251.6. PréventionLa prévention <strong>de</strong> la CPE commence chez la femme enceinte car la mère est la principa<strong>le</strong>source d’infection (Law et al., 2007). Durant sa grossesse, la femme est dans une pério<strong>de</strong><strong>de</strong> gran<strong>de</strong> motivation pour améliorer sa santé. Le moment est propice pour se défaire <strong>de</strong>smauvaises habitu<strong>de</strong>s et mettre en place <strong>de</strong>s comportements favorisant une bonne santébucco<strong>de</strong>ntaire (Herzig et al., 2006). Les mesures préventives qui sont recommandées durantla grossesse contribuent à diminuer <strong>le</strong> réservoir bactérien maternel afin <strong>de</strong> prévenir ouretar<strong>de</strong>r la transmission du SM à l’enfant (Koh<strong>le</strong>r et al., 1983; Koh<strong>le</strong>r et al., 1994;Berkowitz, 2003). Les facteurs <strong>de</strong> risque pour la CPE sont importants quand la mèreprésente un niveau é<strong>le</strong>vé <strong>de</strong> SM, <strong>de</strong>s caries actives et qu’el<strong>le</strong> consomme une diète riche enaliments sucrés (Smith et al., 2002; Ersin et al., 2006). Les applications <strong>de</strong> vernis fluoré,l’utilisation <strong>de</strong> rince-bouches à la chlorhexidine et au fluorure <strong>de</strong> sodium ainsi que <strong>le</strong>s soinsd’hygiène professionnel<strong>le</strong> ont entraîné une amélioration significative <strong>de</strong> la santé <strong>de</strong>ntaire<strong>de</strong>s femmes durant la grossesse (Brambilla et al., 1998; Vasiliauskiene et al., 2007). Lagomme à mâcher au xylitol est aussi recommandée car el<strong>le</strong> a été associée à une réductionsignificative <strong>de</strong> la transmission du SM à l’enfant (So<strong>de</strong>rling et al., 2000; Autio-Gold, 2002;Thorild et al., 2003).Chez <strong>le</strong>s jeunes enfants, certains comportements alimentaires sont à éviter car ilsaugmentent <strong>le</strong> risque <strong>de</strong> CPE. Le fait <strong>de</strong> mettre <strong>de</strong>s boissons sucrées dans <strong>le</strong> biberon, <strong>de</strong>laisser l’enfant dormir avec son biberon, <strong>de</strong> <strong>le</strong> laisser boire au biberon à volonté durant lajournée, <strong>de</strong> servir <strong>de</strong>s boissons sucrées avant <strong>le</strong> coucher et entre <strong>le</strong>s repas, et <strong>de</strong> donner <strong>de</strong>scollations sucrées entre <strong>le</strong>s repas, sont <strong>de</strong>s comportements qui créent un environnementfavorab<strong>le</strong> à la croissance bactérienne et à la production d’aci<strong>de</strong>s (Weinstein et al., 1996;Barber et Wilkins, 2002; Hal<strong>le</strong>tt et O’Rourke, 2002; Tiberia et al., 2007).Plusieurs instances professionnel<strong>le</strong>s par exemp<strong>le</strong>, l’Association <strong>de</strong>ntaire canadienne,l’Ordre <strong>de</strong>s <strong>de</strong>ntistes du Québec, l’Ordre <strong>de</strong>s Hygiénistes <strong>de</strong>ntaires du Québec, l’AmericanDental Association, recomman<strong>de</strong>nt l’utilisation d’une débarbouil<strong>le</strong>tte humi<strong>de</strong> pour nettoyerl’intérieur <strong>de</strong> la bouche du nouveau-né. Le brossage <strong>de</strong>s <strong>de</strong>nts débute avec l’arrivée <strong>de</strong> lapremière <strong>de</strong>nt en bouche. L’âge auquel <strong>le</strong> brossage <strong>de</strong>s <strong>de</strong>nts est commencé ainsi que la
