Scopo. Verificare l’efficacia del trattamento <strong>endovascolare</strong> <strong>degli</strong> <strong>aneurismi</strong><br />
<strong>dell</strong>’arteria splenica (AAS).<br />
Materiale e metodi. Nel periodo compreso tra maggio 2000 e giugno<br />
2003 sono stati trattati 11 AAS veri in 9 pazienti (7 femmine e 2 maschi;<br />
età media 58 anni), 8 sacciformi e 3 fusiformi, 4 localizzati al tratto<br />
medio, 5 al tratto distale e 2 intrasplenici. La diagnosi è stata effettuata<br />
con eco color Doppler e/o angio-TC ed è risultata occasionale in 7 pazienti<br />
e conseguente a dolore in ipocondrio sinistro in 1 caso; un AAS è stato<br />
riscontrato in fase di fissurazione. Quattro AAS sono stati esclusi<br />
mediante embolizzazione <strong>dell</strong>a sacca con microspirali, con preservazione<br />
<strong>dell</strong>a continu<strong>it</strong>à <strong>dell</strong>’asse vascolare; in 2 casi è stata associata l’iniezione<br />
transcatetere di cianoacrilato. In 4 casi è stata effettuata una<br />
legatura <strong>endovascolare</strong>, con ischemia settoriale <strong>dell</strong>a milza. Un AAS fissurato<br />
è stato trattato in urgenza con embolizzazione massiva mediante<br />
cianoacrilato <strong>dell</strong>’arteria splenica. I 2 <strong>aneurismi</strong> intrasplenici sono stati<br />
esclusi, l’uno mediante embolizzazione <strong>dell</strong>’arteria afferente con cianoacrilato<br />
e l’altro con iniezione transcatetere di trombina nella sacca<br />
aneurismatica.<br />
Risultati. È stata ottenuta la devascolarizzazione completa di tutti gli<br />
AAS (in 10/11 al termine <strong>dell</strong>a procedura; in 1/11 al controllo TC, effettuato<br />
dopo 3 giorni). Il follow-up (durata media 18 mesi; range 6-36<br />
mesi) è stato espletato con eco color Doppler e/o angio-TC a 3, 6, 12<br />
mesi e successivamente una volta all’anno; la completa esclusione <strong>degli</strong><br />
<strong>aneurismi</strong> è stata confermata in 11/11 casi. Le complicanze riscontrate<br />
sono state: 4 casi di pleur<strong>it</strong>e sinistra di modesta ent<strong>it</strong>à; febbre e dolore<br />
in ipocondrio sinistro il giorno successivo alla procedura (negli stessi 4<br />
pazienti e in un altro caso). Si sono osservati 5 casi di ischemia settoriale<br />
e 1 caso di infarto massivo <strong>dell</strong>a milza con parziale rivascolarizzazione<br />
splenica da parte di circoli collaterali. Non si sono verificate alterazioni<br />
<strong>degli</strong> enzimi pancreatici; è stata rilevata una piastrinosi trans<strong>it</strong>oria<br />
solo nel paziente con ischemia diffusa <strong>dell</strong>a milza.<br />
Conclusioni. Il trattamento <strong>endovascolare</strong> risulta attuabile, con tecniche<br />
differenti, in pressoché tutti gli AAS; garantisce ottimi risultati sia<br />
immediati che a distanza, presentando indubbi vantaggi nei confronti<br />
del trattamento chirurgico, in relazione alla minore invasiv<strong>it</strong>à e alla conservazione<br />
<strong>dell</strong>a funzional<strong>it</strong>à splenica.<br />

Endovascular treatment of splenic artery<br />
aneurysms<br />
Purpose. To assess the feasibil<strong>it</strong>y and effectiveness of<br />
endovascular treatment of splenic artery aneurysms<br />
(SAAs).<br />
Materials and methods. Between May 2000 and June<br />
2003 we treated 11 true SAAs in 9 patients (7 females and<br />
2 males; mean age 58 years), 8 saccular and 3 fusiform, 4<br />
located at the middle tract of the splenic artery, 5 at the<br />
distal tract and 2 intra-parenchymal. The diagnosis was<br />
performed w<strong>it</strong>h colour-Doppler ultrasound and/or CTangiography;<br />
7 patients were symptomless, 1 had left<br />
hypochondriac pain, and 1 had acute abdomen caused by<br />
a ruptured SAA. Four SAAs were treated by microcoil<br />
embolization of the aneurysmal sac w<strong>it</strong>h preservation of<br />
splenic artery patency; in 2 cases this was associated w<strong>it</strong>h<br />
transcatheter injection of N-butyl-2-cyanoacrylate. Four<br />
cases were treated by endovascular ligature, w<strong>it</strong>h sectoral<br />
spleen ischaemia. One ruptured SAA received emergency<br />
treatment w<strong>it</strong>h splenic artery cyanoacrylate embolization.<br />
Two intra-parenchymal SAAs were excluded, one by cyanoacrylate<br />
embolization of the afferent artery and the other<br />
by transcatheter thrombin injection in the aneurysmal sac.<br />
Results. Technical success was observed in all cases (in<br />
10/11 at the end of the procedure; in 1/11 at CT performed<br />
3 days after the procedure). The follow-up (mean 18<br />
months; range 6-36) was performed by colour-Doppler<br />
ultrasound and/or CT-angiography 3, 6 and 12 months<br />
after the procedure and subsequently once a year; the complete<br />
exclusion of the aneurysms was confirmed in 11/11<br />
cases. The complications were: 4 cases of mild left pleur<strong>it</strong>is;<br />
fever and left hypochondriac pain 1 day after the procedure<br />
(in the same 4 patients and in one other case); 5<br />
cases of sectoral spleen ischaemia and 1 case of diffuse<br />
spleen infarction w<strong>it</strong>h partial revascularization by collateral<br />
vessels. No alteration of the levels of pancreatic<br />
enzymes was found; a trans<strong>it</strong>ory increase in platelet count<br />
occurred only in the patient w<strong>it</strong>h diffuse spleen infarction.<br />
Conclusions. Using different techniques, endovascular<br />
treatment is feasible in nearly all SAAs. It ensures good<br />
immediate and long term results, and no doubt presents<br />
some advantages in comparison to surgical treatment, as<br />
<strong>it</strong> is less invasive and allows the preservation of splenic<br />
function.<br />

