XII. URGENCES CHIRURGIE INTRA-CRANIENNEPHYSIOPATHOLOGIE-La situation la plus trompeuse est sans doute celle d’un patient ayant destroubles modérés de la conscience (score de Glasgow entre 11 et 13) que l’on vousprésente comme une urgence ? L’état clinique semble rassurant mais ces patientssont souvent à la limite de la décompensation cérébrale. Une très faibleaugmentation du volume intracrânien peut provoquer une HTIC majeure.Il est donc parfois indispensable de discuter avec le chirurgien responsable pours’organiser au mieux …toujours au bénéfice du patient !!-Le facteur temps doit être présent à l’esprit-Anticiper les complications per-op et de la période péri-opératoire ( gravité del’engagement cérébral préopératoire, contrôle de l’HTIC postopératoire précoce)-Souvent pas à jeun, état de conscience peut changer très vite !!- PaCO2, Inhalation-Modifications hémodynamiques de l’induction-!! Chute TA lors de l’ouverture de la dure-mère et/ou de ladécompression cérébrale (parfois amélioration spectaculaire de la PaO2)-Garder PPC optimale à tout moment (infusion de noradrénaline sinécessaire !! flush inopportun, une toute petite dose est parfoissuffisante)-!! Implication importante du SNC dans la régulation de la pressionartérielle-La phase aiguë d’une agression cérébrale peut s’accompagner d’un étathyperdynamique avec augmentation du débit cardiaque, tachycardie, HTA.Parfois d’anomalies de l’ECG (troubles de la repolarisation pouvant évoquer uneischémie myocardique).Les signes de bradycardie, HTA, bradypnée si pas encore intubé = signe degravité = signes d’engagement (réponse de Cushing à l’HTIC)133
