I dati relativi all’attività lavorativa, all’esposizione professionale a fattori <strong>di</strong> rischio e ad eventuali fattori <strong>di</strong> confon<strong>di</strong>mento sono stati rilevati dalla cartella clinica e valutati congiuntamente da un equipe <strong>di</strong> me<strong>di</strong>ci specialisti in me<strong>di</strong>cina <strong>del</strong> lavoro. I pazienti sono stati <strong>di</strong>visi in categorie lavorative, considerando quella svolta più a lungo; si segnala che la maggior parte dei pazienti aveva svolto un’unica attività lavorativa. I fattori <strong>di</strong> rischio sono stati considerati solo se l’esposizione è durata per almeno un anno. Le me<strong>di</strong>e <strong>del</strong>le variabili continue sono state confrontate con test non parametrici. Le variabili non continue sono state analizzate con tavole <strong>di</strong> contingenza nelle quali sono stati calcolati X 2 , Odds Ratio e intervallo <strong>di</strong> confidenza al 95% secondo Mantel-Haenszel. Le ra<strong>di</strong>ografie più recenti sono state acquisite e rilette. Tutte le ra<strong>di</strong>ografie sono state lette in doppio cieco da due me<strong>di</strong>ci certificati B-readers secondo la classificazione ILO-BIT (Ginevra 1980) (1). Le prove <strong>di</strong> funzionalità respiratoria sono state eseguite, con spirometro a acqua con campana leggera senza contrappeso, da personale infermieristico adeguatamente formato. Per l’interpretazione <strong>del</strong>le prove <strong>di</strong> funzionalità respiratoria ci si è attenuti alla proposta riportata dall’European Respiratory Society e dall’American Thoracic Society (ATS) (2). Il Volume Residuo è stato misurato con metodo <strong>di</strong> <strong>di</strong>luizione intrapolmonare <strong>di</strong> un gas inerte (elio). La misurazione <strong>del</strong>la <strong>di</strong>ffusione alveolo-capillare (DLCO) è stata eseguita con la tecnica <strong>del</strong> respiro singolo. Per ogni lavoratore inoltre è stato effettuato il confronto dei dati <strong>di</strong> sorveglianza sanitaria con la durata <strong>del</strong>l’esposizione (Total Time Exposure TTE) (3). RISULTATI Sono state spe<strong>di</strong>te 372 lettere; degli 82 lavoratori che hanno aderito allo stu<strong>di</strong>o, 49 rispondevano ai requisiti richiesti, 3 non hanno effettuato le prove spirometriche perché controin<strong>di</strong>cate. Delle 46 spirometrie eseguite, 4 non rispondevano ai criteri ATS, 14 sono risultate nella norma, 15 presentavano un MEF ridotto, 12 deponevano per quadri ostruttivi <strong>di</strong> vario grado, per una la <strong>di</strong>agnosi era <strong>di</strong> restrizione moderatamente severa. La DLCO è risultata alterata in 3 casi. Degli accertamenti ra<strong>di</strong>ografici effettuati, 10 sono risultati positivi o dubbi alla classificazione ILO-BIT; tra questi in 2 casi la classificazione ILO-BIT era 1/0 con patologia professionale in corso <strong>di</strong> riconoscimento da parte <strong>del</strong>l’INAIL, gli altri 8 presentavano un quadro 0/1. Sono stati messi in correlazione i dati <strong>di</strong> anzianità lavorativa per i lavoratori con quadro ra<strong>di</strong>ografico borderline o positivo come riportato nella Tabella I. Analizzando il rapporto tra quadri ra<strong>di</strong>ologici e prove <strong>di</strong> funzionalità respiratoria, si osservava una normalità dei valori <strong>di</strong> funzionalità polmonare come osservato in molti casi <strong>di</strong> silicosi iniziale (Tab. Si è osservato che nei positivi e nei soggetti borderline si ha una funzionalità respiratoria comunque nei limiti <strong>del</strong>la norma. Il piccolo numero <strong>di</strong> positivi non permette <strong>di</strong> fare considerazioni definitive relativamente a tale riscontro, anche se si può sicuramente affermare che questo non è in contrasto con quanto noto degli effetti <strong>del</strong>la silice cristallina con alterazioni ra<strong>di</strong>ografiche associate ad una funzionalità respiratoria nei limiti <strong>del</strong>la norma nei casi iniziali (4). Inoltre si è osservato per i soggetti in esame, rispetto ai lavoratori con ren<strong>di</strong>ta per silicosi osservati in un altro stu<strong>di</strong>o da noi effettuato, un innalzamento <strong>del</strong>l’anzianità lavorativa associato ai quadri ra<strong>di</strong>ografici 0/1 e 1/0. Questo potrebbe essere spiegato con il fatto che i lavoratori esaminati nel primo stu<strong>di</strong>o avevano un’età anagrafica maggiore e un inizio <strong>del</strong>l’esposizione alla silice in epoche antec<strong>ed</strong>enti all’attuazioni <strong>del</strong>le norme <strong>di</strong> prevenzione e protezione nei luoghi <strong>di</strong> lavoro per l’abbattimento <strong>del</strong>le polveri, quando le con<strong>di</strong>zioni lavorative nelle aziende erano sicuramente peggiori. ABSTRACT. This study assess<strong>ed</strong> the smoking habit and smoking cessation stages in a cohort of nurses (N) and nurses’ aides (NA). During a health surveillance check-up all N and NA who were employ<strong>ed</strong> by a Hospital Board answer<strong>ed</strong> a questionnaire. Smoking cessation stages was assess<strong>ed</strong> accor<strong>di</strong>ng to the Prochaska et al. mo<strong>del</strong> (1992). The cohort includ<strong>ed</strong> 1,540 hospital workers, 81% of whom were nurses and 74.8% females. The prevalence of smokers was 32.4% in female workers, 35.9% in males, 32.8% in the N and 35.0% in the NA. Analysis of smoking cessation stages show<strong>ed</strong> 17.4% of smokers were in the precontemplation stage, 71.7% were in the contemplation stage, 7.6% were in the preparation stage and 3.4% in the action stage. No significant <strong>di</strong>fferences emerg<strong>ed</strong> in prevalences stratifi<strong>ed</strong> for gender, schooling, marital status, job, nicotine ad<strong>di</strong>ction, number of cigarettes or age. The prevalence of smokers among hospital employees is higher than in the general Italian population. Most of the employees were in the first stages of change, i.e 17% in the pre-contemplation stage and over 70% in the contemplation stage. Occupational Physicians who are engag<strong>ed</strong> in Health Surveillance should encourage workers to progress through the various stages to cessation by offering appropriate counselling. INTRODUZIONE Il tabagismo è la principale causa prevenibile <strong>di</strong> mortalità e <strong>di</strong> morbilità nei Paesi industrializzati e tutti gli operatori sanitari dovrebbero offrire alla popolazione non solo interventi <strong>di</strong>agnostici e terapeutici per le malattie ad esso correlati, ma anche un comportamento esemplare a tal riguardo. Peraltro, stu<strong>di</strong> svolti su <strong>di</strong>versi gruppi <strong>di</strong> operatori sanitari in Italia hanno rilevato una elevata prevalenza <strong>di</strong> fumatori, in particolare nel personale infermieristico (4, 6). I Me<strong>di</strong>ci <strong>del</strong> <strong>Lavoro</strong> incaricati <strong>di</strong> effettuare la sorveglianza sanitaria possono giocare un ruolo rilevante nel favorire la cessazione <strong>del</strong>l’abitu<strong>di</strong>ne al fumo tra gli operatori sanitari.
