.4.2 Pseudochylothorax
Pseudochylothorax komt voor bij patiënten met verdikte en soms verkalkte pleura bij wie sprake
is van chronisch pleuravocht als gevolg van eerder doorgemaakte tuberculose, therapeutische
pneumothorax (in het kader van tuberculosebehandeling) of reuma. 14,16 Het aspect van het
pleuravocht kan melkachtig tot geel zijn; in tegenstelling tot chylus is het triglyceridegehalte
laag (< 0,56 mmol/l) en het cholesterolgehalte hoog (> 5,2 mmol/l). Behandeling is geïndi-
ceerd bij dyspnoe of verminderde inspanningstolerantie en bestaat uit drainage en incidenteel
decorticatie. 2,14,18

De primaire behandeling van chylothorax bestaat uit behandeling van de
onderliggende oorzaak, drainage en een MCT-dieet.
