CHECKLIST OF DOCUMENTS FOR RENEWAL OF OCI
STICKER/BOOKLET /CHANGE OF PARTICULAR ON FOREIGN PASSPORT
Online OCI Regestration application form to be filled in English. Completed form to be
submitted along with documents.
Please fill the online application form completely, as per the instruction on the form
(Part A & Part B).
The requisite amount of OCI, ICWF Fee & VFS Service charges to be paid.
Photographs - 4 Four recent color passport size(about 51 mm x 51mm) photograph,
showing full frontal view of face against a white background, 01 photograph to be
pasted on the application form & other 03 to be attached with the PIO form.
Copy of old foreign passport.
New foreign passport (valid for 180 days) in original, along with a photocopy.
Original OCI card along with the photocopy (front & back).
Old original OCI sticker along with the photocopy.
Original Uitterksel issued by respective Gemeente in Netherlands for residence proof,
date of Dutch nationality & showing previously nationality as Indian.
Renunciation/ Surrender cetificate of Old Indian Passport from Indian Embassy
should be submitted in Original with a photocopy. Please get the certificate from
Indian Embassy before applying at India Application Centre.OR Declaration
statment(affidavit) in english declaring & affirming about the surender of passport to the
Indian Embassy
NON Dutch national who are resident for more than 01 YEAR are applicable & can submit
the below additional documents

Telex form without fee to be submitted.
Original Uitterksel issued by the respective Gemeente in the country of origin showing
date of becoming the national of respective country & showing previously nationality as
Indian
Original Uitterksel issued by respective Gemeente in Netherlands for residence proof.
Above requirements are only indicative and attempt has been made to cover requirements for a
major section of applicants. However, the Consular Officer may ask for additional documents and
/or could call the applicant for an interview also after examining the application for issuance of
OCI Card
In special cases when the name of the applicant is changed. A certificate from Gemeente in
Netherlands is needed.
