390 G Ital Med Lav Erg 2007; 29:3 www.gimle.fsm.it salute, sono stati utilizzati sistemi d’e<strong>la</strong>borazione informatica, previa introduzione <strong>dei</strong> <strong>da</strong>ti raccolti nel programma statistico SPSS 10, dopo co<strong>di</strong>fica numerica delle <strong>di</strong>agnosi, secondo i modelli ISTAT <strong>per</strong> le in<strong>da</strong>gini sullo stato <strong>di</strong> salute degli Italiani. RISULTATI Nel campione totale, con <strong>la</strong> so<strong>la</strong> variabile del sesso, le più cospicue e statisticamente significative <strong>di</strong>fferenze tra i sessi hanno riguar<strong>da</strong>to le aritmie car<strong>di</strong>ache, 3.0% nelle donne vs. 1.7% negli uomini, le varicosità agli arti inferiori, 7.7% vs. 1.2%, le ma<strong>la</strong>ttie del<strong>la</strong> pelle, 3.6% vs. 0.6%, le neop<strong>la</strong>sie, <strong>la</strong> cefalea ricorrente, 15% vs. 0.6%, i <strong>di</strong>sturbi psichici, 16.7% vs. 2.3%, le spon<strong>di</strong>lopatie nei tre tratti, 11.4 vs. 6.3, e le artropatie, 5% vs. 3.9%. Negli uomini sono state significativamente più alte le prevalenze delle patologie croniche respiratorie, 14.5% vs. 4.7%, e <strong>la</strong> car<strong>di</strong>opatia ischemica, 2% vs. 0.3%. La variabile età ha <strong>di</strong>mostrato influenza simile tra donne ed uomini, con poche <strong>di</strong>fferenze. Va detto, innanzi tutto, che le donne hanno avuto <strong>un</strong>’età me<strong>di</strong>a significativamente inferiore a quel<strong>la</strong> degli uomini, 42.15 vs. 44.17 anni, P

L’applicazione <strong>di</strong> <strong>un</strong> intervento focalizzato solo su aspetti fisiologici conduce inevitabilmente a sottostimare i problemi connessi con l’esigenza <strong>di</strong> tute<strong>la</strong> del<strong>la</strong> salute delle donne, tanto più <strong>per</strong>ché spesso le patologie sfuggono al meccanismo binomiale del rapporto <strong>rischi</strong>o <strong>la</strong>vorativo/ma<strong>la</strong>ttia. Non è, <strong>per</strong>tanto, semplice esercizio retorico cominciare a progettare <strong>un</strong>a Me<strong>di</strong>cina del Lavoro “<strong>di</strong> genere”. Obiettivo: Analizzare i principali fattori associati al<strong>la</strong> depressione 8 mesi dopo il parto, con partico<strong>la</strong>re attenzione al ruolo dell’attività professionale del<strong>la</strong> donna. Materiali e meto<strong>di</strong>: 352 donne hanno risposto ad <strong>un</strong> questionario faccia-a-faccia pochi giorni dopo il parto, presso l’Ospe<strong>da</strong>le Infantile “Burlo Garofolo” <strong>di</strong> Trieste. Fra queste, 292 hanno risposto ad <strong>un</strong> secondo questionario, telefonico, 8 mesi dopo. La depressione 8 mesi dopo il parto è stata misurata con il General Health Questionnaire (GHQ). Risultati: 8 mesi dopo il parto il 5% delle donne era depresso. Il 32% delle donne si <strong>di</strong>chiarava non sod<strong>di</strong>sfatta del<strong>la</strong> sua situazione professionale, senza <strong>di</strong>fferenze fra quelle che <strong>la</strong>voravano e quelle che stavano a casa. Il 13% riportava problemi re<strong>la</strong>tivi al <strong>la</strong>voro (es. licenziamento, scarsa flessibilità <strong>di</strong> orario). La situazione <strong>la</strong>vorativa non era associata al<strong>la</strong> depressione, mentre lo era <strong>la</strong> congruenza fra ciò che <strong>la</strong> donna stava facendo (<strong>la</strong>vorare/stare a casa) e ciò che avrebbe desiderato fare. La re<strong>la</strong>zione <strong>per</strong>maneva anche control<strong>la</strong>ndo <strong>per</strong> gli altri fattori associati al<strong>la</strong> depressione. Conclusioni: L’insod<strong>di</strong>sfazione legata al<strong>la</strong> propria situazione <strong>la</strong>vorativa incide negativamente sul<strong>la</strong> salute delle donne dopo <strong>la</strong> nascita <strong>di</strong> <strong>un</strong> bambino. WORK AND DEPRESSION IN WOMEN AFTER THE BIRTH OF A CHILD. ABSTRACT. Objective: To analyze the main factors associated to depression at 8 months postpartum, looking more particu<strong>la</strong>rly at women’s employment. Method: 352 women responded to a face-to-face questionnaire few <strong>da</strong>ys after the birth, at the Maternity Hospital “Burlo Garofolo” in Trieste, and 292 of them responded to a telephone interview 8 months <strong>la</strong>ter. Psychological <strong>di</strong>stress 8 months after delivery was evaluated with the General Health Questionnaire (GHQ). Results: 8 months postpartum, 5% of women were depressed. 