E TIROIDITE DI HASHIMOTO<br />
Il ruolo della <strong>trombofilia</strong> e dell’autoimmunità tiroidea<br />
nell’infertilità inspiegata, nel fallimento dell’impianto<br />
ovocitario e nell’<strong>aborto</strong> spontaneo <strong>ricorrente</strong> è<br />
affascinante da stu<strong>di</strong>are ma ancora controverso.<br />
Sembra comunque assodato che la <strong>tiroi<strong>di</strong>te</strong><br />
autoimmune <strong>di</strong> Hashimoto non è , <strong>di</strong> per sé, causa <strong>di</strong><br />
<strong>aborto</strong>.<br />
Tant’è vero che seguo una coppia <strong>di</strong> ragazzi entrambi<br />
(papà e mamma) affetti da <strong>tiroi<strong>di</strong>te</strong> autoimmune che<br />
hanno avuto una bellissima bambina e tantissime altre<br />
ragazze con autoimmunità tiroidea che hanno<br />
regolarmente portato a termine gravidanze plurime,<br />
anche gemellari, sotto stretto e frequente controllo<br />
endocrinologico.<br />
Tuttavia l’associazione tra poliabortività ripetuta (due aborti consecutivi) e <strong>ricorrente</strong> (tre o più aborti<br />
consecutivi) e <strong>tiroi<strong>di</strong>te</strong> autoimmune è significamente elevata (8-12%).<br />
Inoltre nelle donne con TAI (tireopatia autoimmune) si è riscontrato un aumento da 3 a 5 volte del rischio <strong>di</strong><br />
<strong>aborto</strong>.<br />
Ed ancora, nelle gestanti eutiroidee con anticorpi antitireoperossidasi (abTPO) e antitireoglobulina (abTg) si<br />
è evidenziata un aumentata frequenza <strong>di</strong> aborti nel primo trimestre e, nelle donne con abortività <strong>ricorrente</strong><br />
la prevalenza <strong>di</strong> anticorpi abTPO raggiunge il 36%.<br />

Ciò non implica che l’autoimmunità tiroidea sia causa <strong>di</strong> <strong>aborto</strong> né che il titolo autoanticorpale (AbTPO e<br />
AbTg) correli con la probabilità <strong>di</strong> <strong>aborto</strong>.<br />
Infatti il riscontro <strong>di</strong> autoimmunità in genere e tiroidea in particolare sono la spia <strong>di</strong> un <strong>di</strong>sor<strong>di</strong>ne<br />
immunologico più generale che pre<strong>di</strong>spone a tutte le malattie autoimmuni.<br />
Chi ha una malattia autoimmune, come la <strong>tiroi<strong>di</strong>te</strong> <strong>di</strong> Hashimoto, è pre<strong>di</strong>sposto ad averne altre (artrite<br />
reumatoide, lupus, psoriasi, celiachia, vitiligine, alopecia, retto colite ulcerosa, sclerosi multipla,<br />
piastrinopenia autoimmune etc., ed anche l’abortività su base autoimmunitaria).<br />
Quin<strong>di</strong> il riscontro <strong>di</strong> movimento autoanticorpale tiroideo è in<strong>di</strong>cativo <strong>di</strong> <strong>di</strong>sregolazione immunitaria<br />
generale e <strong>di</strong> pre<strong>di</strong>sposizione alle malattie autoimmuni compresa l’abortività autoimmune.<br />
In base alle conoscenze attuali sappiamo, infatti, che la sopravvivenza del feto semi-allogenico è dovuta alla<br />
immunosoppressione materna.<br />
Il trofoblasto riesce ad eludere l’attacco da parte del sistema immunitario materno con vari meccanismi<br />
locali, tra cui la particolare configurazione dell’espressione delle molecole HLA <strong>di</strong> superficie che, come<br />
risaputo, sono coinvolte nella presentazione antigenica al sistema immunitario materno.<br />
In particolare il trofoblasto non esprime molecole MCH <strong>di</strong> classe Ia (HLA-A e HLA-B) con l’eccezione <strong>di</strong><br />
molecole HLA-C, ma soprattutto esprime molecole non classiche quali le HLA-G e le HLA-E.<br />
