Post-HD, TDI-derived S was significantly reduced both at basal and mid-ventricle<br />
level. Conversely, 2DSTE-derived S was not affected by acute preload reduction<br />
at any level of the LV.<br />
In conclusion, our study provides evidence that S assessed by 2DSTE is a<br />
preload independent estimate of LV myocar<strong>di</strong>al deformation, and that 2DSTE<br />
may be a useful tool for clinicians to evaluate LV performance in terms of S also<br />
in changeable preload con<strong>di</strong>tions.<br />

In presenza <strong>di</strong> car<strong>di</strong>opatia il tempo <strong>di</strong> transito <strong>di</strong> un mezzo <strong>di</strong> contrasto nel circolo<br />
polmonare (TTP) è inversamente proporzionale alla funzione contrattile del<br />
ventricolo sinistro (VS), essendo questa relazione me<strong>di</strong>ata dall’incremento delle<br />
resistenze vascolari polmonari (RVP). È ipotizzabile che il TTP aumenti anche<br />
con il ridursi della funzione contrattile del ventricolo destro (VD), per venir meno<br />
della forza propulsiva del sangue nel circolo polmonare. Questa relazione non è<br />
stata finora verificata, né è noto se la correlazione tra il TTP e la funzione<br />
contrattile del VS sia <strong>di</strong> tipo lineare. Scopo della ricerca è indagare la relazione<br />
tra TTP e funzione sistolica e <strong>di</strong>astolica del VS, funzione sistolica del VD e<br />
pressioni polmonari.<br />
Meto<strong>di</strong> e Risultati. Abbiamo stu<strong>di</strong>ato 47 pazienti consecutivi (41 m, 6 f; 62±11<br />
anni), sottoposti a risonanza magnetica car<strong>di</strong>aca (RMC) con mezzo <strong>di</strong> contrasto<br />
e ad ecocar<strong>di</strong>ografia transtoracica (ECO-TT) per car<strong>di</strong>opatia ischemica. Sono<br />
state utilizzate sequenze SSFP <strong>di</strong>namiche per calcolare i parametri <strong>di</strong>mensionali<br />
e funzionali <strong>di</strong> entrambi i ventricoli, e sequenze turbo-FLASH per misurare il TTP,<br />
inteso come tempo necessario ad un bolo <strong>di</strong> gadolinio per transitare dal VD al VS<br />
normalizzato per la FC. L’ECO-TT, eseguita entro 1 settimana dalla RMC, è stata<br />
utilizzata per valutare la funzione <strong>di</strong>astolica del VS e la pressione arteriosa<br />
polmonare sistolica (PAPs). La tabella riporta il tipo e il grado <strong>di</strong> correlazione<br />
esistente tra il TTP ed i parametri considerati. Il TTP è influenzato soprattutto dalla<br />
funzione sistolica del VS e del VD con una relazione, illustrata in figura, <strong>di</strong> tipo<br />
rispettivamente inverso e cubico.<br />

Objectives. We evaluated IA, EPS and PSS, as interrogated by dobutamine, to<br />
identify myocar<strong>di</strong>al viability in dysfunctional segments of left ventricle.<br />
Methods. 110 patients (mean age±SD, 58±9 years) with severe left ventricular<br />
dysfunction (mean ejection fraction: 32±11%) underwent DSE. The six-segment<br />
model (posterior and anterior septum, lateral, inferior, anterior and posterior) of<br />
longitu<strong>di</strong>nal wall shortening was performed by TDI during DSE. IA, EPS, PSS<br />
variables were measured (cm/s, cm/s 2 ). Myocar<strong>di</strong>al viability in segments with<br />
severe dyssynergy was defined by biphasic, sustained improvement or ischemic<br />
response by DSE and by re<strong>di</strong>stribution uptake of 99m Tc-tetrofosmin at SPECT.<br />
Results. At low dose dobutamine IA, EPS and PSS were accurate to pre<strong>di</strong>ct<br />
myocar<strong>di</strong>al viability: sensitivity: 89%, 83%, 87%, specificity: 48%, 50%, 49% (K<br />
values: 0.37, 0.34, 0.36). The area under ROC-curve of IA, EPS and PSS<br />
improved from rest to low dose dobutamine with respective cut-off: >175 cm/s 2 ,<br />
>4 cm/s, >6 cm/s and p values:
