Popa” Iaşi Rezumat Episodul <strong>de</strong>presiv din cadrul tulburării afective bipolare reprezintă un domeniu al psihiatriei, încă, insuficient cunoscut şi cercetat, <strong>de</strong>şi afectează 1-5% din populaţie. Pacienţii bipolari, care experimentează un episod maniacal sunt mult mai frecvent spitalizaţi, <strong>de</strong>cât cei cu episod <strong>de</strong>presiv, <strong>de</strong>oarece simptomatologia este mult mai zgomotoasă. Astfel, componenta <strong>de</strong>presivă, cronică a tulburării afective bipolare este neglijată, <strong>de</strong>şi ea reprezintă, în general, prima manifestare a bolii, fiind asociată cu importante disfuncţionalităţi socio-familiale pe termen lung, precum şi cu un risc suicidar crescut. În plus, costurile pentru tulburarea afectivă bipolară şi, în special, pentru <strong>de</strong>presia bipolară, precum şi cele pentru dizabilităţile pe care le <strong>de</strong>termină, fac din această tulburare psihică una dintre cele mai costisitoare boli, costuri <strong>de</strong>păşite doar <strong>de</strong> cele pentru schiz<strong>of</strong>renie. Opţiunile terapeutice pentru <strong>de</strong>presia bipolară, conform rezultatelor <strong>of</strong>erite <strong>de</strong> trialuri clinice recente, au <strong>de</strong>monstrat eficacitatea Lamotriginei, Olanzapinei sau a asocierii SSRI cu antipsihoticele atipice (ex.Olanzapină-Fluoxetină) ca primă linie terapeutică, alături <strong>de</strong> Litiu şi, <strong>de</strong> asemenea, au permis actualizarea algoritmului terapeutic anterior. Astfel, este necesară o revizuire a posibilităţilor terapeutice ale <strong>de</strong>presiei bipolare, pentru a se asigura o mai bună îngrijire a pacienţilor, în condiţiile în care psihiatrii vor putea face alegeri terapeutice a<strong>de</strong>cvate. THERAPEUTIC MANAGEMENT OF THE DEPRESSIVE EPISODE IN BIPOLAR AFFECTIVE DISORDER Abstract Bipolar <strong>de</strong>pression represents a neglected area <strong>of</strong> psychiatry, <strong>de</strong>spite the fact that it affects 1-5% <strong>of</strong> general population. The acute presentation <strong>of</strong> manic episo<strong>de</strong>s in bipolar disor<strong>de</strong>r, with frequent hospitalization, has diverted attention away from the <strong>de</strong>pressive component <strong>of</strong> the disease, which is associated with long-term social dysfunction and a high risk <strong>of</strong> suici<strong>de</strong>. The costs <strong>of</strong> both the illness and its associated disability rank bipolar disor<strong>de</strong>r among the most expansive illness, together with schizophrenia. Nowadays, much research can improve the diagnosis and can i<strong>de</strong>ntify efficacious treatment for bipolar <strong>de</strong>pression, which is still un<strong>de</strong>rdiagnosed and un<strong>de</strong>rtreated. Until recently, the treatment <strong>of</strong> bipolar disor<strong>de</strong>r focused on the control <strong>of</strong> mania, whereas the symptoms <strong>of</strong> <strong>de</strong>pression are the prime clinical symptoms in bipolar disor<strong>de</strong>r and the treatment options are limited.
