510toepassing van ketamine dient ook in een kinderoncologische afdeling rekeninggehouden te worden met een prikkelarme omgeving rondom procedure en uitslapen.- De ketamine waarvan hierboven sprake, is – tenzij anders vermeld – racemischeketamine. In Nederland is racemische Ketamine niet meer verkrijgbaar, maar wordt demeer potente isomeer S-ketaminehydrochloride (ook wel Ketamine-S, merknaamKetanest) gebruikt. Hoewel niet volledig bewezen, wordt bij gebruik van S-ketaminehydrochloride de helft van de dosering van de racemische Ketamineaangehouden.Aanbevelingen1. Gezien de aanwijzingen die bestaan dat pijnervaringen bijdragen aan negatievepsychische late effecten van de behandeling bij overlevenden van kinderkanker, moettijdens de behandeling maximaal geïnvesteerd worden in een effectieve procedurelepijn- en angstbestrijding die rekening houdt met de individuele wensen en behoeftenvan de patiënt.2. Pijnlijke kinderoncologische procedures (zoals lumbaal punctie, beenmergpunctie enbotbiopsie) dienen bij voorkeur te gebeuren onder algehele anesthesie of onder eendiepe PSA. Indien gekozen wordt voor diepe PSA dan gaat de voorkeur uit naarkrachtige middelen met een hoge mate van effectiviteit (zoals propofol of ketamine)zodat er geen noodzaak bestaat tot extra stevige fixatie of immobilisatie van depatiënt. Bij voorkeur wordt geen gebruik gemaakt van een PSA met eenbenzodiazepine, al dan niet in combinatie met lokale anesthesie, omdat hiermee in demeeste gevallen geen optimaal comfort kan worden gegarandeerd.In individuele gevallen kan van deze aanbeveling worden afgeweken waarbij deprocedure verricht wordt onder lokale anesthesie, eventueel aangevuld met een lichtevorm van sedatie (bijvoorbeeld een benzodiazepine of lachgas). Daarbij is het welnoodzakelijk dat:a. Er een volledige informed consent is van de patiënt en de patiënt de voorkeurgeeft aan deze manier van werken.b. Er tijdens de procedure geen gebruik dient te worden gemaakt van geforceerdeimmobilisatie of fixatie (restraint).3. Indien gekozen wordt voor PSA dan:a. Dient voldaan te worden aan de randvoorwaarden ten aanzien vanpatiëntenselectie, de keuze van middelen, skills, competenties, monitoring enoverige randvoorwaarden die in subdeel I van deze richtlijn zijn verwoord.b. Dient maximaal gebruik te worden gemaakt van lokale anesthesie van deprikplaats (bijvoorbeeld met een verdovende creme).c. Dient standaard gebruik te worden gemaakt van niet-farmacologischetechnieken (informatie, voorbereiding, afleiding, gedragsgerichte therapie), zoalsdie in subdeel I van deze richtlijn worden beschreven.4. Lokale anesthesie in de vorm van bijvoorbeeld Emla® of Rapydan® dient bij hetaanprikken van de port-a-cath® te worden toegepast ter verdoving van de huid.5.