Introduzione<br />
L’aneurisma <strong>dell</strong>’arteria splenica (AAS) è il più frequente<br />
<strong>degli</strong> <strong>aneurismi</strong> viscerali, rappresentandone circa il 60%<br />
[1]; ha una maggiore incidenza nel sesso femminile, con<br />

rapporto variabile da 2:1 a 5:1 [2]. Gli AAS possono essere<br />
distinti in <strong>aneurismi</strong> “veri” e pseudo<strong>aneurismi</strong>. I primi comprendono:<br />
gli AAS displastici (13%), correlati a debolezza<br />
congen<strong>it</strong>a <strong>dell</strong>a parete arteriosa; gli AAS gravidici (57%),<br />
per i quali si ipotizza una patogenesi ormonale ed emodinamica;<br />
gli AAS associati ad ipertensione portale (10%); gli<br />
AAS aterosclerotici (15%) più frequenti nei maschi con patologia<br />
aterosclerotica diffusa. Gli pseudo<strong>aneurismi</strong> (5%) sono<br />
per lo più conseguenti a pancreat<strong>it</strong>i, ulcere gastriche perforate,<br />
embolizzazioni settiche o traumi [2-4].<br />
Dal punto di vista morfologico l’AAS può assumere aspetto<br />
fusiforme o sacciforme; presenta diametro variabile da<br />
pochi mm fino a 7-8 cm, con riduzione progressiva <strong>dell</strong>e<br />
dimensioni procedendo dall’origine <strong>dell</strong>’arteria verso i rami<br />
intrasplenici [5]; si localizza preferenzialmente al tratto<br />
medio-distale del vaso e può essere multiplo nel 20-30% dei<br />
casi [6].<br />
L’AAS è spesso asintomatico, di riscontro occasionale o<br />
autoptico [4]; più raramente (3-9.6% dei casi) viene diagnosticato<br />
in fase di fissurazione o di rottura con quadro di<br />
addome acuto, emoper<strong>it</strong>oneo fino allo shock emorragico [7].<br />
Per lungo tempo il trattamento è stato esclusivamente chirurgico<br />
ed ha comportato la legatura <strong>dell</strong>’arteria splenica<br />
associata alla splenectomia: rare sono le segnalazioni di<br />
aneurismectomia e ricostruzione vasale [2, 4, 8, 9].<br />
ratio from 2:1 to 5:1 [2]. SAAs can be classed as ‘true’<br />
aneurysms and pseudoaneurysms. The former include: dysplastic<br />
SAAs (13%) linked to congen<strong>it</strong>al weakness of the<br />
arterial wall; post- pregnancy SAAs (57%), probably related<br />
to hormonal and haemodynamic causes; SAAs associated<br />
w<strong>it</strong>h portal hypertension (10%); atherosclerotic SAAs<br />
(15%), more common in males w<strong>it</strong>h diffuse atherosclerosis.<br />
Pseudoaneurysms (5%) are more frequent as a result<br />
of pancreat<strong>it</strong>is, perforated gastric ulcers, septic embolizations<br />
or traumas [2, 3, 4].<br />
Morphologically, a SAA can be fusiform or saccular. It<br />
can have a diameter that varies from few mm to 7-8 cm,<br />
w<strong>it</strong>h a progressive dimensional reduction from the origin<br />
of the artery towards the intrasplenic branches [5]. The<br />
more frequent localization is the middle-distal third of the<br />
vessel; multiple occurrences are possible in 20-30% of<br />
cases. [6].<br />
A SAA is often asymptomatic, discovered incidentally or<br />
by autopsy [4]. It is more rarely diagnosed during rupture<br />
(3-9.6% of cases) in the presence of acute abdomen,<br />
haemoper<strong>it</strong>oneum and finally haemorrhagic shock [7].<br />
For a long time, treatment was exclusively surgical and<br />
involved the ligature of the splenic artery w<strong>it</strong>h splenectomy:<br />
reports of aneurysmectomy and vessel reconstruction are<br />
rare [2, 4, 8, 9].

