei la adolescenți sunt similare<br />
celor găsite în migrenă la copii, în timp ce simptomele<br />
concomitente erau mai puț<strong>in</strong> frecvente decât în migrena<br />
copiilor sau adulților (26). Battistella și colab. au exam<strong>in</strong>at<br />
243 de pacienți și au ajuns la următoarele concluzii:<br />
declanșarea mai timpurie, durata mai scurtă, asocierea<br />
cu alte simptome (foto sau fonofobie, greață) mai scăzută<br />
și absența amplificării durerii în timpul activității<br />
fizice se manifestă mai degrabă la copiii preșcolari decât<br />
la cei de vârstă pre-pubertară (27). Rezultate similare au<br />
fost arătate la populația d<strong>in</strong> Vojvod<strong>in</strong>a (28).<br />
CONCLUZII:<br />
Diagnosticarea migrenei, în special la copii, nu<br />
este o sarc<strong>in</strong>ă ușoară. Diagnosticul migrenei dep<strong>in</strong>de<br />
de recunoașterea caracteristicilor care dep<strong>in</strong>d de vârstă,<br />
specifice stării. Durerea pulsatorie este cea mai<br />
dom<strong>in</strong>antă începând cu vârsta de 12 ani. Presiunea<br />
dureroasă nedef<strong>in</strong>ită și durerea ascuțită tip ‘mengh<strong>in</strong>ă’<br />
sunt prezente în mod egal până la vârsta de 7 ani.<br />

copii mai mari. La copiii cu vârsta de până la 11 ani,<br />
cefaleea tip migrenă este în mare parte bilaterală.<br />
Semnificația ridicată a durerii unilaterale în migrenă<br />
justifică în mod clar <strong>in</strong>cluderea ei pr<strong>in</strong>tre criteriile de<br />
diagnosticare ale migrenei la copii după împl<strong>in</strong>irea<br />
vârstei de 12 ani. Cele mai multe crize de migrenă se<br />
petrec de mai multe ori pe an la copiii cu vârsta medie<br />
de 11 ani și 1,5 luni cu debutul de la vârsta de 5<br />
ani și 1,8 luni. Durerea în migrenă apare săptămânal<br />
îna<strong>in</strong>te de împl<strong>in</strong>irea vârstei de 10 ani și chiar zilnic,<br />
îna<strong>in</strong>te de împl<strong>in</strong>irea vârstei de 9 ani.<br />
Cefaleea tip migrenă care afectează copii mai<br />
mici au o durată semnificativ mai scurtă. Procentul<br />
de cefalee tip migrenă care are o durată de până la 2<br />
ore (33%) exclude durata migrenei d<strong>in</strong>tre criteriile<br />
de diagnosticare. Greața, senzația de vomă și vărsăturile,<br />
care de obicei sunt evidente ca simptome care<br />
însoțesc crizele de migrenă la copii sunt mult mai<br />
frecvente la copiii mai mici. Doar 1,3% d<strong>in</strong>tre copiii<br />
cu migrenă la vârsta medie de 6 ani și 4.8 luni nu<br />
prez<strong>in</strong>tă greață, senzație de vomă și vărsături.<br />
Nu s-au descoperit corelații legate de vârstă pentru<br />
alte simptome care însoțesc cefaleea tip migrenă (fotofobia,<br />
fonofobia, amețeli, paloare tegumentară, cearcăne,<br />
transpirație, respirație neregulată, dureri precordiale<br />
și alte simptome vegetative, schimbări evidente<br />
în comportament).<br />
Migra<strong>in</strong>e headache is largely underestimated and<br />
misdiagnosed <strong>in</strong> children, because of the lack of anatomical<br />
changes, specific biological markers and specific<br />
research tools or bra<strong>in</strong> imag<strong>in</strong>g techniques (1,2,3,4).<br />
Diagnosis of migra<strong>in</strong>e headache <strong>in</strong> children depends<br />
on systematic exclusion of secondary disorders<br />
and systematic identification of the specific age dependent<br />
migra<strong>in</strong>e headache caracteristics. Migra<strong>in</strong>e<br />
phenomena: sensory, vegetative and affective, found<br />
only <strong>in</strong> humans, with marked age dependent quantitative<br />
and qualitative variations, are outl<strong>in</strong><strong>in</strong>g the<br />
children migra<strong>in</strong>e headache (5,6,7).<br />
There is not enough knowledge on signs and<br />
symptoms <strong>in</strong> children with migra<strong>in</strong>e headaches, to<br />
dist<strong>in</strong>guish clearly all cl<strong>in</strong>ical entities of migra<strong>in</strong>e<br />
syndrome (8,9,10).<br />
