Obiettivi : provare l’efficacia e la sicurezza della integrazione<br />
di L -c<strong>it</strong>rullina orale per migliorare la durata di erezione in<br />
pazienti con disfunzione erettile lieve (ED). L’ integrazione di<br />
L-arginina migliora la funzione n<strong>it</strong>rica mediatica dell’ossido<br />
n<strong>it</strong>rico nella vasodilazione endoteliale; tuttavia, la<br />
somministrazione orale è stata ostacolata dal metabolismo<br />
esteso presistemico. Al contrario, la L-c<strong>it</strong>rullina sfugge dal<br />
metabolismo presistemico ed è convert<strong>it</strong>o in L-arginina,<br />
così da indurre logicamente per la supplementazione di<br />
L-c<strong>it</strong>rullina orale come un precursore per il L-arginine/<br />
n<strong>it</strong>rico ossido erezione pathway del pene.<br />
METODI: Nello studio attuale in singolo cieco,gli uomini<br />
con ED lieve ( punteggio di durezza di erezione di ) 3) ha<br />
ricevuto un placebo per mese 1 e la L-c<strong>it</strong>rullina, 1.5 g/d,<br />
per un altro mese. Sono stati registrati il punteggio di<br />
durezza di erezione, il numero di rapporti per mese, per<br />
soddisfazione di trattamento ed eventi avversi.<br />
RISULTATI: Un totale di 24 pazienti, di età tra 56.5 9.8<br />
anni ±, è stato incluso ed ha concluso lo studio senza<br />
eventi avversi. Il miglioramento nel punteggio di durezza di<br />
erezione da 3 (ED dolce) a 4 (erettile funzione normale) si<br />
è verificato nel 2 (l’8.3%) dei 24 uomini quando prendeva<br />
placebo e 12 (il 50%) dei 24 uomini quando prendeva<br />
l-c<strong>it</strong>rullina (P

Proprietà farmacocinetiche e farmacodinamiche di<br />
L-c<strong>it</strong>rullina e L-arginina orali: incidenza sul metabolismo<br />
dell’ ossido n<strong>it</strong>rico.<br />
Scopi di sommario l’integrazione orale di L-arginina è<br />
stata utilizzata in numerosi studi per migliorare l’ endoteliodipendenza,l’<br />
ossido n<strong>it</strong>rico (no) - ha mediato vasodilatazione.<br />
Il trattamento con L-Arginina è ostacolato da estesa<br />
eliminazione presistemica a causa dell’attiv<strong>it</strong>à intestinale dell’<br />
arginasi. Al contrario,la L-c<strong>it</strong>rullina è assorb<strong>it</strong>a prontamente e<br />
almeno in parte convert<strong>it</strong>a in L-arginina. Lo scopo del nostro<br />
studio era valutare questa conversione metabolica e i suoi<br />
effetti farmacodinamici. successivi.<br />
METODI: In uno studio doppio-cieco, randomizzato,<br />
placebo-controllato incrociato, 20 volontari sani hanno<br />
ricevuto sei diverse dosando regimi di placebo, di c<strong>it</strong>rullina<br />
e arginina. I parametri farmacocinetici (C (massimo), T<br />
(massimo), C (minimo), AUC) sono stati calcolati dopo la<br />
settimana 1 di integrazione orale. Il rapporto del plasma<br />
L-arginina su dimethylarginine asimmetrico, un inib<strong>it</strong>ore<br />
endogeno dell’ossido n<strong>it</strong>rico sintetasi (rapporto arginine/<br />
ADMA), tassi e flow-mediated vasodilation di escrezione di<br />
guanosine monophosphate (cGMP) e n<strong>it</strong>rato (FMD) ciclico<br />
urinario era misurato per valutare effetti farmacodinamici.<br />
RISULTATI: La L-C<strong>it</strong>rullina dose-dependente ha aumentato<br />
AUC e C (massimi) del plasma L-arginina concentrazione più<br />
efficacemente del L-arginina (P < 0.01). ). La dose più alta di<br />
c<strong>it</strong>rullina (3 offerta g) ha aumentato il C (minimo) del plasma<br />
