METODOLOGIA CERCETĂRII<br />
Complexitatea sindromului disfunctional al ATM rezidă într-un<br />
cumul factorial plurivalent, individualizarea etiopatogenică a fiecărei<br />
<strong>la</strong>turi se constituie într-o abordare terapeutică ţintită. Core<strong>la</strong>rea<br />
examenului clinic cu evaluările paraclinice <strong>de</strong>celează cauzele<br />
<strong>de</strong>c<strong>la</strong>nşatoare ale întregului areal pathogenic al , prevalenţa afectărilor <strong>de</strong><br />
acest tip se constituie într-o realitate clinică frecvent întâlnită în activitatea<br />
practică.<br />
IV.1. MOTIVAŢIA ALEGERII TEMEI: Studiile efectuate au urmărit<br />
<strong>de</strong>ru<strong>la</strong>rea unor cercetări complexe cu profund caracter epi<strong>de</strong>miologic ,<br />
terapeutic stabilind cu precizie prevalenţa şi inci<strong>de</strong>nţa sindromului<br />
disfunctional avind ca etiologie factorul muscu<strong>la</strong>r <strong>la</strong> pacientii diagnosticati<br />
cu e<strong>de</strong>ntatie partial intinsa, e<strong>de</strong>ntatie subtota<strong>la</strong> si e<strong>de</strong>ntatie tota<strong>la</strong>.<br />
IV.2. DIRECŢII DE CERCETARE<br />
1. Studiu epi<strong>de</strong>miologic privind impactul şi necesitatea <strong>de</strong>pistării<br />
sindromului disfuncţional <strong>de</strong> etiologie muscu<strong>la</strong>ră –pe un lot reprezentativ<br />
<strong>de</strong> pacienti diagnosticati cu e<strong>de</strong>ntatie partial intinsa, e<strong>de</strong>ntatie subtota<strong>la</strong>,<br />
e<strong>de</strong>ntatie tota<strong>la</strong>, urmarindu-se prevalenta sindromului algo-disfunctional<br />
in contextul coroborativ al unui areal <strong>de</strong> factori ce influienteaza<br />
rezultatele finale.<br />
2. Algoritm terapeutic <strong>de</strong> electie in ve<strong>de</strong>rea reg<strong>la</strong>rii <strong>de</strong>zechilibrelor<br />
muscu<strong>la</strong>re-urmareste selectarea etapelor terapeutie specifice fiecarei<br />
entitati clinice studiate, o atentie <strong>de</strong>osebita este caordata inregistrarii<br />
re<strong>la</strong>tiilor mandibulo-craniene utilizind arcul facil si articu<strong>la</strong>torul<br />
programabil in cadrul sistemului N.O.R.<br />
3. Monitorizare si terapia <strong>la</strong> pacientii cu e<strong>de</strong>ntatie partial intinsa,<br />
e<strong>de</strong>ntatie tota<strong>la</strong> si cu e<strong>de</strong>ntatie subtota<strong>la</strong> <strong>la</strong> pacientii cu <strong>de</strong>zechilibre<br />
muscu<strong>la</strong>re-se materializeaza prin aplicarea Sistemului <strong>Gr</strong>incare.<br />
IV.3. SCOPUL ŞI OBIECTIVELE CERCETĂRII<br />
Având în ve<strong>de</strong>re complexitatea şi varietatea patologiei sindromului algodisfunctional<br />
studiul a avut ca scop i<strong>de</strong>ntificarea prevalenţei şi inci<strong>de</strong>nţei<br />
acestei entitati clinice <strong>de</strong> etiologie muscu<strong>la</strong>ra <strong>la</strong> pacientii cu e<strong>de</strong>ntatie<br />
partial intinsa, e<strong>de</strong>ntatie tota<strong>la</strong> si cu e<strong>de</strong>ntatie subtota<strong>la</strong>, urmata <strong>de</strong><br />

e<strong>la</strong>borarea <strong>la</strong>goritmului specific terapeutic dictat <strong>de</strong> particu<strong>la</strong>ritatea cazuui<br />
clinic.<br />
Cercetarea a urmărit materializarea practică a următoarelor obiective:<br />
1. Deru<strong>la</strong>rea unui studiu epi<strong>de</strong>miologic privind impactul şi<br />
necesitatea <strong>de</strong>pistării sindromului disfuncţional <strong>de</strong> etiologie muscu<strong>la</strong>ră –<br />
pe un lot reprezentativ <strong>de</strong> pacienti diagnosticati cu e<strong>de</strong>ntatie partial<br />
intinsa, e<strong>de</strong>ntatie subtota<strong>la</strong>, e<strong>de</strong>ntatie tota<strong>la</strong>, urmarindu-se prevalenta<br />
sindromului algo-disfunctional in contextul coroborativ al unui areal <strong>de</strong><br />
factori ce influienteaza rezultatele finale.<br />
2. E<strong>la</strong>borarea si particu<strong>la</strong>rizarea unui algoritm terapeutic <strong>de</strong> electie<br />
in ve<strong>de</strong>rea reg<strong>la</strong>rii <strong>de</strong>zechilibrelor muscu<strong>la</strong>re-urmareste selectarea etapelor<br />
terapeutie specifice fiecarei entitati clinice studiate, o atentie <strong>de</strong>osebita<br />
este caordata inregistrarii re<strong>la</strong>tiilor mandibulo-craniene utilizind arcul<br />
facil si articu<strong>la</strong>torul programabil in cadrul sistemului N.O.R.<br />
3. Monitorizare si terapia <strong>la</strong> pacientii cu e<strong>de</strong>ntatie partial intinsa,<br />
e<strong>de</strong>ntatie tota<strong>la</strong> si cu e<strong>de</strong>ntatie subtota<strong>la</strong> <strong>la</strong> pacientii cu <strong>de</strong>zechilibre<br />
muscu<strong>la</strong>re-se materializeaza prin aplicarea Sistemului <strong>Gr</strong>incare<br />
IV.4. MATERIAL ŞI METODĂ<br />
Material<br />
Loturile incluse în studiu sunt variabile ca număr <strong>de</strong> cazuri, în<br />
funcţie <strong>de</strong> studiul realizat.<br />
Criteriile <strong>de</strong> inclu<strong>de</strong>re în lot:<br />
• au fost incluşi pacienţi care au semnat şi datat formu<strong>la</strong>rul<br />
<strong>de</strong> consimţământ informat;<br />
• 452 pacienti diagnosticaţi cue<strong>de</strong>ntatie partial intinsa,<br />
e<strong>de</strong>ntatie subtota<strong>la</strong> si e<strong>de</strong>ntatie tota<strong>la</strong> si sindrom<br />
disfunctional al atm<br />
Criterii <strong>de</strong> exclu<strong>de</strong>re:<br />
• cei ce nu pot să participe <strong>la</strong> studiu pe întreaga perioadă<br />
pacienţi necooperanţi<br />
Loturile studiate sunt reprezentative statistic.<br />
Meto<strong>de</strong>le statistice <strong>de</strong>scriptive şi analitice utilizate au permis<br />
analiza, compararea şi prelucrarea fi<strong>de</strong>lă a datelor obţinute. În cadrul<br />
acestei cercetări s-a folosit pentru prelucrarea statistică a datelor<br />
programul STATISTICA, <strong>de</strong>dicat cercetării medicale. În cadrul studiului<br />
s-au aplicat teste specifice diverselor tipuri <strong>de</strong> date analizate dintre care<br />
putem aminti teste <strong>de</strong> compararea valorilor medii a unui parametru<br />