It is hard to def<strong>in</strong>e children migra<strong>in</strong>e headaches,<br />
mostly because they cannot clearly def<strong>in</strong>e their nuisances<br />
as well as they have age specific features (11).<br />
In the op<strong>in</strong>ion of a great number of authors today,<br />
children migra<strong>in</strong>e headache, as a very complex<br />
syndrome, is still a matter of deduction. Different criteria<br />
are suggested as diagnostic criteria of migra<strong>in</strong>e<br />
headache <strong>in</strong> children (12).<br />
Children migra<strong>in</strong>e headache is a chronic neurological<br />
disorder mostly characterized by episodic attacks<br />
of headaches with different <strong>in</strong>tensity, frequency<br />
and duration, commonly unilateral and associated<br />
with anorexia, nausea, vomit<strong>in</strong>g, phonophobia and<br />
photophobia. Very often preceded or accompanied by<br />
sensory, motor, vegetative, affective and various comb<strong>in</strong>ations<br />
of neurological, gastro<strong>in</strong>test<strong>in</strong>al and vegetative<br />
changes. The degree of their expression and the<br />
time of appearance vary age dependent, qualitatively<br />
and quantitatively <strong>in</strong> children. Accord<strong>in</strong>g to the literature<br />
data, generally, pa<strong>in</strong> quality and localization,<br />

headache duration, behavioral markers accompany<strong>in</strong>g<br />
symptoms and remission occurrence rate seem to be<br />
age depend (13,14).<br />
MATERIAL AND METHODS:<br />
This research was carried out <strong>in</strong> the territory of<br />
Vojvod<strong>in</strong>a, North Prov<strong>in</strong>ce of Serbia, which has a<br />
total population of 2,031,992, accord<strong>in</strong>g to the last<br />
census (<strong>in</strong> 2002). Dur<strong>in</strong>g the study, which lasted<br />
from 1988 to 2008 each year 2000 questionnaires<br />
were given to the participants, who were drawn from<br />
23 preschools and 42 grade schools <strong>in</strong> 9 cities <strong>in</strong><br />
Vojvod<strong>in</strong>a (Novi Sad, Subotica, Kik<strong>in</strong>da, Zrenjan<strong>in</strong>,<br />
Vrsac, Bela Crkva, Melenci, Futog and Temer<strong>in</strong>).<br />
In total 30363 children aged 3-17 years have been<br />
<strong>in</strong>cluded. Recurrent headaches were established <strong>in</strong><br />
27.46% (8456), def<strong>in</strong>ed as migra<strong>in</strong>e headaches <strong>in</strong><br />
8.63% (2644) of <strong>in</strong>terviewed children.<br />
Children were selected accord<strong>in</strong>g to their month<br />
and year of birth, and the first 3 letters of their first<br />
name by a multistage, stratified, clustered sampl<strong>in</strong>g<br />
procedure. This ensured that children could not enter<br />
the study twice dur<strong>in</strong>g the long research period.<br />
The subjects and/or their parents were asked to<br />
fill out a questionnaire <strong>in</strong> their places of residence.<br />
Questionnaires were distributed to children and/or<br />
their parents, selected by random sampl<strong>in</strong>g. The semi<br />
structured questionnaire, which was specially developed<br />
for this study by the author, was designed accord<strong>in</strong>g<br />
to the International Headache Society criteria. It<br />
was a screen<strong>in</strong>g questionnaire, which was completed<br />
by children and/or their parents together. It <strong>in</strong>cluded<br />
3 sections: (1) items about the child’s sociodemographic<br />
characteristics and his/her family and school; (2)<br />
items about the child’s development, and (3) items<br />
about headaches <strong>in</strong>clud<strong>in</strong>g all characteristics, signs<br />
and symptoms.<br />
The questionnaire was developed <strong>in</strong> 3 phases. In<br />
the first, semi structured <strong>in</strong>terviews with pediatricians<br />
and researchers were organized to select relevant doma<strong>in</strong>s.<br />
