OPLEIDING The ESBGH Knowledge Based Examination The European Section and Board of Gastroenterology and Hepatology (ESBGH) examination has run annually since its start in 2013, with a total of 375 entries to date. In 2017, 133 candidates entered the examination. In Switzerland and the Netherlands it has become mandatory to participate. This article describes how the ESBGH and its partners, the Federation of UK Medical Royal Colleges and the British Society of Gastroenterology, ensure the high quality of this validated examination. The Format The test consists of 200 questions to be taken in two three-hour long sessions. The questions are in multiple-choice format. This form assesses the candidates’ ability to interpret information and to solve clinical problems, in addition to core knowledge and comprehension. There are always five options: one correct answer and four alternatives. The four distractors are closely related to the preferred option but less accurate, therefore acting as plausible alternatives. Sample questions are available on the EBGHE website http://www.ebghe.eu. The examination is delivered simultaneously in multiple venues throughout Europe. Validation Examiners are recruited into a question writing group, a standard setting group and an examining board. The application window will be open from December 27th 2017– January 24th 2018. Results will be available 4 weeks after the examination date and Certificates will be posted in mid-June 2018. to the ESBGH. The examiners are all experienced Trainers and Examiners, trained in equality and diversity, have understanding of and experience with the statistical methods used in the examination. Optimally, they work in centres where appraisal is used in quality improvement and have active training roles. Finally, they are fellows of the ESBGH. All question writers must attend a training workshop before they join the question writing group. Question writing meetings are held annually, and here all new questions are peer reviewed before use. Standard Setting Group (SSG) members are trained by the Chair of the group and provided with a handbook which addresses the aims and objectives of the examination and the SSG. They also get instructed on item response theory, the Angoff and Hofstee methods to set the pass mark, and the concept of the just passing candidate. Examining Board members receive written guidance and are paired with more expe rienced colleagues. In all cases, new examiners are invited to attend an induction session which provides an overview of all the pass mark setting techniques. The current European representatives have built up their knowledge of these systems through their involvement over the past five years. Succession planning ensures that board and committee membership is refreshed over time, without impacting the stability and consistency of the group. Examiners are surveyed yearly and asked for confirmation of still meeting the criteria. Who should take the Examination? The ESBGH exam is centred on the Blue Book curriculum but will test a wide range of knowledge in gastroenterology and hepatology. Prospective candidates are expected to be widely read, covering gastroenterology and hepatology text books, guidelines and journals. Attending international and national conferences and speciality meetings with academic content are also encouraged. Knowledge of current European Guidelines and up-to-date recommendations for common GI and liver diseases will be helpful to the candidate. It is recommended that candidates have completed at least three years of specialist GI training before they take the examination. Pass mark The pass mark is set each year by the SSG. The pass rate of trainees in Europe is 64%. The best and worst subject areas are Pancreatic Disease (73% correct) and Nutrition (50% correct). Conclusion The ESBGH knowledge based examination is a mature, high quality, validated examination, which provides a reliable benchmark of the candidate’s knowledge in Gastroenterology and Hepatology.