Il ruolo delle HLA-G è, ad oggi, parzialmente conosciuto ma, si presuppone, secondo l’ipotesi più<br />
accre<strong>di</strong>tata, che l’HLA-G, riveste un ruolo preminente nella resistenza del trofoblasto non villoso alla lisi<br />
me<strong>di</strong>ata dalle cellule NK (natural killer), presenti in gran numero a livello uterino, inibendone l’attività<br />
citolitica e la loro migrazione transplacentare. Anche le molecole HLA-E inibirebbero le cellule NK a livello<br />
dell’interfaccia materno fetale.<br />
Secondo tale teoria, una risposta immunitaria materna eccessiva contro gli antigeni paterni, porterebbe alla<br />
genesi abnorme <strong>di</strong> cellule immunitarie e <strong>di</strong> citochine responsabili <strong>di</strong> abortività ripetuta.<br />
Più precisamente, secondo stu<strong>di</strong> recenti, esistono <strong>di</strong>fferenze significative dei livelli <strong>di</strong> cellule NK nel sangue<br />
periferico nelle donne con abortività ripetuta/<strong>ricorrente</strong> rispetto ai gruppi <strong>di</strong> controllo.<br />
Cellule NK sono state rinvenute anche nell’endometrio e nella decidua <strong>di</strong> donne con abortività ripetuta,<br />
anche se non si conosce il loro ruolo nel meccanismo <strong>di</strong> placentazione.<br />
Secondo queste ipotesi, le cellule NK della mucosa uterina contribuiscono alla risposta citochinica a livello<br />
dell’interfaccia materno fetale.<br />
Tale risposta è sia <strong>di</strong> tipo T-helper-1 (Th1) con sintesi <strong>di</strong> IL-2, IFN (interferone), TNF-alfa, sia <strong>di</strong> tipo T-helper-<br />
2(Th2) con sintesi <strong>di</strong> IL-4, IL-6 e IL-10.<br />

La gravidanza fisiologica sarebbe il risultato <strong>di</strong> una risposta citochinica <strong>di</strong> tipo prevalentemente Th-2, in cui il<br />
blocco degli anticorpi nasconderebbe gli antigeni del trofoblasto fetale dal riconoscimento immunologico da<br />
parte delle cellule citotossiche materne Th1 me<strong>di</strong>ate.<br />
Di contro, le donne con abortività <strong>ricorrente</strong>, avrebbero una risposta citochinica <strong>di</strong> tipo Th-1, sia durante il<br />
periodo dell’impianto che durante la gravidanza a <strong>di</strong>mostrazione del fatto che le alterazioni della tolleranza<br />
immunologica nei confronti dell’unità feto-placentare sarebbero alla base della poliabortività.<br />
Da questa prima <strong>di</strong>samina si può dedurre quin<strong>di</strong> che la positività degli anticorpi <strong>di</strong>retti contro antigeni<br />
tiroidei sia un epifenomeno che riflette un’alterazione del sistema immunitario che comporterebbe il rigetto<br />
del feto.<br />
Inoltre è possibile pure che, in donne con autoimmunità tiroidea e alterazione della fertilità, il<br />
concepimento avvenga in età più avanzata e che questo rappresenti un altro fattore prognostico negativo<br />
aggiuntivo.<br />
Dal punto <strong>di</strong> vista della terapia, invece, in letteratura vi sono lavori che <strong>di</strong>mostrano che in donne con<br />
autoimmunità tiroidea, la terapia con levo-tiroxina, a dosi appropriate tali da mantenere il TSH in un range<br />
ottimale, può ridurre l’incidenza <strong>di</strong> <strong>aborto</strong> <strong>ricorrente</strong>. Inoltre, nelle donne con positività degli anticorpi<br />
anticar<strong>di</strong>olipina e/o alterazioni trombofiliche, il trattamento con eparine a basso peso molecolare + aspirina<br />
