THE PUBLIC HEALTH SYSTEM IN SPAIN AND ITS AUTONOMOUS COMMUNITIES: SUSTAINABILITY<br />

AND REFORMS (2011-2012)<br />

The purpose of this project is to analyze the sustainability of the Spanish public health system as well as to<br />

assess the reform measures. To carry out this research a database has been built, homogeneous and larger<br />

than others built so far, which contains territorial series of public health spending at regional level and by public<br />

agents. It is on the basis of this information that the following tasks have been undertaken:<br />

⎯ An analysis of the evolution of health expenditure in Spain in comparison to the OECD countries and<br />

of the most significant features of the dynamics and composition (by categories and function) of<br />

health spending in Spain and its regions for the 1991-2010 period.<br />

⎯ A description of the different behaviors of the regional health systems, as well as the territorial<br />

differences in the expenditure levels and structure of the public health services.<br />

⎯ The identification of the determining factors of health expenditure and its growth, combining the<br />

traditional residual approach with econometrics.<br />

⎯ A medium-term projection on the future evolution of health spending.<br />

⎯ A descriptive analysis of the recent developments of the financing of the autonomous communities<br />

and of the problem of fiscal sustainability of public health care spending in Spain.<br />

⎯ The study of three areas in which the debate about the reforms needed for the Spanish public health<br />

system takes place, taking in mind: (i) the insurance system and the recent amendment set out in the<br />

Royal Legislative Decree 16/2012; (ii) the changes in the management models and new organizational<br />

forms for the delivery of health services; and (iii) the mechanisms for users’ participation in the cost of<br />

health care (copayment).<br />

A PRODUCTION MODEL AND CHALLENGES FOR SPANISH FIRMS: PRODUCTIVITY, COMPETITION<br />

AND INNOVATION (2012-2013)<br />

This study aims at the analysis of competitiveness of the Spanish economy focusing on the role of the firms in<br />

contrast to previous studies on productivity, competitiveness and innovation carried out in the <strong>Ivie</strong>, which<br />

concentrated more on macroeconomic or sectoral aggregates. This collective work offers an in-depth analysis<br />

of entrepreneurial behavior in relation to some of the factors that literature considers relevant to the dynamics<br />

of productivity: knowledge-based assets such as: human capital, expenditure on R&D or investments in<br />

intangible assets; the size and demography of firms; the intensity of competition in the sectors; outsourcing<br />

and offshoring strategies; or the ability of firms to generate profits and maintain the levels of profitability.<br />
