La combinazione di probabilità<br />

clinica, US e D-dimeri è raccomandata<br />

nell’approccio diagnostico<br />

alla TVP (vedi allegati<br />

1,2,3).<br />

L’ecografia rappresenta l’esame<br />

di prima scelta per la diagnosi<br />

di trombosi venosa dell’arto<br />

superiore. La flebografia<br />

può trovare indicazione nei casi<br />

dubbi.<br />

La diagnosi definitiva di trombosi<br />

cavale si deve basare su<br />

angio-TC o RM, particolarmente<br />

indicate in presenza di malattia<br />

neoplastica.<br />

L’eco-color-doppler può essere<br />

considerato utile quale primo<br />

esame, soprattutto in presenza<br />

di TVP degli arti inferiori, ma un<br />

esito normale non esclude la<br />

diagnosi, soprattutto in presenza<br />

di un forte sospetto clinico.<br />

L’uso della cavografia dovrebbe<br />

essere limitata al planning<br />

chirurgico, in assenza di adeguata<br />

strumentazione TC o RM.<br />

Pur in assenza di studi di confronto<br />

con altre metodiche, la<br />

RM è il test diagnostico raccomandato<br />

per la diagnosi di<br />

trombosi venosa cerebrale.<br />

L’ecografia rappresenta il test<br />

di primo impiego per la diagnosi<br />

di trombosi portale. La TC e<br />

l’angiografia portale sono metodiche<br />

di approfondimento.<br />
