Ufrivillig vekttap over en periode på 3-6 mnd er en mer akutt risikofaktor for underernæring enn KMI Hvis vekten ikke er journalført, spør pasienten hva han/hun veide før de ble syke, eller for 3-6 mnd siden. Sammenlign dette med aktuell vekt. Spørsmål 3. (vil situasjonen forverres?) Spist lite: JA/NEI. Har pasienten spist mindre enn normalt de siste dagene/ukene før sykehusinnleggelsen? Har pasienten kostrestriksjoner som medfører et ensidig kosthold? Har pasienten svelgproblemer? Har pasienten redusert appetitt? Spørsmål 4. (vil sykdomsprosessen akselerere situasjonen?) Alvorlig syk: JA/NEI Har pasienten en økt stressmetabolisme i forbindelse med aktuell sykdom (traume, intensivpasient). Resultat av innledende kartlegging: Dersom svaret er JA på ett eller flere spørsmål, gå videre til hovedvurdering Dersom svaret er NEI på alle spørsmål skal man ikke sette i gang noen ernæringstiltak. Den innledende vurderingen repeteres om en uke dersom pasienten fremdeles er inneliggende. Gjennomføres hos pasienter som fyller minst ett av kriteriene i innledende kartlegging. Hovedscreening - vurdering av risikograd Score Ernæringstilstand Score Sykdommens alvorlighetsgrad 0 Normal ernæringstilstand 0 Ikke syk 1 Vekttap 5-10% siste 3 mnd. og/eller Matinntak 50-75% av behov i mer enn en uke 2 Vekttap > 5% siste 2 mnd. eller BMI= 18.5-20.5 + redusert allmenntilstand eller Matinntak 25-50% av behov i mer enn en uke. 3 Vekttap > 5% siste måned eller BMI < 18.5 + redusert allmenntilstand eller Matinntak 0-25% av behov siste uke. 1 En pasient med kronisk sykdom eller en pasient som har gjennomgått et mindre kirurgisk inngrep. Leverchirrose, nyresvikt, kronisk lungesykdom, kreftpasienter, pasienter med collum femoris fraktur, etter cholecystectomi og laparaskopiske operasjoner. 2 En pasient med tydelig redusert allmenntilstand pga sin sykdom. Alvorlig pneumoni, inflammatorisk tarmsykdom med feber, akutt nyresvikt, større kirurgiske inngrep som kolektomi og gastrektomi, ileus, anastomoselekkasje og gjentatte operasjoner. 3 En pasient som er alvorlig syk.