corespunzător mai multor loturi <strong>de</strong> date dintre care testul ANOVA,<br />
Scheffé, Spjotvol/Stoline, teste specifice <strong>de</strong> core<strong>la</strong>ţie pentru variabile<br />
cantitative cât şi pentru variabile calitative dintre care putem menţiona<br />
Pearson, Chi–pătrat (2), Mantel-Haenszel, Fisher, Spearman, Kendall<br />
tau, Gamma.<br />
În urma aplicării acestor teste s-au luat în discuţie principalii<br />
parametrii <strong>de</strong> interes iar în funcţie <strong>de</strong> valorile acestora s-au stabilit<br />
concluziile. Astfel p parametrul <strong>de</strong> referinţă calcu<strong>la</strong>t în cadrul testelor<br />
reprezintă nivelul <strong>de</strong> semnificaţie al testului, care s-a comparat cu p=0,05<br />
corespunzător unei încre<strong>de</strong>ri <strong>de</strong> 95%, acesta având valori semnificative<br />
pentru pcalcu<strong>la</strong>t
La baza cercetării au stat fişele <strong>de</strong> observaţie ale clinicii, funcţie <strong>de</strong><br />
a căror coduri s-au extras pacienţii cu diagnosticul <strong>de</strong> malre<strong>la</strong>ţie craniomandibu<strong>la</strong>ră.<br />
Rezultatele examenului clinic şi a examenelor paraclinice au<br />
constituit sursa unei baze <strong>de</strong> date care a fost analizată în studiul <strong>de</strong> faţă cu<br />
ajutorul programelor Excel şi Statistica 6.0.<br />
REZULTATE ŞI DISCUŢII<br />
Chiar dacă literatura <strong>de</strong> specialitate abundă <strong>de</strong> teorii şi concepte<br />
care încearcă explicaţii ale etiopatogeniei malre<strong>la</strong>ţiilor cranio-mandibu<strong>la</strong>re,<br />
rămâne permanent loc pentru studiu, respectiv aprofundare a fenomenelor<br />
disfuncţionalizante ale sistemului stomatognat, mai ales că aproape<br />
întot<strong>de</strong>auna aceste teorii au căzut pradă şi unui oarecare grad <strong>de</strong><br />
controversă. Teoria etiopatogenică dishomeostazică are meritul <strong>de</strong> a fi cea<br />
mai amplă şi precisă în ace<strong>la</strong>şi timp, iar cercetarea <strong>de</strong> faţă vine în sprijinul<br />
susţinerii ei. Există o serie <strong>de</strong> factori, elemente ale sistemului stomatognat,<br />
care afectaţi uni<strong>la</strong>teral sau simultan se pot constitui în surse <strong>de</strong> malre<strong>la</strong>ţii<br />
cranio-mandibu<strong>la</strong>re. Factorii majori implicaţi sunt: muscu<strong>la</strong>r, osos,<br />
articu<strong>la</strong>r, <strong>de</strong>ntar şi ocluzal, parodontal, respectiv funcţional (40), iar ei<br />
cunosc un nivel homeostazic atât sistemic, cât şi suprasistemic.<br />
Macroorganismul prin echilibrul sau dimpotrivă <strong>de</strong>zechilibrul constantelor<br />
sale influenţează buna funcţionare a factorilor mai sus menţionaţi. De<br />
asemenea am încercat să <strong>de</strong>monstrăm că atingerea acestor factori nu este<br />
niciodată uni<strong>la</strong>terală, acest lucru fiind extraordinar <strong>de</strong> greu <strong>de</strong> surprins,<br />
poate doar <strong>la</strong> începutul bolii; <strong>de</strong> obicei afectările sunt ale mai multor<br />
elemente, între ele existând o homeostazie perfectă, astfel încât perturbarea<br />
unuia atrage inevitabil funcţionarea <strong>de</strong>fectuaoasă a celor<strong>la</strong>lte, modificând<br />
întregul echilibru.<br />
5.1. Factorul muscu<strong>la</strong>r<br />
Sistemul stomatognat dispune <strong>de</strong> un subsistem muscu<strong>la</strong>r care prin<br />
tonus şi contracţii echilibrate participă <strong>la</strong> conturarea unei re<strong>la</strong>ţii <strong>de</strong> postură,<br />
centrice şi a unei dinamici mandibu<strong>la</strong>re corecte. Dezechilibrarea din diferite<br />

motive a tonusului sau a contracţiilor muscu<strong>la</strong>re produce alterarea re<strong>la</strong>ţiilor<br />
statice sau dinamice, fapt posibil <strong>de</strong> constatat prin modificarea reperelor<br />
muscu<strong>la</strong>re ale acestor re<strong>la</strong>ţii.<br />
Astfel se explică apariţia malre<strong>la</strong>ţiilor <strong>la</strong> persoane cu vicii <strong>de</strong><br />
postură <strong>la</strong> nivelul muşchilor centurii scapu<strong>la</strong>re, gât şi chiar lombar în unele<br />
situaţii.De exemplu s-a <strong>de</strong>scris cazul unei paciente internată în serviciul <strong>de</strong><br />
reumatologie pentru dureri lombare care nu răspun<strong>de</strong>a <strong>la</strong> nici un fel <strong>de</strong><br />
tratament şi care s-a vin<strong>de</strong>cat după o reabilitare orală complexă; durerea era<br />
referită <strong>de</strong> <strong>la</strong> nivelul sistemului stomatognat (241).<br />
Dar malre<strong>la</strong>ţia se poate insta<strong>la</strong> şi datorită hipotoniei muscu<strong>la</strong>turii<br />
manducatoare, fenomen întâlnit mai ales <strong>la</strong> persoanele în vârstă, cu<br />
afecţiuni cronice sau neurologice, endocrine (40, 41). Muşchii pot fi<br />
afectaţi în ceea ce priveşte nivelul tonusului, dar mai ales contracţia, cauza<br />
fiind inf<strong>la</strong>matorie (miozite), traumatică (stupoare, zdrobire, plăgi),<br />
tumorală.<br />
Tulburarea tonusului şi contracţiei muscu<strong>la</strong>re vor antrena în timp<br />
solicitări articu<strong>la</strong>re, odontale, parodontale, toate agravând malre<strong>la</strong>ţia. În<br />
cercetarea <strong>de</strong> faţă am încercat să <strong>de</strong>monstrăm influenţa elementelor<br />
suprasistemice (boli generale care pot modifica starea muşchilor sistemului<br />
stomatognat), precum şi semnele şi simptomele apărute <strong>la</strong> nivelul<br />
muşchilor. Din anamneza pacienţilor am extras bolile cu potenţial să<br />
influenţeze muşchii sistemului stomatognat, astfel încât să <strong>de</strong>termine<br />
insta<strong>la</strong>rea malre<strong>la</strong>ţiei cranio-mandibu<strong>la</strong>re. Un factor suprasistemic care <strong>de</strong><br />
obicei este neglijat, dar care influenţează activitatea muscu<strong>la</strong>ră, în special<br />
nivelul tonusului muscu<strong>la</strong>r este stresul (96, 144, 204, 278, 288). Din<br />
capitolul foii <strong>de</strong> observaţie referitor <strong>la</strong> condiţiile <strong>de</strong> viaţă şi muncă am<br />
extras <strong>de</strong>c<strong>la</strong>raţiile pacienţilor privind prezenţa stresului în viaţa lor.<br />
Durerea muscu<strong>la</strong>ră este simptomul care se asociază cel mai<br />
frecvent cu factorul stres, anxietatea şi <strong>de</strong>presia conducând <strong>la</strong> un cerc vicios<br />
(144, 244) prin care un element îl <strong>de</strong>termină, apoi îl întreţine pe celă<strong>la</strong>lt. În<br />

cadrul lotului durerea muscu<strong>la</strong>ră are o distribuţie după cum reiese din<br />
graficul:<br />

