8.3 Syncope
definitie: tijdelijk verlies van bewustzijn en posturale tonus door een abrupte, transiente en diffuse
cerebrale dysfunctie. Vertigo is frequent “syncope en mineur”.
etiologie:
1/ primair neurologisch: convulsie, migraine, hysterie, drugs
2/ transient metabool: hyperkaliemie, hypoglycemie, hypercalcemie, hyperventilatie, ...
3/ cardiovasculair:
3.1/ vasodepressor syncope - vasovagaal - common faint:
• goedaardig
• prodroom: voelt het aankomen: vertogo, ijlhoofdig, bleek, zweten, nausea, hyperventilatie
• uitlokkende factor: warm, bedrukt, emotie (naald, bloed), honger
• syncope: cerebrale ischemie door hypotensie, cerebrovasculaire vasoconstrictie door hypocapnie;
zelden contusie; snelle recuperatie
• R/ bij prodroom liggen; NaCl belasting
3.2/ cardiale syncope: alarm: bij inspanning, thoracale pijn, palpitaties, familiale plotse dood,
recidiverend, contusie, post-op, medicatie (antiarythmica, antihypertensiva, diuretica)
a/ primair cardiaal:
low cardiac output: valvulair AS, HCMP, ernstig PS, PHT, klepprothese dysfunctie, myxoom, coronaire
insufficientie (Kawasaki, ALCAPA)
b/ arithmie:
AV conductie stoornissen: AV block bij L-TGA, SSS
supra ventriculaire ritmestoornissen: zeer snelle SVT, WPW + VK flut of VKF
ventrikeltachycardie VT: long QT syndroom, Brugada syndroom, arrhytmogene RV dysplasie,
cardiomyopathie (HCMP & CCMP), PO Fallot, extreme bradycardie
3.3/ vasculaire syncope:
orthostatische syncope: volume depletie (zoutarm dieet vooral bij groeispurt, bloedverlies), lange
bedlegerigheid, deshydratatie, perifere neuropathie (diabetes), spinaal letsel, syringomyelie
vasovagale syncope, vagale hypertonie
carotis sinus syncope
hoest syncope
post mictie syncope
preventie: recidief voorkomen
1/ schade bij vallen aan patient en/of derden
2/ permanente neurologische schade
