92 รัชนี แกวมงคล และคณะวัสดุและ วิธีการวัสดุ1. การ เตรียม complement coated red bloodcells1.1 การ เตรียม เซลลสําหรับ ใชทดสอบ ใน หลอดทดลอง ( In vitro )1.1.1 ใชhumancomplementcomponent coated บน เซลลเม็ด โลหิต แดง (rbc) หมูโลหิตO( rbc group O ) ตาม วิธีของ Jenkin DE 6 ( C 3b/C 4b และC 3dKAF ),ChaplinH 7 (C 4b) และFruitstone 8 (C 3balternate pathway และC 3d(AP) Tryp ) และ เตรียมเปน 3-5% cell suspension ใน 0.15 M NaCl1.1.2 เตรียม เซลล Le a ( C ) 9 โดย ใช RBC หมู O Le +a sensitized ดวย anti-Le a +complement1.2 เตรียม เซลลสําหรับ ใชทดสอบ กับ เซลลที่ไดจาก ผูปวย ( In vivo )1.2.1 ใชเม็ด โลหิต แดง จาก ผูปวย ที่มีผลบวกdirectantiglobulintest จํานวน100 ตัวอยาง นํา มาลาง 3 ครั้ง ดวย 0.15M NaCl เพื่อ กําจัด แอนติบอดีที่อยูใน ซีรัม ออก ใหหมด และ เตรียม เปน 3-5% cell suspension ใน 0.15 M NaCl2. น้ํา ยา anticomplement ที่ใชทดสอบ2.1 เตรียม monoclonal anti-C 3d ( 5C 4) ที่ เจือจาง 1/100 ,1/200 และ 1/300 ดวย 1% bovine albumin ( BA ) in Imidazole Buffer Solution ( IBS )2.2 Rabbit anti-C 3, anti-C 3d และ anti-C 4(Ortho)2.3 น้ํา ยาAHS ของ ศูนยฯ (lot.43025) และAHS ของ ตาง ประเทศ 2 บริษัทวิธีการ ศึกษา1. การ ทดสอบ ความ แรง ( ไต เตอร) ดวย hemagglutination test1.1 โดย วิธีหลอด ทดลอง มาตราฐาน ( standarddirect antiglobulin tube test) โดย ใช 100 µL ของmonoclonal anti-C 3d ( 5C 4) ผสม ดวย 100 µL ของ3-5% complement component coated RBC ใน0.15 M NaCl หรือ Low Ionic Strength Solution(LISS) ปน อาน ผล ทันที ( immediately spin tube test)ที่ 3,400 rpm 15 วินาที และ ตั้ง ทิ้ง ไว5 นาที แลว ปน อานผล อีก ครั้ง2. การ ทดสอบ ความ จําเพาะ ( specificity test )2.1 ศึกษา ปฏิกิริยา ความ จําเพาะ ของ การ จับ กลุมโดย เจือ จาง monoclonal anti-C 3d ( 5C 4) ดวย 1%bovine serumalbumin (BSA) ในimidazolebarbital saline ( IBS ) นํา มา ทดสอบ ดวย สาร ละลาย เม็ดโลหิต แดง ที่เตรียม เปน 3-5% cell suspension ใน 0.15M NaCl จาก ผู บริจาค โลหิต ที่ เปน clotted blood ,citrate blood ( ACD หรือ CPD ) ที่เก็บ4 ํซ. 24 ชั่วโมงเปน หมูโลหิต A , B , AB และO หมูละ 20 ตัวอยาง และที่เก็บ 4 ํซ.28 วัน เปน หมูโลหิตA ,B,AB และO หมูละ 20 ตัวอยาง2.2 ทดสอบ heterospecific antibodies โดยใช clotted blood 24 ชั่วโมง เตรียม เปน 3-5% ใน NSSเปน หมู โลหิต A , B , AB และ O หมู ละ 40 ตัวอยางผสม กับ monoclonal anti-C 3d ( 5C 4) ที่เจือ จาง1/300ทดสอบ ที่อุณหภูมิหอง ( 20-30 ํซ. ),37 ํซ. และ ที่ 2-8 ํซ.