1= durere uni<strong>la</strong>terală muşchi maseter,<br />
2= durere uni<strong>la</strong>terală muşchi maseter şi pterigoidian extern,<br />

6= durere uni<strong>la</strong>terală muşchi maseter, temporal, pterigoidian<br />
nu=absenţa durerii muscu<strong>la</strong>re.<br />
De asemenea din baza <strong>de</strong> date am extras din capitolul “Antece<strong>de</strong>nte<br />
generale”, în ce măsură au apărut şi au avut influenţă bolile generale<br />
(neurologice, psihice, endocrine, metabolice) asupra activităţii muşchilor<br />
sistemului stomatognat.<br />

Dar muşchii sistemului stomatognat pot fi afectaţi prin traumatisme<br />
extrinseci, inf<strong>la</strong>maţii, infecţii, tumori, dar şi prin suprasolicitări datorate<br />
celor<strong>la</strong>lţi factori articu<strong>la</strong>r, osos, respectiv ocluzal.Semnele şi simptomele<br />
muscu<strong>la</strong>re sunt frecvente <strong>la</strong> pacienţii cu diagnostic <strong>de</strong> malre<strong>la</strong>ţie craniomandibu<strong>la</strong>ră.<br />
Ca şi concluzie, cel mai frecvent spasmul a apărut <strong>la</strong> muşchiul pterigoidian<br />
extern uni<strong>la</strong>teral, iar obosea<strong>la</strong> <strong>la</strong> muşchii coborâtori, rezultat logic dacă<br />
asociem procentul mare <strong>de</strong> hipotonii care afectează aceşti muşchi.<br />
Limitarea excursiilor mandibu<strong>la</strong>re datorită hipertoniei muscu<strong>la</strong>re,<br />
spasmului, durerii, oboselii muscu<strong>la</strong>re s-a regăsit <strong>la</strong> un procent <strong>de</strong> 3,67% din<br />
pacienţii lotului studiat, fapt ce <strong>de</strong>monstrează că ei îşi aleg anumite tipare <strong>de</strong><br />
mişcare, astfel încât să evite cele<strong>la</strong>lte siptome pe care le acuză.<br />

Dinamica mandibu<strong>la</strong>ră, aşa cum se ştie, se află sub influenţa şi a<br />
<strong>de</strong>terminantului funcţional (muscu<strong>la</strong>r) şi <strong>de</strong> aceea modificarea traiectoriilor<br />
<strong>de</strong> dinamică mandibu<strong>la</strong>ră este unul din semnele cel mai frecvent întâlnite.<br />
Procentul <strong>de</strong> 64,45% al pacienţilor care prezintă acest tip <strong>de</strong> tulburări,<br />
<strong>de</strong>monstreză influenţa c<strong>la</strong>ră a elementului neuro-muscu<strong>la</strong>r asupra dinamicii<br />
mandibu<strong>la</strong>re. Nu se poate minimiza influenţa <strong>de</strong>terminantului anatomic<br />
posterior (articu<strong>la</strong>r), dar rezultatele confirmă i<strong>de</strong>ea conform căreia cei doi<br />
<strong>de</strong>terminanţi se disfuncţionalizează simultan, sau unul este cauza perturbării<br />
celui<strong>la</strong>lt.<br />
Toate modificările factorului muscu<strong>la</strong>r se observă cel mai pregnant în<br />
diagramele care reflectă alterările reperelor muscu<strong>la</strong>re atât ale re<strong>la</strong>ţiei <strong>de</strong><br />
postură cât şi a re<strong>la</strong>ţiei centrice.<br />
Afectarea în proporţie <strong>de</strong> 67,65% a reperului muscu<strong>la</strong>r al re<strong>la</strong>ţiei <strong>de</strong> postură<br />
tră<strong>de</strong>ază o atingere <strong>de</strong>stul <strong>de</strong> importantă a factorului muscu<strong>la</strong>r în cadrul<br />
malre<strong>la</strong>ţiilor cranio-mandibu<strong>la</strong>re, el fiind implicat în <strong>de</strong>c<strong>la</strong>nşarea acestei<br />
patologii. Asocierea cu procente importante ale afectării şi a celor<strong>la</strong>lţi factori<br />
<strong>de</strong>monstrează teoria etiopatogenică dishomeostazică, conform căreia există o<br />
atingere a mai multor factori şi extrem <strong>de</strong> rar uni<strong>la</strong>terală.<br />
Rezultatele obţinute în cazul investigării reperului muscu<strong>la</strong>r al re<strong>la</strong>ţiei<br />
centrice confirmă afectarea masivă a factorului muscu<strong>la</strong>r, procentul <strong>de</strong> doar<br />
14,70% al normalităţii lui este a<strong>la</strong>rmant <strong>de</strong> redus, el reflectând pon<strong>de</strong>rea<br />
mare a malre<strong>la</strong>ţiei <strong>la</strong> pacienţii care se prezintă practic zi <strong>de</strong> zi în cabinetul<br />
<strong>de</strong>ntar.<br />
5.2. Factorul osos<br />
Morfologia osoasă poate fi afectată din motive variate, traumatismele (17,<br />
150, 252), mai ales cele care afectează centrii <strong>de</strong> creştere, distrofiile<br />
maxi<strong>la</strong>relor, dismorfozele cranio-faciale (107, 163), ma<strong>la</strong>diile osoase<br />

