S c r e e n i n gNiveau 4Niveau 4Niveau 4Niveau 4Verwijzing naar oogarts binnen een maand bij proliferatieve retinopathie,bij macula-oedeem of harde exsudaten binnen één papildiameter van defovea en bij onverklaarbare visusdaling of onverklaarbare bevindingen.Verwijzing naar oogarts binnen een week bij vaatnieuwvormingen,preretinale of glasvochtbloeding en bij rubeosis iridis.Verwijzing naar oogarts binnen een dag bij plotselinge visusdaling ofbij gezichtsveldbeperking.Verwijzing naar oogarts ook geïndiceerd in geval van cataract.Overige overwegingen• In hoofdstuk 3 wordt de literatuur met betrekking tot de aanwezigheid van retinopathieals risicofactor voor het optreden van visusbedreigende retinopathie besproken; opbasis daarvan kan worden gesteld dat er aanwijzingen zijn voor progressie, mogelijkbij meer dan enkele microaneurysmata. Bij enkele microaneurysmata is nog geenoogheelkundige behandeling nodig, maar dienen de risicofactoren voor retinopathiezoveel mogelijk te worden behandeld. Patiënten met afwijkingen – gevonden bij fundusfotografieof funduscopie dienen naar de oogarts te worden verwezen (afspraken LPO).* Visusklachten, bijvoorbeeld floaters, komen voor bij patiënten met een goede visus,ondanks een preretinale bloeding: in dergelijke gevallen en bij door de huisarts nietverklaarbare afwijkingen of visusklachten, is verwijzing raadzaam.• De werkgroep is van mening dat niet alleen bij cataract maar ook bij andere storendemediatroebelingen beoordeling van de fundus door de oogarts dient te geschieden.• Oogonderzoek bij kinderen (en pubers) en bij zwangeren bij voorkeur door deoogarts.• Ook bij een moeilijk of niet te beoordelen screening en bij bekende oogheelkundigecomorbiditeiten gaat de voorkeur van de werkgroep uit naar onderzoek door de oogarts.Aanbevelingen4.10 Verwijs naar een oogarts bij enige vorm van diabetische retinopathie,wanneer screening niet goed uitvoerbaar is en bij niet verklaarbare visusklachten.Spoedverwijzing bij plotselinge visusdaling of gezichtsveldbeperking.4.11 Kinderen, zwangeren en patiënten met diabetes mellitus met bekendeoogheelkundige comorbiditeiten dienen naar de oogarts te worden verwezen,waarbij in acht dient te worden genomen dat zwangeren elke drie maandendienen te worden gezien.53
