competenze della medicina spaziale possono già interessare
una molteplicità di eventi che riguardano il nostro pianeta,
legati alla sua dinamica astrofisica e geologica, in quanto
parte di un sistema cosmico.
La cronoastrobiologia, disciplina emergente, ci aiuta a comprendere
e considerare gli effetti che fenomeni astrofisici
hanno sulla salute, e, per quel che ci riguarda, sulla salute
mentale.
Ma di maggiore importanza e attualità sono le grandi emergenze
del pianeta, legate a fenomeni sismici, atmosferici, o
quant’altro riguarda esso come entità fisica (fenomeni astrofisici,
radiazioni, magnetismo, composizione atmosferica
ecc.).
Le catastrofi naturali vedono oggigiorno impegnata l’intera
umanità, e hanno un impatto economico, sociologico,
psicologico su vaste popolazioni, su chi è vittima e su chi
soccorre e deve essere tempestivamente preparato a questo.

La Disaster Psychiatry rientra tra le competenze della medicina
spaziale e in particolare di chi intende occuparsi di
Space Psychiatry.
Appartiene all’ambito specialistico della medicina spaziale
la telemedicina satellitare con le possibili applicazioni nell’assistenza,
oltre che nell’emergenza. La tecnologia satellitare
consente di raggiungere rapidamente ed a costi sostenibili
numerose utenze, nella formazione e aggiornamento degli
operatori, nella consultazione specialistica, e nell’assistenza.
È importante sottolineare come proprio nelle situazioni
di maggiore disagio e/o disabilità, sia determinante ed
efficace giungere più vicino possibile all’utente e a chi lo assiste.
Ecco che la telemedicina satellitare potrebbe e dovrebbe
trovare applicazione prioritaria proprio nell’assistenza
agli anziani, ai disabili e ai malati mentali. Il volano tecnologico
ed economico che verrebbe attivato darebbe forza
e interesse alle categorie sanitarie più socialmente svantaggiate
e stigmatizzate.

Treatment of Obsessive Compulsive
Disorder (ROC): from controlled trials
to reality

Background: despite the growing interest during the last
two decades, Obsessive Compulsive Disorder (OCD) still a
complex and difficult to treat condition. Serotonergic antidepressants
were developed in the treatment of OCD, as
well as some augmentation and combination strategies for
resistant cases, ROC. However, few studies were dedicated
to explore the phenomenology of ROC. These studies are
limited by the selection criteria imposed by strict protocols.
In this context, the AFTOC, French Association of subjects
with OCD, was concerned by a significant rising number of
people suffering from ROC.
Results: the new survey of AFTOC “TOC & ROC” have selected
a sample of 360 patients, who are members of the association.
The rate of ROC was 44.2%, 25.3% of Good responders
(GR), and 30.5% in between. Inter-group comparisons
(vs. GR) showed that the ROC group was significantly
characterized by: higher rates of psychiatric admissions (49%
vs. 28%), suicide attempts (26% vs. 13%), higher numbers of
doctors consulted (5.5 vs. 3.2), compulsions (4.6 vs. 3.4), and
psychiatric comorbidity (2.8 vs. 2.0, especially agoraphobia,

social anxiety and worry about appearance). Assessment by
full TEMPS-A scale revealed, in ROC group, significant
higher rates of cyclothymic Temperament (63% vs. 43%, p =
.0003), depressive Temperament (72% vs. 53%, p = .004),
and Irritable Temperament (21% vs. 9%, p = .02). Logistic regression
analyses (ROC method) were used to identify predictive
factors of ROC. Data showed that the most powerful
factors were worsening under SRIs and worry about appearance
(both OR = 2.85), followed by current age above 40 yrs
(OR = 2.68), and psychiatric admission (OR = 2.23).
Conclusion: we submit the hypothesis that cases with ROC
should be explored through specific comorbidity (obsession
of appearance, social anxiety, and Cyclothymic temperament),
and especially worsening with serotonergic antidepressants.
More vigilance is needed toward suicide risk in
this condition.