The doma<strong>in</strong>s for the section about headaches<br />
were selected based on the International Classification<br />
of Headache<br />
Disorders – II criteria. More than 150 possible<br />
items were identified. Precise, comprehensive and<br />
appropriate items were <strong>in</strong>cluded <strong>in</strong> the first form. The<br />
possible responses were open-ended options or categorical<br />
judgments.<br />
In the second phase, the questionnaire was pretested<br />
<strong>in</strong> semi structured <strong>in</strong>terviews on a small group<br />
of children who either did or did not suffer from<br />
headaches (16 families were <strong>in</strong>cluded). This phase<br />
aimed to evaluate the face and content validity of<br />
the questionnaire. Additionally, the sensitivity was<br />
evaluated by correlat<strong>in</strong>g the data from the questionnaire<br />
and the medical records of the children<br />
who had headaches. This phase resulted <strong>in</strong> a revised<br />
version, which was evaluated only on healthy<br />
children. Fifty children and adolescents completed<br />
the questionnaire twice <strong>in</strong> 3 weeks. The nonresponsive<br />
rate, response distributions, graphical response<br />
presentation (response <strong>in</strong>consistency) and questionnaire<br />
burdens (time to complete, <strong>format</strong>t<strong>in</strong>g, etc.)<br />
were analyzed. A number of items were modified<br />
or elim<strong>in</strong>ated and the f<strong>in</strong>al form <strong>in</strong>cluded 93 items<br />
which required 20 m<strong>in</strong> to complete.<br />
The <strong>in</strong>clud<strong>in</strong>g criteria were: age 3–17 years, signed<br />
<strong>in</strong>formed consent from parents. The exclud<strong>in</strong>g criterion<br />
was a previous diagnosis of a disease that has<br />
headache as a symptom.<br />
The accuracy of the questionnaire used <strong>in</strong> this survey<br />
was based on classification and diagnostic criteria<br />
for headache disorders, cranial neuralgias and facial<br />
pa<strong>in</strong>, and the revised International Headache Society<br />
criteria (15).<br />
The mean age of the study subjects was 9 years<br />
and 2,5 months (range 3–17 years, standard deviation<br />
(SD) 3.44. The large number of children surveyed<br />
allowed for def<strong>in</strong>ite conclusions. The study was conducted<br />
<strong>in</strong> 2 phases: completion of the questionnaire<br />
and, for those with recurrent headache, a face-to-face<br />
<strong>in</strong>terview. Based on data gathered by the questionnaire,<br />
children who had recurrent headache underwent<br />
an extended <strong>in</strong>terview and neurological exam<strong>in</strong>ation.<br />
The accuracy of the questionnaire used <strong>in</strong> this survey<br />
was based on International Headache Society criteria.<br />
Us<strong>in</strong>g the society’s classification codes, migra<strong>in</strong>e<br />
was accepted as 1.1–1.7, migra<strong>in</strong>e with aura was<br />
1.2.2–1.2.6, migra<strong>in</strong>e without aura was 1.1 and other<br />
migra<strong>in</strong>e syndromes were 1.3–1.7. recurrent headache<br />
was accepted as all headache types that appeared 1to<br />
3 times per month, without separat<strong>in</strong>g them due to<br />
specific characteristics. All types of recurrent headaches<br />
(idiopathic or cryptogenic recurrent headaches)<br />
that were not migra<strong>in</strong>e were considered as non-migra<strong>in</strong>e<br />
headaches (16,17,18).<br />
Of the questionnaires, 1.5% due to possible double<br />
<strong>in</strong>terview and miss<strong>in</strong>g data <strong>in</strong> the questionnaires have<br />

dache were analyzed for children who had completely<br />
answered the questionnaire. Compar<strong>in</strong>g characteristics<br />
of <strong>in</strong>dividual headaches <strong>in</strong> children with migra<strong>in</strong>e and<br />
non-migra<strong>in</strong>e headaches, specific age dependent features<br />