(osteomielita), osteoporozele, osteoma<strong>la</strong>cie, rahitismul, atingerile părţilor<br />
osoase ale ATM constituie factori etiopatogenici care antrenează şi tulburări<br />
<strong>la</strong> nivelul altor elemente sistemice stomatognatice, rezultatul fiind malre<strong>la</strong>ţia<br />
cranio-mandibu<strong>la</strong>ră statică sau dinamică.<br />
Asimetria morfologică care apare <strong>la</strong> nivelul factorului osos va antrena<br />
asimetria funcţională, suprasolicitând şi muşchii sistemului stomatognat,<br />
parodonţiul, modificând ocluzia.<br />
De asemenea modificările osoase pot fi şi consecinţa unor ma<strong>la</strong>dii ale<br />
macroorganismului cu influenţe şi <strong>la</strong> nivelul sistemului osos.<br />
În studiul pe care l-am realizat, am cercetat factorul osos prin urmărirea<br />
modificărilor reperelor osoase ale re<strong>la</strong>ţiilor fundamentale craniomandibu<strong>la</strong>re.<br />
Rezultatele studiului arată o modificare şi mai dramatică a reperului osos în<br />
cazul re<strong>la</strong>ţiei centrice, instabilitatea acesteia fiind cea mai frecventă printre<br />
pacienţii care s-au adresat clinicii noastre (75,73%), iar normalitatea<br />
caracterizând doar un procent <strong>de</strong> 15,44% din lot. Este practic un semnal <strong>de</strong><br />
a<strong>la</strong>rmă având în ve<strong>de</strong>re că re<strong>la</strong>ţia centrică este vitală pentru buna funcţionare<br />
a sistemului stomatognat, ea fiind punctul <strong>de</strong> <strong>la</strong> care pornesc şi se finalizează<br />
toate mişcările mandibu<strong>la</strong>re.<br />
Studiul ne-a mai <strong>de</strong>monstrat şi importanţa dimensiunii verticale posterioare<br />
(Zy-Go) care trebuie să aibă dimensiuni egale stânga-dreapta; procentul <strong>de</strong><br />
inegalitate <strong>de</strong> doar 3,67% poate pare mic, dar dacă ne referim <strong>la</strong> pacienţii cu<br />
suficiente stopuri ocluzale pentru <strong>de</strong>terminarea re<strong>la</strong>ţiei centrice el creşte <strong>la</strong><br />
15.15%.<br />
Am putut <strong>de</strong>ce<strong>la</strong> cu siguranţă cauza traumatică care să constituie sursă <strong>de</strong><br />
malre<strong>la</strong>ţii cranio-mandibu<strong>la</strong>re, <strong>la</strong> 1,47% din subiecţii lotului.<br />
5.3. Factorul articu<strong>la</strong>r<br />
Articu<strong>la</strong>ţia temporo-mandibu<strong>la</strong>ră este caracterizată <strong>de</strong> asimetrie<br />
morfologică (316), funcţională, sau apărută datorită unor afecţiuni specifice<br />
(314), fapt care o transformă în factor etiologic al malre<strong>la</strong>ţiilor craniomandibu<strong>la</strong>re.<br />
Asimetria morfologică articu<strong>la</strong>ră este o stare biologică foarte frecventă,<br />
neexistând condili simetrici perfect, angu<strong>la</strong>ţii i<strong>de</strong>ntice, înălţimi condiliene<br />
temporale simi<strong>la</strong>re, rezultă <strong>de</strong>ci că încă din etapa formării sale ATM este<br />
pre<strong>de</strong>stinată a se disfuncţionaliza. Asimetria se poate datora şi unor solicitări<br />
funcţionale asimetrice în<strong>de</strong>lungate.<br />
Articu<strong>la</strong>ţia temporo-mandibu<strong>la</strong>ră poate fi afectată prin ma<strong>la</strong>dii specifice<br />
<strong>de</strong>generative, traumatice, inf<strong>la</strong>matorii, tumorale.<br />

5.4. Factorul <strong>de</strong>ntar şi ocluzal<br />
În momentul în care “cheia întregii stomatologii”, ocluzia, este afectată se<br />
pot <strong>de</strong>c<strong>la</strong>nşa modificări <strong>la</strong> nivelul întregului sistem stomatognat. Pot apare<br />
atriţia, abraziunea care duc <strong>la</strong> distrugerea completă a reliefului ocluzal,<br />
leziunile odontale coronare netratate sau tratate incorect duc <strong>la</strong> modificarea<br />
contactelor inter<strong>de</strong>ntare (290) şi a transmiterii forţelor axiale, dar mai ales<br />
tangenţiale. E<strong>de</strong>ntaţia duce <strong>la</strong> migrări <strong>de</strong>ntare verticale şi orizontale cu<br />
anu<strong>la</strong>rea unor stopuri ocluzale şi crearea contactelor premature precum şi a<br />
interferenţelor ocluzale (81-84).<br />
Factorii ocluzali consi<strong>de</strong>raţi direct implicaţi în <strong>de</strong>c<strong>la</strong>nşarea malre<strong>la</strong>ţiilor<br />
cranio-mandibu<strong>la</strong>re sunt ocluzia <strong>de</strong>schisă, overjetul mai mare <strong>de</strong> 6-7 mm,<br />
diferenţa dintre poziţiile <strong>de</strong> intercuspidare maximă şi ocluzie centrică mai<br />
mare <strong>de</strong> 4 mm, ocluzia încrucişată (crossbite), e<strong>de</strong>ntaţia <strong>la</strong>terală întinsă şi<br />
extinsă (peste 5 unităţi odonto-parodontale absente) (40, 262).<br />
Astfel prin abrazie relieful ocluzal se modifică, <strong>de</strong>venind asimetric.Faţetele<br />
<strong>de</strong> abrazie pot avea dispoziţie verticală, orizontală sau oblică, indicând<br />
frecvenţa anumitor mişcări care o produc.Procesul poate avea loc doar <strong>la</strong><br />
nivelul unui grup <strong>de</strong>ntar, sau poate fi extins pe toată arcada. Abraziunea<br />
conduce cel mai frecvent <strong>la</strong> malre<strong>la</strong>ţii cranio-mandibu<strong>la</strong>re statice prin<br />
bascu<strong>la</strong>re, cu afectarea dimensiunii verticale centrice anterioare sau<br />
posterioare.<br />