32% of women were non-satisfied with their current working status, without <strong>di</strong>fferences among those working and those staying at home. 13% of sample reported problems re<strong>la</strong>ted to work (such as being fired or no flexible working hours). The women’s actual working status was not associated with depression, while the congruence between what the woman was doing (working/at home) and her wishes was. The re<strong>la</strong>tionship was still significant after adjusting for other factors associated with depression. Conclusions: Employment <strong>di</strong>ssatisfaction is negatively associated with women’s health after childbirth. To promote mothers’ wellbeing, the question of women’s employment after birth should be addressed. Key words: Depression, work, post-partum, psychosocial factors INTRODUZIONE La depressione post-partum (DPP) è <strong>un</strong> <strong>di</strong>sturbo molto <strong>di</strong>ffuso e stu<strong>di</strong>ato, che colpisce <strong>da</strong>l 5 al 15% delle madri (Nielsen-Forman et al., 2000; Stewart et al., 2003). L’anedonia, <strong>la</strong> tristezza, i sensi <strong>di</strong> colpa, i sentimenti <strong>di</strong> inadeguatezza che lo caratterizzano provocano <strong>un</strong>a profon<strong>da</strong> sofferenza in<strong>di</strong>viduale e familiare. Il costo <strong>di</strong> questo <strong>di</strong>sturbo in termini <strong>di</strong> salute pubblica aumenta ancora se consideriamo gli effetti negativi del<strong>la</strong> DPP sui figli <strong>da</strong>l p<strong>un</strong>to <strong>di</strong> vista cognitivo, emotivo e comportamentale (Murray & Coo<strong>per</strong>, 1997). Diversi stu<strong>di</strong> hanno in<strong>da</strong>gato i fattori <strong>di</strong> <strong>rischi</strong>o associati al<strong>la</strong> DPP (Dennis et al., 2004; Nielsen-Forman et al., 2000; Watt et al., 2002) evidenziando l’importanza degli aspetti psicosociali: in partico<strong>la</strong>re, le <strong>di</strong>fficoltà nel<strong>la</strong> re<strong>la</strong>zione <strong>di</strong> coppia, <strong>la</strong> mancanza <strong>di</strong> supporto sociale, l’insod<strong>di</strong>sfazione rispetto al<strong>la</strong> propria situazione <strong>la</strong>vorativa risultano significativamente associati al<strong>la</strong> DPP (Romito et al., 1999). Gli effetti del <strong>la</strong>voro extra-domestico sul<strong>la</strong> salute delle donne con bambini piccoli è <strong>un</strong> argomento ancora controverso (Romito, 1994). Le donne <strong>la</strong>voratrici hanno complessivamente <strong>un</strong>a salute migliore rispetto alle casalinghe, ma dormono <strong>di</strong> meno e hanno meno tempo libero (Artazcoz et al., 2004; Istat, 2007), sono sod<strong>di</strong>sfatte del proprio <strong>la</strong>voro ma non dello spazio <strong>da</strong> de<strong>di</strong>care a se stesse (Istat, 2004). L’arrivo <strong>di</strong> <strong>un</strong> figlio comporta <strong>per</strong> <strong>la</strong> donna <strong>un</strong>a <strong>di</strong><strong>la</strong>tazione del tempo de<strong>di</strong>cato al <strong>la</strong>voro familiare e <strong>un</strong>a riduzione <strong>di</strong> quello retribuito, in modo <strong>di</strong>ametralmente opposto a quanto avviene <strong>per</strong> gli uomini (Istat, 2007). Gli esiti sul<strong>la</strong> salute psicofisica delle donne, poi, <strong>di</strong>pendono anche <strong>da</strong> altre variabili come il livello <strong>di</strong> istruzione, il red<strong>di</strong>to, <strong>la</strong> c<strong>la</strong>sse sociale (Romito, 1994; Goldberg et al., 2004). Scopo <strong>di</strong> questo stu<strong>di</strong>o è analizzare il ruolo <strong>dei</strong> fattori psicosociali associati al<strong>la</strong> depressione 8 mesi dopo il parto, con partico<strong>la</strong>re attenzione al ruolo dell’attività professionale del<strong>la</strong> donna. PROCEDURA E METODI È <strong>un</strong>o stu<strong>di</strong>o longitu<strong>di</strong>nale svolto a Trieste presso l’I.R.C.C.S. “Burlo Garofolo”, approvato <strong>da</strong>l Comitato <strong>per</strong> <strong>la</strong> Bioetica dell’Istituto. Da gennaio ad aprile 2004 è stato chiesto a tutte le puer<strong>per</strong>e <strong>di</strong> partecipare ad <strong>un</strong>a ricerca sul<strong>la</strong> salute delle donne. Coloro che accettavano, erano intervistate con <strong>un</strong> questionario, e veniva chiesto loro se volevano rispondere ad <strong>un</strong> altro questionario, telefonico, 8 mesi dopo. Le intervistatrici erano competenti sul tema del<strong>la</strong> DPP e informate sui servizi sociosanitari a Trieste. STRUMENTI E MISURE Il primo questionario include domande su: aspetti socio-demografici, intenzionalità del<strong>la</strong> gravi<strong>da</strong>nza, salute del<strong>la</strong> donna e del bambino, parto. Il secondo questionario include domande su: salute del bambino, al<strong>la</strong>ttamento, ripresa dell’attività sessuale, salute fisica e mentale del<strong>la</strong> donna, situazione <strong>la</strong>vorativa, re<strong>la</strong>zione <strong>di</strong> coppia, con<strong>di</strong>visione del <strong>la</strong>voro domestico e <strong>di</strong> cura del bambino, ed es<strong>per</strong>ienze <strong>di</strong> violenza. Salute mentale del<strong>la</strong> donna e DPP: <strong>la</strong> DPP è stata valutata con il General Health Questionnaire (GHQ) nel<strong>la</strong> versione con 12 domande. Abbiamo adottato <strong>un</strong> cut-off >5 <strong>per</strong> in<strong>di</strong>viduare le donne con <strong>un</strong>a sinto-