of children migra<strong>in</strong>e headaches were established.<br />
The demographic, cl<strong>in</strong>ical and social characteristics<br />
were described by age and sex accord<strong>in</strong>g to headache<br />
presence and type. The Hi 2 test, Lev<strong>in</strong> test and<br />
ANOVA were used as statistical methods. A significance<br />

Nausea, vomit<strong>in</strong>g impulse and vomit<strong>in</strong>g are<br />
much more frequent <strong>in</strong> younger children. Only<br />
1.3% of children with migra<strong>in</strong>e headaches of average<br />
age 6 years and 4.8 months do not have nausea,<br />
vomit<strong>in</strong>g <strong>in</strong>st<strong>in</strong>ct and vomit<strong>in</strong>g (Table 5. Headache<br />
associated nausea and vomit<strong>in</strong>g <strong>in</strong> regard to the age<br />
of children).<br />
There was no age dependency for other accommpan<strong>in</strong>g<br />
migra<strong>in</strong>e headache symptoms (photophobia,<br />
phonohobia, vertigo, sk<strong>in</strong> pallor, dark circles under<br />
the eyes, sweat<strong>in</strong>g, irregular breath<strong>in</strong>g, precordial<br />
pa<strong>in</strong> and other vegetative symptoms, noticable behavior<br />
changes), as well as <strong>in</strong> cop<strong>in</strong>g with migra<strong>in</strong>e<br />
report<strong>in</strong>g <strong>in</strong> observed children.<br />
DISCUSSION<br />
Out of 30636 observed migra<strong>in</strong>e headache was<br />
reported <strong>in</strong> 8.63%, (43.95% boys and 56.05% girls).<br />
Our f<strong>in</strong>d<strong>in</strong>gs are very simillar to other European<br />
study groups. Wober B<strong>in</strong>gol’s population study of<br />
migra<strong>in</strong>e <strong>in</strong> children and adolescents, encountered<br />
prevalence of migra<strong>in</strong>e <strong>in</strong> 1,1-5% of children and<br />
3-17,6% <strong>in</strong> adolecents (19). In 1983. Sillanapaa<br />
described higher headache presence: 37% of 2941<br />
F<strong>in</strong>land children aged 7-14 (20). Hershey f<strong>in</strong>ds pediatric<br />
migra<strong>in</strong>e occurs <strong>in</strong> up to 10.6% of children<br />
between the ages of 5 and 15 years and <strong>in</strong> up to 28%<br />
of adolescents between the ages of 15 and 19 years<br />
(21). In Turckey Zecir described migra<strong>in</strong>e prevalence<br />
was 8.8%: 6.7% <strong>in</strong> boys and 11.0% <strong>in</strong> girls (OR:<br />
1.7; 95% CI: 1.3 to 2.3) (22). Raieli V. et al found <strong>in</strong><br />
1598 medical records of children who visited Headache<br />
Center <strong>in</strong> the study period, accord<strong>in</strong>g to the<br />
IHS criteria 35.2% migra<strong>in</strong>e headaches (23).<br />
In the observed group of children at the average<br />
9 years 2.5 months (range 3-17), with standard<br />
deviation 3.44, first migra<strong>in</strong>e headache attacks vere<br />
reported <strong>in</strong> children of age 5 years 1,8 months (SD<br />
1,47 standard error of the aritmetical mean 0,03).<br />
Balott<strong>in</strong> et al analyzed group of children aged 12<br />
months-6 years; with mean age at the first observation:<br />
4 years and 7 months, and described mean age<br />
at onset: 4 years and 2 months, range: 10 months-6<br />
years (24).<br />
Pulsat<strong>in</strong>g pa<strong>in</strong> is most frequent <strong>in</strong> child migra<strong>in</strong>e<br />
headaches, dom<strong>in</strong>at at age of 12, whereas undef<strong>in</strong>ed,<br />
pressure and squeez<strong>in</strong>g pa<strong>in</strong> are equally present up<br />
to age of 7. The younger the child, the more likely<br />
it is that the pa<strong>in</strong> is described as undef<strong>in</strong>ed. There<br />
is a clear and statistically significant correlation<br />
between the type of pa<strong>in</strong> and the child’s age, which<br />
questions the validity of <strong>in</strong>sit<strong>in</strong>g on <strong>in</strong>clud<strong>in</strong>g type<br />