Capitolul VI.<br />
METODE CLINICE ŞI PARACLINICE DE DETERMINARE A<br />
DEZECHILIBRULUI MUSCULO-LIGAMENTAR LA PACIENŢII<br />
EDENTAŢI PARŢIAL ÎNTINS, SUBTOTAL ŞI TOTAL.<br />
SCOPUL CERCETĂRII<br />
Ocluzia reprezintă stâlpul <strong>de</strong> bază al întregii medicini stomatologice şi <strong>de</strong><br />
aceea echilibrarea ocluzală, respectiv repoziţionarea cranio-mandibu<strong>la</strong>ră se<br />
regăsesc frecvent printre cercetările specialiştilor. Un motiv în plus să aloc<br />
un capitol aparte cercetării pon<strong>de</strong>rii în care reechilibrarea ocluzală intervine<br />
în rezolvarea malre<strong>la</strong>ţiilor cranio-mandibu<strong>la</strong>re, dacă aceasta poate fi soluţia<br />
unică pentru rezolvarea simptomelor, sau este necesară asocierea altor<br />
terapii. Multitudinea <strong>de</strong> posibilităţi terapeutice care stau <strong>la</strong> dispoziţia<br />
practicianului nu reprezintă neapărat un avantaj, ci mai <strong>de</strong>grabă o încercare<br />
în plus în alegerea celei mai bune soluţii. O trecere în revistă a posibilităţilor<br />
<strong>de</strong> terapie ocluzală, a importanţei lor şi sublinierea rezultatelor obţinute este<br />
scopul capitolului <strong>de</strong> faţă.<br />
MATERIAL ŞI METODĂ<br />
Studiul are <strong>la</strong> bază un lot <strong>de</strong> pacienţidin Baza Clinica <strong>de</strong> Invatamint<br />
a Facultatii <strong>de</strong> Medicina Dentara Iasi , care au beneficiat <strong>de</strong> tratament <strong>de</strong><br />
repoziţionare cranio-mandibu<strong>la</strong>ră în ultimii trei ani (2002-2004). Lotul<br />
numără 952 subiecţi din cei 1451 care au beneficiat <strong>de</strong> tratamente <strong>de</strong><br />
specialitate, 462 <strong>de</strong> bărbaţi şi 490 femei, cu vârste cuprinse între 13 şi 93 ani.<br />
Pentru colectarea datelor necesare am avut <strong>la</strong> dispoziţie fişele <strong>de</strong><br />
observaţie ale pacienţilor clinicii, care ne-au oferit rezultatele examenului<br />
clinic, examenelor paraclinice, diagnosticele complete, tratamentele<br />
efectuate, precum şi diagnosticul final. Totul s-a cules în nişte tabele special<br />
create care să urmărească toţi parametrii <strong>de</strong> interes pentru malre<strong>la</strong>ţiile craniomandibu<strong>la</strong>re.<br />
Datele culese au fost introduse în computer şi cu ajutorul<br />
programului „Statistica 2000” s-au prelucrat astfel încât să se poată extrage<br />
concluziile studiului.<br />
Am urmărit <strong>de</strong>pistarea pacienţilor care au fost supuşi procedurilor<br />
<strong>de</strong> terapie ocluzală, sub diversele ei forme, modul cum au reacţionat şi<br />
persistenţa rezultatelor în timp.<br />

REZULTATE ŞI DISCUŢII<br />
În cadrul oricărei terapii ocluzale este necesară o abordare sistematică,<br />
disciplinată a tratamentului, <strong>de</strong> fiecare dată urmărind o serie <strong>de</strong> obiective<br />
capitale:<br />
1. Obţinerea unei ocluzii centrice cu repere corecte,<br />
2. Ghidaj anterior optim,<br />
3. O înclinare a pantei retroincisive în armonie cu înclinarea pantelor<br />
condiliene temporale, înclinarea şi înălţimea cuspizilor, convexitatea,<br />
respectiv concavitatea curbelor <strong>de</strong> compensaţie, distanţa intercondiliană,<br />
diametrul transversal al arca<strong>de</strong>i <strong>de</strong>nto-alveo<strong>la</strong>re maxi<strong>la</strong>ră şi mandibu<strong>la</strong>ră, etc.<br />
(Huffman şi Regenos)<br />
4. Obţinerea şi menţinerea unei stabilităţi ocluzale statice şi dinamice,<br />
5. Amendarea simptomelor disfuncţionale asociate problemelor<br />
ocluzale (12).<br />
Pentru atingerea acestor obiective este necesară alegerea unei variante <strong>de</strong><br />
tratament potrivită care să se bazeze pe abordarea cea mai puţin distructivă<br />
pentru sistemul stomatognat. Finalul tratamentului trebuie să conducă <strong>la</strong><br />
amendarea tuturor semnelor disfuncţionale.<br />
1.Pentru atingerea primului obiectiv şi anume o ocluzie centrică cu toţi<br />
parametrii corecţi este necesară obţinerea unei distribuţii optime a forţelor<br />
exercitate prin contactele <strong>de</strong>nto-<strong>de</strong>ntare ocluzale. Astfel contactele din zona<br />
<strong>la</strong>terală trebuie să se producă într-un asemenea mod încât să ofere stabilitate<br />
atât în sens mezio-distal cât şi vestibulo-oral, fapt posibil datorită calităţii<br />
punctelor <strong>de</strong> contact interarcadic.<br />
2. Pentru obţinerea unui ghidaj anterior optim trebuie obligatoriu satisfăcut<br />
primul obiectiv ce va împiedica contactele puternice în zona anterioară a<br />
arca<strong>de</strong>i, care este mult mai vulnerabilă.<br />
Ghidajul anterior are rol fundamental în mişcarea <strong>de</strong> protruzie oferind<br />
realizarea unei mişcări <strong>de</strong> propulsie corecte, fără interferenţe pe partea<br />
nelucrătoare.<br />
3. Înclinarea pantei retroincisive nu trebuie să fie foarte restrictivă,<br />
astfel încât fricţiunea să fie cât mai scăzută. Înclinarea pantei retroincisive<br />
este dictată <strong>de</strong> fapt <strong>de</strong> înclinarea pantei articu<strong>la</strong>re, ele fiind armonice şi cu<br />
înălţimea cuspidiană, curbura sagitală şi înclinarea p<strong>la</strong>nului <strong>de</strong> ocluzie.<br />
Studiile au arătat că înclinarea pantei retroincisive trebuie să fie cu maxim 5<br />
gra<strong>de</strong> mai mare <strong>de</strong>cât a pantei articu<strong>la</strong>re.<br />
4. Stabilitatea ocluziei poate fi atinsă numai prin obţinerea unor<br />
contacte în zonele <strong>la</strong>terale ale arca<strong>de</strong>lor multiple, simultane, uniform<br />

distribuite, stabile, cu localizare precisă, realizate între suprafeţe nete<strong>de</strong> şi<br />
convexe, <strong>de</strong> tip cuspid-fosetă şi cuspid- ambrazură.<br />
5. În ceea ce priveşte obiectivul remiterii semnelor şi simptomelor<br />
disfuncţionale acest lucru va fi posibil după satisfacerea primelor patru<br />
obiective.<br />
Realizarea ghidajului anterior în armonie cu toţi parametrii ocluziei statice<br />
În cazul lotului <strong>de</strong> pacienţi cercetat s-a aplicat procedura <strong>de</strong> şlefuire<br />
selectivă, respectiv remo<strong>de</strong><strong>la</strong>re coronară atât pentru echilibrare ocluzală<br />
propriu-zisă, cât şi ca etapă preprotetică a tratamentului protetic. Răspunsul<br />
favorabil al pacienţilor <strong>la</strong> tratamentul <strong>de</strong> şlefuire selectivă vine să<br />
<strong>de</strong>monstreze necesitatea reechilibrării ocluzale post terapie ortodontică.<br />

