maart '10 nederlands tijdschrift voor anesthesiologie 5 | samenvatting Ondanks de sterk verbeterde kans op overleven na een multi (neuro) trauma door opvang middels het Advanced Trauma Life Support principe blijft traumatisch letsel van het centrale zenuwstelsel een belangrijke oorzaak van sterfte en invaliditeit. Tevens blijft adequate behandeling van pijn in de trauma setting ernstig onderbelicht. Het moeten toedienen van algehele anesthesie bij een traumapatiënt brengt meerdere risico’s met zich mee. De optimale behandeling van een neurotrauma met cervicaal letsel zou moeten bestaan uit continue neurologische beoordeling, continue immobilisatie van het (cervicale) wervelkolom en dus vermijden van intubatie, en optimale pijnstilling. Deze optimale behandeling is haalbaar middels het gebruik van locoregionale technieken. De veiligheid en effectiviteit van locoregionale technieken is de laatste tien jaar dramatisch verbeterd door het gebruik van neurostimulatoren, echodoppler geleide puncties en steriele en aseptische technieken. Onze indruk is helaas dat deze techniek weinig wordt toegepast in de trauma setting. Aan de hand van de volgende casus waarbij met succes een locoregionale techniek is toegepast bij een neurotrauma patiënt met een instabiele cervicale 3 fractuur bespreken wij de risico’s van dergelijke casuïstiek en de voordelen van het toepassen van een locoregionale techniek. abstract Despite the improved chances of survival after a multi trauma since the implementation of the Advanced Trauma Life Support protocol, trauma to the central nervous system remains an important cause of death and invalidity. Unfortunately treatment of pain in the trauma setting also remains inadequate. General anesthesia in the trauma setting does not go without risk. Optimal treatment of a neurotrauma patient should include continous neurological monitoring, continous immobilization of the (cervical) spinal cord, thus no need for intubation, and optimal pain treatment. This optimal treatment is possible by means of employing a locoregional technique. The safety and efficacy of locoregional techniques has dramatically improved over the last decade due to the use of neurostimulators, ultrasound, and sterile and aseptic techniques. Unfortunately we have the impression that within the trauma setting locoregional anesthesia is infrequently used. The following case describes the successful use of locoregional anesthesia in a patient with a neurotrauma and an instable cervical 3 fracture. We will discuss the risks in such trauma patients and the advantages of locoregional anesthesia. Keywords: cervical spine, locoregional anesthesia, pain, trauma. Inleiding In Nederland zijn sinds 1999 elf level- 1- traumacentra welke ongevalslachtoffers opvangen volgens het ‘advanced trauma life support’ ( = ATLS) principe. Dit heeft geleid tot een vergrote kans op overleven van traumapatiënten [1]. Daar letsels van het centrale zenuwstelsel een hoge kans op mortaliteit en sterfte hebben [2] is een optimale observatie van het neurologische beeld hierbij van cruciaal belang. Alcohol- en drugsgebruik, maar ook (anesthesiologische) medicatie kan deze beoordeling bemoeilijken. Hemodynamische effecten van de anesthesie en directe laryngoscopie kunnen ruggenmergletsel veroorzaken bij instabiele cervicale fracturen [3]. Het belang en de mogelijkheden van regionaal anesthesie in dit soort casuïstiek worden in de hier volgende casus belicht. Casus Een 82-jarige aanspreekbare patiënt werd de traumakamer binnengebracht door de ambulance dienst na een hoog energetische trauma (80 km/h), waarbij hij als voetganger was aangereden door een motor. Bij analyse volgens Advanced Trauma Life Support (ATLS) protocol had de patiënt een vrije ademweg ( A= airway) met adequate ademhaling (B= breathing) en was hemo-dynamisch
