→ Il flusso ematico renale rappresenta un quarto della<br />
gettata cardiaca ed è il più alto dell’organismo in rapporto<br />
alle dimensioni dell’organo<br />
→ eterogeneo(principalmente diretto alla corticale per<br />
ottimizzare la filtrazione glomerulare ed il riassorbimento dei<br />
soluti mentre il flusso per la midollare è basso volto a<br />
preservare il gradiente osmotico e mantenere la<br />
concentrazione dell’urina)<br />
→ particolare vulnerabilità del tratto sottile ascendente<br />
della midollare alta(si realizza il gradiente osmotico con<br />
l’attivo riassorbimento del sodio che richiede una grande<br />
quantità di ossigeno)<br />

ha osservato una significativa riduzione dei mediatori<br />
dell’apoptosi nel sangue circolante di pazienti settici<br />
trattati con HVHF rispetto ai pazienti trattati con dosi<br />
convenzionali, dimostrando perciò degli effetti biologici<br />
positivi sulla risposta immunitaria cellulare<br />
Anche il Drotrecogin alfa (attivato), tra le sue attività, ha<br />
proprietà anti-apoptotiche evidenti:sarebbe<br />
interessante combinare il suo utilizzo<br />
con l’HVHF .

Perché il trattamento extracorporeo del sangue<br />
dovrebbe avere un impatto positivo sui diversi<br />
organi?<br />
….una possibile risposta potrebbe essere perché<br />
tutti gli organi hanno in comune il contatto con il<br />
sangue ,e…tutte le terapie extracorporee hanno<br />
come fine quello di depurare il sangue!<br />
Da queste osservazioni e dalla conoscenza della<br />
biologia molecolare della sepsi,acquista significato<br />
la “teoria umorale” della sindrome da disfunzione<br />
multiorgano (MODS); diventa cosi corollario<br />
considerare le terapie extracorporee come terapia<br />
di supporto multiorgano (MOST) e non come<br />
supporto di un singolo organo. (Bellomo 2004)
