Updated Recommendations on
postmenopausal hormone therapy (HT)
Budapest February 2007
Governing Principles
IMS 2007 Updated Recommendations
-HT must be individualized and tailored according to symptoms and the
need of prevention…..
- HT includes a wide range of hormonal products and routes of
administaration with potentially different benefits and risks
- Menopause before the age of 45 years ……..will benefit from
Hormone….
- HT should not be recommended without a clear indication…..
- Women….. should have at lest an annual consultation….
- …no reasons to place mandatory limitations on the length of
treatment…
- Dosage should be titrated to the lowest effective dose.
- …progestogen should be added to systemic estrogen……Natural
progesterone ed some progestogens have specific beneficial effects….
- Effetti Benefici Generali
- Sintomi vasomotori
- Sintomi urogenitali
- Disturbi psico-emozionali e dell’umore
- Disturbi del comportamento
- Disturbi sessuali e della sessualità
- Dolori articolari e muscolari
- Osteoporosi Postmenopausale
- Patologia Cardiovascolare
IMPORTANZA DEGLI ORMONI
EVENTI AVVERSI DELLA TERAPIA ORMONALE
- Tromboembolismo Venoso
- Cancro Mammario
- Eventi Cardiovascolari
PER LA SCELTA FINALE DEL FARMACO E’
SEMPRE NECESSARIO EFFETTUARE UN
BILANCIO TRA GLI EFFETTI BENEFICI ED
EVENTUALI EVENTI AVVERSI COLLEGABILI,
TENENDO PRESENTE IL QUADRO CLINICO
COMPLESSIVO
In 50-59 year-old women a “window of opportunity” for
a benefit in cardiovascukar disease displays a high
plausibility.
EMAS strongly recommend evaluating individual risk
factors before prescription of an HRT.
Lifestyle counselling is also strongly recommended
since exercise, loosing weight and low alcohol intake
can decrease the risk of breast cancer
Maturitas, 2008
EVIDENZE PER GLI EFFETTI BENEFICI GENERALI
HT remains the most effective theraphy for
“vasomotor” and estrogen deficient “urogenital
symptoms”
Other menopause-related complain (joint and muscle
pains… mood swings…sexual dysfunction………..may
improuve during HT
Individualizated HT ……..…improuves overall quality
of life
EVIDENZE PER L’OSTEOPOROSI
HT is effective in preventing the bone loss
associated with the menopause
HT decreases the incidence of all osteoporosis-related
fractures
HT is an appropriate firstline (?) therapy in postmenopausal women
presenting with an increased risk for fracture…under the age of 60
years and for the prevention of bone loss in women with premature
menopause
Estrogeni: riduzione rischio fratture
Estrogeni e Osteoporosi
13 Trials selezionati:
Estrogeni e Osteoporosi
L’effetto protettivo è dipendente dalle dosi
Le dosi standard sono:
Estradiolo Micronizzato 1 mg
Estradiolo Emidrato 1 mg
Estradiolo Valerato 1-2 mg
Estradiolo 2 mg
Estrogeni Coniugati 0.625 mg
Estradiolo TransDermico 50 μμμμgggg
Estradiolo Transd. Gel 1 mg
Estradiolo TransNasale 200 µg
Tibolone 2.5 mg
l’osteoporosi
Progestinici e Osteoporosi
In vitro studies provides some evidence for a beneficial effect of
progestins on bone.
This effecxt appears to be mediated by PR expression in
osteoblasts as through glucocorticoid receptor
Progestins do not add or subtract much of the protective action
of oestrogens on the bones
There are no consistent data demonstrating relevant differences
in bone-sparing effects between the various progestins used
in clinical practice
IMS 2007 Updated Recommendations
7 studi caso-controllo, 1 studio di coorte e 9 trials randomizzati controllati
ORMONI:
- 17β estradiolo, CEE, estrogeni esterificati (studi osservazionali)
- CEE/ CEE + MPA/ MPA / progesterone micronizzato (PEPI)
- CEE + MPA (HERS)
- Estradiolo + Norethisterone acetato (EVTET)
- CEE/ CEE + MPA (ERA)
- 17β estradiolo (WEST)
- Estradiolo valerato (ESPRIT)
- CEE / CEE + MPA (WHI)
- CEE / CEE + MPA (WISDOM)
CONCLUSIONI:
8 studi osservazionali e 9 trials randomizzati controllati
8 studi osservazionali e 9 trials randomizzati controllati
- i Norpregnani (nomegestrolo acetato e promegestone) appaiono trombogenici
- il progesterone micronizzato e i Pregnani (didrogesterone, medrogestone,
clormadinone acetato, ciproterone acetato, medrossiprogesterone acetato)
appaiono sicuri rispetto al rischio tromboembolico venoso
- necessari ulteriori studi per confermare tali dati

CONCLUSIONI:
10 tibolone 2.5 mg
18 CEE+MPA (0.625 mg+2.5 mg)
. nessun effetto su AT III, Proteina S, TFPI (tissue factor pathway inhibitor)
……the possible risk of breast cancer associated with
HT is small (less than 0.1% per annum).
….WHI study indicate that no increased risk is observed in women
initiating HT, for up to 7 years:
… WHI and Nurses’ Health Study suggest that long-term
estrogen-only administration for 7 to 15 years, respectively, does
not increase the risk
There are insufficient data to evaluate the possible differences
in the incidence of BC ….. using different types and routes of
estrogen, natural progesterone and progestogens and
androgen administration
IMS 2007 Updated Recommendations
Studio caso-controllo:
CONCLUSIONI:
