G Ital Med Lav Erg 2007; 29:3 409 www.gimle.fsm.it fettuati a tutti i <strong>la</strong>voratori senza <strong>di</strong>stinzione <strong>di</strong> età o selezione tramite anamnesi considerando, oggigiorno, anche <strong>la</strong> significativa mobilità degli OS fra i <strong>di</strong>versi reparti delle strutture ospe<strong>da</strong>liere. Lo screening <strong>per</strong> <strong>la</strong> sieroprevalenza degli anticorpi <strong>per</strong> V, R, P e M appare molto giustificato in quanto può anche portare <strong>dei</strong> vantaggi che possono influire positivamente sul rapporto costo/beneficio. Infatti, sono <strong>da</strong> considerare, <strong>da</strong> <strong>un</strong>a parte, l’elevato numero <strong>di</strong> soggetti suscettibili presente nel<strong>la</strong> popo<strong>la</strong>zione <strong>la</strong>vorativa e <strong>la</strong> potenziale gravità e frequenza degli effetti col<strong>la</strong>terali <strong>di</strong> tali ma<strong>la</strong>ttie (più gravi e più frequenti negli adulti) e, <strong>da</strong>ll’altra, <strong>la</strong> sicurezza degli attuali vaccini ed il basso costo sia <strong>dei</strong> vaccini che <strong>dei</strong> test <strong>di</strong> screening. Ulteriori vantaggi possono arrivare se tali strategie vengono adottate su ampie popo<strong>la</strong>zioni e in partico<strong>la</strong>r modo se le campagne vaccinali sono mirate su sottogruppi a maggior <strong>rischi</strong>o, in<strong>di</strong>rizzando preliminarmente i programmi ai soggetti <strong>di</strong> giovane età, alle donne, ai me<strong>di</strong>ci specializzan<strong>di</strong> e più in generale agli OS che <strong>la</strong>vorano in aree a maggior <strong>rischi</strong>o. Effettuare lo screening <strong>per</strong> tutti gli OS in ass<strong>un</strong>zione, compresi i me<strong>di</strong>ci specializzan<strong>di</strong>, sembra quin<strong>di</strong> <strong>un</strong>a strategia <strong>di</strong> intervento appropriata. Alc<strong>un</strong>i stu<strong>di</strong> internazionali avvalorano molto <strong>la</strong> tesi che <strong>la</strong> promozione <strong>di</strong> programmi vaccinali possa influire positivamente sul rapporto costo-beneficio. Il presente stu<strong>di</strong>o segue le in<strong>di</strong>cazioni <strong>di</strong> autorevoli linee gui<strong>da</strong> internazionali e nazionali. Pressoché tutti gli stu<strong>di</strong> <strong>di</strong> sieroprevalenza <strong>per</strong> M, V, R e P condotti su OS concor<strong>da</strong>no nel raccoman<strong>da</strong>re lo screening e <strong>la</strong> successiva vaccinazione <strong>dei</strong> soggetti suscettibili <strong>per</strong> <strong>la</strong> prevenzione del<strong>la</strong> trasmissione nosocomiale delle citate infezioni. Tali in<strong>di</strong>cazioni sono compatibili con quanto previsto <strong>da</strong>l<strong>la</strong> attuale legis<strong>la</strong>zione italiana, che obbliga i <strong>da</strong>tori <strong>di</strong> <strong>la</strong>voro ad attuare delle strategie preventive <strong>per</strong> le patologie trasmesse <strong>da</strong> agenti biologici e <strong>per</strong> quelle patologie <strong>per</strong> le quali esistano delle vaccinazioni efficaci, nonché a fornire gratuitamente i vaccini ai <strong>la</strong>voratori. Tra i vantaggi del presente stu<strong>di</strong>o va sottolineato l’elevato numero <strong>di</strong> soggetti sottoposti a screening, che ha portato ad <strong>un</strong>a elevata potenza statistica, <strong>la</strong> comparabilità con gli stu<strong>di</strong> presenti in letteratura scientifica, l’attuazione <strong>di</strong> buone pratiche <strong>di</strong> Me<strong>di</strong>cina del Lavoro, il rispetto <strong>di</strong> autorevoli linee gui<strong>da</strong> e del<strong>la</strong> normativa vigente. Fra le limitazioni deve essere rilevata <strong>la</strong> selezione o<strong>per</strong>ata tramite l’anamnesi, che ha escluso <strong>da</strong>llo screening i soggetti che riferivano <strong>di</strong> aver contratto in passato V, R, P e M o <strong>di</strong>chiaravano <strong>un</strong>a pregressa vaccinazione. Ciò ha verosimilmente limitato l’in<strong>di</strong>viduazione <strong>di</strong> alc<strong>un</strong>i soggetti suscettibili. Le strategie fin qui esposte <strong>per</strong>mettono al MC <strong>di</strong> poter o<strong>per</strong>are seguendo buone pratiche e gli attuali stan<strong>da</strong>rd scientifici <strong>per</strong> <strong>la</strong> tute<strong>la</strong> del<strong>la</strong> salute e