1733 cazuri <strong>de</strong> cancere digestive in diferite stadii, pentru care<br />
există stocate: țesuturi, flui<strong>de</strong> biologice și informații clinice<br />
corelate (aproximativ 22 000 <strong>de</strong> esantioane). Biobanca mai<br />
cuprin<strong>de</strong> o colecție <strong>de</strong> blocuri <strong>de</strong> parafină, lame histologice și<br />
imagini histologice reprezentative pentru diagnostic ale<br />
cazurilor incluse. Pentru evi<strong>de</strong>nța cazurilor bancate sunt concepute<br />
și utilizate fișiere Excel un<strong>de</strong> informațiile sunt codificate și<br />
anonimizate. Fiecare eșantion (caz) bancat este asociat cu informații<br />
clinice complete, inclusiv diagnostic histopatologic.<br />
Implementarea <strong>de</strong> noi tehnologii mo<strong>de</strong>rne <strong>de</strong> biologie<br />
moleculară în afecțiunile gastrointestinale reprezintă unul din<br />
obiectivele principale ale acestei direcții <strong>de</strong> cercetare.<br />
Un alt obiectiv pe termen lung va fi acela <strong>de</strong> a i<strong>de</strong>ntifica și<br />
caracteriza un panel <strong>de</strong> markeri care va eficientiza strategiile <strong>de</strong><br />
<strong>de</strong>tecţie în stadii incipiente a neoplaziilor pancreatice, hepatice,<br />
colon și rect. Transpunerea în aplicaţii medicale, prin aplicarea<br />
meto<strong>de</strong>lor mo<strong>de</strong>rne <strong>de</strong> investigare cu orientare terapeutică țintită<br />
în tumorile digestive va duce la creșterea posibilității<br />
aplicării unui tratament <strong>de</strong> înaltă specificitate.<br />
Cercetările cele mai relevante efectuate până în prezent <strong>de</strong><br />
către echipa CCGTH au fost în domeniul neoplasmului <strong>de</strong> pancreas.<br />
Actualmente nu există meto<strong>de</strong> screening standardizate<br />
pentru neoplasmul <strong>de</strong> pancreas și nici biomarkeri specifici,<br />
care să realizeze diferențierea între neoplasmul pancreatic și alte<br />
afecțiuni majore pancreatice, cum ar fi pancreatita cronică.<br />
Rezultatele preliminare obținute în cadrul proiectului<br />
CEEX 56/2005, Genopact (o valoare <strong>de</strong> finanțare pentru<br />
CCGTH <strong>de</strong> 740000 Ron) arată că genele i<strong>de</strong>ntificate prin<br />
tehnica <strong>de</strong> microarray au un nivel <strong>de</strong> expresie semnificativ<br />
diferit între a<strong>de</strong>nocarcinom pancreatic și țesut normal. Deoarece<br />
multe din genele i<strong>de</strong>ntificate <strong>de</strong> noi sunt implicate în procesul <strong>de</strong><br />
fibroză este foarte importantă diferențierea acestora <strong>de</strong> genele<br />
implicate în carcinogeneza pancreatică. Proiectul MOLPANC-<br />
PNII 41-024 (o valoare <strong>de</strong> finanțare pentru CCGTH <strong>de</strong> 800 000<br />
Ron) își propune o comparare a datelor <strong>de</strong> microarray pe un lot<br />
<strong>de</strong> pacienți cu pancreatită cronică cu cele pentru a<strong>de</strong>nocarcinomul<br />
pancreatic obținute anterior pentru cancerul <strong>de</strong> pancreas și<br />
obținerea unui set <strong>de</strong> gene specifice pentru carcinogeneza pancreatică.<br />
După analiza căilor <strong>de</strong> semnalizare implicate vor fi selectate<br />
proteine cheie, care vor fi testate ulterior prin meto<strong>de</strong><br />
diferite <strong>de</strong> fosforilare. Activitățile <strong>de</strong>sfășurate în parteneriat (Institutul<br />
Național <strong>de</strong> Cercetare și Dezvoltare în Informatică, Institutul<br />
Național <strong>de</strong> Cercetare-Dezvoltare pentru Microbiologie<br />
și Imunologie “Cantacuzino”, SC Rntech, SC Genetic Lab) în<br />
cadrul acestui proiect vor duce la consolidarea unui grup interdisciplinar<br />
ce va folosi tehnici avansate <strong>de</strong> genomică și proteomică:<br />
microarray, phosphoproteomica, siARN pentru gene<br />
candidat și analiza bioinformatică integrată complexă.<br />
Într-un consorțiu constituit în cadrul proiectului CEEX<br />
62/2006 (Profile Genice Induse <strong>de</strong> Supresia Transcripțională a<br />