a basse dosi (80-100 mg) porta ad un decremento nell’incidenza <strong>di</strong> <strong>aborto</strong>, specie se associato anche all’uso<br />
<strong>di</strong> progestinico (progeffik cp 200 mg) come trattamento empirico, specie in caso <strong>di</strong> insufficienza luteinica.<br />
Un altro interessante lavoro condotto in una Clinica Ginecologica Universitaria italiana ha <strong>di</strong>mostrato che il<br />
trattamento con dosi appropriate <strong>di</strong> levo tiroxina, anche in caso <strong>di</strong> eutiroi<strong>di</strong>smo materno, porta ad un<br />
miglioramento dell’outcome riproduttivo con un aumento del “delivery rate” dall’11 all’86% e riduzione del<br />
“miscarriage (<strong>aborto</strong>) rate” dall’89 al 13,5% dopo trattamento con tiroxina.<br />
Un altro piccolo stu<strong>di</strong>o <strong>di</strong> coorte ha invece riportato che il pretrattamento con prednisone in donne con<br />
<strong>tiroi<strong>di</strong>te</strong> autoimmune <strong>di</strong> Hashimoto migliora la riuscita dell’impianto ovocitario mentre non apporta alcun<br />
beneficio sull’incidenza <strong>di</strong> abortività.<br />

La sindrome da anticorpi antifosfolipi<strong>di</strong> è un’altra<br />
con<strong>di</strong>zione patologica trombofilica, caratterizzata da<br />
ipercoagulabilità, strettamente associata alla <strong>tiroi<strong>di</strong>te</strong><br />
autoimmune <strong>di</strong> Hashimoto che, se non <strong>di</strong>agnosticata in<br />
tempo può determinare abortività ripetuta/<strong>ricorrente</strong>,<br />
preeclampsia, iposviluppo del feto e morte intrauterina.<br />

E’ caratterizzata da trombosi venose e o arteriose, aborti ricorrenti e anticorpi antifosfolipi<strong>di</strong>ci.<br />
Gli anticorpi antifosfolipi<strong>di</strong>ci (anticar<strong>di</strong>olipina IgG e IgM, anti-beta2-glicoproteina I, lupus anticoagulant),<br />
interagendo con varie proteine della coagulazione, piastrine o con le cellule endoteliali possono contribuire<br />
alla patogenesi della malattia. L'incidenza non è nota, comunque la prevalenza degli anticorpi antifosfolipi<strong>di</strong><br />
nella popolazione generale è bassa (1-4,5%) e aumenta con l'età. I sintomi clinici principali associati all'APS<br />
sono la trombosi, l'<strong>aborto</strong>, la trombocitopenia, i sintomi neurologici, la livedo reticularis e l'anemia<br />
emolitica. Gli anticorpi antifosfolipi<strong>di</strong> sono presenti in circa 1/3 dei pazienti con lupus eritematoso sistemico<br />
(LES). Elevati titoli <strong>di</strong> anticorpi anticar<strong>di</strong>olipina, anticorpi LAC (lupus anticoagulant) e, soprattutto, anti-beta2<br />
GPI sono importanti marcatori pre<strong>di</strong>ttivi dei sintomi clinici della APS nei pazienti con SLE. Il trattamento<br />
della trombosi è basato su una terapia a lungo termine e intensiva con warfarina (rapporto internazionale<br />
normalizzato - INR - superiore o uguale a 3). Per il trattamento dell'<strong>aborto</strong> <strong>ricorrente</strong> si raccomanda una<br />
terapia a basso dosaggio <strong>di</strong> aspirina (80 mg/giorno), più eparina sottocutanea, non frazionata o a basso<br />
peso molecolare. (Autore: Dott. M. Tektonidou - Maggio 2004). Le altre altrazioni trombofiliche, che<br />
possono associarsi alla poliabortività sono le mutazioni della MTHFR (metatetraidrofolato-reduttasi) C677T<br />
e A1298C, sia in etero che in omozigosi con conseguente iperomocisteinemia, trattabili con acido<br />
tetraidrofolico (prefolic), ma anche le mutazioni del fattore V <strong>di</strong> Leiden (G1691A) con resistenza alla proteina<br />
