EG > 28 settimane:
3,5–5 mg/kg in bolo, poi, dopo 12 ore dal bolo,
2,5 mg/kg ogni 12 ore, in 1ª settimana
3,5–5 mg/kg in bolo, poi, dopo 8 ore dal bolo,
2,5 mg/kg ogni 8 ore, dopo la 1ª settimana
Vancomicina

EG 29-35 settimane: 15 mg/kg ogni 12 ore
