als bij de voorkomende comorbide psychopathologie. In de meeste gevallen ligt de
combinatie van verschillende interventies voor de hand.
Samenvattend kan, over de psychologische behandeling van PTSS, het volgende worden
gezegd:
Gelet op de bestaande evidentie verdient het zonder meer de voorkeur te behandelen
met IE ofwel EMDR.
De keuze te starten voor IE dan wel EMDR zal in de praktijk vooral worden gemaakt
op basis van de beschikbare kennis en kunde bij een individuele therapeut of de desbetreffende
instelling.
Het feit dat EMDR een relatief sterk non-verbaal karakter heeft, doet vermoeden dat
bij de behandeling van getraumatiseerde vluchtelingen het beste kan worden gestart
met EMDR.
Bedacht moet worden dat PTSS een hoge mate van co-morbiditeit kent en in verschillende
gradaties van complexiteit voorkomt. Behalve in gevallen van ‘schone’ PTSS ten
gevolge van type 1-trauma zullen interventies als CGT en EMDR altijd (onder)deel
moeten uitmaken van een bredere, veelal gefaseerde behandeling.

8.3 Combinatietherapie
8.3.1 Keuze en verantwoording literatuur
Voor de onderstaande tekst is gebruik gemaakt van literatuur die gevonden is door
middel van een gecomputeriseerd literatuuronderzoek. In Medline werden geen studies
gevonden op combinaties met de volgende trefwoorden: posttraumatic stress
disorder en treatment. Handmatig zoeken leidde tot één meta-analyse, die gebruikt
werd om de effectiviteit van antidepressiva met cognitieve gedragstherapie te vergelijken.
Tot op heden geen studies verricht waarin antidepressiva direct met cognitieve
gedragstherapie zijn vergeleken. Ook is de combinatiebehandeling niet op effectiviteit
geëvalueerd.