C attivata, l’ATIII (antitrombina III), del fattore II (protrombina II G20210A) e il deficit delle proteine C ed S<br />
della coagulazione e gli elevati valori <strong>di</strong> fattore VIII della coagulazione.<br />

Tutte queste con<strong>di</strong>zioni genetiche, se misconosciute e non trattate portano alla trombosi placentare e<br />
all’<strong>aborto</strong>.<br />
In conclusione, è doveroso che in caso <strong>di</strong> <strong>aborto</strong> ripetuto, già dal 2° <strong>aborto</strong>, venga intrapreso uno stu<strong>di</strong>o<br />
approfon<strong>di</strong>to della gestante per escludere tutti i fattori <strong>di</strong> rischio e le cause secondarie conosciute <strong>di</strong> <strong>aborto</strong>.<br />
Bisogna eliminare i fattori <strong>di</strong> rischio come il fumo <strong>di</strong> sigaretta, perché la nicotina inibisce la sintesi del<br />
progesterone e il rilascio <strong>di</strong> PGE2, allontanarsi da ev. esposizioni ambientali nocive per la salute, eliminare il<br />
consumo <strong>di</strong> alcoolici e <strong>di</strong> cocaina per l’effetto avverso sulla fertilità, limitare l’uso <strong>di</strong> FANS e anti depressivi<br />
nel periodo preconcezionale. Escludere la presenza <strong>di</strong> infezioni <strong>di</strong> virus, batteri ed effettuare lo screening<br />
delle infezioni TORC (toxoplasmosi, rosolia, citomegalovirus, herpes), clami<strong>di</strong>e, micoplasmi etc, che<br />
possono, non curate, contribuire ad innescare contrazioni uterine espulsive e <strong>aborto</strong>.<br />
Ancora, nello stu<strong>di</strong>o della poliabortibvità vanno escluse le cause cromosomiche materne e/o paterne con<br />
esame del cariotipo con ricerca <strong>di</strong> traslocazioni parentali e <strong>di</strong> errori nell’inattivazione dell’X, trisomie<br />
16,18,21,22, triploi<strong>di</strong>e e poliploi<strong>di</strong>e, monosomia X, traslocazioni sbilanciate etc.<br />
Non meno importante è la ricerca <strong>di</strong> anomalie anatomiche uterine: utero setto, bicorne ,unicorne, presenza<br />
<strong>di</strong> sinechie o peggio s.<strong>di</strong> Asherman. L’isteroscopia <strong>di</strong>agnostica associata alla laparoscopia rimangono gli<br />
accertamenti fondamentali per lo stu<strong>di</strong>o delle anomalie anatomiche della cavità uterina onde provvedere<br />
alla loro eliminazione .<br />
Non ultime vanno ricercate altre endocrinopatie quali il già citato ipotiroi<strong>di</strong>smo/<strong>tiroi<strong>di</strong>te</strong> autoimmune <strong>di</strong><br />
Hashimoto, anche nelle forme cosiddette “milde”, il <strong>di</strong>abete mellito, l’insufficienza luteinica con inadeguata<br />
secrezione <strong>di</strong> progesterone da parte del corpo luteo, l’iperprolattinemia e la PCOS (sindrome dell’ovaio<br />
policistico).<br />
In caso <strong>di</strong> insufficiente/inadeguata secrezione progestinica vi è razionale nell’uso dell’ormone progesterone<br />
per via vaginale o orale o intramuscolare in particolare nel primo trimestre <strong>di</strong> gravidanza.<br />
Qualcuno ha segnalato, quale possibile effetto avverso, la ipospa<strong>di</strong>a nei neonati maschi.<br />
E’ ancora <strong>di</strong>battuta la dose e la durata del trattamento progestinico ma, una metanalisi evidenzia una<br />
significativa riduzione <strong>di</strong> aborti in donne con poliabortività, trattate con progesterone rispetto a quelle<br />
trattate con placebo e non trattate.<br />