CONCLUZII<br />
1. Terapia ocluzală are <strong>la</strong> bază o serie <strong>de</strong> obiective care trebuie să fie<br />
urmărite, astfel încât rezultatele să conducă <strong>la</strong> stabilitate şi funcţionalitate.<br />
2. Indicaţiile reechilibrării ocluzale sunt multiple cu implicaţii în<br />
terapia sindromului disfuncţional, ortodontică, protetică.<br />
3. I<strong>de</strong>al ar fi să se acor<strong>de</strong> un interval <strong>de</strong> timp (pretratament) ce să<br />
constea în îngrijire <strong>la</strong> domiciliu şi care să ofere confortul şi re<strong>la</strong>xarea<br />
componentei neuro-muscu<strong>la</strong>re, rezultând un sistem stomatognat apt să ofere<br />
informaţiile diagnostice necesare şi să permită transferul datelor pe<br />
simu<strong>la</strong>tor.<br />
4. Evi<strong>de</strong>nţierea contactelor premature şi a interferenţelor ocluzale este<br />
<strong>de</strong> o importanţă covârşitoare, ne<strong>de</strong>terminarea lor cu acurateţe conducând<br />
inevitabil <strong>la</strong> eşec.<br />
5. Instrumentarul folosit este variat şi a<strong>de</strong>cvat tehnicii permiţând<br />
obţinerea unor rezultate <strong>de</strong>osebite.<br />
6. Există o multitudine <strong>de</strong> meto<strong>de</strong> <strong>de</strong> şlefuire selectivă cu<br />
caracteristici care sugerează apartenenţa <strong>la</strong> un concept sau altul al ocluziei<br />
i<strong>de</strong>ale.<br />
7. Şcoa<strong>la</strong> ieşeană propune o metodă <strong>de</strong> şlefuire selectivă complexă,<br />
riguroasă, bazată pe mo<strong>de</strong>lul ocluziei naturale, aplicabilă tuturor problemelor<br />
ocluzale.<br />
8. Reechilibrarea ocluzală în ortodonţie este esenţială conferind<br />
stabilitate mai rapid, iar perioada <strong>de</strong> contenţie se va reduce.<br />
9. Terapia ocluzală în parodontologie se regăseşte ca etapă <strong>de</strong><br />
tratament propriu-zis, eliminând traumatismul ocluzal şi ajutând <strong>la</strong> reducerea<br />
mobilităţii <strong>de</strong>ntare.<br />
10. În tratamentul protetic reechilibrarea ocluzală se regăseşte atât în<br />
cadrul pregătirii pre- şi proprotetice, dar şi în etapa <strong>de</strong> adaptare imediată,<br />
primară, secundară a aparatelor gnatoprotetice conjuncte şi / sau adjuncte.<br />
11. Tratamentul <strong>de</strong> echilibrare a ocluziei nu este consi<strong>de</strong>rat ca încheiat<br />
niciodată, în cadrul dispensarizării pacientului trebuind să se continue<br />
menţinerea rezultatelor obţinute.<br />
12. Sănătatea orală nu se poate menţine fără i<strong>de</strong>ntificarea şi eliminarea<br />
problemelor ocluzale a căror prezenţă vor atenta <strong>la</strong> homeostazia sistemului<br />
stomatognat.<br />

STUDIU COMPARATIV PRIVIND DIFERITE METODE DE<br />
DEPISTARE<br />
A ECHILIBRULUI MUSCULO-LIGAMENTAR LA PACIENŢII<br />
EDENTAŢI PARŢIAL ÎNTINS, SUBTOTAL ŞI TOTAL<br />
SCOPUL CERCETĂRII<br />
Studiul <strong>de</strong> faţă este <strong>de</strong>osebit <strong>de</strong> vast şi are ca obiective principale urmărirea<br />
corelării etapelor p<strong>la</strong>nului <strong>de</strong> tratament protetic complex cu meto<strong>de</strong>le <strong>de</strong><br />
repoziţionare cranio-mandibu<strong>la</strong>ră.<br />
De asemenea am căutat să creionez locul şi rolul terapiei gnatoprotetice în<br />
schema <strong>de</strong> tratament a malre<strong>la</strong>ţiilor cranio-mandibu<strong>la</strong>re. Existenţa unei<br />
malre<strong>la</strong>ţii cranio-mandibu<strong>la</strong>re nu presupune neapărat şi tratament protetic,<br />
dar un tratament protetic implică într-o proporţie <strong>de</strong>stul <strong>de</strong> mare, poate chiar<br />
<strong>de</strong> 100%, aplicarea unei meto<strong>de</strong> repoziţionare cranio-mandibu<strong>la</strong>ră, chiar dacă<br />
aceasta are doar caracter profi<strong>la</strong>ctic.<br />

Am urmărit <strong>de</strong>pistarea pacienţilor care au fost supuşi tratamentelor<br />
protetice diverse, tipul <strong>de</strong> protezare realizat, modul cum s-a rezolvat<br />
malre<strong>la</strong>ţia diagnosticată şi ce s-a remarcat <strong>de</strong>osebit în perioada <strong>de</strong><br />
dispensarizare.<br />
REZULTATE ŞI DISCUŢII<br />
Tratamentele protetice posedă un algoritm clinic c<strong>la</strong>r, matematizat chiar,<br />
dar care în ace<strong>la</strong>şi timp <strong>la</strong>să loc individualizării fiecărui caz în parte, oferind<br />
pacientului stabilitate şi funcţionalitate optimă. Indiferent <strong>de</strong> tipul <strong>de</strong><br />
e<strong>de</strong>ntaţie diagnosticat etapele care trebuie parcurse sunt aceleaşi şi anume:<br />

• pregătirea organismului şi a cavităţii orale: care cuprin<strong>de</strong> o gamă <strong>de</strong><br />
subetape ca educaţia sanitară, pregătirea generală a organismului, pregătirea<br />
locală, numită şi periprotetica (=pre- şi pro-protetică)<br />
• tratamentul protetic propriu-zis: pregătirea substructurilor organice,<br />
amprentarea, înregistrarea re<strong>la</strong>ţiilor fundamentale cranio-mandibu<strong>la</strong>re şi<br />
transferul pe simu<strong>la</strong>tor, adaptarea aparatelor gnatoprotetice, fixarea,<br />
• tratamentul post-protetic: dispensarizarea.<br />
Fiecare din aceste etape sunt absolut indispensabile în realizarea unui<br />
tratament protetic <strong>de</strong> calitate şi cu rezultate a <strong>la</strong> long. Chiar dacă afirmaţia<br />
poate părea <strong>la</strong> prima ve<strong>de</strong>re puţin exagerată, omiterea unei etape poate duce<br />
<strong>la</strong> eşec imediat sau în timp. De asemenea în fiecare din etapele enumerate<br />
mai sus se poate realiza tratament <strong>de</strong> repoziţionare cranio-mandibu<strong>la</strong>ră.<br />
Evi<strong>de</strong>nt că nu <strong>la</strong> toţi pacienţii se va realiza repoziţionare în fiecare etapă <strong>de</strong><br />
tratament, dar măcar în câteva se pot regăsi elemente care se adresează<br />
malre<strong>la</strong>ţiei existente.<br />

