mario composito di morte, infarto e stroke è risultato signicativamente ridotto da Ticagrelor<br />
rispetto a Clopidogrel. Anche tutti gli obiettivi secondari escluso lo stroke e la trombosi su<br />
stent sono stati signicativamente meno nel gruppo trattato con Ticagrelor. Questa maggior<br />
efcacia è stata osservata qualunque fosse la dose da carico di Clopidogrel impiegata.<br />
Tutto ciò non è stato gravato da maggiori sanguinamenti.<br />
Nelle linee guida ESC 2011 relative ai pazienti con SCA senza STEMI Ticagrelor viene raccomandato<br />
per tutti i pazienti a rischio moderato od alto di eventi ischemici qualunque sia<br />
il trattamento precedente e qualunque sia la strategia prevista.<br />
Conclusioni<br />
Nel corso degli ultimi anni è stata riconosciuta l’importanza dell’inibizione del recettore<br />
P2Y12 piastrinico quale target chiave per la terapia antiaggregante delle sindromi coronariche<br />
acute. Il Clopidogrel, che si è andato affermando quale antiaggregante di riferimento<br />
non è in grado di garantire in tutti i pazienti una inibizione piastrinica efcace ed il suo<br />
semplice aumento di dosaggio non appare una strada che garantisca l’obiettivo terapeutico.<br />
Pertanto la ricerca farmacologica e clinica hanno individuato nuovi farmaci in grado<br />
di migliorare la risposta terapeutica antiaggregante. Prasugrel e Ticagrelor sono entrati a<br />
tutti gli effetti tra i farmaci raccomandati seppure per indicazioni diverse. Ticagrelor, nuovo<br />
farmaco già attivo n dal suo assorbimento, inibisce in modo reversibile il recettore P2Y12,<br />
favorisce un’inibizione piastrinica rapida e potente, e si è dimostrato assai efcace nelle<br />
SCA con rischio moderato ed elevato. Ticagrelor , in particolare, ha ridotto la mortalità cardiovascolare<br />
e quella per tutte le cause senza causare un aumento di mortalità per sanguinamento.<br />
Esistono differenze nel disegno degli studi dei nuovi antiaggreganti che<br />
possono rendere difcile il confronto dei dati di mortalità .<br />
Con la dovuta attenzione questi nuovi farmaci sono in grado di offrire una potente azione<br />
antiaggregante e di migliorare l’outcome rispetto al trattamento con Clopidogrel.<br />
