Ook levonorgestrel releasing-IUD werd recent voorgesteld voor pijnklachten geassocieerd met rectovaginale endometriose. Indien deze medicatie niet aangewezen of onvoldoende werkzaam is, kan gekozen worden voor GnRH-analogen, danazol of gestrinone. Recidieven zijn frequent na het stoppen van de hormonale therapie en er is geen bewijs van verhoging van de vruchtbaarheid. Onderzoek naar nieuwe medicatie is gericht op factoren die de ontwikkeling en progressie van endometriose beinvloeden en dit omvat progesteronreceptormodulatoren, gonadotropin releasing hormone (GnRH)-analogen, aromatase-inhibitoren alsook tumor necrosis factor-a (TNF-a)-inhibitoren, angiogenesisinhibitoren, matrixmetalloproteïnase-inhibitoren en oestrogeenreceptor b-agonisten. G1356N_2010 Gebruik van een hormonaal spiraaltje (Mirena ® ) verhoogt het risico op borstkanker niet In een grote veiligheidsstudie bij meer dan 5.000 vrouwen met borstkanker en 20.000 controlevrouwen werd geen verschil in het risico op borstkanker waargenomen tussen vrouwen die Mirena ® gebruiken of vroeger gebruikt hebben, in vergelijking met vrouwen die een koperspiraaltje gebruiken. Deze studie bevestigt dat het hormoon levonorgestrel (LNG) in Mirena ® het risico op borstkanker niet verhoogt. De studie werd uitgevoerd door het centrum voor epidemiologie en gezondheidsresearch in Berlijn, als een zogeheten casuscontrolestudie. In tumorregisters werden 5.113 vrouwen met een bewezen borstkanker opgespoord en die vergeleken met 20.452 controlevrouwen (vrouwen zonder borstkanker) van dezelfde leeftijd en uit dezelfde streek. Van de 5.113 vrouwen met een bewezen borstkanker (= 21,2%) hadden er 1.085 vóór de diagnose Mirena ® gebruikt en 1.130 (= 22,1%) een koperspiraaltje. Er was geen verschil in het tumortype, de histologie, tumorgrootte of metastasering tussen de groepen. De niet-gecorrigeerde Odds Ratio (OR) voor vrouwen die Mirena ® hadden gebruikt, versus de vrouwen met een koperspiraaltje was 1,04 [95% betrouwbaarheidsinterval (BI) 0,93-1,17]. De gecorrigeerde OR was 0,99 (95% BI 0,88-1,12). Soortgelijke resultaten werden gevonden bij de vrouwen die een spiraaltje gebruikten op het ogenblik van de studie: niet-gecorrigeerde OR 0,90 (95% BI 0,58-1,41) en gecorrigeerde OR 0,85 (0,52-1,39). De auteurs besluiten dat vrouwen die Mirena ® gebruiken, en vrouwen die een koperspiraaltje gebruiken, eenzelfde risico op borstkanker lopen. Huidig of vroeger gebruik van Mirena ® of een koperspiraaltje verhoogt het risico op borstkanker niet. Gardasil ® geselecteerd door de Vlaamse Gemeenschap Gardasil ® werd geselecteerd door de Vlaamse Gemeenschap voor het gratis vaccinatieprogramma tegen Humaan papillomavirus voor meisjes in het eerste jaar Vlaams secundair onderwijs. De keuze van de Vlaamse overheid om voor dit vaccinatieprogramma vanaf het schooljaar 2010- 2011 Gardasil ® te gebruiken, bevestigt het belang van de voordelen die verwacht worden van deze primaire preventiemaatregel tegen HPV infecties. Gardasil ® is het enige vier-type (6, 11, 16, 18) HPV vaccin dat naast bescherming tegen baarmoederhalskanker, ook bescherming kan bieden tegen precancereuze letsels van de baarmoederhals, de vulva en de vagina veroorzaakt door bepaalde oncogene HPV types, en tegen genitale wratten veroorzaakt door bepaalde HPV types. De HPV types waartegen Gardasil ® gericht is, veroorzaken de overgrote meerderheid van genitale ziekte te wijten aan HPV. Gardasil ® blijft in België nog steeds terugbetaald door het RIZIV indien voorgeschreven voor meisjes die op het ogenblik van de eerste toediening minstens 12 jaar zijn maar nog geen 19 jaar oud. Er verandert niets voor meisjes die onderwijs volgen georganiseerd of gesubsidieerd door de Franstalige Gemeenschap. Ze blijven genieten van de terugbetaling van Gardasil ® via het RIZIV. Hetzelfde geldt voor meisjes vanaf het tweede jaar Vlaams secundair onderwijs omdat er geen inhaalvaccinatie wordt voorzien door de Vlaamse overheid. Noodpil ulipristalacetaat (ellaOne ® ) significant doeltreffender dan levonorgestrel Het Europese geneesmiddelenagentschap (EMA) heeft op basis van recente studies bevestigd dat ulipristalacetaat (ellaOne ® ) significant doeltreffender is dan levonorgestrel om het risico op zwangerschap te verminderen. Het EMA evalueerde hiervoor alle resultaten van een omvangrijke en gerandomiseerde fase 3 studie vs. levonorgestrel en van een metaanalyse van twee vergelijkende studies vs. Levonorgestrel, waaraan bijna 3.500 vrouwen deelnamen. Deze studies, gepubliceerd in The Lancet van februari 2010, kwamen tot de bevinding dat ulipristalacetaat, vergeleken met levonorgestrel, het zwangerschapsrisico bijna halveerde wanneer ingenomen binnen 72 uur (OR 0,58 [0,33; 0,99], p = 0,0462) na onbeschermd seksueel contact en met bijna twee derde verminderde wanneer ingenomen binnen de eerste 24 uur (OR 0,35, [0,11; 0,93], p = 0,0346). Kort na de bevestiging van deze resultaten door het EMA heeft ook de Amerikaanse Food and Drug Administration FDA dit ge- neesmiddel goedgekeurd voor noodanticonceptie in de VS. De Europese Commissie heeft in mei 2009 ellaOne ® een vergunning afgeleverd voor het in de handel brengen als noodanticonceptie binnen 120 uur (5 dagen) na onbeschermde geslachtsgemeenschap of falende anticonceptie.