Più recentemente è stato proposto il trattamento <strong>endovascolare</strong>:<br />
in letteratura sono segnalati sinora poco più di 60<br />
casi (la maggior parte trattati con spirali [10-32] - a volte in<br />
associazione con Gelfoam [11, 33, 34] - ; 1 caso mediante<br />
un palloncino a distacco [32] e solo alcuni mediante endoprotesi<br />
[32, 35-37]). In questo articolo viene presentata la<br />
serie personale di 11 AAS trattati con approccio <strong>endovascolare</strong>,<br />
confrontandone i risultati con quelli riportati in letteratura.<br />
Materiale e metodi<br />
Nel periodo compreso tra maggio 2000 e giugno 2003 (Tab.<br />
I) sono stati trattati 11 AAS veri in 9 pazienti (7 femmine, 2<br />
maschi), di età compresa tra 40 e 72 anni (età media di 58 anni).<br />
In 7 pazienti – asintomatici - la diagnosi è stata occasionale<br />
nel corso di esami ecografici o di Tomografia Computerizzata<br />
(TC) espletati per altri motivi; 1 caso si è appalesato con dolore<br />
in ipocondrio sinistro; nell’altro caso la diagnosi di AAS<br />
fissurato è stata formulata con un esame TC effettuato in urgenza<br />
per quadro di addome acuto e anemizzazione.<br />
Otto AAS erano sacciformi e 3 fusiformi, con diametro compreso<br />
tra 1,5 cm e 4 cm (diametro medio di 2,7 cm); per quanto<br />
concerne la sede, 4 AAS erano localizzati al tratto medio, 5<br />
AAS al tratto distale, 2 AAS erano intrasplenici. In entrambi<br />
i pazienti in cui vi era una doppia localizzazione, un AAS era<br />
localizzato al terzo distale, l’altro era intrasplenico.<br />
La valutazione pre-trattamento è stata completata mediante<br />
studio angiografico preliminare attuato nella stessa seduta<br />
<strong>dell</strong>a procedura interventistica.<br />
Le procedure sono state espletate con approccio percutaneo<br />
transfemorale destro in anestesia locale (lidocaina al<br />
2%), con cateterismo selettivo <strong>dell</strong>’arteria splenica.<br />
In 4 casi (2 AAS del terzo medio: fig. 1A-D; 2 AAS del<br />
terzo distale) si è proceduto alla sola esclusione aneurismatica<br />
preservando la continu<strong>it</strong>à <strong>dell</strong>’asse vascolare arterioso<br />
senza provocare infarto splenico, mediante il cateterismo<br />
superselettivo <strong>dell</strong>’AAS con microcatetere e successivo posizionamento,<br />
nel contesto <strong>dell</strong>a sacca, di spirali (microspirali<br />
in platino altamente radiopache con fibre in poliestere<br />
che ne aumentano la trombogenic<strong>it</strong>à). In 2/4 casi, al fine di<br />
stabilizzare l’agglomerato di spirali, è stata associata un’emulsione<br />
di Lipiodol e Cianoacrilato (rapporto 3:1) in soluzione<br />
glucosata ipertonica al 50%.<br />
In 4 casi (1 AAS del terzo medio; 3 AAS del terzo distale:<br />
figg. 2A-C; 3A-F) si è effettuata la cosiddetta “legatura<br />
<strong>endovascolare</strong>” (mediante il posizionamento di spirali in platino,<br />
di calibro adeguato al lume del vaso, prima a valle e<br />
poi a monte <strong>dell</strong>’AAS) con infarto <strong>dell</strong>a milza che è risultato<br />
soltanto settoriale.<br />
In 1 caso (AAS del terzo medio fissurato e tamponato, in<br />
paziente emodinamicamente stabile) trattato in urgenza in<br />
sala operatoria, è stata embolizzata l’arteria splenica mediante<br />
emulsione di Lipiodol e Cianoacrilato con infarto massivo<br />
<strong>dell</strong>a milza (fig. 4A-D).<br />
I 2 <strong>aneurismi</strong> intrasplenici sono stati esclusi l’uno mediante<br />
occlusione <strong>dell</strong>’arteria afferente con un’emulsione di<br />
Lipiodol e Cianoacrilato, l’altro mediante iniezione endoaneurismatica<br />
trans-catetere di trombina, previo cateterismo<br />
superselettivo <strong>dell</strong>a sacca con microcatetere (fig. 3A-D).<br />

More recently, endovascular treatment has been proposed,<br />
and to date, just over 60 cases have been reported in the l<strong>it</strong>erature<br />
(the major<strong>it</strong>y treated w<strong>it</strong>h coils [10-32], sometimes<br />
associated w<strong>it</strong>h Gelfoam [11, 33, 34]; 1 case w<strong>it</strong>h a detach-

Risultati<br />
In 10/11 <strong>degli</strong> AAS trattati (tab. I) si è ottenuta la completa<br />
e immediata esclusione, documentata al controllo angioable<br />
balloon [32] and only a few w<strong>it</strong>h stent-grafting [32, 35,<br />
36, 37]). This paper presents our experience of 11 SAAs<br />
treated w<strong>it</strong>h endovascular approach and compares the results<br />
to those reported in l<strong>it</strong>erature.

grafico effettuato al termine <strong>dell</strong>a procedura (figg. 1D, 2C,<br />
3D) ed all’eco color Doppler e/o all’angio-TC espletati entro<br />
7 giorni dal trattamento (figg. 2D, 4B-C), sub<strong>it</strong>o prima <strong>dell</strong>a<br />
dimissione; 1 AAS, ancora parzialmente perfuso sub<strong>it</strong>o<br />
dopo l’embolizzazione, è apparso totalmente escluso al controllo<br />
TC effettuato a distanza di 3 giorni (fig. 3F-G). I controlli<br />
successivi - effettuati con eco color Doppler e/o angio-<br />
TC a 3, 6, 12 mesi e successivamente 1 volta all’anno (durata<br />
media del follow-up: 18 mesi; range: 6-36 mesi) hanno<br />
confermato la persistenza <strong>dell</strong>’esclusione <strong>degli</strong> AAS (figg.<br />
1E-F, 2E-F, 3H-I).<br />
Il tempo medio di degenza è risultato di 3 giorni (range:<br />
1-7 giorni).<br />
Le complicanze riscontrate nella serie personale sono state:<br />
versamento pleurico sinistro di modesta ent<strong>it</strong>à (verificatosi<br />
in 4 pazienti); febbre (T < 38°) e dolore in ipocondrio<br />
sinistro il giorno successivo alla procedura (negli stessi 4<br />
pazienti e in un altro caso): tale sintomatologia è stata agevolmente<br />
controllata con terapia medica (antipiretici e analgesici).<br />
Non si sono mai verificate complicanze ascessuali<br />
in sede splenica.<br />
Il mon<strong>it</strong>oraggio bioumorale non ha mai evidenziato alterazione<br />
<strong>degli</strong> enzimi pancreatici e ha fatto rilevare una piastrinosi<br />
trans<strong>it</strong>oria nel paziente con ischemia massiva <strong>dell</strong>a<br />
milza; in tale evenienza la funzional<strong>it</strong>à splenica è stata parzialmente<br />
ripristinata, dal momento che il conteggio piastrinico<br />
si è normalizzato dopo 3 mesi e gli esami TC di follow<br />
up hanno evidenziato la progressiva involuzione <strong>dell</strong>a<br />
milza che -al controllo a 12 mesi- è risultata sensibilmente<br />
ridotta di volume, con “isole” di parenchima residuo vascolarizzate<br />
attraverso circoli collaterali (fig. 4E).<br />
Discussione<br />
L’AAS è una patologia rara, asintomatica per lungo tempo,<br />
spesso rilevata occasionalmente in corso di indagini diagnostiche<br />
esegu<strong>it</strong>e per altri motivi [10]. La storia naturale<br />
<strong>dell</strong>’AAS è il suo progressivo aumento di dimensioni sino<br />
alla rottura; pertanto – con l’eccezione <strong>degli</strong> <strong>aneurismi</strong> veri<br />
con diametro inferiore a 2 cm, per i quali è proponibile il<br />
follow-up ecografico e/o TC – si rende sempre necessario il<br />
trattamento [3, 32].<br />
Gli pseudo<strong>aneurismi</strong>, viceversa, in relazione al loro elevato<br />
rischio di rottura richiedono un provvedimento terapeutico<br />
in tutti i casi, indipendentemente dalle loro dimensioni<br />
[17, 32].<br />
La terapia chirurgica consiste generalmente nella legatura<br />
<strong>dell</strong>’arteria splenica associata a splenectomia per via laparotomica<br />
o laparoscopica e comporta in elezione una morbil<strong>it</strong>à<br />
del 9% ed una mortal<strong>it</strong>à <strong>dell</strong>’1,3%, che sale al 20% in<br />
urgenza e al 70% in gravidanza [17].<br />
Per quanto riguarda l’approccio <strong>endovascolare</strong>, il trattamento<br />
embolizzante può essere realizzato con due tecniche:<br />
embolizzazione mediante spirali lim<strong>it</strong>ata alla sacca aneurismatica,<br />
con mantenimento <strong>dell</strong>a continu<strong>it</strong>à <strong>dell</strong>’arteria splenica<br />
[11, 32] (fig. 1); “legatura <strong>endovascolare</strong>”, che prevede<br />
il posizionamento di spirali metalliche a monte e a valle<br />
<strong>dell</strong>’AAS al fine di ottenerne una completa devascolarizzazione<br />
(figg. 2, 3): nella seconda evenienza, l’afflusso ematico<br />
alla milza è mantenuto - almeno parzialmente- dalla pre-<br />

Materials and methods<br />
Between May 2000 and June 2003 (Tab. I) we treated 11<br />
true SAAs in 9 patients (7 female, 2 male), of an age ranging<br />
from 40 to 72 (mean age of 58). In 7 asymptomatic patients<br />
the diagnosis was made incidentally during Ultrasound or<br />
Computerized Tomography (CT) examinations carried out<br />
for other reasons. One case presented w<strong>it</strong>h left hypochondriac<br />
pain, and in the other case, the diagnosis of a ruptured<br />
SAA was achieved by a CT examination urgently carried out<br />
due to acute abdomen and anaemia.

