446 G Ital Med Lav Erg 2007; 29:3 www.gimle.fsm.it di SM sono stati adottati i criteri ATPIII modificati: presenza di almeno 3 delle seguenti condizioni sono presenti contemporaneamente nello stesso soggetto: circonferenza vita ≥ 102 cm negli uomini e≥ 88 cm nelle donne, trigliceridi serici ≥ 150 mg/dl, colesterolo HDL ≥ 40 mg/dl negli uomini e ≥ 50 mg/dl nelle donne, pressione arteriosa sistolica ≥130 mmHg, pressione diastolica ≥ 85 mmHg, glicemia ≥ 100 mg/dl. Sono stati inoltre considerati fumo, uso di alcool, livello di scolarità e attività fisica, come fattori di confondimento. La popolazione è stata suddivisa in 2 gruppi in base allo svolgimento di turni notturni. Risultati: l’incidenza di SM è risultata significativamente maggiore tra i lavoratori a turni rispetto agli altri lavoratori: (OR: 4.10, 95% CI 1.34-12.55, p=0.01). La differenza era più evidente nei soggetti al di sotto dei 40 anni (OR=6.6, 95% CI 1.05- 40.85, p=0.04) rispetto a quelli al di sopra di 40 anni: OR= 3.8, 95% CI= 0.92-15.81, p=0.06). La differente incidenza di sindrome metabolica tra i due gruppi era rilevabile anche dopo analisi multivariata, che ha preso in considerazione tutti i fattori di confondimento valutati: OR=3.66, 95% CI 1.25-10.53, p=0.001. Conclusioni in questo studio si dimostra per la prima volta che la turnazione notturna si associa ad un aumentato rischio di sviluppare la sindrome in lavoratori esenti da essa in condizioni di base. Objective: to assess whether shift work has a causeeffect nexus with the incidence of metabolic syndrome. Design: retrospective follow-up study, case-control type. Research Methods and Procedures: a total of 202 female and male nurses, aged 23-60 years, having a 1 year minimal follow-up, and without any metabolic syndrome criterion at the first visit were evaluated. The ATP modified criteria were applied for the diagnosis of metabolic syndrome. Moreover smoking habitus, alcohol consumption, educational level and physical activity were considered as bias factors. The.sample was divided in two groups doing or not night shifts. Results: the metabolic syndrome incidence resulted significantly greater in shift workers than in other workers (OR: 4.10, 95% CI 1.34-12.55, p=0.01). ANALISI STATISTICA Le variabili sono espresse come media e deviazione standard. Il confronto di variabili continue è stato eseguito con il test del t di Student o, quando appropriato, con il test esatto di Fischer. Per il confronto delle variabili categoriche è stato utilizzato il test del Chi quadro. Le differenze nell’incidenza della sindrome metabolica tra i lavoratori in turno notturno ed il gruppo di controllo sono state espresse come Odds Ratio (OR) ed intervalli di confidenza al 95% (95% CI). La differenza è stata inoltre valutata anche con analisi multivariata mediante regressione logistica multipla, inserendo come possibili fattori di confondimento il fumo, la scolarità, l’età ed il consumo di alcool. Infine, per verificare l’andamento temporale dei singoli componenti della sindrome metabolica, è stata utilizzata l’ANOVA per misure ripetute. L’analisi statistica è stata eseguita utilizzando il software SPSS versione 13. Risultati. L’incidenza di sindrome metabolica in corso di follow-up è risultata significativamente maggiore tra i lavoratori impegnati in turno notturno in confronto agli altri lavoratori: gruppo con turno notturno: 19 su 115 (16.52%); altro gruppo: 4 su 87 (4.59%) (OR: 4.10, 95% CI 1.34- 12,55; p=0.01.) La differenza era più evidente nei soggetti al di sotto dei 40 anni; infatti, in questa fascia di età, l’incidenza di sindrome metabolica è risultata pari a 14 su 84 (16.66%) nel gruppo dei lavoratori in turno, e ad 1 su 31 (3.2%) nel secondo gruppo di lavoratori (OR=6.6, 95% CI 1.05-40.85, p=0.04) (Fig. 1). Il confronto tra i due gruppi limitato ai lavoratori con più di 40 anni di età ha mostrato una differenza al limite della significatività statistica: gruppo dei lavoratori con turno notturno: 5 su 31(16.1%); altro gruppo:3 su 56 (5.35%)(OR= 3.8, 95% CI= 0.92-15.81, p=0.06) Figura 2 La differente incidenza di sindrome metabolica tra i due gruppi era rilevabile anche dopo analisi multivariata, che ha preso in considerazione età, fumo, alcool, familiarità e scolarità come possibili fattori di confondimento nell’incidenza della sindrome. Nella grande maggioranza dei sottogruppi considerati ciascun componente della sindrome metabolica è risultato maggiormente alte-

Inoltre, significative variazioni peggiorative in corso di follow-up sono state riscontrate solo per i lavoratori in turnazione notturna per tutte le componenti della sindrome, eccettuata la glicemia. CONCLUSIONI I dati del presente studio mostrano una associazione tra sindrome metabolica e attività lavorativa in turno notturno, sia nei soggetti al di sotto dei che al di sopra dei 40anni di età. Inoltre, un progressivo peggioramento dei fattori associati alla sindrome è stato riscontrato in modo omogeneo nei lavoratori con turnazione notturna. Un possibile effetto negativo dell’attività lavorativa notturna sui parametri della sindrome metabolica è stato riportato in precedenti studi 8-10 . Il nostro è però il primo studio a dimostrare che la turnazione notturna si associa ad un aumentato rischio di sviluppare la sindrome in lavoratori esenti da essa in condizioni di base, e pertanto rafforza l’ipotesi che tra i due eventi esista un nesso di causalità. Va rilevato che la popolazione lavorativa presa in considerazione nel presente studio è caratterizzata da un’attività lavorativa notturna di notevole impegno, e non è pertanto certo che i dati siano estrapolabili ad altra personale lavorativo impegnato in attività notturna con un carico minore di stress. L’analisi multivariata che ha preso in considerazione alcune di tali attività quali il fumo e l’alcool, non ha modificato sostanzialmente l’associazione rilevata, suggerendo un ruolo non di primaria importanza per tali componenti. Poiché l’attività fisica svolta sul posto di lavoro può essere considerata simile per i due gruppi, la sedentarietà sul posto di lavoro può essere esclusa. L’incremento ponderale rilevato nei lavoratori in turnazione notturna può pertanto essere attribuito ad aumentato introito calorico e/o a ridotta attività fisica al di fuori del contesto lavorativo. Esistono dati contrastanti circa le possibili conseguenze sulla salute del lavoro sedentario. Esso potrebbe essere associato alla sindrome metabolica, che è data dalla presenza contemporanea di una serie di fattori di rischio cardiovascolare (ipertensione, iperglicemia, obesità, dislipidemia). Pertanto, abbiamo investigato la prevalenza della sindrome in 1547 videoterminalisti (lavoratori molto sedentari) e in 892 controlli con minore livello di sedentarietà. ANALISI STATISTICA<br />

Risultati. L’incidenza <strong>di</strong> sindrome metabolica in corso <strong>di</strong> follow-up è<br />

notturno in confronto agli altri <strong>la</strong>voratori: gruppo con turno notturno: 19<br />