L-arginine e ha migliorato il rapporto L-arginina/ADMA da 186<br />
+/-8 (linea di base) con 278 +/-14 [l’intervallo di fiducia 0.01, il<br />
95% < P (CI) 66, 121]. Inoltre, il n<strong>it</strong>rato e il cGMP urinari sono<br />
stati aumentati da 92 +/-da 10 a 125 +/-15 micromol mmol<br />
(-1) creatinine P = il 0.01, il 95% CI 8, 58 ) e da 38 +/-da 3.3<br />
a 50 +/-6.7 nmol mmol (-1) creatinine ( P = il 0.04, il 95% CI<br />
0.4, 24 ), rispettivamente. Nessun trattamento ha migliorato<br />
FMD sopra linea di base. L’analisi tuttavia, raggruppata di<br />
tutti i dati FMD ha rivelato una correlazione fra l’aumento di<br />
rapporto e miglioramento arginina/ADMA di FMD.<br />
CONCLUSIONE: I nostri dati mostrano per la prima volta<br />
che l’ integrazione di L-c<strong>it</strong>rullina orale alza la concentrazione<br />
plasmatica di L-arginina e aumenta la segnalazione NOdipendente<br />
in un modo dose-dipendente.<br />
AIMS: <strong>Oral</strong> L-arginine <strong>supplementation</strong> has<br />
been used in several studies to improve<br />
endothelium-dependent, n<strong>it</strong>ric oxide (NO)mediated<br />
vasodilation. L-Arginine treatment<br />
is hampered by extensive presystemic<br />
elimination due to intestinal arginase activ<strong>it</strong>y.<br />
In contrast, L-<strong>c<strong>it</strong>rulline</strong> is readily absorbed<br />
and at least in part converted to L-arginine.<br />
The aim of our study was to assess this<br />
metabolic conversion and <strong>it</strong>s subsequent<br />
pharmacodynamic effects.<br />
METHODS: In a double-blind, randomized,<br />
placebo-controlled cross-over study, 20<br />
healthy volunteers received six different<br />
dosing regimes of placebo, <strong>c<strong>it</strong>rulline</strong>, and<br />
arginine. Pharmacokinetic parameters<br />
(C(max), T(max), C(min), AUC) were calculated<br />
after 1 week of oral <strong>supplementation</strong>. The<br />
ratio of plasma L-arginine over asymmetric<br />
dimethylarginine, an endogenous inhib<strong>it</strong>or of<br />
n<strong>it</strong>ric oxide synthase (arginine/ADMA ratio),<br />
urinary cyclic guanosine monophosphate<br />
(cGMP) and n<strong>it</strong>rate excretion rates, and flowmediated<br />
vasodilation (FMD). was measured<br />
to assess pharmacodynamic effects.<br />
RESULTS: L-C<strong>it</strong>rulline dose-dependently<br />
increased AUC and C(max) of plasma<br />
L-arginine concentration more effectively<br />
than L-arginine (P < 0.01). The highest dose<br />
of <strong>c<strong>it</strong>rulline</strong> (3 g bid) increased the C(min) of<br />
plasma L-arginine and improved the L-arginine/<br />
ADMA ratio from 186 +/- 8 (baseline) to 278<br />
+/- 14 [P < 0.01, 95% confidence interval (CI)<br />
66, 121]. Moreover, urinary n<strong>it</strong>rate and cGMP<br />
were increased from 92 +/- 10 to 125 +/- 15<br />
micromol mmol(-1) creatinine (P = 0.01, 95%<br />
CI 8, 58) and from 38 +/- 3.3 to 50 +/- 6.7<br />
nmol mmol(-1) creatinine (P = 0.04, 95% CI<br />
0.4, 24), respectively. No treatment improved<br />
FMD over baseline. However, pooled analysis<br />
of all FMD data revealed a correlation between<br />
the increase of arginine/ADMA ratio and<br />
improvement of FMD.<br />
CONCLUSION: Our data show for the first<br />
time that oral L-<strong>c<strong>it</strong>rulline</strong> <strong>supplementation</strong><br />
raises plasma L-arginine concentration and<br />
augments NO-dependent signalling in a dosedependent<br />
manner.