senza di circoli collaterali [10, 22, 32]. La scelta del tipo di<br />
embolizzazione dipende essenzialmente dalla morfologia e<br />
dalla sede <strong>degli</strong> AAS: il flusso arterioso può essere preservato<br />
negli AAS sacciformi del tratto medio e del terzo distale<br />
a monte <strong>dell</strong>a biforcazione <strong>dell</strong>’arteria splenica stessa,<br />
mentre negli <strong>aneurismi</strong> fusiformi e in quelli del terzo distale<br />
coinvolgenti la biforcazione o localizzati in corrispondenza<br />
dei rami di suddivisione arteriosa deve essere esegu<strong>it</strong>a<br />
la “legatura <strong>endovascolare</strong>”.<br />
Gli AAS a sede intraparenchimale vengono preferenzial-<br />
Eight SAAs were saccular and 3 were fusiform, w<strong>it</strong>h a<br />
diameter between 1.5 cm and 4 cm (mean diameter of 2.7<br />
cm). Four SAAs were located in the middle third, 5 SAAs in<br />
the distal third and 2 SAAs were intrasplenic. In both patients<br />
who had double aneurysm localization, one SAA was located<br />
in the distal third, the other was intrasplenic.<br />
Pre-treatment evaluation was completed using a preliminary<br />
angiographic study which was carried out at the time<br />
of the operative procedure.<br />

mente embolizzati mediante cateterizzazione superselettiva,<br />
ottenuta con microcatetere coassiale, <strong>dell</strong>’arteria afferente e<br />
iniezione nella sacca di materiale embolizzante defin<strong>it</strong>ivo (trombina<br />
o cianoacrilato: fig 3C-D ). Qualora tale procedura non<br />
sia tecnicamente fattibile, si può ricorrere all’embolizzazione<br />
del ramo arterioso afferente all’aneurisma.<br />
Gli AAS localizzati al terzo prossimale <strong>dell</strong>’arteria, in prossim<strong>it</strong>à<br />
<strong>dell</strong>a sua emergenza dal tripode celiaco, fino a qualche<br />
anno fa rappresentavano una controindicazione al trattamento<br />
<strong>endovascolare</strong>: la sede infatti non consente la legatura endovaneous<br />
transfemoral approach under local anaesthetic (2%<br />
lidocaine), w<strong>it</strong>h selective catheterization of the splenic artery.<br />
In 4 cases (2 SAAs of the middle third: fig. 1A-B-C-D; 2<br />
SAAs of the distal third) we aimed for aneurysm exclusion<br />
conserving the continu<strong>it</strong>y of the vascular artery axis w<strong>it</strong>hout<br />
causing a spleen infarction. This involved the superselective<br />
catheterization of the SAA w<strong>it</strong>h a microcatheter and<br />
the subsequent pos<strong>it</strong>ioning of coils (highly radiodense platinum<br />
microcoils w<strong>it</strong>h polyester fibres which increase thrombogenic<strong>it</strong>y)<br />
in the sac. In 2/4 cases, in order to stabilise the<br />
agglomerate of coils, an emulsion of Lipiodol and<br />
Cyanoacrylate was added (3:1) in a hypertonic glucose solution<br />
of 50%.<br />
In 4 cases (1 SAA of the middle third; 3 SAAs of the distal<br />
third: fig. 2A-B-C; fig. 3A-B-D-E-F) the so-called<br />
“endovascular ligature” was performed (by pos<strong>it</strong>ioning the<br />
coils of appropriate calibre for the vessel lumen, first downstream<br />
and then upstream of the SAA) w<strong>it</strong>h only sectoral<br />
spleen infarction.<br />
In 1 case (a ruptured SAA of the middle third in a haemodynamically<br />
stable patient) treated urgently in the operating<br />
theatre, the splenic artery was embolized using Lipiodol<br />
and Cyanoacrylate emulsion w<strong>it</strong>h massive infarction of the<br />
spleen (fig. 4A-B-C-D).<br />
The intrasplenic aneurysms were excluded, one by occlusion<br />
of the afferent artery w<strong>it</strong>h Lipiodol and Cyanoacrylate<br />
emulsion, the other by an endoaneurysmal transcatheter<br />
injection of thrombin, after superselective catheterization of<br />
the sac w<strong>it</strong>h a microcatheter. (fig. 3A-B-C-D).<br />
Results<br />
In 10/11 of the SAAs treated (tab. I), complete and immediate<br />
exclusion was observed at angiography carried out at<br />
the end of the procedure (fig. 1D; fig. 2C; fig. 3D) and at<br />
colour-Doppler ultrasound and/or CT-angiography performed<br />
w<strong>it</strong>hin 7 days (fig. 2D; fig. 4B-C) immediately before<br />
discharge. One SAA was still partially perfused after immediate<br />
embolization, and appeared completely excluded at<br />
the CT carried out after 3 days (fig. 3F-G). Colour-Doppler<br />
ultrasound and/or CT-angiography follow-up at 3, 6, 12<br />
months and subsequently once a year (mean length: 18<br />
months, range: 6-36 months) confirmed the persistence of<br />
SAA exclusion (fig. 1E-F; fig. 2E-F; fig. 3H-I).<br />
The average hosp<strong>it</strong>alisation time was 3 days (range: 1-7<br />
days).<br />
The complications encountered in our experience were:<br />
a modest amount of left pleural effusion (four patients); fever<br />
(T < 38°) and left hypochondriac pain the following day (in<br />
the same 4 patients and in one other case). Such symptoms<br />
were easily controlled w<strong>it</strong>h medical therapy (antipyretic and<br />
analgesic drugs). No abscess complications in the splenic<br />
area were ever noted.<br />
Laboratory mon<strong>it</strong>oring never showed alterations of the<br />
pancreatic enzymes but revealed a transient increase in<br />
platelet levels in the patient w<strong>it</strong>h diffuse spleen ischemia. In<br />
this case, splenic function was partially restored and the<br />
platelet count returned to normal after 3 months; at the fol-

<strong>dell</strong>a pervietà vascolare [10]; gli stents ricoperti<br />
sarebbero teoricamente preferibili per il trattamento sia <strong>degli</strong><br />
<strong>aneurismi</strong> fusiformi che di quelli sacciformi [35-37, 39]: l’imlow-up<br />

spleen was observed; at the 12 month follow-up the volume<br />
of the spleen was found to be considerably reduced, w<strong>it</strong>h<br />
‘islands’ of residual vascularized parenchyma supplied by<br />

Discussion<br />
SAA is a rare disease, asymptomatic for a long time and<br />
often only incidentally discovered during imaging carried

piego di tali devices peraltro è ostacolato dalla tortuos<strong>it</strong>à e dal<br />
ridotto calibro <strong>dell</strong>’arteria splenica [31]. Attualmente l’unica<br />
controindicazione al trattamento <strong>endovascolare</strong> è cost<strong>it</strong>u<strong>it</strong>a dagli<br />
AAS rotti in cui il paziente è emodinamicamente instabile e<br />
pertanto, in rapporto alla s<strong>it</strong>uazione di emergenza, deve essere<br />
sottoposto ad immediato intervento chirurgico di splenectomia.<br />
Nell’esperienza personale sono stati trattati 11 AAS in 9<br />
pazienti, tutti con le caratteristiche di <strong>aneurismi</strong> veri; va sottolineato<br />
che la nostra serie è tra quelle numericamente più<br />
consistenti riportate in letteratura insieme a quelle di<br />
out for other reasons. [10]. The natural history of the SAA<br />
is a progressive increase in size until rupture. Therefore, w<strong>it</strong>h<br />
the exception of true aneurysms w<strong>it</strong>h a diameter less than 2<br />
cm which can be followed up by Ultrasound and/or CT, treatment<br />
is always necessary [3, 32].<br />
By contrast, given the high risk of rupture of pseudoaneurysms,<br />
these always require treatment regardless of size<br />
[17, 32].<br />
Surgical therapy generally involves the ligature of the<br />
splenic artery w<strong>it</strong>h splenectomy by way of laparotomy or<br />
laparoscopy; the procedure carries a morbid<strong>it</strong>y of 9% and<br />
a mortal<strong>it</strong>y of 1.3%, which rises to 20% in cases of emergency<br />
and to 70% during pregnancy [17].<br />
W<strong>it</strong>h regard to the endovascular approach, embolization<br />
can be carried out using two techniques: embolization<br />
using coils lim<strong>it</strong>ed to the aneurysmal sac, maintaining<br />
continu<strong>it</strong>y in the splenic artery [11, 32] (fig. 1);<br />
“endovascular ligature”, which requires the pos<strong>it</strong>ioning<br />
of the metal coils both upstream and downstream of the<br />
SAA in order to attain a complete devascularization (figs.<br />
2, 3). In the second occurrence, blood flow to the spleen<br />
is at least in part maintained by the collateral vessels [10,<br />
22, 32]. The choice of embolization type essentially depends<br />
on the morphology and location of the SAA. Arterial flow<br />
can be preserved in saccular SAAs of the middle third and<br />
distal third upstream of the bifurcation of the splenic artery.<br />
Conversely, fusiform aneurysms and those in the distal<br />
third involving the bifurcation must be treated w<strong>it</strong>h<br />
‘endovascular ligature’.<br />
Ideally, intraparenchymal SAAs are embolized by superselective<br />
catheterization, achieved w<strong>it</strong>h a coaxial microcatheter<br />
in the afferent artery and an injection of defin<strong>it</strong>ive<br />
embolizing material (thrombin or Cyanoacrylate: fig 3C-D)<br />
in the sac. Should such procedure not be feasible, <strong>it</strong> is possible<br />
to turn to embolization of the afferent artery branch of<br />
the aneurysm.<br />
Until a few years ago, localization of a SAA in the proximal<br />
third of the artery, near <strong>it</strong>s emergence from the celiac<br />
trunk, represented a contraindication to endovascular<br />
treatment. The location does not allow endovascular ligature,<br />
as there is not enough space to place the coils<br />
upstream of the aneurysm. Moreover, should the SAA have<br />
a large neck, embolization of the aneurysmal sac is not<br />
advised, as the coils can migrate to other branches of the<br />
celiac artery. Embolization using three-dimensional volumetric<br />
coils w<strong>it</strong>h controlled release have recently been<br />
proposed for proximal saccular SAAs, w<strong>it</strong>h preservation<br />
of vascular patency [10]. Covered stents would be preferred<br />
when treating both fusiform and saccular aneurysms<br />
[35, 36, 37, 39]; yet the use of such devices is hindered<br />
by the tortuos<strong>it</strong>y and the small diameter of the splenic<br />
artery [31].<br />
The only current contraindication of endovascular treatment<br />
is represented by ruptured SAAs when the patient is<br />
haemodynamically unstable; this emergency s<strong>it</strong>uation requires<br />
an immediate surgical splenectomy.<br />
In our experience, 11 SAAs were treated in 9 patients, all<br />
w<strong>it</strong>h characteristics of true aneurysms. It should be noted

