Posologia
Per os 2 mg/kg/dose ogni 8 ore (max 12 mg/kg/die)
Ev Bolo EG > 36 settimane 1,5 mg/kg/dose ogni 8 ore
EG < 36 settimane 0,5 mg/kg/dose ogni 12 ore
Ev Continua EG > 36 settimane 0,2 mg/kg/h
EG < 36 settimane 0,06 mg/kg/h
Ridurre la posologia in caso di insufficienza epatica o renale
In previsione di sospensione ridurre il dosaggio del 30% ogni giorno per
evitare iperacidità reattiva
• Inibitori della pompa protonica: Omeprazolo
0,7-3,3 mg/kg /die in 1-2 dosi, 30 minuti prima del pasto