CONCLUZII<br />
1. Tratamentul gnato-protetic complex este specific pacienţilor cu<br />
malre<strong>la</strong>ţii cranio-mandibu<strong>la</strong>re consecinţa unor e<strong>de</strong>ntaţii, dar poate reprezenta<br />
soluţia unică în cazul terapiei unor dizarmonii <strong>de</strong>nto-alveo<strong>la</strong>re primare sau<br />
secundare cu influenţe asupra poziţiei mandibulei faţă <strong>de</strong> craniu.<br />
2. Tratamentul <strong>de</strong> repoziţionare cranio-mandibu<strong>la</strong>ră se poate realiza în<br />
diferite etape ale tratamentului gnato-protetic pornind <strong>de</strong> <strong>la</strong> educaţia sanitară<br />
şi până <strong>la</strong> tratamentul protetic propriu-zis.<br />
3. În cazul asocierii diagnosticului <strong>de</strong> e<strong>de</strong>ntaţie cu cel <strong>de</strong> malre<strong>la</strong>ţie<br />
cranio-mandibu<strong>la</strong>ra este obligatorie protezarea provizorie, <strong>de</strong> temporizare şi<br />
respectiv <strong>de</strong> stabilizare.<br />
4. Nu se recurge <strong>la</strong> protezare <strong>de</strong> lungă durată până în momentul<br />
atingerii unui nivel <strong>de</strong> homeostazie sistemică optim.<br />
5. Tratamentul gnatoprotetic <strong>de</strong> lungă durată poate îmbraca una din<br />
formele: conjunctă, adjunctă, mixtă, hibridă, pe suport imp<strong>la</strong>ntar.<br />
6. Este obligatorie individualizarea fiecarui caz, selectând din schema<br />
propusă calea cea mai eficientă.<br />

CONCLUZII FINALE<br />
1. Există trei intervale <strong>de</strong> vârstă <strong>la</strong> care afectarea prin malre<strong>la</strong>ţii<br />
cranio-mandibu<strong>la</strong>re este mare: 20-25 ani, 55-60 ani, 70-80 ani, toate grupele<br />
fiind asociate cu perioada <strong>de</strong> formare a tinerilor, <strong>de</strong> stres maxim, cu apariţia<br />
e<strong>de</strong>ntaţiilor întinse, respectiv a e<strong>de</strong>ntaţiei totale, asociate cu malre<strong>la</strong>ţia.<br />
2. Se remarcă o uşoară predominenţă a sexului feminin, iar grupul <strong>de</strong><br />
pensionari este cel mai afectat atât datorită vârstei cât şi condiţiilor socioeconomice<br />
care nu permit un tratament a<strong>de</strong>cvat şi <strong>la</strong> timp a bolilor din sfera<br />
stomatologică.<br />
3. 11% din pacienţii lotului <strong>de</strong> studiu au recunoscut prezenţa stresului<br />
cotidian social şi familial,<br />
4. Bolile generale (neurologice, psihice, endocrine metabolice)<br />
influenţează <strong>factorii</strong> sistemici şi predominant pe cel muscu<strong>la</strong>r.<br />
5. Există factori etiologici sistemici care acţionează singu<strong>la</strong>r extrem<br />
<strong>de</strong> rar, efectul lor fiind rezultatul interesării lor simultane. Se<br />
individualizează <strong>factorii</strong> muscu<strong>la</strong>r, osos, articu<strong>la</strong>r, <strong>de</strong>ntar şi ocluzal,<br />
parodontal, funcţional.<br />
6. Semnele şi simptomele muscu<strong>la</strong>re sunt expresia c<strong>la</strong>ră a interesării<br />
factorului muscu<strong>la</strong>r; marcherul cel mai fi<strong>de</strong>l este reperul muscu<strong>la</strong>r al<br />
re<strong>la</strong>ţiilor <strong>de</strong> postură şi centrică, a căror valori modificate s-au regăsit în<br />
procente <strong>de</strong> 67,65%, respectiv 85, 30%).<br />
7. Factorul osos <strong>de</strong>vine etiologic singu<strong>la</strong>r, dar cel mai frecvent în<br />
asociere cu afecţiuni ale celor<strong>la</strong>lţi factori sistemici. Modificarea reperului<br />
osos al re<strong>la</strong>ţiilor fundamentale cranio-mandibu<strong>la</strong>re este în raport proporţional<br />
cu al factorului muscu<strong>la</strong>r.<br />
8. Patologiile ATM pot reprezenta cauza malre<strong>la</strong>ţiilor craniomandibu<strong>la</strong>re,<br />
constituind un factor etilogic articu<strong>la</strong>r. Normalitatea reperelor<br />
articu<strong>la</strong>re s-a constatat doar <strong>la</strong> un procent <strong>de</strong> 19, 85% din lot.<br />

9. Cele mai importante modificări se regăsesc <strong>la</strong> nivelul factorului<br />
<strong>de</strong>ntar şi ocluzal care cunoaşte şi cele mai frecvente şi variate interesări:<br />
abrazie <strong>de</strong>ntară, LOC cu <strong>de</strong>sfiinţarea contactelor ocluzale, obturaţii, aparate<br />
gnatoprotetice neadaptate ocluzal, migrări <strong>de</strong>ntare, dizarmonii <strong>de</strong>ntoalveo<strong>la</strong>re,<br />
e<strong>de</strong>ntaţia, tulburarea parametrilor ocluziei statice, a re<strong>la</strong>ţiilor<br />
interarcadice, a ocluziei dinamice, toate antrenând în timp, în cazul lipsei <strong>de</strong><br />
tratament, tulburarea şi a celor<strong>la</strong>lţi factori, în special muscu<strong>la</strong>r şi parodontal.<br />
Afectarea reperului <strong>de</strong>ntar al re<strong>la</strong>ţiilor fundamentale cranio-mandibu<strong>la</strong>re se<br />
regăseşte în proporţie <strong>de</strong> 81,8%.<br />
10. S-a evi<strong>de</strong>nţiat şi factorul etiologic funcţional care nu acţionează<br />
singu<strong>la</strong>r, ci <strong>de</strong> cele mai multe ori este consecinţa celor<strong>la</strong>lţi factori, dar are rol<br />
în accentuarea lor. Afectarea uneia sau mai multor funcţii s-a produs într-un<br />
procent surprinzător <strong>de</strong> mare (97.06%),semnal <strong>de</strong> a<strong>la</strong>rmă privind tratamentul<br />
optim al malre<strong>la</strong>ţiilor.<br />
11. Malre<strong>la</strong>ţiile cranio-mandibu<strong>la</strong>re prezintă numeroase forme clinice,<br />
organizate într-o c<strong>la</strong>sificare riguroasă, diagnosticul reprezentând o<br />
combinaţie a acestor forme clinice. Cele mai frecvente combinaţii sunt cele<br />
dintre malre<strong>la</strong>ţiile statice extraposturale prin rotaţia mandibulei în p<strong>la</strong>n<br />
sagital cu scă<strong>de</strong>rea dimensiunii etajului inferior şi excentrică mixtă,<br />
malre<strong>la</strong>ţii dinamice fără contact <strong>de</strong>ntar în mişcarea <strong>de</strong> <strong>la</strong>teralitate şi cu<br />
afectarea funcţiilor.<br />
12. Tratamentul trebuie început în prezenţa semnelor şi simptomelor<br />
specifice, fiind caracterizat prin complexitate, abordare simultană a tuturor<br />
factorilor interesaţi, rigurozitate, cooperare interdisciplinară în cele mai<br />
multe cazuri (gnatolog, protetician, chirurg, ortodont, ORL-ist)<br />
13. În consecinţă tratamentul trebuie să cunoască un profund caracter<br />
etiologic, completat <strong>de</strong> terapia simptomatică.<br />
14. Terapia ocluzală are <strong>la</strong> bază o serie <strong>de</strong> obiective care trebuie să fie<br />
urmărite, astfel încât rezultatele să conducă <strong>la</strong> stabilitate şi funcţionalitate.<br />
15. Indicaţiile reechilibrării ocluzale sunt multiple cu implicaţii în<br />
terapia sindromului disfuncţional, ortodontică, protetică.<br />