Gabelmann et al. (10 osservazioni, di cui 5 pseudo<strong>aneurismi</strong><br />
correlati a pancreat<strong>it</strong>e) [27] e di Guillon et al. (12 osservazioni,<br />
di cui 2 pseudo<strong>aneurismi</strong>) [32].<br />
In 10/11 <strong>degli</strong> AAS trattati si è ottenuto un successo tecnico<br />
immediato; l’eco color Doppler e l’angio-TC hanno documentato<br />
la devascolarizzazione di tutti gli AAS già al controllo<br />
espletato nella prima settimana dal trattamento; i controlli<br />
successivi (follow-up compreso tra 6 mesi e 36 mesi;<br />
valore medio di 18 mesi) ne hanno confermato la persistente<br />
completa esclusione. L’Angio-TC ha forn<strong>it</strong>o altresì informazioni<br />
accurate sulla vascolarizzazione del parenchima<br />
splenico, nonostante la frequente concom<strong>it</strong>anza di importanti<br />
artefatti da indurimento del fascio dovuti alle spirali.<br />
Dalla revisione <strong>dell</strong>a letteratura, a partire dal 1978, risultano<br />
pubblicati 61 casi sottoposti a trattamento <strong>endovascolare</strong>:<br />
in 56 sono state impiegate le spirali [10-32], in 3 casi<br />
associate a Gelfoam [11, 33, 34]; solo in 4 pazienti sono stati<br />
utilizzati stents ricoperti [32, 35-37]; 1 caso infine è stato<br />
embolizzato mediante palloncino a distacco [32]). La percentuale<br />
di successo è del 90%; tra le cause di insuccesso<br />
sono state riportate la ricanalizzazione da parte di circoli<br />
collaterali [16, 28] e la migrazione <strong>dell</strong>o stent-graft [32].<br />
Per quanto riguarda il follow up (compreso tra 2 e 69 mesi;<br />
valore medio di 35 mesi) sono riportati unicamente alcuni<br />
casi di riperfusione a pochi giorni dal trattamento [17, 32];<br />
non sono peraltro descr<strong>it</strong>ti casi di ricanalizzazione <strong>degli</strong> AAS<br />
a medio - lungo termine [32]. Pertanto parrebbe sufficiente,<br />
anche alla luce <strong>dell</strong>’esperienza personale, un follow up<br />
con imaging non superiore a 6 mesi, con eventuali ulteriori<br />
controlli motivati solo dalla comparsa di sintomi.<br />
Tra le complicanze del trattamento <strong>endovascolare</strong>, riportate<br />
in letteratura, vanno rammentate: il danno ischemico<br />
<strong>dell</strong>a milza [12]; il dolore in ipocondrio sinistro, la febbre e<br />
l’aumento trans<strong>it</strong>orio <strong>degli</strong> enzimi pancreatici (che configurano<br />
la sindrome post-embolizzazione [32]); il versamento<br />
pleurico sinistro [16]; la migrazione accidentale <strong>dell</strong>e spirali<br />
o <strong>dell</strong>o stent-graft [31, 39]; le complicanze nella sede di<br />
accesso arterioso.<br />
Le complicanze riscontrate nella serie personale non sono<br />
dissimili: febbre e dolore addominale in 5 pazienti, in 4 dei<br />
quali concom<strong>it</strong>ava pleur<strong>it</strong>e sinistra di modesta ent<strong>it</strong>à. In particolare<br />
va sottolineato che la funzional<strong>it</strong>à splenica è stata preservata<br />
in tutti i casi; anche nella paziente in cui è stata embolizzata<br />
l’intera arteria splenica, con conseguente ischemia parenchimale<br />
massiva, si è verificata solo una tromboc<strong>it</strong>osi trans<strong>it</strong>oria-<br />
con normalizzazione del conteggio piastrinico a 3 mesi<br />
di distanza dalla procedura- grazie ai circoli collaterali che hanno<br />
garant<strong>it</strong>o la vascolarizzazione del parenchima splenico residuo<br />
e ne hanno determinato la progressiva ipertrofia (fig. 4).<br />
Il trattamento <strong>endovascolare</strong> offre numerosi vantaggi<br />
rispetto alla tradizionale terapia chirurgica [11, 15, 17, 40]:<br />
minore invasiv<strong>it</strong>à, ricorso all’anestesia locale, approccio più<br />
semplice in quegli AAS in cui l’esposizione chirurgica è<br />
indaginosa (per esempio in conseguenza di pregressi interventi),<br />
riduzione dei tempi intercorrenti fra diagnosi e terapia,<br />
riduzione dei tempi di ospedalizzazione con conseguente<br />
riduzione dei costi per la struttura san<strong>it</strong>aria; il vantaggio più<br />
importante consiste peraltro nella conservazione <strong>dell</strong>a funzione<br />
splenica, che viene perduta nella maggioranza <strong>degli</strong><br />
interventi chirurgici, in cui la legatura <strong>dell</strong>’arteria splenica<br />
è associata alla splenectomia.<br />
that our series is one of the largest reported in the l<strong>it</strong>erature<br />
along w<strong>it</strong>h that of Gabelmann et al. (10 observations, 5 of<br />
which were pseudoaneurysms related to pancreat<strong>it</strong>is) [27]<br />
