We are here to answer your questions about our health plan and the benefits for you or your child. Please call us at the<br />

This directory is in two parts.<br />

The first part lists the doctors and clinics who can provi<strong>de</strong> your prenatal care. It also lists the hospitals where you<br />

may <strong>de</strong>liver your baby.<br />

You do not need a referral to make an appointment with a prenatal care provi<strong>de</strong>r. You can call one on the list<br />

yourself. If you need help making an appointment, call Member Services at the number below.<br />

4. The second part lists the primary care provi<strong>de</strong>rs, specialists and hospitals that can provi<strong>de</strong> medical care for your<br />

newborn baby. You must choose a primary care provi<strong>de</strong>r for your baby and let CHIP and <strong>Community</strong> <strong>First</strong> know<br />

who it is. Be sure to give us the primary care provi<strong>de</strong>r ID# next to the doctor’s name. If you do not, one will be<br />

chosen for you.<br />

5. You do not need a referral from your baby’s primary care provi<strong>de</strong>r to make an appointment with a behavioral<br />

health provi<strong>de</strong>r. You can call one on the list yourself. Member Services can help you. Call them at the number<br />

below.<br />

6. Your Prescription Drug Benefits<br />

<strong>Community</strong> <strong>First</strong> does not cover your prescription drugs. These drugs are covered by the CHIP Prescription<br />

Drug Benefit (PDB). You can use any pharmacy taking part in the CHIP PDB. Call CHIP if you need help finding a<br />

pharmacy. Their telephone number is 1-866-274-9154.<br />

Your baby’s doctor<br />

Your baby’s primary care provi<strong>de</strong>r is his or her own doctor, who will provi<strong>de</strong> or arrange for medical care. Even if your<br />

child has a true emergency, the primary care provi<strong>de</strong>r will manage his or her treatment so that your child gets the right<br />

kind of care. If your child needs to see a specialist, the primary care provi<strong>de</strong>r will arrange it. If your child needs to go<br />

into the hospital, the primary care provi<strong>de</strong>r will arrange it and manage his or her care while there. If your child has<br />

a medical emergency (in or out of the service area):<br />

• Go to the nearest emergency room.<br />

• Contact your child’s primary care provi<strong>de</strong>r within 24 hours, or as soon as possible, so that he or she can arrange for<br />

follow-up care. If you cannot call, have someone call for you.<br />

• Coordinate all follow-up care through your child’s primary care provi<strong>de</strong>r. You can call your child’s primary care<br />

provi<strong>de</strong>r any time of the day or night. Someone will always be able to take your call.<br />
