RISK OF OCCUPATIONAL ACCIDENT: COMPARISON<br />
BETWEEN DIFFERENT WORK SHIFT SCHEDULES AND JOB<br />

ABSTRACT. Recent stu<strong>di</strong>es show<strong>ed</strong> a close relationship between<br />
number of working hours, their position in the work day, duration of<br />
previous waking period, drowsiness and accidents at work. In our study<br />
we compar<strong>ed</strong> three <strong>di</strong>fferent kinds of shift workers (178 nurses, 174<br />
policemen, and 126 train drivers) by means of a validat<strong>ed</strong> questionnaire<br />
(Standard Shiftwork Index). Statistical analysis was carri<strong>ed</strong> out using<br />
Kolmogorov-Smirnov test and Cox proportional regression, then<br />
calculating the Fitt<strong>ed</strong> Hazard Function (FHF) and the Simulat<strong>ed</strong> FHF.<br />
Accor<strong>di</strong>ng to our data, the occurrence of occupational accidents seems<br />
to depend both on the job and the work shift sch<strong>ed</strong>ule. Train drivers<br />
have a lower risk of accident than the others workers; yet the<br />
policemen, whose work sch<strong>ed</strong>ule consists of four periods, the last of<br />
which at night, have a much higher risk during the third and especially<br />
the fourth period when compar<strong>ed</strong> with workers whose work sch<strong>ed</strong>ule<br />
consists of only three periods before the rest. The knowl<strong>ed</strong>ge of these<br />
aspects can help the occupational physician to a correct management in<br />
order to r<strong>ed</strong>uce accident risk at shift work: firstly by information and<br />
<strong>ed</strong>ucation of workers (fee<strong>di</strong>ng habits, lifestyle etc.), then by me<strong>di</strong>cal<br />
surveillance aim<strong>ed</strong> to point out eventual clinical situations pre<strong>di</strong>sposing<br />
to accident and to identify criteria for fitness judgment, finally<br />
suggesting the employer ergonomic measures, such as shift changes.<br />
INTRODUZIONE<br />
Il sistema <strong>di</strong> turno adottato, la mansione e le con<strong>di</strong>zioni <strong>di</strong> lavoro<br />
possono con<strong>di</strong>zionare notevolmente la capacità <strong>di</strong> adattamento al lavoro<br />
notturno. Recenti stu<strong>di</strong> mostrano una stretta correlazione fra numero <strong>di</strong><br />
ore lavorative, posizione temporale <strong>del</strong>le stesse, durata <strong>del</strong> tempo <strong>di</strong> veglia<br />
prec<strong>ed</strong>ente e sonnolenza da una parte <strong>ed</strong> infortuni sul lavoro dall’altra.<br />
Scopo <strong>del</strong>lo stu<strong>di</strong>o è stato quello <strong>di</strong> confrontare tre <strong>di</strong>verse categorie<br />
<strong>di</strong> lavoratori turnisti al fine <strong>di</strong> valutare come la mansione lavorativa <strong>ed</strong> il<br />
sistema <strong>di</strong> turnazione adottato influenzino separatamente l’occorrenza <strong>di</strong><br />
infortuni, <strong>ed</strong> adottare le conseguenti strategie <strong>di</strong> prevenzione.<br />
MATERIALI E METODI<br />
Lo stu<strong>di</strong>o è stato condotto in una grande città me<strong>di</strong>ante l’uso <strong>di</strong> un<br />
questionario assistito e validato (Standard Shifwork Index-SSI), somministrato<br />
ad un campione <strong>di</strong> 478 lavoratori turnisti appartenenti a tre <strong>di</strong>fferenti<br />
categorie: 178 infermieri (I), 174 operatori <strong>del</strong>la Polizia <strong>di</strong> Stato (P)<br />
e 126 macchinisti <strong>del</strong>le ferrovie (F).<br />
I sistemi <strong>di</strong> turnazione adottati sono i seguenti: Infermieri “turno in<br />
quarta” con notte prolungata (pomeriggio 14-21; mattino 7-14; notte 21-<br />
7; riposo); Ferrovieri “turno in quarta” (pomeriggio 14-22; mattino 6-14;<br />
notte 22-6; riposo); Poliziotti “turno in quinta” (sera 19-24; pomeriggio<br />
13-19; mattino 7-13; notte 24-7; riposo). Tutti e tre i sistemi sono in senso<br />
antiorario, regolari, con velocità <strong>di</strong> rotazione rapida.<br />
Sono state confrontate le <strong>di</strong>stribuzioni <strong>del</strong> numero <strong>di</strong> infortuni per categoria<br />
lavorativa me<strong>di</strong>ante il test <strong>di</strong> Kolmogorov-Smirnov (KS). L’influenza<br />
<strong>del</strong>la categoria lavorativa e <strong>del</strong>le caratteristiche personali <strong>del</strong> lavoratore<br />
(età, sesso, BMI, anzianità <strong>di</strong> turnismo) sul rischio <strong>di</strong> infortunio<br />
all’interno <strong>di</strong> un ciclo <strong>di</strong> turnazione sono state valutate per mezzo <strong>del</strong>la<br />
regressione proporzionale <strong>di</strong> Cox, che stima l’Hazard Ratio (HR) <strong>di</strong> ogni<br />
variabile inserita nell’analisi e la sua significatività. Il rischio <strong>di</strong> infortunio<br />
HF (Hazard Function) risulta dalla sovrapposizione degli HR <strong>del</strong>le<br />
variabili emerse significative nell’analisi <strong>di</strong> Cox; il suo andamento in funzione<br />
<strong>del</strong> tempo trascorso dall’inizio <strong>del</strong> piano <strong>di</strong> turnazione (Fitt<strong>ed</strong> Hazard<br />
Function - FHF) è stato ben interpolato me<strong>di</strong>ante <strong>del</strong>le funzioni matematiche<br />
strettamente crescenti nel tempo. Infine, per separare gli effetti<br />
<strong>del</strong>la mansione e <strong>del</strong> sistema <strong>di</strong> turnazione, sono state simulate per le tre<br />
categorie lavorative sistemi <strong>di</strong> turnazione <strong>di</strong>fferenti da quelli effettivamente<br />
seguiti; la stima <strong>del</strong> rischio è espressa dalla funzione SFHF (Simulat<strong>ed</strong><br />
FHF).<br />
Per ogni analisi effettuata il livello <strong>di</strong> significatività è stato fissato a 0.005.<br />
RISULTATI<br />
Il campione è prevalentemente costituito da soggetti <strong>di</strong> sesso maschile;<br />
i F sono più anziani <strong>ed</strong> hanno anzianità <strong>di</strong> turnismo significativamente<br />
più elevate rispetto ad I e P.<br />
Il 26% degli I, il 34% dei P <strong>ed</strong> il 19% dei F riporta <strong>di</strong> aver avuto un<br />
infortunio sul lavoro; rispettivamente il 28%, il 7% <strong>ed</strong> il 21% <strong>di</strong> questi lo<br />
attribuiscono all’eccessiva sonnolenza. I P hanno un numero <strong>di</strong> infortuni<br />
significativamente più elevato degli altri (p