A Roma test su una sostanza in grado di risolvere nell’85% dei casi una forma di impotenza che<br />
colpisce tre milioni di <strong>it</strong>aliani. La sperimentazione, condotta per ora su un campione ridotto e<br />
per un periodo lim<strong>it</strong>ato, ha escluso effetti collaterali. I risultati dello studio saranno presentati al<br />
congresso della Società <strong>it</strong>aliana di andrologia<br />

ROMA- Non cura l’effetto, ma la causa. Si<br />
chiama c<strong>it</strong>rullina ed è un integratore che, stando<br />
ai risultati di una sperimentazione <strong>it</strong>aliana,<br />
promette di combattere la disfunzione erettile, un<br />
disturbo della sessual<strong>it</strong>à che in Italia affligge circa<br />
3 milioni di uomini. La sostanza e lo studio che<br />
ne sostiene gli effetti pos<strong>it</strong>ivi saranno presentati<br />
da domani al ventiseiesimo congresso nazionale<br />
della Società <strong>it</strong>aliana di andrologia (Sia). Secondo<br />
lo studio, la c<strong>it</strong>rullina ha dimostrato di funzionare<br />
nell’85% dei casi. Non essendo un farmaco, non<br />
rientra fra i prodotti già in commercio che vanno<br />
assunti in vista dell’occorrenza, ma esplica i suoi<br />
effetti dopo un mese di assunzione regolare.<br />
A condurre la sperimentazione è stato Ermanno<br />
Greco, direttore del Centro di medicina e<br />
biologia della riproduzione presso l’European<br />
Hosp<strong>it</strong>al di Roma: “Per arrivare a questi risultati<br />
- spiega Greco - siamo part<strong>it</strong>i dall’origine della<br />
malattia. La disfunzione erettile, e in particolare<br />
quella che dipende da problemi vascolari<br />
che si verifica nel 60-70% dei casi, viene<br />
determinata essenzialmente da una carenza di<br />
ossido n<strong>it</strong>rico a livello della cellula muscolare<br />
liscia intracavernosa; la cellula cioè da cui<br />
dipende l’erezione”. La somministrazione di<br />
c<strong>it</strong>rullina, secondo il test, rest<strong>it</strong>uisce ai pazienti<br />
l’equilibrio biologico che garantire all’organismo<br />
la concentrazione di ossido n<strong>it</strong>rico necessaria<br />
a raggiungere e mantenere l’erezione.<br />
La sperimentazione è stata condotta su 75<br />
uomini con un’età media di 48 anni. Il campione<br />
è stato diviso in tre gruppi da 25 persone.<br />
“Al primo gruppo abbiamo somministrato<br />
del placebo - spiega ancora Greco - , al<br />
secondo invece l’arginina, un aminoacido che<br />
insieme all’ossigeno forma l’ossido n<strong>it</strong>rico,<br />
ed al terzo la c<strong>it</strong>rullina”. Il test ha mostrato<br />
che l’arginina da sola non basta a risolvere il<br />
problema perché l’organismo la metabolizza<br />
sub<strong>it</strong>o, e quell’1% che resta in circolazione<br />
non riesce a produrre l’effetto. La c<strong>it</strong>rullina,<br />
invece, continua il ricercatore, ha mostrato di<br />
avere la capac<strong>it</strong>à di trasformarsi in arginina.<br />
Ma, a differenza della sostanza originale,<br />
non viene metabolizzata e resta in circolo.<br />
Per arrivare ai risultati finali sono stati verificati<br />
i parametri biochimici del campione preso in<br />
esame. “Controllando i valori ematici - dice Greco<br />
- abbiamo visto che la c<strong>it</strong>rullina effettivamente<br />
viene convert<strong>it</strong>a in arginina e che, in una<br />
dose pari a 6 grammi al giorno, ha l’effetto di<br />
aumentarne i livelli. Inoltre, questo integratore fa<br />
crescere ‘l’attiv<strong>it</strong>à’ delle cellule che garantiscono<br />
l’erezione e non ha alcun effetto collaterale.<br />
Per questo può essere utilizzato anche dai<br />
pazienti a rischio, con patologie vascolari,<br />
cardiache, ischemiche e può essere assunto<br />
anche con altre molecole anti-impotenza”.<br />
La sostanza è stata brevettata e lo studio,<br />
dopo la presentazione al Congresso della Sia,<br />
verrà sottoposto all’attenzione della comun<strong>it</strong>à<br />
scientifica internazionale. “Nel nostro campione<br />
di sperimentazione - dice Ermanno Greco -<br />
abbiamo avuto una risposta pos<strong>it</strong>iva nell’85% dei<br />
pazienti coinvolti. Speriamo, in questa maniera,<br />
di aprire una strada nuova per la ricerca”.<br />
Secondo Greco, solo 2 dei 3-4 milioni di <strong>it</strong>aliani<br />
che soffrono di disfunzione erettile si rivolgono<br />
allo specialista; gli altri convivono con il problema<br />
e lo tengono nascosto in camera da letto. “Il<br />
70% ha più di 40 anni - afferma lo studioso - ;<br />
generalmente si tratta di pazienti con problemi<br />
cardiovascolari, alcuni in sovrappeso e perlopiù<br />
fumatori. Tre su dieci assumono regolarmente<br />
farmaci anti-impotenza”.

Abstract<br />
PURPOSE: To investigate the pharmacokinetics of L-carn<strong>it</strong>ine (LC) and <strong>it</strong>s analogues, acetyl-L-carn<strong>it</strong>ine (ALC) and<br />
propionyl-L-carn<strong>it</strong>ine (PLC) in healthy volunteers after single L-carn<strong>it</strong>ine administration.METHODS: Liquid L-carn<strong>it</strong>ine<br />
(2.0 g) was administered orally as a single dose in 12 healthy subjects. Plasma and urine concentrations of L-carn<strong>it</strong>ine,<br />
ALC and PLC were detected by HPLC.<br />
RESULTS: The maximum plasma concentration (Cmax) and area under the curve (AUC 0-infin<strong>it</strong>y) of L-carn<strong>it</strong>ine was<br />
84.7+/-25.2 micromol x L(-1) x h and 2676.4+/-708.3 micromol x L(-1) x h, respectively. The elimination half-life of<br />
L-carn<strong>it</strong>ine and the time required to reach the Cmax (Tmax) was 60.3+/-15.0 and 3.4+/-0.46 h, respectively. The Cmax<br />
of ALC (12.9+/-5.5 micromol x L(-1)) and PLC (5.08+/-3.08 micromol x L(-1)) was lower than L-carn<strong>it</strong>ine (P < 0.01),<br />
so as the AUC 0-infin<strong>it</strong>y (166.2+/-77.4 and 155.6+/-264.2 micromol x L(-1) x h, respectively, P < 0.01). The half-life of<br />
ALC (35.9+/-28.9h) and PLC (25.7+/-30.3 h) was also shorter than L-carn<strong>it</strong>ine (P < 0.01). The 24h accumulated urinary<br />
excretion of L-carn<strong>it</strong>ine, ALC and PLC were 613.5+/-161.7, 368.3+/-134.8 and 61.3+/-37.8 micromol, respectively.<br />
CONCLUSION: L-carn<strong>it</strong>ine has a greater maximum plasma concentration than ALC and PLC. L-carn<strong>it</strong>ine also has a<br />
longer half-life than ALC and PLC. These data may have important implications in the designing of dosing regimens for<br />
L-carn<strong>it</strong>ine or <strong>it</strong>s analogues, such as ALC or PLC.
L-<strong>c<strong>it</strong>rulline</strong>-malate influence over branched chain<br />

Abstract<br />
Exhaustive exercise induces disturbances in metabolic homeostasis which can result in amino acid catabolism and<br />
lim<strong>it</strong>ed L-arginine availabil<strong>it</strong>y. <strong>Oral</strong> L-<strong>c<strong>it</strong>rulline</strong> <strong>supplementation</strong> raises plasma L-arginine concentration and augments<br />
NO-dependent signalling. Our aim was to evaluate the effects of diet <strong>supplementation</strong> w<strong>it</strong>h L-<strong>c<strong>it</strong>rulline</strong>-malate prior<br />
to intense exercise on the metabolic handle of plasma amino acids and on the products of metabolism of arginine as<br />
creatinine, urea and n<strong>it</strong>r<strong>it</strong>e and the possible effects on the hormonal levels. Seventeen voluntary male pre-professional<br />
cyclists were randomly assigned to one of two groups: control or supplemented (6 g L-<strong>c<strong>it</strong>rulline</strong>-malate 2 h prior<br />
exercise) and participated in a 137-km cycling stage. Blood samples were taken in basal cond<strong>it</strong>ions, 15 min after the race<br />
and 3 h post race (recovery). Most essential amino acids significantly decreased their plasma concentration as a result<br />
of exercise; however, most non-essential amino acids tended to significantly increase their concentration. C<strong>it</strong>rullinemalate<br />
ingestion significantly increased the plasma concentration of <strong>c<strong>it</strong>rulline</strong>, arginine, orn<strong>it</strong>hine, urea, creatinine and<br />
n<strong>it</strong>r<strong>it</strong>e (p < 0.05) and significantly decreased the isoleucine concentration from basal measures to after exercise (p <<br />
0.05). Insulin levels significantly increased after exercise in both groups (p < 0.05) returning to basal values at recovery.<br />
Growth hormone increased after exercise in both groups, although the increase was higher in the <strong>c<strong>it</strong>rulline</strong>-malate<br />
supplemented group (p < 0.05). L-<strong>c<strong>it</strong>rulline</strong>-malate <strong>supplementation</strong> can enhance the use of amino acids, especially the<br />
branched chain amino acids during exercise and also enhance the production of arginine-derived metabol<strong>it</strong>es such as<br />
n<strong>it</strong>r<strong>it</strong>e, creatinine, orn<strong>it</strong>hine and urea.
