J’exécute ensuite un snap-back-test (pression entre deux doigts) pour vérifier l’absence de laxité de la peau au niveau de la paupière inférieure. Pour finir, je demande au patient de regarder vers le haut pendant que j’exerce une pression sur l’os malaire. Cela me permet d’évaluer la quantité à injecter pour corriger le creux. Si je remarque que mon patient a un problème d’hyperpigmentation, je le préviens que cet acte n’aura pas d’impact sur la couleur du cerne, mais permettra de combler l’ombre causée par le creux. If I notice that my patient has issues with hyperpigmentation, I tell them that the procedure will not affect the colour of the under-eye area, but will fill the shadows caused by the cavity. The danger zones to avoid are the angular vein and artery, which are found in the inner corner of the eye, as well as the infraorbital nerve that comes out of the infraorbital foramen, which is easily located by drawing a vertical line through the centre of the pupil (eyes facing forward). Le traitement à la canule de 25 G et un point d’entrée latéral au foramen permettent d’éviter ces dangers. Pour l’injection, je réalise un pré-trou 1 à 2 cm en-dessous du canthus externe de l’œil. Avec une canule de 25 G 40 mm, je me dirige en profondeur vers le coin interne de l’œil de façon très délicate. Puis je réalise des rétrotraçantes très lentement dans le SOOF médial au contact du pré-périosté le long du sillon palpébro-nasal. Enfin et si nécessaire, depuis le même point d’entrée, je change la direction de ma canule pour injecter cette fois-ci vers le canthus externe de l’œil, toujours en rétrotraçante, le long du sillon orbito-malaire, dans le SOOF latéral et au contact du pré-périosté. J’injecte vraiment par petites quantités et au grand maximum 0,5 mL par cerne. Je préfère sous-corriger et revoir mon patient 3 semaines après pour une retouche éventuelle. Et pour finir il est important de choisir le bon produit. Le cerne est une zone très délicate qui nécessite un gel léger, modelable et faiblement hygroscopique pour ne pas surcharger le réseau lymphatique et éviter l’œdème. Pour ma part j’injecte toujours cette zone avec TEOSYAL Redensity ® 2, seul produit sur le marché qui a été conçu spécifiquement pour l’injection du cerne creux. Finally, it is important to choose the right product. The under-eye is a very delicate area that requires a very light gel that is mouldable and mildly hygroscopic so as not to overcharge the lymphatic system and avoid any swelling.