sicurezza degli OS e <strong>dei</strong> terzi. Va sottolineato infine il ruolo fon<strong>da</strong>mentale del MC nel<strong>la</strong> promozione e gestione <strong>di</strong> queste tematiche <strong>per</strong> quanto riguar<strong>da</strong> <strong>la</strong> <strong>valutazione</strong> del <strong>rischi</strong>o biologico sia livello in<strong>di</strong>viduale che collettivo, <strong>per</strong> <strong>la</strong> stesura <strong>di</strong> adeguati protocolli <strong>di</strong> sorveglianza sanitaria, <strong>per</strong> l’apporto all’informazione/formazione degli OS, nel<strong>la</strong> gestione <strong>dei</strong> giu<strong>di</strong>zi <strong>di</strong> idoneità <strong>per</strong> gli OS suscettibili, anche in rapporto alle eventuali in<strong>di</strong>cazioni o controin<strong>di</strong>cazioni al<strong>la</strong> vaccinazione, e nel<strong>la</strong> gestione delle campagne vaccinali. Obiettivi. È stata pre<strong>di</strong>sposta <strong>un</strong>a revisione del<strong>la</strong> letteratura <strong>per</strong> incrementare le conoscenze circa il valore pre<strong>di</strong>ttivo positivo del test tubercolinico (TT) nel<strong>la</strong> sorveglianza degli o<strong>per</strong>atori sanitari (OS). Meto<strong>di</strong>. Nel<strong>la</strong> banca-<strong>da</strong>ti PubMed e negli atti <strong>dei</strong> Congressi Nazionali SIMLII, sono state raccolte pubblicazioni che hanno descritto <strong>la</strong> sorveglianza degli OS in ospe<strong>da</strong>le me<strong>di</strong>ante TT nel <strong>per</strong>iodo 1997 - 6/2007, escludendo le ricerche effettuate in nazioni con me<strong>di</strong>o-elevata incidenza <strong>di</strong> TB. Risultati. Sono stati selezionati 25 stu<strong>di</strong>. La compliance degli OS al<strong>la</strong> sorveglianza con TT varia <strong>da</strong> 80 a 100%. È stata stimata <strong>un</strong>’incidenza <strong>di</strong> cuticonversioni tra 0,07 e 3,7% ed <strong>un</strong>a prevalenza <strong>di</strong> cutipositività tra 5,7 e 57%, entrambe significativamente associate con <strong>la</strong> vaccinazione con BCG e <strong>la</strong> nascita all’estero. Nel complesso, quasi tutti gli autori hanno <strong>di</strong>mostrato l’associazione tra fattori occupazionali e cutipositività o cuticonversione. L’anzianità <strong>la</strong>vorativa in ospe<strong>da</strong>le non incrementa <strong>la</strong> prevalenza <strong>di</strong> cuticonversioni. Discussione. Il TT positivo e <strong>la</strong> cuticonversione, considerando opport<strong>un</strong>amente i fattori non occupazionali, appaiono associati all’esposizione a Mycobacterium Tuberculosis in ospe<strong>da</strong>le e possono rappresentare <strong>un</strong> utile strumento <strong>per</strong> <strong>la</strong> sorveglianza degli OS. Parole chiave: test tubercolinico, o<strong>per</strong>atori sanitari, sorveglianza sanitaria TUBERCULIN SKIN TEST (TST) SURVEY AMONG HEALTHCARE WORKERS (HCWS) IN HOSPITAL: A SYSTEMATIC REVIEW OF THE LITERATURE ABSTRACT. Objectives: to summarize the evidence on the Tuberculin Skin Test (TST) survey among healthcare workers (HCWs) in hospital, in the <strong>la</strong>st decade. Methods: a systematic search was conducted in PubMed and in the Procee<strong>di</strong>ngs of the congresses of the Italian Society of Occupational Health and Industrial Hygiene (SIMLII); researches conducted in areas with me<strong>di</strong>um-high incidence of Tuberculosis were excluded. Fin<strong>di</strong>ngs: 25 publications were identified. The compliance of HCWs to TST surveys ranged from 80 to 100%, the incidence of TST-conversion from 0,07 to 3,7% and the prevalence of positive TST from 5,7 to 57%. TST-conversion and positive TST were significantly associated to BCG vaccination and foreign-born. Most stu<strong>di</strong>es fo<strong>un</strong>d an association between TST-conversion and occupational exposure in certain location (emergency facilities, internal me<strong>di</strong>cine, infectious <strong>di</strong>seases, intensive care <strong>un</strong>its, pathology, microbiology) and occupational categories (nurses, <strong>la</strong>boratory technicians). In some stu<strong>di</strong>es, <strong>per</strong>iod of hospital employment do not increased TST conversion rate. Discussion: Positive TST and TST conversion rate seem to be associated with occupational exposure to Mycobacterium Tuberculosis in hospital, considering non-occupational factors. Evidence from literature seemed to support the use of TST in the tuberculosis screening programme of HCWs in hospital.