โดย incubate 30 นาที ใน แตละ อุณหภูมิที่ทดสอบ ปนอาน ผล ดวย ตา เปลา และ กลอง จุลทรรศน2.3 ทดสอบindirectantiglobulintest โดยใชหมูโลหิตA ,B,AB และO ในCPD ไมเกิน5 วันหมู ละ 20 ตัวอยาง ลาง เซลล 3 ครั้ง ดวย NSS เตรียมเปน 3-5 % ใน NSS ใชซีรัมใหม ( 1-2 วัน ) จาก หมูโลหิตA , B , AB และO หมูละ4 ตัวอยาง โดย ใชซีรัมใหมแตละ หมูโลหิต ทํา ปฏิกิริยา กับ เซลลหมูเดียว กัน5 ตัวอยาง( ซีรัมโลหิต หมู A 1 ตัวอยาง ใชทดสอบ กับ เซลลเม็ด โลหิต1แดง หมูA5 ตัวอยาง ) แลว นํา ไป incubate ที่ 37 ํซ. 30นาที นํา มา ปน ลาง3 ครั้ง แลว หยด monoclonal anti-Thai Journal of Hematology and Transfusion Medicine Vol. 12 No. 2 April-June 2002

Monoclonal Anti-C 3dAntibody93C 3d ( 5C 4) ที่ เจือ จาง 1/300 2 หยด ผสม ให เขากัน ปนอาน ผล ดวย ตา เปลา และ กลอง จุลทรรศน2.4 ทดสอบ กับ trypsin treated cells andpapain treated cells โดย ใช citrate blood เก็บ ที่4 ํซ. 24 ชั่วโมง เปน หมูโลหิต A , B , AB และO หมูละ20 ตัวอยาง นํา มาsensitized ดวย เอ็น ไซมTrypsinและ Papain และ เตรียม เปน 3-5% cell suspensionแลว นํา มา ทดสอบ กับ monoclonal anti- C 3d ปน อาน ผลทันที และ ตั้ง ทิ้ง ไวที่อุณหภูมิหอง 10 นาที ปน อาน ผล ดวยตา เปลา และ กลอง จุลทรรศนผล การ ทดลองไดนํา monoclonal anti-C 3d ( 5C 4) มา ทดสอบ ความแรง ( ไต เตอร และ score ) กับ complement component coated rbc ( C 3b,C 3d,C 4b และ uncoated ) ซึ่ง ผลการ ทดลอง ดัง แสดง บน ตาราง ที่ 1จาก ตาราง ที่1 พบ วา monoclonal anti-C 3d (5C 4)มีความ แรง ( ไต เตอร) เทากัน กับ ทุกC 3 coated RBC ที่ใช ทดสอบ แต ความ แรง ที่ อาน เปน คะแนน ( score )10แตก ตางกัน ซึ่ง ความ แรง เปน score ดวยC 3b/ C 4b และตาราง ที่ 1 แสดง ความ แรง ( ไต เตอร และ score ) ของmonoclonal anti-C 3d ( 5C 4) ดวย complementcoated rbcRed blood cellscoated withTitre ScoreC 3b/ C 4bC 3d/ C 4d Tryp.C 3b ( AP )C 3d ( AP ) Tryp.Le a CC 4bUncoated409640964096409640960011712411712411200C 3b ( AP ) coated rbc ไดคะแนน เทากัน คือ 117 คะแนนและ ความ แรง ปฏิกิริยา ดวย C 3d/ C 4d Tryp. และ C 3d(AP) Tryp. ไดคะแนน เทากับ 124 คะแนน เทากัน สวนความ แรง เปน score ที่ทดสอบ ดวย Le a C เทากับ 112คะแนน ซึ่ง ต่ํา กวา เซลล อื่น ๆ เล็ก นอย จาก ผล การทดสอบ นี้สรุป ไดวา ความ แรง ( ไต เตอร) ที่ทดสอบ ดวยเซลลตางๆ มีความ แรง เทากัน คือ 1/4096 และ ความ แรงเปนscore มีความ แตก ตางกัน ไมมาก ดังนั้น จึง ไดนําmonoclonal anti-C 3d ( 5C 4) ที่ เจือ จาง 1/100, 1/200และ 1/300 มา ทดสอบ ความ แรง ( ไต เตอร) เปรียบ เทียบกับ AHS ของ ศูนยฯ ( lot. 43025 ) และ ของ ตาง ประเทศ2 บริษัท ผล การ ทดลอง ดัง แสดง บน ตาราง ที่ 2จาก ตาราง ที่2 พบ วา ความ แรง ( ไต เตอร) ที่ทดสอบดวย C 3b และC 3d coated rbc เทากัน ทุก ชนิด ของ น้ํา ยาที่ใชทดสอบ คือ ไต เตอร 16 แตความ แรง เปน score แตกตางกัน พบ วา ที่ เจือ จาง 1/100 ปฏิกิริยา ความ แรง เปนscore ดวย C 3b และ C 3d coated RBC สูงสุด สวน ที่เจือ จาง 1/200 ปฏิกิริยา ความ แรง score ดวย C 3bcoated rbc มีความ แรง score ใกลเคียง กับ น้ํา ยา AHSของ ศูนย ฯ และ ของ ตาง ประเทศ ทั้ง 2 บริษัท และ ที่ เจือจาง 1/300 ปฏิกิริยา ความ แรง score ดวยC 3b coatedrbc มีความ แรง score นอย กวา น้ํา ยา AHS ของ ศูนยฯและ ของ ตาง ประเทศ ทั้ง2 บริษัท สวน ปฏิกิริยา ความ แรงscore ดวยC 3d coated RBC มีความ แรง ใกลเคียง กับน้ํา ยา ของ ศูนยฯ และ ของ ตาง ประเทศ ทั้ง 2 บริษัทจาก ตาราง ที่3 พบ วา ที่การ เจือ จาง 1/100 ใหผลบวกกับ บาง เซลลทุก การ ทดสอบ ( 4 การ ทดสอบ ) และ ทุก เซลลที่ใชทดสอบ มอง เห็น ดวย ตา เปลา สวน การ เจือ จาง ที่ 1/200 พบ วา ใหผลบวก กับ ทุก การ ทดสอบ และ ทุก เซลลที่ใชทดสอบ ทั้ง ดูดวย กลอง จุลทรรศนและ ดูดวย ตา เปลา และที่การ เจือ จาง 1/300 ใหผล ลบ กับ ทุก การ ทดสอบ และ ทุกเซลลที่ใชทดสอบ เหมือน กับ น้ํา ยา AHS ของ ศูนยฯดังนั้น ผล การ ทดลอง จาก ตาราง ที่ 2 และ ตาราง ที่ 3จึง ได คัด เลือก monoclonal anti-C 3d ( 5C 4) ที่ ความวารสารโลหิตวิทยาและเวชศาสตรบริการโลหิต ปที่ 12 ฉบับที่ 2 เมษายน-มิถุนายน 2545

It was high potency (titer) andspecificity which reacted with complement coated red bloodcells bothin vitro and in vivo. Anti-C 3d secreting clone 5C 4 can be used at afinal dilution of 1 to300 toprovideanexcellentantiglobulinreagentwhenmixedwithasuitabledilutionofrabbitanti-IgG serumto prepare anti -human globulin reagent (AHS). Toevaluatethepotencyandspecificity.It was tested the potency and specificity to comparewith polyclonal (rabbit ) antiglobulin reagentconventionally preparedbyNBC,TRCand2commercialreagents. Itwasfoundthattheblendedantiglobulin reagentgaveasgoodqualityaspolyclonal (rabbit)antiglobulinreagent.
