Atteinte spécifique du faisceau cortico spinal auniveau de la capsule interne et du tronc cérébralqui est caractéristique du neuro behçet avec unebonne corrélation clinique avec le syndromepyramidal (manifestation clinique la plusfréquente).
L’atteinte de la substance blanche est beaucoupplus fréquente que celle de la substance grise.Les noyaux rouges sont épargnés.Le tegmentum est rarement atteint.
