Abstract<br />

Po<strong>te</strong>ntial contribut<strong>in</strong>g factors <strong>in</strong> the creation of a therapeutic milieu for<br />

Sesotho/Setswana elderly persons <strong>in</strong> residential care facilities <strong>in</strong> the Free Sa<strong>te</strong><br />

Occupational therapy practice is based on the paradigm of an <strong>in</strong>tr<strong>in</strong>sic connection between a<br />

human’s fundamental need to participa<strong>te</strong> <strong>in</strong> a wide range of occupations as both a means and<br />

an end <strong>in</strong> order to atta<strong>in</strong>, restore or ma<strong>in</strong>ta<strong>in</strong> health. Choices for occupational participation<br />

that give mean<strong>in</strong>g and purpose are of<strong>te</strong>n culturally bound. For that reason the occupational<br />

therapy profession views the client-centred approach as a very important avenue to pr<strong>om</strong>o<strong>te</strong><br />

wellbe<strong>in</strong>g and enhance the quality of life for each person as a unique <strong>in</strong>dividual. The clientcentred<br />

approach entails participation and engagement <strong>in</strong> occupations that have particular<br />

significance for such an <strong>in</strong>dividual. As part of the occupational therapy process occupations<br />

are selec<strong>te</strong>d and presen<strong>te</strong>d <strong>in</strong> collaboration with the client. Occupational therapy practice is<br />

<strong>in</strong>formed by, and to a certa<strong>in</strong> ex<strong>te</strong>nt <strong>in</strong>formative to, its young foundational discipl<strong>in</strong>e, viz.<br />

occupational science. However, s<strong>om</strong>e essential concepts with<strong>in</strong> occupational science are<br />

founded on Wes<strong>te</strong>rn philosophies and world views. Locally and <strong>in</strong><strong>te</strong>rnationally, li<strong>te</strong>rature<br />

regard<strong>in</strong>g cultural aspects of activity participation lacks an <strong>in</strong>-depth understand<strong>in</strong>g.<br />

Occupational therapists serv<strong>in</strong>g the Free Sta<strong>te</strong> c<strong>om</strong>munity currently c<strong>om</strong>prise ma<strong>in</strong>ly a whi<strong>te</strong>,<br />

female population who were usually also educa<strong>te</strong>d and tra<strong>in</strong>ed <strong>in</strong> a hegemonic environment<br />

<strong>in</strong> <strong>te</strong>rms of gender and race. This presents a great challenge with regard to <strong>in</strong>sightful clientcentred<br />

practice to the Sesotho/Setswana c<strong>om</strong>munity. With<strong>in</strong> this Sesotho/Setswana<br />

c<strong>om</strong>munity numerous elderly <strong>in</strong>dividuals are forced by modernisation and urbanisation to<br />

embrace the unconventional environment of a residential care unit as a h<strong>om</strong>es<strong>te</strong>ad. The<br />

recognition of <strong>in</strong>dividual needs <strong>in</strong> <strong>te</strong>rms of cust<strong>om</strong>s, values, convictions and beliefs is<br />

overshadowed by the fulfilment of seem<strong>in</strong>gly other important needs, such as medical and<br />

physical care. Occupational therapy practice is also bound by the concept of occupational<br />

justice, which is an ex<strong>te</strong>nsion of social justice and human dignity. The restoration of social<br />

justice by means of client-centred <strong>in</strong><strong>te</strong>rvention and fulfill<strong>in</strong>g occupational engagement is<br />

therefore c<strong>om</strong>pr<strong>om</strong>ised by a lack of cultural awareness and by preconceived ideas.<br />

The aim of this study, therefore, was the exploration of factors that could contribu<strong>te</strong> to the<br />

creation of a more culturally acceptable residential care environment. A qualitative study<br />

design was applied dur<strong>in</strong>g this study. N<strong>om</strong><strong>in</strong>al Group Technique (NGT) <strong>in</strong><strong>te</strong>rviews were<br />

conduc<strong>te</strong>d with two groups of professionals who had previously been <strong>in</strong>volved, or who were<br />

currently <strong>in</strong>volved, <strong>in</strong> Sesotho/Setswana elderly care. The aim of these NGT <strong>in</strong><strong>te</strong>rviews was<br />

not to produce scientific or theoretical facts, but rather to provide a platform to participants to<br />

voice their op<strong>in</strong>ions. The question posed to the groups was: “What can be done to crea<strong>te</strong><br />

more appropria<strong>te</strong> experiences for the Sesotho/Setswana group of elderly <strong>in</strong>dividuals <strong>in</strong> an<br />

old-age h<strong>om</strong>e?” S<strong>in</strong>ce the participants were regarded as experts <strong>in</strong> the field of elderly care,<br />

their knowledge and experience produced a wealth of <strong>in</strong>formation concern<strong>in</strong>g the pr<strong>om</strong>otion<br />

of mean<strong>in</strong>gful activity participation for Sesotho/Setswana elderly persons <strong>in</strong> residential care<br />

units.<br />

Of all the given responses, five were prioritised by each group as the most significant. Rather<br />

than suggestions for cultural adaptations <strong>in</strong> residential care units or suggestions for cultural<br />

activities, the central theme of the feedback was clear: Acknowledg<strong>in</strong>g the human dignity of<br />

every elderly <strong>in</strong>dividual is non-negotiable. Human dignity is, however, regarded as a<br />

universal need of every human be<strong>in</strong>g. The real issue that needed further <strong>in</strong>vestigation, then,<br />
