DIFFERENT EFFECTS OF CARDIAC RESYNCHRONIZATION THERAPY ON<br />

LEFT ATRIAL FUNCTION IN PATIENTS WITH EITHER IDIOPATHIC OR<br />

ISCHEMIC DILATED CARDIOMYOPATHY: A TWO-DIMENSIONAL SPECKLE<br />

Background. In <strong>di</strong>lated car<strong>di</strong>omyopathy (DCM), attenuation of left atrial (LA)<br />

booster pump function has been observed, and attributed both to altered LA<br />

loa<strong>di</strong>ng con<strong>di</strong>tions owing to left ventricular (LV) <strong>di</strong>astolic dysfunction, and to LA<br />

involvement in the myopathic process.<br />

Aims of the study. To detect by speckle-tracking two-<strong>di</strong>mensional strain (2DSE)<br />

LA systolic dysfunction in DCM, and to assess effects of car<strong>di</strong>ac resynchronization<br />

therapy (CRT) on LA myocar<strong>di</strong>al strain during 6-month follow-up.<br />

Methods. Ninety patients (52.4±10.2 yrs) with either i<strong>di</strong>opathic (47 patients) or<br />

ischemic (43 patients) DCM underwent standard Doppler echo and 2DSE<br />

analysis of atrial longitu<strong>di</strong>nal strain in the basal segments of LA septum and LA<br />

lateral wall, and in LA roof.<br />

Results. The two groups were comparable for clinical variables (NYHA class: III<br />

in 72.2%; IV in 27.8%). LV volumes, ejection fraction, stroke volume, and mitral<br />

valve ERO were similar between the two groups. No significant <strong>di</strong>fferences were<br />

evidenced in Doppler transmitral inflow measurements. Also LA <strong>di</strong>ameter and<br />

maximal volume were similar between the two groups. Conversely, LA active<br />

emptying volume and fraction were both lower in patients with i<strong>di</strong>opathic DCM.<br />

Peak systolic myocar<strong>di</strong>al atrial strain was significantly compromised in patients<br />

with i<strong>di</strong>opathic DCM compared with ischemic DCM in all the analyzed atrial<br />

segments (p15%).<br />

A significant improvement in LA systolic function was obtained only in patients<br />

with ischemic DCM responders to CRT (p
