Samenvatting
Hydatide cysten van Morgagni zijn kleine, benigne
cysten en zijn restanten van de buizen van Wolff.
Ze komen in het kleine bekken veel voor en zijn
zelden symptomatisch.
Wij beschrijven een casus waarin een zwangere
patiënte een acute buik ontwikkelde op basis van
een partiële torsio tubae, veroorzaakt door een hydatide
cyste. Patiënte onderging een laparoscopie
waarbij het fimbriële uiteinde van de tuba om een
omentum streng was gekruld. Dit beeld is nog niet
eerder beschreven.
Wij concludeerden dat een benigne, kleine cyste
een acute buik kan veroorzaken en dat een laparoscopie
zal moeten worden verricht, wanneer diagnostische
hulpmiddelen geen richting geven aan
de differentiaaldiagnose - ook tijdens een zwangerschap.

Summary
Hydatid cysts of Morgagni – remnants of the Wolffian
ducts - are benign, cystic structures and are
usually of no clinical significance. They are located
below the fallopian tube near the fimbria. Strangulation
of a hydatid of Morgagni is rarely reported.
In this case, a 28-year-old pregnant woman was referred
to our hospital with acute abdominal pain of
rapid onset. Despite preoperative examination, laboratorial
tests and ultrasonographic imaging, we were
not able to clearly locate the origin of the pain and
decided to perform a diagnostic laparoscopy.
We found an adnexal torsion of the right fallopian
tube caused by a hydatid cyst which appeared at

the end of the fallopian tube. The tube was strangulated
through the omentum, partly necrotic and
therefore extracted. Both maternal and fetal outcome
were good.
In this case, an adnexal torsion - caused by a small
hydatid cyst of Morgagni – caused an acute abdomen
in a pregnant patient: a case which has not
been described before. A hydatid cyst is difficult
to recognize and when preoperative examinations
are inconclusive in a patient with acute abdominal
pain, laparoscopy should be performed – also
during pregnancy.