of pa<strong>in</strong> <strong>in</strong> diagnostic criteria of children migra<strong>in</strong>e<br />
headache.<br />
Children with migra<strong>in</strong>e headaches of mean age<br />
9 years 2,5 months assessed their pa<strong>in</strong> severity us<strong>in</strong>g<br />
a Pa<strong>in</strong> Assessment Scale of 0 – 5 with 3.12. The<br />
assessment of migra<strong>in</strong>e headache severity depends<br />

cl<strong>in</strong>ical study<br />
(79.4%), but <strong>in</strong> child migra<strong>in</strong>e is up to the 11 th<br />
birthday mostly bilateral. At age of 11 it is mostly<br />
unilateral but with bilateral spread<strong>in</strong>g, and after<br />
12 th birthday it is clearly unilateral. Dist<strong>in</strong>ct pa<strong>in</strong><br />
location <strong>in</strong> migra<strong>in</strong>e headaches is significantly connected<br />
with the age of children and their ability to<br />
<strong>in</strong>terpret it. Maytal found unilateral pa<strong>in</strong> <strong>in</strong> 34.1%<br />
pediatric migra<strong>in</strong>e (25).<br />
The frequency of migra<strong>in</strong>e head pa<strong>in</strong> attacks, as<br />
well as other characteristics of migra<strong>in</strong>e syndrome<br />
<strong>in</strong> children, depend on children’s age. Most migra<strong>in</strong>e<br />
hradache attacks (78,1%) occur several times per<br />
year. <strong>in</strong> children with average age of 11 years and<br />
1.8 months.<br />
Migra<strong>in</strong>e headaches last much longer (from several<br />
m<strong>in</strong>utes <strong>in</strong> 0.89% to 2 h <strong>in</strong> 27.8%, and whole<br />
day or longer <strong>in</strong> 21.8% children. The percentage<br />
of migra<strong>in</strong>e headaches which last a few m<strong>in</strong>utes<br />
(0.89%) up to 1 h (4.4%) and up to 2 h (27.8%), excludes<br />
length of migra<strong>in</strong>e headache from diagnostic<br />
criteria. Migra<strong>in</strong>e headaches which affect younger<br />
children last significantly shorter: at average age of<br />
8 years few m<strong>in</strong>utes, at average 9 years 4.5 months<br />
up to 1 h , and after 12 years 4 months more than<br />
2 h . Wober-B<strong>in</strong>gol <strong>in</strong> 1995, analyzung 409 children<br />
found duration of migra<strong>in</strong>e attacks was less than 2<br />
h <strong>in</strong> 19.0% of the migra<strong>in</strong>e patients (19). Maytala<br />
described headache duration of 2 hours or longer <strong>in</strong><br />
55.7% children with migra<strong>in</strong>e with aura (25).<br />
Migra<strong>in</strong>e is usually evident by the accompany<strong>in</strong>g<br />
symptoms which dom<strong>in</strong>ate the cl<strong>in</strong>ical picture, especially<br />
at younger age. Nausea is present <strong>in</strong> 98,7%,<br />
vomit<strong>in</strong>g impulse <strong>in</strong> 97,7%, vomit<strong>in</strong>g <strong>in</strong> 77,2%, and<br />
nausea and/or vomit<strong>in</strong>g impulse and/or vomit<strong>in</strong>g <strong>in</strong><br />
83.7% of children migra<strong>in</strong>e episodes. This clearly<br />
def<strong>in</strong>es nausea, vomit<strong>in</strong>g <strong>in</strong>st<strong>in</strong>ct and vomit<strong>in</strong>g as<br />
basic elements of migra<strong>in</strong>e attacks <strong>in</strong> children. Nausea,<br />
vomit<strong>in</strong>g <strong>in</strong>st<strong>in</strong>ct and vomit<strong>in</strong>g are much more<br />
frequent <strong>in</strong> younger children. Only 1.3% of children<br />
with migra<strong>in</strong>e headaches of average age 6 years and<br />
5.5 months have no nausea, vomit<strong>in</strong>g <strong>in</strong>st<strong>in</strong>ct and<br />
vomit<strong>in</strong>g. Maytala described 47.7% children with<br />
migra<strong>in</strong>e without aura vomit dur<strong>in</strong>g migra<strong>in</strong>e attacks<br />
(25). Analyz<strong>in</strong>g 409 children Wober-B<strong>in</strong>gol<br />
found that <strong>in</strong> the differential diagnosis of migra<strong>in</strong>e<br />
