Samenvatting Aanleiding: Vanuit de beroepsgroep fysiotherapie is naar voren gekomen dat het gebruik van meetinstrumenten gestimuleerd moet worden. Doel: Het ontwikkelen en uittesten van een implementatiestrategie voor het gebruik van de PSK en 6- MWT, deze twee testen worden het meest aanbevolen in de KNGF richtlijnen. Vraagstellingen: 1. Welke bevorderende en belemmerende factoren spelen een rol bij het (niet) gebruiken van de meetinstrumenten PSK en 6-MWT in de eerstelijns fysiotherapiepraktijk? 2. Welke implementatiestrategie sluit het beste aan bij de bevorderende en belemmerende factoren? 3. Zijn de implementatiestrategieën toepasbaar en effectief binnen de eerstelijns fysiotherapiepraktijk? Methode: In dit project is gewerkt volgens het implementatiemodel van Grol en Wensing (2006). In de analysefase werd dmv een literatuurstudie en via interviews nagegaan wat de belemmerende en bevorderende factoren zijn voor het gebruik van meetinstrumenten in de dagelijkse praktijk. Op basis van de bevindingen in de analysefase en literatuur en in overleg met de klankbordgroep werden diverse implementatiestrategieën ontwikkeld, rekening houdend met de verschillende fasen van gedragsverandering. Deze strategieën zijn uitgetest bij vier eerstelijns fysiotherapiepraktijken, waarna evaluatie en bijstelling plaatsvond. Resultaten: Belemmerende factoren waren op het gebied van deskundigheid: gebrek aan kennis, scholing, routine en ervaring; attitude: weerstand tegen verandering, niet overtuigd zijn van de meerwaarde en het gevoel overladen te worden met informatie; organisatie: gebrek aan tijd en ruimte, ontbreken van financiële tegemoetkoming, praktijkbeleid en intercollegiaal overleg; patiënt: andere verwachtingen, onvoldoende cognitie; meetinstrumenten: onvoldoende beschikbaarheid, onduidelijke instructie en een te groot aanbod. Op basis van deze factoren werden verschillende implementatiestrategieën ontwikkeld: het aanpassen van de meetinstrumenten, het ontwikkelen van een zelfanalyselijst en een scholingsmodule. De strategieën werden bij twee groepen fysiotherapeuten uitgetest en op basis daarvan vonden aanpassingen plaats. Conclusie en discussie: Het aanpassen van de meetinstrumenten vergrootte de praktische toepassingsmogelijkheden in de praktijk. De zelfanalyse en scholingsmodule werden door de cursisten als waardevol ervaren. Positieve elementen waren bewustwording, zelfreflectie, praktisch oefenen, interactieve werkvormen, coachende stijl, discussies met collega‟s, inzicht in gebruiksmogelijkheden en prikkeling tot toepassing in het methodisch handelen. Uitproberen van de strategieën in de kleine en selecte groep fysiotherapeuten staat echter niet garant voor een generalisatie van de resultaten naar alle eerstelijns fysiotherapeuten. Effectiviteit van de strategieën op langere termijn zijn in dit één jaar durende project niet gemeten kunnen worden. De ontwikkelde strategieën kunnen verder worden uitgewerkt en ingezet in toekomstige implementatieprojecten van de beroepsvereniging. Summary Objective: The physiotherapists‟ use of clinimetrics is a topic of the Dutch professional group. The aim of this study is to develop and test an implementation strategy for the use of the Patient Specific Complaints (PSK) and the Six Meter Walking Test (6-MWT), the most recommended tests in the Dutch clinical practise guidelines. Research questions: 1. Which barriers and facilitators contribute to the use of the PSK and 6-MWT in the primary health care physiotherapists‟ practise? 2. Which implementation strategy fit the best to these barriers and facilitators? 3. Are these strategies applicable and effective in the primary health care physiotherapists‟ practise? Method: In this project we used the implementation model of Grol and Wensing (2006). To identify the barriers and facilitators for the use in everyday practise, literary study and interviews have been done. These data formed the base for the development of different implementation strategies, in which also the phase of behaviour change was considered. The strategies have been tested in four primary health care physiotherapists‟ practises, after which evaluation and readjustment took place. Results: Barriers were found at different levels: professionalism contain: lack of knowledge, education, routine and experience; attitude: resistance against change, not being convinced of added value, the feeling of being overloaded with information; organisation: lack of time and room, not being paid, lack of practise policy and meetings with colleges; patient: different expectations, lack of cognition; measurement instruments: insufficient availability and instructions, to much of them. Out of these data, together with a feedback group the following strategies have been developed: adjustment of the measurement instruments, development of a self-analysis list and an education module. These strategies were tested in two different groups of physiotherapists after which adjustment took place. Conclusion and discussion: The adjustment of the instruments led to a better use in everyday care. The participants appreciated the self- analysis and education in case of consciousness, self-reflection, practical rehearsing, interactive study groups, coaching style, colleague‟s discussions and understanding of the probability to use the instruments in methodical action. The try out of these strategies in this little and select groups can not be generalised to all primary health care practises. The long-term efficacy couldn‟t be worked out in this one year project. The strategies here developed are further to be worked out and can be used in future implementation programs of the Dutch professional physiotherapist‟s group. Objective: The physiotherapists‟ use of clinimetrics is a topic of the Dutch professional group. The aim<br />