Ets 1 în Cancerul <strong>de</strong> Pancreas - cu o valoare <strong>de</strong> finanțare pentru<br />
CCGTH <strong>de</strong> 500 000 Ron) cu o echipă <strong>de</strong> la UMF Iași, coordonată<br />
<strong>de</strong> către dr Lefter Liviu s-a urmărit <strong>de</strong>monstrarea<br />
potenţialului Ets-1 ca agent antiangiogenic și i<strong>de</strong>ntificarea<br />
mecanismului specific prin care Ets1 poate fi manipulat în cancerul<br />
pancreatic. Rezultatele acestui studiu s-au concretizat prin<br />
created a collection of more than 1733 cases of GI cancers in<br />
different stages, for which are stored: tissues, biological fluids<br />
and clinical-related information(approximately 22 000 samples).<br />
The biobank also inclu<strong>de</strong>s a collection of paraffin blocks, histological<br />
smears and images representative of the histological diagnosis<br />
of all inclu<strong>de</strong>d cases.<br />
Excel files are <strong>de</strong>signed and used for the bank records and<br />
the information is encrypted and anonymous. Each sample<br />
(case) is associated with complete clinical information, including<br />
histopathological diagnosis.<br />
Implementation of new technologies in molecular biology<br />
of GI diseases is one of the main directions of this research.<br />
Another long-term objective will be to i<strong>de</strong>ntify and characterize<br />
a panel of markers that will improve <strong>de</strong>tection strategies<br />
in early stages of pancreatic, liver, colon and rectum neoplasia.<br />
The transposition in medical applications, by applying mo<strong>de</strong>rn<br />
methods of investigation with therapeutic targeted guidance in<br />
GI tumors will increase the possibility of applying a treatment<br />
of high specificity.<br />
The most relevant research conducted so far by the team of<br />
CGSLT has been in pancreas neoplasm field.<br />
There are currently no effective biomarkers useful for early<br />
<strong>de</strong>tection of pancreatic cancer or even to differentiate between<br />

pancreatic a<strong>de</strong>nocarcinoma and another major pancreatic disease,<br />
chronic pancreatitis. The preliminary results of the<br />
GENOPACT project, CEEX 56 /2006 (CCGTH budget of<br />
740000 Ron) showed genes whose expression levels differ significantly<br />
between PDAC and normal pancreatic tissue. Since<br />
many of these genes are involved in the fibrotic response, it is<br />
crucial to be able to separate the genes involved in more benign<br />
fibrosis also encountered in CP from the genes implicated in carcinogenesis.<br />
The aim of MOLPANC, PNII 41-024 (CCGTH<br />
total budget of 800000) project is to compare microarrays data<br />
from a group of patients with chronic pancreatitis with those<br />
from a group of patients with pancreatic a<strong>de</strong>nocarcinoma and to<br />
provi<strong>de</strong> specific genes for pancreatic carcinogenesis. Key-proteins<br />
will be selected after the analysis of the involved signal<br />
pathway, which will be tested by different way of phosphorilation.<br />
The activity from this project will lead to the build-up of a<br />
multidisciplinary medical approach (partners: (Institutul National<br />
<strong>de</strong> Cercetare si Dezvoltare in Informatica, Institutul National<br />
<strong>de</strong> Cercetare-Dezvoltare pentru Microbiologie si<br />
Imunologie “Cantacuzino”, SC Rntech, SC Genetic Lab) and<br />
the use of advanced techniques of genomic and proteomic such<br />
as: microarray, phosphoproteomica, and RNA for the candidate<br />
genes and complex integrated bioinformatical analysis.<br />
In the consortium formed in the project CEEX 62/2006<br />
(gene profiles induced by transcriptional suppression of Ets 1 in<br />
pancreatic cancer - with a funding value of 500 000 Ron for<br />
CGSLT)in cooperation with a team from UMF Iasi, coordinated<br />
by Dr Liviu Lefter we <strong>de</strong>monstrate the potential Ets-1 as antiangiogenic<br />
agents and i<strong>de</strong>ntify the specific mechanism by<br />
which Ets1 can be manipulated in pancreatic cancer. The results<br />
of this study have been published in 2009 in Cancer Gene Therapy:<br />
