We are here to answer your questions about our health plan and the benefits for you or your child. Please call us at the

This directory is in two parts.
The first part lists the doctors and clinics who can provide your prenatal care. It also lists the hospitals where you
may deliver your baby.
You do not need a referral to make an appointment with a prenatal care provider. You can call one on the list
yourself. If you need help making an appointment, call Member Services at the number below.
4. The second part lists the primary care providers, specialists and hospitals that can provide medical care for your
newborn baby. You must choose a primary care provider for your baby and let CHIP and Community First know
who it is. Be sure to give us the primary care provider ID# next to the doctor’s name. If you do not, one will be
chosen for you.
5. You do not need a referral from your baby’s primary care provider to make an appointment with a behavioral
health provider. You can call one on the list yourself. Member Services can help you. Call them at the number
below.
6. Your Prescription Drug Benefits
Community First does not cover your prescription drugs. These drugs are covered by the CHIP Prescription
Drug Benefit (PDB). You can use any pharmacy taking part in the CHIP PDB. Call CHIP if you need help finding a
pharmacy. Their telephone number is 1-866-274-9154.
Your baby’s doctor
Your baby’s primary care provider is his or her own doctor, who will provide or arrange for medical care. Even if your
child has a true emergency, the primary care provider will manage his or her treatment so that your child gets the right
kind of care. If your child needs to see a specialist, the primary care provider will arrange it. If your child needs to go
into the hospital, the primary care provider will arrange it and manage his or her care while there. If your child has
a medical emergency (in or out of the service area):
• Go to the nearest emergency room.
• Contact your child’s primary care provider within 24 hours, or as soon as possible, so that he or she can arrange for
follow-up care. If you cannot call, have someone call for you.

• Coordinate all follow-up care through your child’s primary care provider. You can call your child’s primary care
provider any time of the day or night. Someone will always be able to take your call.
