samenvatting Door de introductie van het Helikopter-Mobiel Medisch
Team (H-MMT’s) in Nederland is er een uitbreiding van preklinische
diagnostische mogelijkheden bij vitaal bedreigde patiënten. Prehospitale
echografie kan een belangrijke ondersteuning geven bij het stellen van
diagnoses onder moeilijke omstandigheden. Het werken volgens de ABCDEsystematiek
in de prehospitale traumazorg wordt ondersteund door de
echografische beeldvorming van hart, longen en abdomen. Echografie kan
belangrijke ondersteuning geven bij het uitsluiten van een pneumothorax
en het controleren van de positie van een endotracheale tube. Internationaal
is reeds ervaring met de toepassing van prehospitale echografie in het veld,
een vast stramien voor het verrichten van dit onderzoek is vereist. De auteurs
pleiten voor een prospectief onderzoek naar deze nieuwe techniek door de
vier H-MMT’s.
abstract The introduction of a Helicopter Emergency Medical Service
(HEMS) in the Netherlands improves the diagnostic possibilities for vitally
compromised patients in the field. Prehospital sonography can give important
support in diagnosing under difficult circumstances. Imaging of heart,
lungs and abdomen can support decision making for the provider of trauma
care, especially regarding the ABC’s in the primary survey. Excluding a pneumothorax
and checking the position of the endotracheal tube by means of
sonography, is still rarely applied, even in hospital. Two cases are described,
and a review of relevant literature is given. Further research into the benefit
of prehospital ultrasound by HEMS in the Netherlands is necessary.
