PRINCIPES GÉNÉRAUX D’IMMUNOLOGIE ET D’IMMUNISATION CHAPITRE 11.3 PRODUITS IMMUNISANTSL’immunisation permet de procurer au corps humain les moyens de se défendre contre une agressionbiologique avant qu’elle survienne. Dans l’immunisation active, il s’agit de stimuler le systèmeimmunitaire par un produit immunisant connu et contrôlé en évitant les conséquences liées àl’infection naturelle. Dans l’immunisation passive, il s’agit d’un transfert d’anticorps, appelésimmunoglobulines, provenant d’un sujet immunisé à un autre qui ne l’est pas.1.3.1 VACCINSUn vaccin est un produit biologique fabriqué à partir de bactéries ou de virus complets, de leursconstituants (polysaccharides, protéines) ou de leurs produits (toxines), dont on enlève par différentsprocédés la capacité de produire la maladie tout en conservant celle d’induire une réponseimmunitaire (immunogénicité). Les vaccins peuvent être inactivés ou vivants atténués (voir letableau suivant).L’immunogénicité d’un vaccin dépend des facteurs propres à l’antigène, notamment sa morphologie,sa nature chimique et sa masse moléculaire, de la voie d’administration et de l’utilisationd’adjuvants. De façon générale, les protéines sont les substances immunogènes les plus puissantes.En outre, plus la masse moléculaire est élevée, plus l’antigène est immunogène. Certains vaccinsconstitués de polysaccharides de petit poids moléculaire sont ainsi conjugués à une protéine afin deles rendre plus immunogènes, à un plus jeune âge. Les principales protéines utilisées pour laconjugaison dans la fabrication des vaccins actuels sont les suivantes : l’anatoxine diphtérique,l’anatoxine tétanique, la variante non toxique de la toxine diphtérique (CRM 197 ) et la protéine OMPprovenant de la capsule du Neisseria meningitidis.10 Avril 2009