and tension-type headache the <strong>in</strong>tensity of pa<strong>in</strong>,<br />
aggravation of headache by physical activity, nausea<br />
and vomit<strong>in</strong>g were the most important features The<br />
quality of pa<strong>in</strong>, phono- and phonophobia were less<br />
helpful and location least important (19).<br />
There is no clear age dependency for photophobia,<br />
phonohobia, vertigo, sk<strong>in</strong> pallor, dark circles<br />
under the eyes, sweat<strong>in</strong>g, irregular breath<strong>in</strong>g, precordial<br />
pa<strong>in</strong> and other vegetative symptoms, oticable<br />
behavior changes.<br />
In 2000 analyz<strong>in</strong>g age at onset <strong>in</strong>fluence on migra<strong>in</strong>e<br />
cl<strong>in</strong>ical features. Zambr<strong>in</strong>o found that unilateral<br />
pa<strong>in</strong> location and severe <strong>in</strong>tensity, the presence<br />
of nausea, vomit<strong>in</strong>g, phonophobia and photophobia<br />
were features which varied between migra<strong>in</strong>ous and<br />
non-migra<strong>in</strong>ous subjects. He did not f<strong>in</strong>d the age<br />
at onset is a factor <strong>in</strong>fluenc<strong>in</strong>g the characteristics of<br />
the headache. He concluded, the location and the<br />
quality of pa<strong>in</strong> <strong>in</strong> adolescent migra<strong>in</strong>e are similar<br />
to those found <strong>in</strong> childhood migra<strong>in</strong>e, while the<br />
concomitant symptoms were less frequent than <strong>in</strong><br />
childhood and <strong>in</strong> adult migra<strong>in</strong>e (26). Battistella et<br />
all exam<strong>in</strong>ed 243 patients and found earlier onset,<br />
shorter duration, lower symptom association (photo-<br />
or phonophobia, nausea) and no pa<strong>in</strong> <strong>in</strong>crease<br />
dur<strong>in</strong>g physical activity rather <strong>in</strong> pre school than<br />
<strong>in</strong> prepubertal children (27). Simillar resulths have<br />
been shown <strong>in</strong> population ov Vojvod<strong>in</strong>a (28).<br />
CONCLUSION:<br />
Diagnos<strong>in</strong>g migra<strong>in</strong>e especially <strong>in</strong> children is<br />
not an easy task. The diagnosis of migra<strong>in</strong>e depends<br />
on the recognition of the age dependent features<br />
specific to the condition.<br />
Pulsat<strong>in</strong>g pa<strong>in</strong> is the most dom<strong>in</strong>ate from from<br />
age of 12years. Undef<strong>in</strong>ed pa<strong>in</strong> pressure and squeez<strong>in</strong>g<br />
pa<strong>in</strong> are equally present up to the age of 7.<br />
Higher pa<strong>in</strong> level is described <strong>in</strong> older children.<br />
Head pa<strong>in</strong> <strong>in</strong> child migra<strong>in</strong>e is up to 11 th birthday<br />
mostly bilateral. High significance of unilateral pa<strong>in</strong><br />
<strong>in</strong> migra<strong>in</strong>e headache clearly justifies its <strong>in</strong>clusion<br />
<strong>in</strong>to diagnostic criteria of child migra<strong>in</strong>e after 12 th<br />
birthday. Most migra<strong>in</strong>e attacks occur several times<br />
a year <strong>in</strong> children with average age of 11 years and<br />
1.5 months with onset at the age of 5 years and 1.8<br />
months. Migra<strong>in</strong>e head pa<strong>in</strong> appears weekly before<br />
10 th , even daily before 9 th birthday.<br />
Migra<strong>in</strong>e headaches which affect younger children<br />

and 4.8 months have no nausea, impulse to vomit<strong>in</strong>g<br />
and vomit<strong>in</strong>g.<br />
There is no age dependency for other accommpan<strong>in</strong>g<br />
migra<strong>in</strong>e headache symptoms (photophobia,<br />
phonohobia, vertigo, sk<strong>in</strong> pallor, dark circles under<br />
the eyes, sweat<strong>in</strong>g, irregular breath<strong>in</strong>g, precordial<br />
pa<strong>in</strong> and other vegetative symptoms, noticable behavior<br />