16. I<strong>de</strong>al ar fi să se acor<strong>de</strong> un interval <strong>de</strong> timp (pretratament) ce să<br />
constea în îngrijire <strong>la</strong> domiciliu şi care să ofere confortul şi re<strong>la</strong>xarea<br />
componentei neuro-muscu<strong>la</strong>re, rezultând un sistem stomatognat apt să ofere<br />
informaţiile diagnostice necesare şi să permită transferul datelor pe<br />
simu<strong>la</strong>tor.<br />
17. Evi<strong>de</strong>nţierea contactelor premature şi a interferenţelor ocluzale este<br />
<strong>de</strong> o importanţă covârşitoare, ne<strong>de</strong>terminarea lor cu acurateţe conducând<br />
inevitabil <strong>la</strong> eşec.<br />
18. Instrumentarul folosit este variat şi a<strong>de</strong>cvat tehnicii permiţând<br />
obţinerea unor rezultate <strong>de</strong>osebite.<br />
19. Există o multitudine <strong>de</strong> meto<strong>de</strong> <strong>de</strong> şlefuire selectivă cu<br />
caracteristici care sugerează apartenenţa <strong>la</strong> un concept sau altul al ocluziei<br />
i<strong>de</strong>ale.<br />
20. Şcoa<strong>la</strong> ieşeană propune o metodă <strong>de</strong> şlefuire selectivă complexă,<br />
riguroasă, bazată pe mo<strong>de</strong>lul ocluziei naturale, aplicabilă tuturor problemelor<br />
ocluzale.<br />
21. Reechilibrarea ocluzală în ortodonţie este esenţială conferind<br />
stabilitate mai rapid, iar perioada <strong>de</strong> contenţie se va reduce.<br />
22. Terapia ocluzală în parodontologie se regăseşte ca etapă <strong>de</strong><br />
tratament propriu-zis, eliminând traumatismul ocluzal şi ajutând <strong>la</strong> reducerea<br />
mobilităţii <strong>de</strong>ntare.<br />
23. În tratamentul protetic reechilibrarea ocluzală se regăseşte atât în<br />
cadrul pregătirii pre- şi proprotetice, dar şi în etapa <strong>de</strong> adaptare imediată,<br />
primară, secundară a aparatelor gnatoprotetice conjuncte şi / sau adjuncte.<br />
24. Tratamentul <strong>de</strong> echilibrare a ocluziei nu este consi<strong>de</strong>rat ca încheiat<br />
niciodată, în cadrul dispensarizării pacientului trebuind să se continue<br />
menţinerea rezultatelor obţinute.<br />
25. Sănătatea orală nu se poate menţine fără i<strong>de</strong>ntificarea şi eliminarea<br />
problemelor ocluzale a căror prezenţă vor atenta <strong>la</strong> homeostazia sistemului<br />
stomatognat.<br />

26. Tratamentul trebuie să fie în primul rând etiologic, ceea ce<br />
presupune o profundă cunoaştere a patologiilor componentei neuromuscu<strong>la</strong>re<br />
a sistemului stomatognat.<br />
27. Este indispensabilă asocierea unei terapii <strong>de</strong> <strong>de</strong>stresare, <strong>de</strong><br />
conştientizare a pacientului asupra bolii sale, <strong>de</strong> evitare a mişcărilor care<br />
provoacă simptomele, toate acestea regăsindu-se într-un pachet <strong>de</strong> “indicaţii”<br />
pe care l-am propus şi a cărui eficacitate am putut să o verific în timp.<br />
28. Tratamentul etiologic al componentei neuro-muscu<strong>la</strong>re constă în<br />
meto<strong>de</strong> <strong>de</strong> re<strong>la</strong>xare substitutivă, activă, BFB-EMG, intercepţie ocluzală<br />
pentru re<strong>la</strong>xare muscu<strong>la</strong>ră.<br />
29. Concluzia pertinentă pe care am <strong>de</strong>sprins-o a fost aceea că ar fi<br />
i<strong>de</strong>ală aplicarea gutierelor <strong>de</strong> re<strong>la</strong>xare muscu<strong>la</strong>ră <strong>la</strong> toţi pacienţii cu malre<strong>la</strong>ţii<br />
cranio-mandibu<strong>la</strong>re, <strong>de</strong>oarece doar în condiţiile unei re<strong>la</strong>xări muscu<strong>la</strong>re se<br />
poate vorbi <strong>de</strong> terapie stomatologică <strong>de</strong> calitate.<br />
30. Terapia simptomatică poate reprezenta uneori unica modalitate<br />
(neradicală) <strong>de</strong> conducere a unui tratament , ea constând în proceduri<br />
balneofizioterapice şi medicamentoase a căror rezultate sunt incontestabile.<br />
31. Tratamentul componentei neuro-muscu<strong>la</strong>re se va asocia în<br />
majoritatea cazurilor cu terapii specifice celor<strong>la</strong>lte elemente ale sistemului<br />
stomatognat.<br />
32. Reducerea <strong>de</strong>zordinilor ATM este tot<strong>de</strong>auna legată <strong>de</strong> modificările<br />
şi patologia articu<strong>la</strong>ră pe care o prezintăpacienţii cu malre<strong>la</strong>ţii craniomandibu<strong>la</strong>re.<br />
33. Tratamentul obligă <strong>la</strong> co<strong>la</strong>borare interdisciplinară în special cu<br />
medicul reumatolog, chirurg, ORL-ist, .<br />
34. Frecvenţa modificărilor ATM este <strong>de</strong> 97%, fapt ce impune o<br />
pregătire a specialiştilor înacest domeniu.<br />
35. Schema <strong>de</strong> tratament propusă este re<strong>la</strong>tiv simplă, dar presupune un<br />
diagnostic diferenţial atent astfel încât să fie a<strong>de</strong>cvată patologiei prezentate.<br />

36. Eficacitatea schemelor <strong>de</strong> tratament atât pentru afectarea<br />
componentei neuro-muscu<strong>la</strong>re cât şi <strong>de</strong>zordinile ATM au fost <strong>de</strong>monstrate în<br />
timp <strong>de</strong> procentul mic al recidivei semnelor şi simptomelor malre<strong>la</strong>ţiei<br />
cranio-mandibu<strong>la</strong>re.<br />
37. Tratamentul gnato-protetic complex este specific pacientilor cu<br />
malre<strong>la</strong>tii cranio-mandibu<strong>la</strong>re consecinta unor e<strong>de</strong>ntatii, dar poate reprezenta<br />
solutia unica in cazul terapiei unor dizarmonii <strong>de</strong>nto-alveo<strong>la</strong>re primare sau<br />
secundare cu influente asupra pozitiei mandibulei fata <strong>de</strong> craniu.<br />
38. Tratamentul <strong>de</strong> repozitionare cranio-mandibu<strong>la</strong>ra se poate realiza in<br />
diferite etape ale tratamentului gnato-protetic pornind <strong>de</strong> <strong>la</strong> educatia sanitara<br />
si pana <strong>la</strong> tratamentul protetic propriu-zis.<br />