26fréquence à laquel<strong>le</strong> il est effectué sont <strong>de</strong>s facteurs <strong>de</strong> protection significatifs contre laprésence <strong>de</strong> CPE (Vanobbergen et al., 2001; Tiano et al., 2009).Selon <strong>le</strong>s recommandations <strong>de</strong> l’Association américaine <strong>de</strong> <strong>de</strong>ntisterie pédiatrique (AAPD,2008), la première visite <strong>de</strong> l’enfant <strong>de</strong>vrait être effectuée 6 mois après l’éruption <strong>de</strong> lapremière <strong>de</strong>nt, au plus tard à l’âge <strong>de</strong> 1 an. La rencontre précoce avec <strong>le</strong> <strong>de</strong>ntiste estsouhaitab<strong>le</strong> car celui-ci pourra expliquer aux parents <strong>le</strong>s comportements à mettre en placeavec <strong>le</strong>ur enfant pour <strong>le</strong> maintien d’une santé <strong>de</strong>ntaire optima<strong>le</strong>. De plus, la rencontre avec<strong>le</strong>s parents permet d’i<strong>de</strong>ntifier et <strong>de</strong> modifier <strong>le</strong>s mauvaises habitu<strong>de</strong>s avant qu’el<strong>le</strong>s nesoient bien établies (Harrison, 2003). Une méta-analyse effectuée sur l’influence <strong>de</strong>l’éducation sur la santé <strong>de</strong>ntaire a révélé que cel<strong>le</strong>-ci améliorait la connaissance maisqu’el<strong>le</strong> avait peu d’impact à long terme sur <strong>le</strong>s changements <strong>de</strong> comportement (Kay etLocker, 1996; Gussy et al., 2006). De plus, <strong>le</strong>s recommandations ont plus <strong>de</strong> chances d’êtresuivies par la population qui est la moins à risque, ce qui contribue à entretenir et renforcer<strong>le</strong>s inégalités socia<strong>le</strong>s en santé <strong>de</strong>ntaire (Baric, 1989; Poutanen et al., 2006; Franco et al.,2008). La perception qu’a <strong>le</strong> parent <strong>de</strong> sa capacité à effectuer <strong>le</strong> brossage <strong>de</strong>s <strong>de</strong>nts et àcontrô<strong>le</strong>r <strong>le</strong>s habitu<strong>de</strong>s <strong>de</strong> collation sucrées, est un prédicteur significatif <strong>de</strong> la mise enapplication <strong>de</strong>s comportements favorab<strong>le</strong>s à une bonne santé <strong>de</strong>ntaire (Adair et al., 2004).Dans une entrevue effectuée auprès <strong>de</strong> parents d’enfants ayant eu <strong>de</strong>s soins <strong>de</strong>ntaires sousanesthésie généra<strong>le</strong>, Amin et Harrison (2009) rapportent que <strong>le</strong>s parents souhaitaient que<strong>le</strong>s professionnels <strong>le</strong>urs donnent <strong>de</strong>s trucs et <strong>le</strong>urs montrent comment s’y prendre pourbrosser <strong>le</strong>s <strong>de</strong>nts <strong>de</strong> <strong>le</strong>ur enfant plutôt que <strong>de</strong> <strong>le</strong>urs transmettre <strong>de</strong> l’information sous forme<strong>de</strong> dépliants. Les recommandations sur l’hygiène bucca<strong>le</strong> sont plus souvent suivies quecel<strong>le</strong>s qui sont effectuées pour changer <strong>le</strong>s comportements alimentaires (Levine, 1996). Lesentrevues motivationnel<strong>le</strong>s lors <strong>de</strong> rencontres personne à personne semb<strong>le</strong>nt être unestratégie efficace pour amener un changement <strong>de</strong> comportement (Pine et al., 2004;Weinstein et al., 2004; Harrison et al., 2007; John, 2008). Dans une étu<strong>de</strong> sur <strong>le</strong>s entrevues<strong>de</strong> motivation, Weinstein et coll. (2006) ont constaté que <strong>le</strong>s enfants <strong>de</strong>s mères qui avaientbénéficié d’une entrevue couplée à six appels téléphoniques <strong>de</strong> suivi, avaient moins <strong>de</strong>nouvel<strong>le</strong>s caries après <strong>de</strong>ux ans, que <strong>le</strong>s enfants du groupe contrô<strong>le</strong> où l’intervention s’étaitlimitée à la distribution <strong>de</strong> dépliants et au visionnement d’un vidéo.