and Guillon et al. (12 observations, 2 of which were pseudoaneurysms)<br />
[32].<br />
In 10/11 of the SAAs treated, technical success was immediate.<br />
Colour-Doppler ultrasound and CT-angiography also<br />
showed devascularization of all the SAAs at the examinations<br />
carried out in the first week after treatment. Followup<br />
(range 6-36 months; mean 18 months) confirmed the complete<br />
exclusion. CT angiography also provided accurate<br />
information about the vascularization of the splenic parenchyma,<br />
desp<strong>it</strong>e the frequent presence of significant artefacts due<br />
to the coils.<br />
A review of the l<strong>it</strong>erature since 1978 identified 61 cases<br />
of endovascular treatment. In 56 of these, coils were used<br />
[10-32], in 3 cases associated w<strong>it</strong>h Gelfoam [11, 33, 34];<br />
covered stents were used in only 4 patients [32, 35, 36, 37],<br />
while one case was embolized using a detachable balloon<br />
[32]. The success rate was 90%. The reported causes of nonsuccess<br />
included recanalization of the collateral vessels [16,<br />
28] and stent-graft migration [32].<br />
Concerning the follow-up (range: 2-69 months; mean 35<br />
months), few cases of reperfusion are reported to have<br />
occurred soon after treatment [17, 32]; no mid-long term<br />
cases of recanalization have been reported [32]. These data,<br />
confirmed by our experience, show that a 6-month followup<br />
by imaging would appear to be enough, w<strong>it</strong>h further examinations<br />
carried out only in case of symptoms.<br />
Amongst the reported complications of endovascular treatment,<br />
the following should be recalled: ischaemic damage<br />
of the spleen [12]; left hypochondrium pain, fever and the<br />
trans<strong>it</strong>ory increase of pancreatic enzymes (which form the<br />
post-embolization syndrome [32]), left pleural effusion [16];<br />
accidental migration of the coils or stent-graft [31, 39] and<br />
complications related to the arterial access.<br />
The complications identified in our experience are not<br />
dissimilar. They include fever and abdominal pain in 5<br />
patients, 4 of whom complained of mild left pleur<strong>it</strong>is. In particular,<br />
splenic function was preserved in all cases; even in<br />
the patient where the whole splenic artery was embolized,<br />
w<strong>it</strong>h diffuse parenchymal ischaemia, only a temporary thrombocytosis<br />
was observed and the platelet count returned to<br />
normal after 3 months; this was due to the collateral vessels<br />
which guaranteed the vascularization of the remaining<br />
splenic parenchyma and caused <strong>it</strong>s progressive hypertrophy<br />
(fig. 4).<br />
Endovascular treatment offers many advantages compared<br />
to surgical therapy [11, 15, 17, 40]. It is less invasive,<br />
uses local anaesthetic, and <strong>it</strong> provides a more simple approach<br />
to SAAs where trad<strong>it</strong>ional surgery is cumbersome (for instance<br />
following previous operations). It also offers a reduction in<br />
time between diagnosis and therapy, shorter hosp<strong>it</strong>al stays<br />
and therefore a reduction in cost for the health system. The<br />
most important advantage is the preservation of splenic function,<br />
which is lost in the major<strong>it</strong>y of surgical cases where<br />
the ligature of the splenic artery is associated w<strong>it</strong>h splenectomy.

Conclusioni<br />
Il trattamento <strong>endovascolare</strong> rappresenta attualmente l’opzione<br />
di prima scelta in condizioni di elezione e di urgenza<br />
- purché il paziente sia emodinamicamente stabile -, in quanto<br />
risulta attuabile, con tecniche differenti, in pressoché tutti<br />
i casi; garantisce ottimi risultati sia immediati sia a distanza;<br />
presenta indubbi vantaggi nei confronti <strong>dell</strong>a terapia chirurgica,<br />
con particolare riguardo alla minore invasiv<strong>it</strong>à e alla<br />
preservazione <strong>dell</strong>a funzional<strong>it</strong>à splenica.<br />
Conclusion<br />
In conclusion, endovascular treatment currently represents<br />
the preferred option in elective cases and in emergency<br />
settings provided the patient is haemodynamically stable. It<br />
is feasible in nearly all cases given that various techniques<br />
can be used; <strong>it</strong> guarantees both good immediate and longterm<br />
results; <strong>it</strong> presents doubtless advantages compared to<br />
surgical treatment, particularly because <strong>it</strong> is less invasive<br />
and preserves splenic function.<br />
