Convulsie tijdens<br />
carotisendarteriëctomie<br />

samenvatting Carotisendarteriëctomie is een frequent uitgevoerde ingreep<br />
bij patiënten met een symptomatische carotisstenose en kan onder een<br />
vorm van locoregionale anesthesie worden uitgevoerd. Wanneer een cervicaal<br />
plexus blok toegepast wordt is het vaak noodzakelijk dat de chirurg tijdens<br />
de procedure lidocaïne infiltreert in de regio van de a. carotis, wegens pijn bij<br />
manipulatie welke niet door het blok gedekt word. De casus betreft een patiënt<br />
onder locoregionale anesthesie die na accidentele injectie van lidocaïne in de a.<br />
carotis, een convulsie doormaakte. Een dergelijke casus is slechts éénmaal eerder<br />
beschreven in de literatuur. Tijdens en na de convulsie ontwikkelde de patiënt<br />
een ernstige gecombineerde lactaat en respiratoire acidose. Er trad een volledig<br />
spontaan herstel op van de acidose en de patiënt hield geen neurologische restverschijnselen<br />
over aan het <strong>voor</strong>val. Lactaatacidose na een gegeneraliseerd insult<br />
is een self limiting conditie welke geen specifieke behandeling behoeft.<br />
abstract Carotid endarterectomy is a procedure frequently performed<br />
on patients with symptomatic internal carotid stenosis, often under regional<br />
anesthesia. Because of additional afferent autonomic innervation not covered by<br />
the regional anesthesia, supplementation with a local anesthetic by the surgeon<br />
is often needed to treat pain or discomfort during manipulation of the carotid<br />
artery. We describe a patient under regional anesthesia who had a convulsion<br />
following infiltration of lidocaine in the region of the internal carotid artery.<br />
Only one similar <strong>case</strong> has been <strong>report</strong>ed before. The patient developed a severe<br />
combined respiratory and lactic acidosis. Complete recovery of the acidosis occurred<br />
spontaneously and there were no residual neurological deficits after the<br />
event. Lactic acidosis following a convulsion is self limiting, no specific treatment<br />
is needed.<br />
